The onset and growth of subependymal giant cell astrocytoma (SEGA) in tuberous sclerosis complex (TSC) typically occurs in childhood. There is minimal information on SEGA evolution in adults with TSC. Of 2,211 patients enrolled in TOSCA, 220 of the 803 adults (27.4%) ever had a SEGA. Of 186 patients with SEGA still ongoing in adulthood, 153 (82.3%) remained asymptomatic, and 33 (17.7%) were reported to ever have developed symptoms related to SEGA growth. SEGA growth since the previous scan was reported in 39 of the 186 adults (21%) with ongoing SEGA. All but one patient with growing SEGA had mutations in TSC2. Fourteen adults (2.4%) were newly diagnosed with SEGA during follow-up, and majority had mutations in TSC2. Summary Objective To present the baseline data of the international TuberOus SClerosis registry to increase disease Awareness (TOSCA) with emphasis on the characteristics of epilepsies associated with tuberous sclerosis complex (TSC). Methods Retrospective and prospective patients’ data on all aspects of TSC were collected from multiple countries worldwide. Epilepsy variables included seizure type, age at onset, type of treatment, and treatment outcomes and association with genotype, seizures control, and intellectual disability. As for noninterventional registries, the study protocol did not specify any particular clinical instruments, laboratory investigations, or intervention. Introduction The mortality from cardiovascular disease in patients with chronic renal failure is much higher than in the general population. In particular, patients with chronic renal failure with replacement therapies (dialysis patients and patients with renal transplantation) show both increased traditional risk factors and risk factors due to the dysfunction of the renal system. In combination with necessary medication for renal insufficiency oxidative stress is elevated. Progression of atherosclerosis is promoted due to increased oxidation of lipids and endothelium damage. This link between lipid oxidation and artherogenesis provides the rationale for the supposed beneficial effect of supplementation with antioxidative vitamins (vitamin A, C and E). Such an effect could not be demonstrated for patients with a history of cardiovascular disease and without kidney diseases. However, in high risk patients with chronic renal failure and renal replacement therapies this could be different. Objectives The objective of this systematic literature review was to assess the clinical effectiveness and cost-effectiveness of supplementation with antioxidative vitamins A, C or E to reduce cardiovascular events in patients with chronic kidney diseases, dialysis-requiring patients and patients after a renal transplantation with or without cardiovascular diseases. Methods A systematic literature review was conducted with documented search and selection of the literature, using a priori defined inclusion and exclusion criteria as well as a documented extraction and assessment of the literature according to the methods of evidence-based medicine. Results 21 publications met the inclusion criteria for the evaluation of clinical effectiveness. No study could be identified for the economic evaluation. Two studies (four publications) analysed the effect of oral supplementation on the secondary prevention of clinical cardiovascular endpoints. Studies analysing the effect on patients without a history of cardiovascular disease could not be identified. 17 studies analysed the effect of oral supplementation or infusion with antioxidative vitamins or the supplementation with dialysis membranes coated with vitamin E on intermediate outcomes like oxidative stress or vessel parameters. The two randomized clinical trials analysing the effect of orally supplemented vitamin E on clinical endpoints in patients with mild-to-moderate renal insufficiency and for haemodialysis patients respectively reported different results. After 4.5 years supplementation with a daily dose of 400 IU vitamin E renal insufficiency patients showed neither a beneficial nor a harmful effect on a combined event rate of myocardial infarction, stroke or death by cardiovascular causes. The second study reported a 50% risk reduction (RR=0.46, 95%-KI: 0.27-0.78, p=0.014) on the combined event rate of fatal myocardial infarction, nonfatal myocardial infarction, stroke, peripheral vascular disease or unstable angina pectoris in the study arm with vitamin E-Supplementation of 800 IU daily. In 16 of 17 studies with intermediate endpoints the supplementation with vitamins was associated with a change of one or several of the examined endpoints in the expected direction. This means that the concentrations of the markers for oxidative stress decreased in the Vitamin E-group, the progression of aortic calcification (only one study) was reduced, the intima media thickness decreased and the lipid profile improved. No studies regarding costs or cost-effectiveness were identified. Discussion A possible explanation for the different results in the two studies with clinical endpoints may be due to the different study populations with different risk profiles, to different dosage during the intervention or to variation by chance. Due to the absence of clinically meaningful endpoints, the relevance of studies analysing the effect of antioxidative vitamins on intermediate endpoints like oxidative stress markers is basically limited to show single intermediate steps of the postulated biological effect mechanisms by which a potentially preventive effect could possibly be mediated. The mainly unsatisfactory planning and reporting quality of the 17 identified studies and a possible "publication bias" are further limitations. Conclusion The available evidence is not sufficient to support or to reject an effect of antioxidative vitamins on secondary prevention for cardiovascular disease for patients with chronic renal insufficiency or renal replacement therapy. There is a lack of randomized, placebo-controlled studies with a sufficient number of cases and clinical endpoints of cardiovascular disease, on the effect of antioxidative vitamins either orally applied or given by vitamin E-modified dialysers. No data are available about supplementation with antioxidative vitamins for primary prevention of cardiovascular disease. Therefore the current evidence does not allow to draw conclusions concerning this subject either. As opposed to patients with a history of cardiovascular disease without kidney diseases where there is enough evidence to exclude a beneficial effect on secondary prevention of cardiovascular disease for patients with chronic renal insufficiency and renal replacement therapy this question remains unanswered. Conclusions about costs and cost-effectiveness also cannot be drawn. Einleitung Das Risiko für niereninsuffiziente Patienten, an einer kardiovaskulären Erkrankung zu sterben, liegt weit über dem der Allgemeinbevölkerung. Insbesondere Nierenerkrankte mit Ersatztherapien (Dialysepatienten und Patienten mit Nierentransplantation) zeigen erhöhte traditionelle kardiovaskuläre Risikofaktoren, aber auch Risikofaktoren, die durch die Dysfunktion des renalen Systems bedingt sind. In Kombination mit der für Niereninsuffiziente nötigen Medikation ergibt sich eine Erhöhung des oxidativen Stresses, der wiederum die Progression der Atherosklerose infolge von erhöhter Lipidoxidation und Endothelschädigung fördert. Diese Verbindung zwischen Lipidoxidation und der Entstehung von Atherosklerose stellt die logische Grundlage für einen angenommenen präventiven Effekt einer Supplementation mit antioxidativen Vitaminen (Vitamin A, C und E) dar. Für Patienten mit kardiovaskulären Vorerkrankungen ohne Nierenerkrankungen ließ sich ein solcher Effekt nicht nachweisen, dies könnte jedoch in der Hochrisikogruppe der Patienten mit chronischen Nierenerkrankungen und Nierenersatztherapien anders sein. Fragestellung In einer systematischen Übersichtsarbeit der Literatur soll untersucht werden, ob eine Supplementation mit den antioxidativen Vitamine A, C oder E bei Patienten mit chronischer Nierenerkrankung, dialysepflichtigen Patienten und Patienten nach Nierentransplantation mit oder ohne kardiovaskulären Erkrankungen ein medizinisch wirksames Verfahren zur Verminderung von kardiovaskulären Ereignissen darstellt. Zudem soll eine systematische Übersichtsarbeit über die Wirtschaftlichkeit dieser Präventionsmaßnahme durchgeführt werden. Methodik Es wird eine systematische Literaturübersicht mit dokumentierter Literaturrecherche und -selektion, vorabdefinierten Ein- und Ausschlusskriterien sowie dokumentierter Extraktion und Bewertung der Literatur nach den Methoden der evidenzbasierten Medizin durchgeführt. Ergebnisse Für die medizinische Bewertung erfüllten 21 Publikationen die Einschlusskriterien, für die ökonomische Bewertung konnte keine geeignete Studie identifiziert werden. Zwei Studien (vier Publikationen) untersuchten den Einfluss von oraler Supplementation zur Sekundärprävention klinischer kardiovaskulärer Endpunkte. Zur Frage der Primärprävention kardiovaskulärer Ereignisse konnten keine Publikationen identifiziert werden. 17 Studien untersuchten den Einfluss einer oralen Supplementation oder Infusion antioxidativer Vitamine oder der Supplementation durch mit Vitamin E-beschichteten Dialysemembranen auf intermediäre Endpunkte wie oxidativen Stress oder gefäßverändernde Parameter. Die beiden randomisierten klinischen Studien zur Sekundärprävention mit oral supplementiertem Vitamin E bei Patienten mit gering- bis mittelgradiger Niereninsuffizienz bzw. bei Hämodialysepatienten zeigten unterschiedliche Ergebnisse. Während sich bei niereninsuffizienten Patienten nach 4,5 Jahren Supplementation mit 400 IU Vitamin E täglich weder ein protektiver noch ein risikoerhöhender Einfluss auf eine Kombination kardiovaskulärer Ereignisse aus Myokardinfarkt, Schlaganfall und Tod durch kardiovaskuläre Ursachen nachweisen ließ, berichtete die zweite Studie eine Halbierung des Risikos, einen tödlichen, einen nicht-tödlichen Myokardinfarkt, einen Schlaganfall, eine periphere vaskuläre Erkrankung oder eine instabile Angina zu erleiden, im Studienarm mit Vitamin E-Supplementation von 800 IU täglich (RR=0,46, 95%-KI: 0,27-0,78, p=0,014). In 16 von 17 Studien mit intermediären Endpunkten war die Vitaminsupplementation bei einer oder mehrerer der untersuchten Zielgrößen mit einer Veränderung in der erwarteten Richtung zu beobachten, d. h. die Konzentrationen der Marker für oxidativen Stress nahmen in der Interventionsgruppe ab, die Progression der Kalzifizierung der Aorten (nur eine Studie) war geringer, die Intima-Media-Dicke nahm ab und das Lipidprofil zeigte positive Veränderungen. Zur Wirtschaftlichkeit liegen keine Ergebnisse aus Studien vor. Diskussion Mögliche Erklärungen für das unterschiedliche Ergebnis der beiden Studien mit klinischen Endpunkten können in den unterschiedlichen Studienpopulationen mit verschieden hohen Erkrankungsrisiken, in der unterschiedlichen Dosierung bei der Intervention oder in zufälliger Variation liegen. Die Aussagekraft von Studien zum Effekt antioxidativer Vitamine auf intermediäre Endpunkte wie oxidative Stressmarker ist aufgrund des Fehlens klinisch relevanter Endpunkte von vornherein darauf beschränkt, einzelne Zwischenschritte der postulierten biologischen Wirkmechanismen, über die möglicherweise ein präventiver Effekt erzielt werden könnte, nachzuweisen. Die überwiegend mangelhafte Planungs- und Berichtsqualität der 17 identifizierten Studien und ein möglicher "publication bias" schränken die Aussagekraft weiter ein. Schlussfolgerung Die Evidenz ist nicht ausreichend, um eine Aussage über einen sekundärpräventiven Effekt antioxidativer Vitamine bei kardiovaskulären Erkrankungen bei Patienten mit chronischer Niereninsuffizienz oder Nierenersatztherapie treffen zu können. Es fehlen randomisierte, placebokontrollierte Studien mit ausreichenden Fallzahlen mit klinischen Endpunkten kardiovaskulärer Erkrankungen, die den Einfluss oral applizierter oder durch beschichtete Hämodialysemembranen verabreichter antioxidativer Vitamine bei dieser Patientengruppe untersuchen. Zur Primärprävention kardiovaskulärer Erkrankungen durch die Supplementation antioxidativer Vitamine bei den genannten Patientengruppen stehen keine Daten zur Verfügung, so dass hier ebenfalls keine Aussagen getroffen werden können. Im Unterschied zur Situation bei Patienten mit kardiovaskulärer Vorerkrankung ohne Nierenerkrankung, wo die Evidenz mittlerweile ausreichend ist, um einen sekundärpräventiven Effekt der untersuchten antioxidativen Vitamine auszuschließen, ist die Frage bei Patienten mit chronischer Niereninsuffizienz und Nierenersatztherapie noch ungeklärt. Zur Wirtschaftlichkeit können keine Aussagen gemacht werden.
