Abstract

PURPOSE: To assess whether the combination of letrozole, metronomic cyclophosphamide and sorafenib (LCS) is well tolerated and shows activity in primary breast cancer (BC). METHODS: Thirteen oestrogen receptor-positive, postmenopausal, T2-4, N0-1 BC patients received the LCS combination for 6 months. In these patients we examined the pharmacokinetics of sorafenib and cyclophosphamide, toxicity of the regimen, the clinical response to therapy and changes in the levels of biologically relevant biomarkers. RESULTS: Adequate plasma concentrations of sorafenib were achieved in patients when it was dosed in combination with L+C. The mean plasma concentrations of C were consistently lower following administration of LCS, compared with administration of L+C only. The most common drug-related grade 3/4 adverse events were skin rash (69.3%), hand-foot skin reaction (69.3%) and diarrhoea (46.1%). According to RECIST Criteria, a clinical complete response was observed in 6 of 13 patients. A significant reduction in tumour size, evaluated with MRI, was also observed between baseline and 14 days of treatment in all 13 patients (P=0.005). A significant reduction in SUV uptake, measured by (18)FDG-PET/CT, was observed in all patients between baseline and 30 days of treatment (P=0.015) and between baseline and definitive surgery (P=0.0002). Using modified CT Criteria, a response was demonstrated in 8 out of 10 evaluable patients at 30 days and in 11 out of 13 evaluable patients at the definitive surgery. A significant reduction in Ki67 expression was observed in all patients at day 14 compared with baseline (P<0.00001) and in 9 out of 13 patients at the definitive surgery compared with baseline (P<0.03). There was also a significant suppression of CD31 and VEGF-A expression in response to treatment (P=0.01 and P=0.007, respectively). CONCLUSIONS: The LCS combination is feasible and tolerable. The tumour response and target biomarker modulation indicate that the combination is clinic

Abstract

To evaluate a patient population affected by Neurofibromatosis type 1 (NF1) with special reference to optic pathway gliomas.A total of 419 NF1 patients (222 men, 197 women) were retrospectively evaluated to show incidence, biological behaviour, neuroradiological and clinical presentation of optic pathway gliomas. Central Nervous System MR and ophthalmologic evaluations were reviewed.Thirty-two patients out of 419 (7.6%) showed the presence of optic pathway gliomas. Of these, 28 (87.2%) patients showed tumoral involvement of the optic chiasma. The right intracranial optic nerve was involved in 15 patients (46.8%), the left intracranial optic nerve in 18 (56.2%), the right extracranial optic nerve in 11 (34.3%), the left extracranial optic nerve in 10 (31.2%) and the right and left optic tracts in 6 (18.7%) patients. Geniculate body and optic radiation were not involved by tumour diffusion. Visual acuity testing was positive in 26/32 (81.2%), the fundus oculi in 11/32 (34.3%) patients and the visual field evaluation in 12/32 (37.5%) patients.In this study we showed an high correlation between optic pathway tumours and ophthalmologic abnormalities confirming the usefulness of an integrated clinical-neuroradiological approach in patients affected by NF1. We also described the biological behaviour of these tumours in our NF1 patient population.

Abstract

To investigate the relationship between Magnetic Resonance (MR) findings and the presence of isolated growth hormone deficiency (IGHD) or multiple pituitary hormone deficiency (MPHD).39 children, 27 boys and 12 girls (mean age 9.6 years) were enrolled. The growth hormone deficiency and MPHD were defined by standard laboratory methods. MR scans of the hypothalamo-hypophyseal tract were carried out on all patients before the therapy. Perinatal histories were reviewed.The pituitary anterior lobe was considered small in 13/39 (33%). Twenty-six of 39 (66%) patients did not show any abnormalities of the hypothalamohypophyseal tract. An ectopic neurohypophysis was found in 8/39 patients (20%). Only 2 patients among those with an ectopic neurohypophysis showed a small anterior pituitary lobe. Thirty-three of 39 (84.6%) patients showed IGHD while 6/39 (15.4%) patients showed MPHD. Twenty-eight of 39 (71%) patients had a severe deficiency while 11/39 (28%) patients had a moderate deficiency.Our study confirms the usefulness of MRI in the diagnostic and therapeutic approach to short stature in children; in fact 48% of patients in our series showed MR findings reasonably related to hormonal deficiency.

Abstract

One thousand mammograms of non-preselected patients were reviewed in order to assess in how many and which cases an equivocal diagnosis can be made unquestionable and to test the value of follow-up exams with direct radiographic magnification. Direct radiographic magnification was used in all questionable cases (3%): 18 lesions were characterized by microcalcifications and 12 by nodules. Four cases were diagnosed as unquestionable (13%)-1 microcalcification and 3 nodules-while 13 patients were reexamined with mammography only. Even though the percentage variation from equivocal to unquestionable diagnoses is modest but significant, direct radiographic magnification is always useful in the follow-up.

Abstract

Cerebrospinal fluid (CSF) waveform evaluation through the use of Phase Contrast Cine-Magnetic Resonance (MR) imaging after pre-mammillary endoscopic ventriculostomy.A descriptive study of 21 patients, 13 men and 8 women (average age 36), submitted to pre-mammillary endoscopic ventriculostomy for treatment of non-communicating hydrocephalus. Evaluation of CSF was accomplished using a Siemens System (1.5 Siemens Magnetom 63SP) with continuous data recording and retrospective cardiac gating. The quantitative study was performed with in-plane and through-plane spatial resolution: sagittal and transverse 2D FISP (70,100 TR/13,16 TE, flip angle 15°), recording the mean velocity at the level of aqueduct, pontine cistern and CSF space in front of C1 as well as mean and maximum velocities and stroke volume at the ventriculostomy site.A Pearson correlation (P<0.001) is present between diastolic and systolic stroke volumes not only at the site the ventriculostomy but also among stroke volumes and velocities in the pontine cistern and aqueduct. The only level where no statistical correlation was found between stroke volumes and velocities was the CSF space before the first cervical vertebral body (C1).Analysis with Phase Contrast Cine-MR imaging shows that pre-mammillary endoscopic ventriculostomy is efficient for restoring cerebrospinal fluid pulsatile waveform.
