Abstract

Myeloid cells, such as neutrophils, are produced in the bone marrow in high quantities and are important in the pathogenesis of vascular diseases such as pulmonary hypertension (PH). Although neutrophil recruitment into sites of inflammation has been well studied, the mechanisms of neutrophil egress from the bone marrow are not well understood. Using computational flow cytometry, we observed increased neutrophils in the lungs of patients and mice with PH. Moreover, we found elevated levels of IL-6 in the blood and lungs of patients and mice with PH. We observed that transgenic mice overexpressing Il-6 in the lungs displayed elevated neutrophil egress from the bone marrow and exaggerated neutrophil recruitment to the lungs, resulting in exacerbated pulmonary vascular remodeling, and dysfunctional hemodynamics. Mechanistically, we found that IL-6-induced neutrophil egress from the bone marrow was dependent on interferon regulatory factor 4 (IRF-4)-mediated CX3CR1 expression in neutrophils. Consequently, Cx3cr1 genetic deficiency in hematopoietic cells in Il-6-transgenic mice significantly reduced neutrophil egress from bone marrow and decreased neutrophil counts in the lungs, thus ameliorating pulmonary remodeling and hemodynamics. In summary, these findings define a novel mechanism of IL-6-induced neutrophil egress from the bone marrow and reveal a new therapeutic target to curtail neutrophil-mediated inflammation in pulmonary vascular disease.

Abstract

Visceral adipose tissue (VAT) is a metabolic organ known to regulate fat mass, and glucose and nutrient homeostasis. VAT is an active endocrine gland that synthesizes and secretes numerous bioactive mediators called 'adipocytokines/adipokines' into systemic circulation. These adipocytokines act on organs of metabolic importance like the liver and skeletal muscle. Multiple preclinical and in vitro studies showed strong evidence of the roles of adipocytokines in the regulation of metabolic disorders like diabetes, obesity and insulin resistance. Adipocytokines, such as adiponectin and omentin, are anti‐inflammatory and have been shown to prevent atherogenesis by increasing nitric oxide (NO) production by the endothelium, suppressing endothelium‐derived inflammation and decreasing foam cell formation. By inhibiting differentiation of vascular smooth muscle cells (VSMC) into osteoblasts, adiponectin and omentin prevent vascular calcification. On the other hand, adipocytokines like leptin and resistin induce inflammation and endothelial dysfunction that leads to vasoconstriction. By promoting VSMC migration and proliferation, extracellular matrix degradation and inflammatory polarization of macrophages, leptin and resistin increase the risk of atherosclerotic plaque vulnerability and rupture. Additionally, the plasma concentrations of these adipocytokines alter in ageing, rendering older humans vulnerable to cardiovascular disease. The disturbances in the normal physiological concentrations of these adipocytokines secreted by VAT under pathological conditions impede the normal functions of various organs and affect cardiovascular health. Abstract

Rubigine® is an anti-rust stain remover containing fluorides which is believed to have been the cause of many deaths in Martinique. However, after the modification of its composition in 2006, serious poisoning from old formulas containing fluorides persisted. Our main objective was to determine the clinical characteristics and prognostic factors of these intoxications. Methods: Any patient admitted to the Martinique University Hospital for acute Rubigine® poisoning was included from 1 January 2000 to 31 December 2016. Usual demographic and clinical data were collected and comparisons between surviving and deceased patients made using a univariate analysis and logistic regression. Results: Fifty-five patients were included (mean age: 43 years; sex ratio M/F: 1.1), and the main clinical characteristics were: changes in electrocardiogram (ECG) (80%), digestive system disorders (75%), and neurological disorders (12%). The main features linked to death were the presence of hydrofluoric acid (p < 0.0001), age over 55 years (p = 0.01), hypocalcemia after the initial intravenous calcium supplementation (p = 0.0003), diarrhea (p < 0.0001), hypersialorrhea (p < 0.0001), myocardial excitability (p < 0.0001), and state of shock (p < 0.0001). Three patients required circulatory support by venous-arterial ECMO. Mortality was 10.9%. Conclusions: Rubigine® poisoning is responsible for significant morbidity and mortality. Fortunately, its incidence as well as mortality has sharply decreased in Martinique thanks to the measures taken by the French state. This retrospective work nevertheless shows that acute intoxication by the old formula of Rubigine® remains the main factor of poor prognosis. [ABSTRACT FROM AUTHOR]

Abstract

Since 2011, there have been ongoing massive unexplained increases of sargassum seaweed strandings along the coastlines of Caribbean countries. The objective of our study was to describe the clinical characteristics of patients exposed to noxious emissions of decomposing sargassum seaweed. This observational study included patients from January 2018 to December 2018 for complaints attributed to decomposing sargassum seaweed. History and geographical characteristics of sargassum seaweed strandings as well as detection of ambient air hydrogen sulfide (H2S) levels were documented during the inclusion period. A total of 154 patients were included. Mean exposure period was 3 months. Neurological (80%), digestive (77%) and respiratory (69%) disorders were the most frequent reasons for medical visit. Temporal distribution of medical visits was related to history of strandings. Geographical origins of patients were consistent with the most impacted areas of strandings as well as the most elevated ambient air H2S levels. The toxicological syndrome induced by sargassum seaweed exposure is close to the toxidrome associated with acute H2S exposure in the range of 0–10 ppm. Our study suggests that patients living in massive stranding areas may be exposed to H2S > 5 ppm for 50 days per year. [ABSTRACT FROM AUTHOR]

Abstract

L'objectif de notre étude était de décrire les caractéristiques cliniques et les complications qui peuvent survenir au décours d'une ingestion de Rubigine® ainsi que de déterminer les facteurs pronostiques de décès. Méthodes: étude rétrospective sur 16 ans (2000 à 2016), incluant tous les patients admis aux urgences et en réanimation du Centre Hospitalier Universitaire (CHU) de Martinique, pour intoxication aiguë par la Rubigine®. Les données démographiques et cliniques usuelles ont été recueillies et les comparaisons entre patients survivants et décédés effectuées à l'aide d'une analyse univariée. Résultats: 55 patients (âge moyen: 43 ans (15-82); sex-ratio homme/femme: 1,1) ont été inclus. 25 des patients n'avaient aucun antécédent notable. La DMS était de 3,3 jours (1- 37). 40 des patients ont présenté une hypocalcémie après la supplémentation initiale par calcium IV. Les complications étaient les suivantes: insuffisance respiratoire aiguë nécessitant une ventilation mécanique invasive (22 des patients ; durée de ventilation : 2,8 jours, (1-16», défaillance rénale (20 ; dont 50 ayant nécessité une épuration extrarénale), défaillance hémodynamique (16), défaillance hépatique (15), défaillance de la coagulation (15), défaillance neurologique (12) et défaillance multiviscéraIe (10,9). Trois patients ont présenté un état de choc réfractaire cardiogénique nécessitant la mise sous ECMO veino-artérielle (5,5) et un autre patient une perforation digestive (1,8). La mortalité était de 10,9, permettant d'identifier des facteurs pronostiques de décès. Conclusions: L'intoxication par la Rubigine® est responsable d'une morbi-mortalité significative, malgré une prise en charge optimale.

Abstract

Near Hatay, the Antakya-Samandag-Cyprus Fault (ASCF), East Anatolian and Dead Sea Fault Zones, the large faults that form the edges of the African, Anatolian, Cyprus and Arabian Plates, all produce large earthquakes, which have decimated Hatay repeatedly. Near Samandag, Hatay, differential vertical displacement on the ASCF has uplifted the southeastern side relative to northwestern side, producing large fault scarps that parallel the Asi (Orontes) River. Tectonic uplift coupled with Quaternary sealevel fluctuations has produced several stacked marine terraces stranded above current sealevel. This study dated 24 mollusc samples from 10 outcrops on six marine terraces near Samandag electron spin resonance (ESR). Ages were calculated using time-averaged and volumetrically averaged external dose rates, modelled by assuming typical water depths for the individual species and sediment thicknesses estimated from geological criteria. Uplift rates were then calculated for each fault block. At all the MagaracA +/- k terraces, the dates suggest that many shells were likely reworked. On the 30 m terrace at MagaracA +/- k IV (UTM 766588-3999880), Lithophagus burrows with in situ shells cross the unconformity. One such shell dated to 62 +/- 6 ka, setting the minimum possible age for the terrace. For all the MagaracA +/- k terraces at similar to 30 m above mean sealevel (amsl), the youngest ages for the reworked shells, which averaged 60 +/- 3 ka for six separate analyses, sets the maximum possible age for this unit. Thus, the terrace must date to 60-62 +/- 3 ka, at the MIS 3/4 boundary when temperatures and sealevels were fluctuating rapidly. Older units dating to MIS 7, 6, and 5 likely were being eroded to supply some fossils found in this terrace. At MagaracA +/- k Dump (UTM 765391-4001048), similar to 103 m amsl, Ostrea and other shells were found cemented in growth position to the limestone boulders outcropping there < 2.0 m above a wave-eroded notch. If the oysters grew at the same time as the wave-cut notch and the related terrace, the date, 91 +/- 13 ka, for the oysters, this fault block has been uplifted at 1.19 +/- 0.15 m ky(-1), since MIS 5c. At Samandag Kurt Stream at 38 m amsl, molluscs were deposited fine sandy gravel, which was likely formed in a large tidal channel. Four molluscs averaged 116 +/- 5 ka. If these molluscs have not been reworked, this fault block has uplifted at 0.34 +/- 0.05 m ky(-1) since the MIS 5d/5e boundary. The differences in these uplift rates suggests that at least one, and possibly two, hitherto undiscovered faults may separate the MagaracA +/- k Dump site from the other MagaracA +/- k sites and from the Samandag Kurt Stream site.
