Wealthy nations must step up support for Africa and vulnerable countries in addressing past, present and future impacts of climate change. The 2022 report of the Intergovernmental Panel on Climate Change (IPCC) paints a dark picture of the future of life on earth, characterised by ecosystem collapse, species extinction, and climate hazards such as heatwaves and floods. These are all linked to physical and mental health problems, with direct and indirect consequences of increased morbidity and mortality. To avoid these catastrophic health effects across all regions of the globe, there is broad agreement—as 231 health journals argued together in 2021—that the rise in global temperature must be limited to less than 1.5oC compared with pre-industrial levels. While the Paris Agreement of 2015 outlines a global action framework that incorporates providing climate finance to developing countries, this support has yet to materialise. COP27 is the fifth Conference of the Parties (COP) to be organised in Africa since its inception in 1995. Ahead of this meeting, we—as health journal editors from across the continent—call for urgent action to ensure it is the COP that finally delivers climate justice for Africa and vulnerable countries. This is essential not just for the health of those countries, but for the health of the whole world.

Background & aim: The process of country lockdown, social distancing and limited access to services as a result of COVID-19 pandemic brought a change to established patterns in the antenatal care services in the primary health care systems. The current article presents the lived experience of utilization of antenatal services among pregnant women during covid-19 pandemic.Methods: A descriptive phenomenological design was used to elicit data from 12 pregnant women. Pregnant women who booked in healthcare services were recruited from Gauteng and Limpopo province in South Africa using a purposive and snowballing sampling method. The data were collected and recorded from July to August 2020, either through the telephone or e-platforms (Zoom and google meet). The recorded data was transcribed and analyzed using the steps outlined by Sundler et all.Results: Four themes emerged from the analysed data. Three themes focused on the experiences of the pregnant women showing the negative impact of the COVID-19 pandemic on antenatal services including: fear of contracting COVID-19 during antenatal; limited available antenatal services and coping with the new normal. The fourth theme is need for digital health education which focused on the antenatal services development.Conclusion: The study showed that the covid-19 pandemic negatively impacted utilization of antenatal services among pregnant women. It is therefore important for the health departments to adopt innovative ways to improve utilization of antenatal services among pregnant women during a pandemic situation such as the COVID-19 pandemic.

Background: Although diagnosis and treatment of diarrhoea are considered easy, statistics show that 525,000 children worldwide die annually due to diarrhoea, 90% of the deaths are in Sub-Saharan Africa and South Asia, and Mozambique account for 6.9%. Assessment of practices of diagnosis and treatment of diarrhoea in children under five were con-ducted in Maputo, Mozambique. Design and method: The study was retrospective – source of information: record books from 2015 to 2019. All statements about age, gender, signs and symptoms, diagnoses, and treatment were collected to assess practices implemented by the health professionals to diagnose diarrhoea in children under five. Results: A total of 9,041 cases were found, where 4,052 (44, 8 %) were female, urban area accounts for 7,668 (74.8 %). Children younger than 6 months 1,013 (11,2%); from 6 to 11 months 1,370 (15,2%); from 12 to 23 months 2,535 (28 %); from 24 to 35 months 1,674 (18.5 %), from 36 to 47 months 1,239 (13.7 %) and from 48 to 59 months 1,210 (13.4 %). About 3,644 (40.3 %) had fever, 3,467 (38 %) vomit, 1,999 (22 %) blood in stool and other symptoms; only 5 (1 %) of the children’s stools were submitted for laboratory analysis. The clinical diagnoses were diarrhoea 3,905 (43 %), diarrhoea and vomit 2,037(22 %) and others. The main treatment was oral rehydration salts 7,118 (79 %) and 21 % antibiotics. Conclusion: Even when the signs and symptoms (fever and blood in stool) suggested or required laboratory exams, this was not done. Nevertheless, the children were treated with antibiotic without the screening of etiological agent.

Effective management of labour pain is an essential aspect of care provided to women during childbirth. The aim of this study was to assess pregnant women’s knowledge of using non-pharmacological pain relief methods during childbirth. Methods: This quantitative descriptive cross-sectional study was conducted at four hospitals in the Tshwane District of South Africa. The sample consisted of 384 pregnant women. Results: The results show that (n = 200, 52.1%) pregnant women lack knowledge regarding non-pharmacological pain relief methods, while a minority (n = 101, 26.3%) had some knowledge, some (n = 73, 19%) were uncertain about these methods and others (n = 10, 2.6%) did not comment. Additionally, most respondents (n = 232, 60%) never received education about the various methods available to manage pain during labour at antenatal care, while others (n = 131, 34%) agreed that they received such education. A significant relationship between the level of education and knowledge about non-pharmacological pain relief methods was noted (p = 0.0082). In conclusion, respondents knew methods such as massage, breathing exercises, movements and birth positions. However, they lacked knowledge of birth companionship, acupuncture, transcutaneous electrical nerve stimulation (TENS), aromatherapy and music. The overall findings revealed that antenatal care services provided by midwives are not effectively preparing pregnant women for pain relief during childbirth.

Introduction Nurses are essential for implementing evidence-based practices to improve patient outcomes. Unfortunately, nurses lack knowledge about research and do not always understand research terminology. This study aims to develop an in-service training programme for health research for nurses and midwives in the Tshwane district of South Africa.Methods and analysis This protocol outlines a codesign study guided by the five stages of design thinking proposed by the Hasso-Plattner Institute of Design at Stanford University. The participants will include nurses and midwives at two hospitals in the Tshwane district, Gauteng Province. The five stages will be implemented in three phases: Phase 1: Stage 1—empathise and Stage 2—define. Exploratory sequential mixed methods including focus group discussions with nurses and midwives (n=40), face-to-face interviews (n=6), and surveys (n=330), will be used in this phase. Phase 2: Stage 3—ideate and Stage 4—prototype. A team of research experts (n=5), nurses and midwives (n=20) will develop the training programme based on the identified learning needs. Phase 3: Stage 5—test. The programme will be delivered to clinical nurses and midwives (n=41). The training programme will be evaluated through pretraining and post-training surveys and face-to-face interviews (n=4) following training. SPSS V.29 will be used for quantitative analysis, and content analysis will be used to analyse qualitative data.Ethics and dissemination The protocol was approved by the Faculty of Health Sciences Research Ethics Committee of the University of Pretoria (reference number 123/2023). The protocol is also registered with the National Health Research Database in South Africa (reference number GP_202305_032). The study findings will be disseminated through conference presentations and publications in peer-reviewed journals.
