Abstract

Recommender Systems have not been explored to a great extent for improving health and subjective wellbeing. Recent advances in mobile technologies and user modelling present the opportunity for delivering such systems, however the key issue is understanding the drivers of subjective wellbeing at an individual level. In this paper we propose a novel approach for deriving personalized activity recommendations to improve subjective wellbeing by maximizing the congruence between activities and personality traits. To evaluate the model, we leveraged a rich dataset collected in a smartphone study, which contains three weeks of daily activity probes, the Big-Five personality questionnaire and subjective wellbeing surveys. We show that the model correctly infers a range of activities that are 'good' or 'bad' (i.e. that are positively or negatively related to subjective wellbeing) for a given user and that the derived recommendations greatly match outcomes in the real-world., Comment: Presented at ACM Conference on Recommender Systems (RecSys) 2019, Copenhagen, Denmark

15. The Teaching Activity and the Generation of Mathematical Learning Opportunities

Abstract

We present a case study that has been developed to inform about the teaching activity of a secondary mathematics teacher in a whole group discussion and the mathematical learning opportunities generated for the students in this classroom context. For data of one lesson we determine the episodes that shape the whole group discussion. For each episode we then examine the effects of the observed actions on the type of learning that can be encouraged. Our research reveals significant relationships between the teaching activity of the teacher and the creation and potential exploitation of mathematical learning opportunities on the part of the students. Abstract

Dental implants offer an effective solution for partial and total edentulism, but mechanical and biological complications exist. Furthermore, high occlusal loads challenge implants and lead to potential failures. This review focuses on impact testing in contrast to incremental and static tests, an underexplored aspect of assessing daily loads on implants, bringing to light potential complications. The review examines studies employing impact forces to assess implant-supported prostheses and natural teeth properties, highlighting their significance in dental research. A systematic search following PRISMA guidelines identified 21 relevant articles out of 224, emphasizing studies employing impact forces to evaluate various aspects of dental implant treatments. The diverse applications of impact forces in dental research were categorized into tooth structure, restorative materials, interface evaluation, implant properties, and finite element models. Some studies showed the significance of impact forces in assessing stress distribution, shock absorption, and biomechanical response. Impact testing is a critical tool for understanding the daily forces on implants. Despite diverse experimental approaches, a lack of standardized protocols complicates the systematization of the results and, therefore, the conclusions. Abstract

Purpose Ventilator associated-lower respiratory tract infections (VA-LRTIs), either ventilator-associated pneumonia (VAP) or tracheobronchitis (VAT), accounts for most nosocomial infections in intensive care units (ICU) including. Our aim was to determine if appropriate antibiotic treatment in patients with VA-LRTI will effectively reduce mortality in patients who had cardiovascular failure. Methods This was a pre-planned subanalysis of a large prospective cohort of mechanically ventilated patients for at least 48 h in eight countries in two continents. Patients with a modified Sequential Organ Failure Assessment (mSOFA) cardiovascular score of 4 (at the time of VA-LRTI diagnosis and needed be present for at least 12 h) were defined as having cardiovascular failure. Results VA-LRTI occurred in 689 (23.2%) out of 2960 patients and 174 (25.3%) developed cardiovascular failure. Patients with cardiovascular failure had significantly higher ICU mortality than those without (58% vs. 26.8%; p < 0.001; OR 3.7; 95% CI 2.6-5.4). A propensity score analysis found that the presence of inappropriate antibiotic treatment was an independent risk factor for ICU mortality in patients without cardiovascular failure, but not in those with cardiovascular failure. When the propensity score analysis was conducted in patients with VA-LRTI, the use of appropriate antibiotic treatment conferred a survival benefit for patients without cardiovascular failure who had only VAP. Conclusions Patients with VA-LRTI and cardiovascular failure did not show an association to a higher ICU survival with appropriate antibiotic treatment. Additionally, we found that in patients without cardiovascular failure, appropriate antibiotic treatment conferred a survival benefit for patients only with VAP. Abstract

Background: Animal models of acute respiratory distress syndrome (ARDS) do not completely resemble human ARDS, struggling translational research. We aimed to characterize a porcine model of ARDS induced by pneumonia—the most common risk factor in humans—and analyze the additional effect of ventilator-induced lung injury (VILI). Methods: Bronchoscopy-guided instillation of a multidrug-resistant Pseudomonas aeruginosa strain was performed in ten healthy pigs. In six animals (pneumonia-with-VILI group), pulmonary damage was further increased by VILI applied 3 h before instillation and until ARDS was diagnosed by PaO2/FiO2 < 150 mmHg. Four animals (pneumonia-without-VILI group) were protectively ventilated 3 h before inoculum and thereafter. Gas exchange, respiratory mechanics, hemodynamics, microbiological studies and inflammatory markers were analyzed during the 96-h experiment. During necropsy, lobar samples were also analyzed. Results: All animals from pneumonia-with-VILI group reached Berlin criteria for ARDS diagnosis until the end of experiment. The mean duration under ARDS diagnosis was 46.8 ± 7.7 h; the lowest PaO2/FiO2 was 83 ± 5.45 mmHg. The group of pigs that were not subjected to VILI did not meet ARDS criteria, even when presenting with bilateral pneumonia. Animals developing ARDS presented hemodynamic instability as well as severe hypercapnia despite high-minute ventilation. Unlike the pneumonia-without-VILI group, the ARDS animals presented lower static compliance (p = 0.011) and increased pulmonary permeability (p = 0.013). The highest burden of P. aeruginosa was found at pneumonia diagnosis in all animals, as well as a high inflammatory response shown by a release of interleukin (IL)-6 and IL-8.
