Cochlear implantation is the common treatment for severe to profound sensorineural hearing loss if there is no benefit from hearing aids. Measuring the electrode impedance along the electrode array at different time points after surgery is crucial in verifying the electrodes’ status, determining the compliance levels, and helping to identify the electric dynamic range. Increased impedance values without proper reprogramming can affect the patient’s performance. The prediction of acceptable levels of electrode impedance at different time points after the surgery could help clinicians during the fitting sessions through a comparison of the predicted with the measured levels. Accordingly, clinicians can decide if the measured levels are within the predicted normal range or not. In this work, we used a dataset of 80 pediatric patients who had received cochlear implants with the MED-EL FLEX 28 electrode array. We predicted the impedance of the electrode arrays in each channel at different time points: at one month, three months, six months, and one year after the date of surgery. We used different machine learning algorithms such as linear regression, Bayesian linear regression, decision forest regression, boosted decision tree regression, and neural networks. The used features include the patient’s age and the intra-operative electrode impedance at different electrodes. Our results indicated that the best algorithm varies depending on the channel, while the Bayesian linear regression and neural networks provide the best results for 75% of the channels. Furthermore, the accuracy level ranges between 83% and 100% in half of the channels one year after the surgery, when an error range between 0 and 3 KΩ is defined as an acceptable threshold. Moreover, the use of the patient’s age alone can provide the best prediction results for 50% of the channels at six months or one year after surgery. This reflects that the patient’s age could be a predictor of the electrode impedance after the surgery.

One of the most widely used measures of scientific impact is the number of citations. However, due to its heavy-tailed distribution, citations are fundamentally difficult to predict but can be improved. This study was aimed at investigating the factors and parts influencing the citation number of a scientific paper in the otology field. Therefore, this work proposes a new solution that utilizes machine learning and natural language processing to process English text and provides a paper citation as the predicted results. Different algorithms are implemented in this solution, such as linear regression, boosted decision tree, decision forest, and neural networks. The application of neural network regression revealed that papers’ abstracts have more influence on the citation numbers of otological articles. This new solution has been developed in visual programming using Microsoft Azure machine learning at the back end and Programming Without Coding Technology at the front end. We recommend using machine learning models to improve the abstracts of research articles to get more citations.

Background: In some patients with cochlear implants, bony resorption at the internal receiver-stimulator site can be observed on postoperative computed tomography. Therefore, it is essential to elucidate the effects of the internal receiver-stimulator on the scalp and bony bed over time. Objective: We aimed to evaluate how the internal receiver-stimulator of a cochlear implant device changed the thickness of the surrounding scalp and skull over time using computed tomography. Methods: This retrospective study evaluated patients who underwent cochlear implantation and received two computed tomography scans postoperatively for different indications at a tertiary referral centre. The main outcomes were scalp thickness and bony bed depth. Results: Fourteen ears were included in this study. There was very good inter-rater reliability, with a Cronbach’s alpha of 0.94. The mean scalp thickness over the internal receiver-stimulator was 6.02 (+/-2.4) mm in the first scan and decreased with no significant change to 5.62 mm (+/-1.64) in the second scan (p = 0.59, paired t-test). The mean depth of the bony bed increased significantly from 1.39 mm (+/-0.93) to 2.62 mm (+/-1.24) (p = 0.03). Conclusion : There was no change in the thickness of the scalp overlying the internal receiver-stimulator indicating that the scalp was more resistant than the bony skull to the tensile pressure exerted by the internal receiver-stimulator. In contrast, the bony bed depth of the internal receiver-stimulator increased over time. This can result in decreased internal receiver-stimulator protrusion and decreased risk of device displacement and migration.

The mathematical equations to estimate cochlear duct length (CDL) using cochlear parameters such as basal turn diameter (A-value) and width (B-value) are currently applied for cochleae with two and a half turns of normal development. Most of the inner ear malformation (IEM) types have either less than two and a half cochlear turns or have a cystic apex, making the current available CDL equations unsuitable for cochleae with abnormal anatomies. Therefore, this study aimed to estimate the basal turn length (BTL) from the cochlear parameters of different anatomical types, including normal anatomy; enlarged vestibular aqueduct; incomplete partition types I, II, and III; and cochlear hypoplasia. The lateral wall was manually tracked for 360° of the angular depth, along with the A and B values in the oblique coronal view for all anatomical types. A strong positive linear correlation was observed between BTL and the A- (r2 = 0.74) and B-values (r2 = 0.84). The multiple linear regression model to predict the BTL from the A-and B-values resulted in the following equation (estimated BTL = [A × 1.04] + [B × 1.89] − 0.92). The manually measured and estimated BTL differed by 1.12%. The proposed equation could be beneficial in adequately selecting an electrode that covers the basal turn in deformed cochleae.

Background and objectives Electrode migration after cochlear implantation (CI) is a rare complication that accounts for 1to 15% of all revision surgery. This study is a systematic review of the literature for investigating the knowledge and approaches to the incidence of electrode migration after CI. Methods A systematic electronic search of the literature was carried out using PubMed, Cochrane, Virtual Health Library, Scopus and Web of Science (ISI). All original articles that reported electrode migration after CI surgery were included. The Newcastle-Ottawa Scale and CARE checklist were utilized for the assessment of the risk of bias. Descriptive data analysis was performed using SPSS software. Results A total of 26 studies including 4,316 patients were included. Out of them, 289 patients had electrode migration following CI. To diagnose electrode migration, traditional computed tomography scan was used in 13 studies, while cone-beam computed tomography was applied in three studies. In addition, electrode migration was detected during intraoperative exploration in eight studies. The most common presenting symptom was change in sound/poor performance (n = 43) followed by pain sensation (n = 15) and facial nerve stimulation (n = 10). Cholesteatoma was the most common associated pathology (n = 10) followed by infection (n = 9) and ossification of the basal turn of the cochlea (n = 8). Conclusion Electrode migration is a major complication of CI and could be more common than previously thought. As it may occur with or without clinical complaints, long-term follow-up through routine radiological scanning is recommended. Further studies are warranted to identify the underlying mechanism of electrode extrusion and the appropriate fixation method.

BACKGROUND: Cochlear implant (CI) recipients with a cochleovestibular malformation (CVM) are at a higher risk of experiencing an intra-operative cerebrospinal fluid (CSF) gusher and, therefore are at greater risk of developing postoperative meningitis than are CI recipients with normal cochlear anatomy. To control CSF gushers, the FORM electrode array was developed. OBJECTIVES: To assess the ability of the FORM24 electrode array in managing intraoperative CSF gushers and preventing postoperative CSF leakage in a population of CI recipients. DESIGN: Retrospective. SETTING: Tertiary health care center. PATIENTS AND METHODS: All CIs in which a FORM24 was used between January 2014 and March 2018 were reviewed for demographic and safety results. MAIN OUTCOME MEASURES: Safety results were assessed as the intraoperative or postoperative presence of an episode of CSF leakage or meningitis. SAMPLE SIZE: 177 CI recipients. RESULTS: Thirty-six (20.3%) had a CVM and 141 had normal anatomy (79.7%). Of the 36 participants with a CVM, 20 (55.6%) experienced an intraoperative CSF gusher, all of which were resolved. No cases of postoperative leakage or meningitis were recorded after a mean follow-up time of 36 months. CONCLUSION: The FORM24 array is able to help surgeons stop intraoperative CSF gushers and prevent postoperative CSF leakage and meningitis in CI recipients with a CVM. OBJECTIVES: To evaluate the feasibility of performing cochlear implantation under conscious sedation (CS) as day surgery with same-day fitting. MATERIALS AND METHODS: All patients underwent cochlear implantation under CS between November 2017 and April 2018. The data collected included demographic information, preoperative clinical characteristics, surgical details, postoperative fitting information, and side effects, if any. RESULTS: Nine patients had 11 cochlear implants (CIs) placed under CS (2 patients received bilateral CIs). One patient’s data were excluded from the audiological results because conversion to general anesthesia (GA) was necessary. One patient (11%) vomited just before the end of the procedure. Seven patients had uneventful procedures. Eight (88%) patients were discharged home the same day. BACKGROUND: Cochlear morphology and cochlear duct length (CDL) play important roles in the selection of appropriate electrodes. Cochlear parameters such as diameter (A value) and width (B value) are used as inputs for calculating the CDL. Current measurements of these parameters are inefficient and time consuming. Recently developed otological planning software (OTOPLAN) allows surgeons to directly measure these parameters and then automatically calculate the CDL. OBJECTIVES: The primary objective was to validate this new software for measuring the cochlear parameters and CDL. The secondary aim was to investigate the correlation between each cochlear parameter with the calculated CDL. DESIGN: Retrospective. SETTINGS: Ear specialist hospital. PATIENTS AND METHODS: The measurement of cochlear diameter (A value) was chosen as the validation parameter. To do this, the A value was measured by a neurotologist on the new OTOPLAN planning software and was validated to the one measured on the currently used DICOM viewer. Upon the validation of the OTOPLAN software, the other two cochlear parameters, namely width (B value) and height (H value) were measured, and CDL was automatically calculated. Finally, the correlation of all parameters with the CDL was statistically analyzed. MAIN OUTCOME MEASURES: Validation of OTOPLAN and CDL estimation. SAMPLE SIZE: 88 ears. RESULTS: There was no significant difference between the A-value measured on the DICOM viewing software and that on the new planning software by the two independent neurotologists (P=.27). Both A-and B-values showed a high positive correlation to the CDL. However, the B-value showed a stronger correlation to the CDL than the A-value (r=0.63 for A, and r=0.96 for B). CONCLUSION: The direct measurement of cochlea parameters and automatic calculation of the CDL could improve the efficiency of clinical workflow and make otology surgeons more independent. Moreover, the cochlear width (B) has a strong correlation to the CDL. Thus, we suggest using the combination of A and B to accurately estimate the CDL rather than using only one. Objective: The objective of this study was to determine the shape of cochlear basal turn through basic cochlear parameters measurement. The secondary aim was to overlay an image of the precurved electrode array on top of the three-dimensional (3D) image of the cochlea to determine which shape of the cochlear basal turn gives optimal electrode-to-modiolus proximity. Materials and Methods: Computed tomography (CT) preoperative image-data sets of 117 ears were made available for the measurements of cochlear parameters retrospectively. Three-dimensional slicer was used in the visualization and measurement of cochlear parameters from both 3D and 2D (2-dimensional) images of the inner ear. Cochlear parameters including basal turn diameter ( A), width of the basal turn ( B), and cochlear height (H) were measured from the appropriate planes. B/ A ratio was made to investigate which ratios correspond to round and elliptical shape of the cochlear basal turn. Results: The cochlear size as measured by A value ranged between 7.4 mm and 10 mm. The B value and the cochlear height ( H) showed a weak positive linear relation with A value. The ratio between the B and A values anything above or below 0.75 could be an indicator for a more round- or elliptical shaped cochlear basal turn, respectively. One sized/shaped commercially available precurved electrode array would not offer a tight electrode-to-modiolus in the cochlea that has an elliptical shaped basal turn as identified by the B/A ratio of Conclusion: Accurate measurement of cochlear parameters adds value to the overall understanding of the cochlear geometry before a cochlear implantation procedure. The shape of cochlear basal turn could have clinical implications when comes to electrode-to-modiolus proximity.

Objectives : To compare the clinical efficiency of the CR220 intraoperative remote assistant device used by the surgical team with that of the custom sound (CS) system used by an audiologist. Methods : Th is was a prospective clinical study in a quaternary care center (King Abdullah Ear Specialist Centre) in Riyadh, Saudi Arabia, between October 2018 and March 2019. We included adult and pediatric patients who underwent cochlear implant (CI) surgeries. For every participant, the intraoperative CI testing was performed via both the aforementioned methods. The time taken to complete the measurements with both approaches, including the time required by the audiologist to reach the operating room (OR) and to complete the test, was recorded. Results : There were no significant differences in the number of responding electrodes between the 2 approaches. For the 25 participants, the time taken for the measurements was 566 minutes with the CS and 173 with the CR220 systems. This significant difference indicates that considerable time can be saved. Conclusion : The CR220 enables intraoperative CI electrode tests and auto-NRT measurements. Its ergonomics and ease-of-use help the surgical team conduct the tests without an audiologist in the OR, resulting in the efficient use of clinical resources. Objectives To determine whether distance from a cochlear implant (CI) center can influences the ages of presentation and intervention for CI, which thus may influence CI outcomes in Saudi Arabia. Methods This study comprised a retrospective cross-sectional analysis of 221 patients who presented to the CI committee between March 2016 and March 2018 at King Abdullah Ear Specialist Center, Riyadh, Saudi Arabia. Through phone interviews and patients' files, age at suspicion, audiology testing, diagnosis, hearing aid fitting, and CI discussion were recorded for prelingually deaf children; additionally, demographic information were recorded. Patients were divided into 3 groups based on the distance between their place of residence and the nearest CI center (residing within 200 km, 200-500 km, and greater than 500 km). Results Patients living within 200 km showed the youngest mean age for all tested checkpoints; however, there were no statistically significant differences among the groups. Conclusion In Saudi Arabia, distance from CI center does not have a significant effect on age of presentation for CI intervention.

Objectives To understand the growth rate of mastoid thickness and skull width associated with the age for both normal and malformed inner-ear anatomy groups. Also, to determine if there is any mathematical relation between cochlear size as measured by the "A" value against the age, mastoid thickness, and skull width. Methods Ninety-two computed tomography image datasets of human temporal bone were made available that contained normal (n = 44) and malformed inner-ear (n = 48) anatomies. The age of the subjects ranged from 6 months to 79 years. CE marked OTOPLAN preplanning otology software was used to load the patient's preoperative images for making all the measurements including mastoid thickness, skull width, and the cochlear size as measured by the "A" value. Mastoid thickness was measured both in axial and coronal planes starting from the cochlear entrance to the skull surface, with the line in plane with the basal turn of the cochlea. Skull width was measured from side to side in both axial and coronal planes from the image slice that gave the highest width. The cochlear size in terms of basal turn diameter "A" was measured from "Cochlear View" in the oblique coronal plane. Results Mastoid thickness and skull width increased with age in a logarithmic manner. The mastoid thickness increased from a minimum of 17 mm to around 34 mm and the skull width increased from 105 mm to around 146 mm as the age increased from 6 months to 20 years. At the age of around 20, both the mastoid thickness and skull width reached the plateau and thereafter with a very little growth. The skull width was linearly correlated with the mastoid thickness conveying the fact that bigger the head size is, thicker will be the mastoid. The size of the cochlea as measured by the "A" value did not have any meaningful correlation with the age, mastoid thickness, and skull width. This conveys the message that the cochlear size is independent of the overall size of head and the age of patient. Conclusions Mastoid thickness and skull width increased with age, while the cochlear size was independent of age, mastoid thickness, and the size of the skull.

The effect of insertion depth and position of cochlear implant (CI) electrode arrays on speech perception remains unclear. This study aimed to determine the relationship between cochlear coverage and speech performance in children with prelingual hearing loss with CI. Pure tone audiometry (PTA) and speech audiometry, including speech reception threshold (SRT) using spondee words and speech discrimination score (SDS) using phonetically balanced monosyllabic words, were tested. The Categories of Auditory Performance (CAP) and Speech Intelligibility Rating (SIR) scales were also used. Thirty-one ears were implanted with the FLEX 28 electrode array, and 54 with the FORM 24 were included in the current study. For the studied ear, the mean cochlear duct length was 30.82 ± 2.24 mm; the mean cochlear coverage was 82.78 ± 7.49%. Cochlear coverage was a significant negative predictor for the mean pure tone threshold across frequecnies of 0.5, 1, 2, and 4 kHz (PTA4) (p = 0.019). Cochlear coverage was a significant positive predictor of SDS (p = 0.009). In children with cochlear coverage ≥ 82.78%, SDS was significantly better than in those with coverage < 82.78% (p = 0.04). Cochlear coverage was not a significant predictor of the SRT, CAP, or SIR. In conclusion, the cochlear coverage of the CI electrode array has an impact on the users’ SDS. Further long-term studies with larger sample sizes should be conducted to address the most critical factors affecting CI recipients’ outcomes.

This study aimed to validate the role of 3D segmentation in measuring the volume of the vestibular aqueduct (VAD), and the inner ear, and to study the correlation between VAD volume and VAD linear measurements at the midpoint and operculum. The correlation with other cochlear metrics was also studied. We retrospectively recruited 21 children (42 ears) diagnosed with Mondini dysplasia (MD) plus enlarged vestibular aqueduct (EVA) from 2009 to 2021 and who underwent cochlear implantation (CI). Patients’ sociodemographic data were collected, and linear cochlear metrics were measured using Otoplan. Vestibular aqueduct width and vestibular aqueduct and inner ear volumes were measured by two independent neuro-otologists using 3D segmentation software (version 4.11.20210226) and high-resolution CT. We also conducted a regression analysis to determine the association between these variables and CT VAD and inner ear volumes. Among the 33 cochlear implanted ears, 13 ears had a gusher (39.4%). Regarding CT inner ear volume, we found that gender, age, A-value, and VAD at the operculum were statistically significant (p-Value = 0.003

Objectives This study aimed to define the best electrode trajectory line in cochlear implant (CI) surgery using the OTOPLAN (otology planning software) reconstructed 3D model and to investigate the surgical distance of the retro-facial approach as a direct access to the round window. Methods Computed tomography (CT) scans of the normal temporal bone were included for analysis in this study. OTOPLAN reconstruction was used to build 3D models with specific ear structures for study analysis. Results Twenty-five scans were included; the average age at the time of CT scan was 6.8±12 years. Twelve scans (48%) were right-sided and thirteen (52%) were left-sided. The best trajectory line to the round window was identified in all scans. The retro-facial approach was the optimal approach for 52% of cases (13/25). In all scans, the safe distance from the facial nerve were in favor of the retro-facial approach (P = 0.0011). Conclusion The OTOPLAN reconstructed imaging provided a good analysis of the retro-facial approach and helped in planning the surgical trajectory line towards the round window. Additionally, calculation of the surgical distance can help the surgeon compare the retro-facial approach to the standard facial recess for preoperative planning. These findings may help in robotic surgery.

Background In early 2021, vaccination against COVID-19 became one of the most important measures needed to control the pandemic. Objectives This study aimed to investigate the levels of acceptance and factors affecting the decisions among Saudi parents and caregivers of children under 12 for getting them vaccinated. Design and setting A cross-sectional survey of 1000 caregivers and parents was carried out from May 2021 to June 2021. The participants were from the eastern, central, southern, western, and northern provinces of Saudi Arabia. Materials and methods The data were collected and managed using Microsoft Excel (Microsoft® Corp., Redmond, WA) and analyzed using SPSS version 23 (IBM Corp., Armonk, NY). Results Of the 1000 respondents, 281 (28.1%) reported they would vaccinate their children, 346 (34.6%) rejected vaccination, and 373 (37.3%) were not sure. Most caregivers (24.1%; n = 241) reported protecting children as the principal reason for accepting vaccination. Regarding those rejecting the vaccine, the most common concern was that children might experience side effects (42.9%; n = 429). Limitations The acceptance of the COVID-19 vaccine is dynamic and changes with legislations and public awareness policies. Conclusions COVID-19 vaccine acceptance for children was low at the time of this study compared to the United Kingdom and United States. To achieve vaccination coverage that is adequate for herd immunity in Saudi Arabia, intense educational and awareness strategies are needed.

BACKGROUND: Congenital sensorineural hearing loss (SNHL) is a common disability in children. It can affect normal language development and educational achievement. Today, the time to cochlear implant is delayed for many children, which in turn delays intervention and impacts outcomes. Lack of knowledge and experience with congenital SNHL in the family are critical factors that can delay identification and intervention. OBJECTIVES: Compare treatment seeking behavior in families for a first and second congenitally deaf child. Design: Analytical, cross-sectional using medical record data. SETTING: Ear specialist hospital in Riyadh. SUBJECTS AND METHODS: All patients who presented to the cochlear implant committee from March 2016 to March 2018 and met criteria were included in the study. Data on when the subjects presented to hospital and were approved for cochlear implant were retrieved from the patient files and through phone calls to the family. The age of first suspicion, audiological testing, diagnosis, hearing aid fitting, and the decision for cochlear implant were compared between the first and second child in families with multiple children with congenital SNHL. MAIN OUTCOME MEASURES: The timing difference between the first and second deaf child in seeking treatment. SAMPLE SIZE: 116 (58 pairs). RESULTS: The second child was suspected to have hearing loss 13.6 months earlier than the first child and presented to the cochlear implant committee for final decision 16.7 months earlier than his\her sibling. Objective To evaluate cochlear implantation in Vogt-Koyanagi-Harada syndrome with regard to surgical difficulties and hearing outcomes. Patients Single case report. Intervention(s) Therapeutic complete electrode insertion during surgery despite evident intra-cochlear fibrosis. Main outcome measure(s) Postoperative speech audiometry, categories of auditory performance, and speech intelligibility rating. Results Vogt-Koyanagi-Harada syndrome is a multisystem autoimmune disease that affects tissues containing melanin. It is characterized by bilateral uveitis with auditory, vestibular, and dermatologic manifestations. Standard treatment comprises aggressive administration of systemic corticosteroids.This report describes the use of bilateral cochlear implants in a 30-year-old Saudi woman who presented with uveitis and was diagnosed with Vogt-Koyanagi-Harada syndrome. She had progressive hearing loss in both ears and experienced minimal improvement with hearing aids. The patient underwent sequential cochlear implantation, which was challenging because it was difficult to insert the electrode in the right ear due to intra-cochlear fibrosis. After more than 5 years of follow-up, she has good hearing in both ears, despite the advancement of ocular disease and recurring visual complaints. Conclusion This study suggests that patients with Vogt-Koyanagi-Harada syndrome could develop intra-cochlear fibrosis during cochlear implantation due to the autoimmune nature of disease. Moreover, cochlear implantation becomes more difficult with disease advancement. Therefore, it is essential that healthcare professionals consider early detection and prompt treatment of hearing loss in patients with this syndrome.

Objective To determine the effect of cochlear coverage on audiological and speech parameters in patients with cochlear implants. Previous work has investigated the effect of tailoring electrode size to a cochlear implant recipient's individual cochlear duct length (CDL). However, no clear relationship has been found between speech development and the extent of electrode insertion, and the benefits of apical stimulation are not yet clear. Methodology In this retrospective study, we assessed the effect of cochlear coverage on audiological and speech performance. Participants were prelingually deaf children who received cochlear implants between June 2013 and December 2014 under the care of a single cochlear implant surgeon. Cochlear coverage was estimated for each ear according to electrode type, depth of insertion, and the number of active electrodes. Electrode type and length were determined by the individual's CDL, measured by computed tomography (CT), and full insertion was documented intraoperatively. The number of active electrodes was recorded using intraoperative audiological response telemetry. Audiological assessments were obtained 6 months and 1 year postoperatively. Results of the categories of auditory performance-II and speech intelligibility rating scales were obtained after 3 years. Patients were divided into two groups based on their cochlear coverage and their audiological and speech outcomes were compared. Results Of the 97 children recruited, 47 were girls. Temporal bone CT scans showed the right and left mean CDLs among girls were 27.7 and 27.9 mm, respectively, and 29.2 mm for both ears in boys. For each sex, the right and left CDLs did not differ significantly (p = 0.07). Twenty patients were lost to follow-up, leaving 77 patients (120 ears), which were divided into groups according to cochlear coverage (complete vs. incomplete). Significant between-group differences were not found in assessments of audiology, categories of auditory performances, or speech intelligibility ratings after 3 years. Conclusion Audiological parameters do not differ according to the degree of cochlear coverage, specifically for low-frequency tones. Speech parameters are also comparable. Therefore, complete cochlear coverage does not appear to provide significant benefit over incomplete coverage for prelingually deaf cochlear implant recipients.

Auditory neuropathy spectrum disorder (ANSD) is a distinct type of SNHL that is characterized by the presence of otoacoustic emissions and/or cochlear microphonics. Cochlear implantation was initially not recommended for ANSD children, later studies showed variable outcomes among those subjects.To assess the auditory and speech performance of cochlear implanted children with auditory neuropathy spectrum disorder (ANSD) and to compare these results to those obtained from cochlear implanted children with sensorineural hearing loss (SNHL) Material and methods: 18 cochlear implanted children with ANSD and 40 cochlear implanted children with SNHL were included in this study. Auditory and speech performance results were compared across both subject groups using the Category of Auditory Performance (CAP) and Speech Intelligibility Rate (SIR) tests, with measurements recorded one year post implantation.Cochlear implanted children with ANSD showed clinically significant improvements that were comparable to those observed from cochlear implanted subjects without ANSD.Children with ANSD benefit from early cochlear implantation and can reach similar auditory and speech performance results as that achieved by children without ANSD.

The A-value used in cochlear duct length (CDL) estimation does not take malformed cochleae into consideration. The objective was to determine the A-value reported in the literature, to assess the accuracy of the A-value measurement and to evaluate a novel cochlear measurement in distinguishing malformed cochlea. High resolution Computer Tomography images in the oblique coronal plane/cochlear view of 74 human temporal bones were analyzed. The A-value and novel C-value measurement were evaluated as predictors of inner ear malformation type. The proximity of the facial nerve to the basal turn was evaluated subjectively. 26 publications report on the A-value; but they do not distinguish normal vs. malformed cochleae. The A-values of the normal cochleae compared to the cochleae with cochlear hypoplasia, incomplete partition (IP) type I, -type II, and -type III were significantly different. The A-value does not predict the C-value. The C-values of the normal cochleae compared to the cochleae with IP type I and IP type III were significantly different. The proximity of the facial nerve to the basal turn did not relate to the type of malformation. The A-value is different in normal vs. malformed cochleae. The novel C-value could be used to predict malformed anatomy, although it does not distinguish all malformation types.

Objectives The aim of this study was to explore: 1) the average use of each device in sequentially implanted cochlear implants; 2) whether the inter-implant duration between implants produced any significant difference in the average use of the second implant; and 3) whether wearing hearing aids before the implantation of the second cochlear implant affects its average use. Materials and methods The study included 20 participants with bilateral Nucleus 24 implants (Cochlear Corporation, Lone Tree, CO). Data regarding various variables were extracted and then analyzed with IBM SPSS Statistics for Mac, version 23 (IBM Corp., Armonk, NY). Results The pediatric group included 14 subjects (average age 7.5 years) while the adult group comprised six subjects (average age 37.5 years). The average use of the second device was 0.9 hours per day more than the first in the pediatric group while it was 1.22 hours per day more in the adult group. We also divided the subjects on the basis of duration between the first and second devices and calculated the average use of each device by them. There was no significant difference (p>0.05). The average use by subjects who did and did not use hearing aids before implantation was also insignificant (p>0.05). Conclusions No significant difference between the average use of the first and second implants, between the inter-implant duration of the first and second implants, the average use of the second implant, and between using hearing aids before the implantation of the second device and the average use was observed.

Objectives To study the changes in the coiled configuration of electrode excess lead in the mastoid cavity in the cochlear implant recipients over time. Methods Post-operative CT scans at two different appointments of fourteen patients with cochlear implants (CI) were retrospectively analyzed using a DICOM viewer software (3D-slicer). Mastoid thickness (MT) was measured in the oblique coronal plane from the round window (RW) entrance to the mastoid edge and inter-cochlear distance (ICD) was measured in the axial plane at the fundus level between two ears. 3D segmentation of the entire inner ear of both sides and coiled electrode excess lead was performed to visually compare the changes in coiled configuration between the two CT scan time points. Result MT and ICD increased logarithmically with the patient’s age, as has been measured from both the 1st and the 2nd CT scans and a weak linear correlation between MT and ICD was observed. Growth in MT and ICT measured between the time of 1st and 2nd CT scans showed a strong linear correlation. In eight cases, changes in the electrode excess lead have been observed in the 2nd CT scan, either a change in the coiling configuration of electrode excess lead or shifted laterally toward the mastoid edge. The ICD growth between the 1st and the 2nd CT scans was >2 mm in only seven cases and all of them were children. All other six cases had no observed changes in the coiled electrode lead. In addition, the mastoid growth between the 1st and the 2nd CT scan was >2.5 mm in only 4 cases. Conclusion Coiled configuration of electrode excess lead could change when the MT and ICD increased over time.

Significance Statement: Pregnancy-related giant nasal masses often present a diagnostic challenge and clinical dilemma due to the obvious restrictions in utilizing imaging and general anesthesia during pregnancy. Therefore, we highlight the importance of clinical judgment, especially during pregnancy, and should not disregard certain clinical cues. In addition, we recommend an in-office polypectomy for giant nasal polyps worsening during pregnancy and nasal steroids to optimize their quality of life while avoiding systemic medications or further interventions.

Objectives: The cochlear implant (CI) recipients and their families form an integral part of the CI team. The decision about CI is challenging for parents and their support is essential during rehabilitation. The families should have realistic expectations from the implantation, as unrealistic expectations cause frustration, negatively impacting the outcome. This study aimed to translate and validate the parental expectation questionnaire by evaluating the outcomes of their children after CI in the Saudi population. Materials and Methods: A questionnaire-based study was conducted at a tertiary hospital from June 2018 to July 2019. Altogether, 100 parents of children undergoing CI answered a questionnaire of 18 questions regarding the expectation of their child's performance and refilled it after a year of surgery. All healthy prelingual pediatric patients accepted by the CI committee for bilateral CI with at least 1 year follow-up postoperatively were included in this study. The main outcome measures were parents' awareness and expectation about CI. Results: The study showed parents having high expectations from their children with CI. The parents were more aware about the rehabilitation demands and stress after CI surgery with 16% of parents unaware of the importance of the rehabilitation process. Conclusions: Proper counseling on realistic expectations should be given at different implantation stages; pre- and post-implantation for an improved outcome and helping the child in reaching full potential.

ObjectiveThis study aimed to examine the impact of trainee involvement in performing tympanoplasty or tympano-ossiculoplasty on outcomes.MethodsA retrospective analysis was performed of a prospective database of all patients undergoing tympanoplasty and tympano-ossiculoplasty in a single centre during a three-year period. Patients were divided into three primary surgeon groups: consultants, fellows and residents. The outcomes of operative time, surgical complications, length of hospital stay, and air–bone gap improvement were compared among the groups.ResultsThe study included 398 tympanoplasty and tympano-ossiculoplasty surgical procedures, 71 per cent of which were performed by junior trainees (residents). The junior trainee group was associated with a significantly longer surgical time, without adverse impact on outcomes.ConclusionTrainee participation in tympanoplasty and tympano-ossiculoplasty surgery was associated with longer surgical time, but did not negatively affect the peri-operative course or hearing outcome. Therefore, resident involvement in these types of surgery is safe.

Background Datalogging feature of the cochlear implant audio processor has been utilized to calculate the average daily wearing hours for cochlear implant devices by patients. Objectives To assess the relationship between the time use of cochlear implant audio processor and speech development as well as to identify the lowest acceptable duration of audio processor use to achieve an acceptable language development. Methods A retrospective study design including prelingual thirty-four ears (24 patients) who received the same electrode array of cochlear implant with 2 years follow up. The audiological and speech evaluations were done for all patients postoperatively and the last postoperative follow-up visit was used for analysis in this study. Results The average daily use of the audio processor was 11.3 ± 2.7 h per day. The pure tone average was 30.55 ± 4.64 dB whereas the speech reception threshold was 30.88 ± 6.12 dB. The average speech discrimination score at 65 dB was 68.59 ± 16.80%. A significant positive correlation (r = 0.54, p value = 0.0009) was found between the daily use of the audio processor and the speech discrimination score. The lowest wearing time needed to have more than 60% of the speech discrimination score was 8.3 h/day. Conclusion The present study revealed a positive correlation between the daily duration of the audio processor usage and speech performance. Moreover, we found that pediatric patients need to use their cochlear implant device for at least 8.3 h/day to achieve acceptable language development.

BACKGROUND: Minimal access surgery has been promoted to reduce the surgical duration and complications of cochlear implant (CI) surgery. A requirement of minimal access surgery is adequate visualization of the surgical cavities. OBJECTIVE: The aim of this study was to evaluate whether a new light-integrated surgical retractor reduced CI surgical time. DESIGN: A retrospective comparative study. SETTING: Tertiary referral otolaryngology clinic. PATIENTS AND METHODS: We reviewed the records of all patients who received implants between January 2013 to May 2014 at King Abdul-Aziz University Hospital before and after use of the illuminated retractor. MAIN OUTCOME MEASURE(S): Duration of surgery before and after the use of the illuminated retractor. RESULTS: There were no statistically significant differences in the surgical duration of CI relating to use of the illuminated retractor in 117 patients. The mean (SD) surgery duration in the unilateral cases was 132.7 (45.5) minutes before and 125.32 (42.76) minutes after use of the illuminated retractor (P=.479). For the bilateral cases, the mean (SD) surgical time was 213.6 (45.7) minutes before and 206.5 (45.5) minutes after use of the retractor (P=.702). There were no adverse events with use of the new tool. CONCLUSION: The similar duration and lack of adverse events suggest that the illuminated retractor is safe to use when drilling the bony recess for the receiver-stimulator package within the subperiosteal pocket. LIMITATIONS: The method used to calculate surgical duration may explain why no reduction in duration was shown.

Background The selection of an appropriate imaging technique for assessment before cochlear implantation is critical for precise diagnosis and management. While magnetic resonance imaging (MRI) is used for the diagnosis of several conditions, such as labyrinthitis ossificans, cochlear nerve deficiency, and neoplasms, high-resolution computed tomography (HRCT) provides excellent details of the temporal bone. However, it remains unclear whether routine MRI provides any additional benefits over HRCT. Objectives To assess the added value of MRI as a screening tool for temporal bone abnormalities in cochlear implant candidates through comparisons of its findings with those of HRCT. Materials and method It is a retrospective analysis of preoperative HRCT and MR images in a tertiary referral center. A total of 308 patients who underwent MRI and HRCT examinations before cochlear implantation between 2013 and 2015 were included. Preoperative HRCT and MR images were screened for temporal bone abnormalities by a senior neurotologist and a neuroradiologist. Results HRCT detected inner ear deformities in 51 of the 308 (16.6%) subjects, whereas MRI revealed abnormalities in only 18 (5.8%) of subjects. HRCT detected the same inner abnormalities in 16 of the 18 (88.9%) subjects diagnosed by MRI, whereas it showed normal results for the remaining two subjects. MRI detected cochlear nerve aplasia/hypoplasia in 13 subjects, 11 of whom had associated inner ear deformities that were detected by HRCT. The MR images of nine subjects showed cochlear fibrosis, which was confirmed by HRCT in all nine subjects. Conclusion In this study, MRI did not exhibit significant additional benefits over HRCT, and its routine use for the preoperative assessment of CI candidates was not justified. However, MRI is warranted for subjects at an increased risk of cochlear nerve aplasia due to an inner ear deformity or a narrow internal auditory canal. The establishment of criteria that facilitate the performance of MRI only when absolutely needed will reduce healthcare costs, prevent unnecessary exposure to the risks associated with general anesthesia, and shorten delays before cochlear implantation.

Objective Cochlear implants (CIs) are typically activated four weeks after the implantation surgery. This delay between device implantation and activation lengthens the implant process and consequently induces personal and financial burdens for some patients who travel from remote regions to receive the surgery. However, fitting the speech processor and eliminating the waiting period could decrease the indirect cost associated with cochlear implantation. The objective of this study was to assess the impact of an early CI fitting on the overall cost paid by patients and their families aiming to improve future care strategies for patients receiving CIs. Methods This retrospective study was conducted in a tertiary referral center. All patients who received any kind of CI with early fitting of the speech processor were included. The total financial benefit for the patients and their families over the standard activation visit was investigated by assessing the cost of the non-medical expense for one hospital visit. Results Our results showed that the non-medical cost for each hospital visit associated with cochlear implantation was higher for those who traveled from remote areas: 81 USD for each patient within 200 km of the implantation center and 748.56 USD for each patient farther than 200 km from the implantation center. Conclusions Using the early fitting approach, some of the financial burden associated with implantation could be alleviated.

Background Hearing impairment is found to be the most prevalent disabling condition worldwide. Early diagnosis is crucial to avoid speech and language delays and to ensure the best performance results after cochlear implant (CI) surgery. Universal newborn hearing screening is a way to recognize newborns with a hearing impairment with or without risk factors. In this article, we have studied the effect of the newborn hearing screening program on early presentation to a healthcare center and, hence, early intervention in patients with congenital hearing loss, and reviewed the international numbers. Objectives The objective of this study was to determine whether neonatal hearing screening in Saudi Arabia helped prelingually deaf children to present earlier or not. Design Retrospective cross-sectional review Setting King Abdullah Ear Specialist Center (KAESC), Riyadh, Saudi Arabia Subjects and methods We included all patients who presented to the CI committee for the first time at KAESC, between March 2016 and March 2018, and met the inclusion criteria. Data were retrieved through phone calls and patient files. The sample size was 242. Main outcomes The timing difference between those who were screened positive for hearing loss at birth versus patients who were screened negative or not screened at all. Results By far, patients who were screened positive for hearing loss presented earlier (p-value >0.001) to a healthcare center than those who were not screened at all or screened negative for hearing loss and they finished the journey to CI 17 months earlier than those who were not screened. On the other hand, those who were screened negative were not found to present later than those who were not screened. Conclusion Going with the international trend, screening was found to have a significant positive effect on age at presentation, diagnosis, hearing aid fitting, surgery, and, hence, performance after implantation. Testing false negative on screening did not show a significant further delay when compared to those who were not screened.

Background: The ADHEAR device, a new nonsurgical bone conduction hearing device, has been developed for patients with conductive hearing loss. Objectives: This study aims to assess the impact of the ADHEAR device on the audiological performance and satisfaction level in subjects with conductive hearing loss. Methods: Twelve patients with conductive hearing loss were included. All patients received the device for 3 months. The audiological outcomes were determined using basic audiological assessments, including pure tone audiometry and sound field measurements of pure tone and speech audiometry with the contralateral ear occluded with a specific earplug. Additionally, the patients were subjectively evaluated using (1) the Speech, Spatial, and Qualities Questionnaire (SSQ), and (2) the custom-made ADHEAR questionnaire. Results: Analysis of the measured audiological outcomes revealed an average improvement in pure tone thresholds (functional gain) of 23 (± 4.4) dB HL when the ADHEAR system was used compared to the unaided condition in the sound field. Moreover, speech reception thresholds improved by an average of 23 (± 15.3) dB SPL in the aided condition with plugged contralateral ear. Additionally, when using ADHEAR in the sound field, subjects’ speech recognition scores improved by 32% (± 17.7) in quiet and 21% (± 15.1) in the presence of interfering noise. The average SSQ questionnaire scores improved from 3.9 at the study initiation to 6.6 after 3 months of device usage. ADHEAR custom questionnaire assessments revealed high satisfaction and acceptance of the device with no pain or skin irritation. Conclusion: During the study period, this new adhesive system yielded improved audiological outcomes with high patient satisfaction and acceptance and no reported skin irritation or pain.
