Abstract

There is no consensus about the best treatment (surgery, chemotherapy and percutaneous treatments) for hepatic cystic echinococcosis (CE). Retrospective studies have addressed the pros and cons of each option, but costs of treatments have never been studied extensively. Aim of this paper is to compare the cost of PAIR and conservative surgery for non-complicated echinococcal cysts of the liver in Italy. Materials and Methods. Costs of hospital stay, operating theatre, disposable items, salaries for health personnel and sonograms, were obtained from the S.Matteo Hospital Administration and pharmacy. The institution is a large tertiary care teaching hospital in Lombardy, a region in Northern Italy. Costs of 1-year follow-up were included. The costs were calculated for a 3-day hospital stay for PAIR procedure and 4-day hospital stay for surgery, both without complications. Results and Discussion. The 2006 mean specific cost of a PAIR treatment for a 3-day hospital stay and 1 year follow-up was 2,072 EUR (average exchange rate: 1 EUR = 1.26 USD). Main cost entries were the following: a) hospital stay (net cost): € 1,500 (72.4% of the total cost); b) disposable items (needles, catheters) € 239 (11.5%); c) personnel: € 156 (7.5%); d) drugs (30-days albendazole administration as prophylaxis of secondary echinococcosis): € 72 (3.5%); e) ultrasound scans (paid by patient): € 105 (5.1%). In the same year and same hospital, the specific cost of a surgical conservative intervention and 1-year follow-up was 3,267 EUR. Main cost entries were the following: a) hospital stay (net cost): € 2,000 (61,2% of the total cost); b) operating theatre (2 hours): € 140 (4,3%); c) disposable items: e.g. :Fibrin Glue € 500 (15,3%); d) personnel: € 300 (9,2%); e) pre-operative tests (Labs, ECG, Chest X-ray, anesthesiological evaluation): € 150 (4,6%); f) drugs (30-days albendazole administration as prophylaxis of secondary echinococcosis): € 72 (2,2%); g) ultrasound scans (paid by patient): € 105 (3,2%). Conclusion PAIR is significantly less expensive than conservative surgery for uncomplicated echinococcal cysts of the liver. …………………………………………………………………………………………………………………

Abstract

The ability to evaluate liver stiffness non-invasively in clinical practice by measuring transient elastography using FibroScan® has resulted in considerable interest and enthusiasm. A core study group, organized by the Italian Association for the Study of the Liver, has assessed the usefulness of FibroScan® in the diagnosis and management of liver disease in clinical practice. The group concluded that FibroScan® is a valuable, non-invasive technique and have developed a consensus report form for registering transient elastography results. In this article, we report the findings of the study group.

Abstract

The aim of the present retrospective study was to assess the incidence of adverse events (AE) of a second-generation ultrasound contrast agent in real clinical practice. A total of 28 Italian Centres provided data on the postmarketing use of SonoVue (Bracco Spa, Milan, Italy) in abdominal examination performed between December 2001 and December 2004. A total of 23 188 investigations were reported. No fatal event occurred. AEs were reported in 29 cases, of which only two were graded as serious; the rest, 27, were nonserious (23 mild, three moderate and one severe). The overall reporting rate of serious AE was 0.0086%. Overall, only four AEs required treatment (two serious, two nonserious including one moderate and one severe AEs). In conclusion, the present large-scale retrospective analysis showed that SonoVue has a good safety profile in abdominal applications, with an AE reporting rate lower than or similar to that reported for radiologic and magnetic resonance contrast agents.

40. Justified concern or exaggerated fear: the risk of anaphylaxis in percutaneous treatment of cystic echinococcosis-a systematic literature review

Abstract

Percutaneous treatment (PT) emerged in the mid-1980s as an alternative to surgery for selected cases of abdominal cystic echinococcosis (CE). Despite its efficacy and widespread use, the puncture of echinococcal cysts is still far from being universally accepted. One of the main reasons for this reluctance is the perceived risk of anaphylaxis linked to PTs. To quantify the risk of anaphylactic reactions and lethal anaphylaxis with PT, we systematically searched MEDLINE for publications on PT of CE and reviewed the PT-related complications. After including 124 publications published between 1980 and 2010, we collected a total number of 5943 PT procedures on 5517 hepatic and non-hepatic echinococcal cysts. Overall, two cases of lethal anaphylaxis and 99 reversible anaphylactic reactions were reported. Lethal anaphylaxis occurred in 0.03% of PT procedures, corresponding to 0.04% of treated cysts, while reversible allergic reactions complicated 1.7% of PTs, corresponding to 1.8% of treated echinococcal cysts. Analysis of the literature shows that lethal anaphylaxis related to percutaneous treatment of CE is an extremely rare event and is observed no more frequently than drug-related anaphylactic side effects.

Abstract

Introduction: Liver metastases often exhibit a hypervascular halo during the arterial phase of contrast-enhanced ultrasonography (CEUS). This finding has no correlates on baseline gray-scale imaging, and it has never been characterized. The aim of this study was to identify the features of this halo and determine whether it should be included in the ablation volume during thermal ablation procedures. Materials and methods: We prospectively enrolled 25 patients referred to our department for thermal ablation of liver metastases. Before treatment all patients underwent CEUS, and the maximum diameter of the metastatic lesion was measured before administration of the ultrasound contrast agent and during the arterial and portal venous phases of the contrast contrast-enhanced study. Maximum diameters in the different vascular phases were compared with the Turkey-Kramer test. Two biopsies were obtained from each lesion with a 21-gauge needle: 1) one from the center of the metastasis to confirm the diagnosis and 2) one from the hypervascular peripheral halo identified in the arterial phase at CEUS. Results: The mean (±standard deviation) maximum lesion diameter was 2.67 ± 1.2 cm before contrast agent injection, 3.50 ± 1.4 cm during the arterial phase, and 2.71 ± 1.2 cm during the venous phase. The difference between maximum diameters measured before contrast enhancement and in the arterial phase was highly significant (mean: 0.84 ± 0.45 cm, p < 0.0001). Histological examination of halo specimens revealed inflammatory infiltrates with no evidence of tumor infiltration in 24/25 (96%) cases and normal hepatic parenchymal tissue in the 25th specimen. Discussion: The hypervascular halo surrounding liver metastases during the arterial phase of CEUS represents a chronic inflammatory infiltrate, not tumor infiltration. Abstract

Human brucellosis commonly involves the liver and manifests as nonspecific or granulomatous hepatitis. Although the development of a brucellar hepatic abscess is rare, it can be difficult to accurately diagnose given that histopathologic analysis is often not diagnostic, and the bacterium is seldom isolated from pus cultures. Therefore, serologic tests and imaging play an important role in the diagnosis of a brucellar abscess. The characteristics of a brucellar abscess on conventional ultrasonography, computed tomography (CT), and magnetic resonance imaging (MRI) have been reported previously. 1,2 However, to the best of our knowledge, the features of a brucellar hepatic abscess on contrast-enhanced ultrasonography (CEUS) have not been described previously. © 2008 by the American Institute of Ultrasound in Medicine.
