Abstract

Systemic antimicrobial treatments are commonly prescribed to dogs with acute diarrhoea, while nutraceuticals (prebiotics, probiotics, and synbiotics) are frequently administered as an alternative treatment. The aim of this systematic review and meta-analysis was to assess the effectiveness of antimicrobials and nutraceutical preparations for treatment of canine acute diarrhoea (CAD). The results of this study will be used to create evidence-based treatment guidelines. PICOs (population, intervention, comparator, and outcome) were generated by a multidisciplinary expert panel taking into account opinions from stakeholders (general practitioners and dog owners). The Grading of Recommendations Assessment, Development and Evaluation (GRADE) methodology was used to evaluate the certainty of the evidence. The systematic search yielded six randomised controlled trials (RCT) for antimicrobial treatment and six RCTs for nutraceutical treatment meeting the eligibility criteria. Categories of disease severity (mild, moderate, and severe) were created based on the presence of systemic signs and response to fluid therapy. Outcomes included duration of diarrhoea, duration of hospitalization, progression of disease, mortality, and adverse effects. High certainty evidence showed that antimicrobial treatment did not have a clinically relevant effect on any outcome in dogs with mild or moderate disease. Certainty of evidence was low for dogs with severe disease. Nutraceutical products did not show a clinically significant effect in shortening the duration of diarrhoea (based on very low to moderate certainty evidence). No adverse effects were reported in any of the studies.

14. Sol-gel synthesis of phosphate-based glasses for biomedical applications

Abstract

This thesis concerns the development of a new and facile sol-gel synthesis route for production of phosphate-based glasses for biomedical applications including; tissue engineering, imaging contrast agents and drug delivery systems. The structure of the prepared samples was probed by XRD, 31P MAS-NMR, EDX and FTIR spectroscopy that confirmed successful synthesis and production of phosphate based glasses via the sol-gel method. In this study, for the first time, quaternary phosphate-based sol-gel derived glasses in the P2O5-CaO-Na2O-TiO2 system with a high TiO2 content of up to 30 mol% were synthesised. While incorporating a high percentage of titanium into the phosphate network is non-trivial via traditional melt quench methods. Investigation of quaternary glasses with the general formula of (P2O5)55-(CaO)25-(Na2O)(20-x)-(TiO2)x, where X = 0, 5, 10 or 15 revealed, substituting titanium in place of sodium significantly improves the stability and prolongs the degradation of these glasses, which opens up a number of potential biomedical applications. Cell studies on titanium-stabilised glasses suggested glasses containing 5 or 10 mol% TiO2 have optimal potential for bone tissue engineering applications. Electrospraying was used to prepare (P2O5)55-(CaO)30- (Na2O)15 glass nanospheres with a diameter size range of 200-500 nm. These glass nano spheres were used as a transient contrast agent for ultrasound imaging to label mesenchymal stem cells and it was determined in vitro and in vivo that these nanospheres had a detection limit of 5 and 9 μg.mL-1, respectively. Cell counts down to 4000 could be measured with ultrasound imaging with no cytotoxicity at doses required for imaging. Glass nanospheres were also used as a carrier for drug delivery applications with a linear release of tetracycline hydrochloride molecules within the first 4 hours of the study period. Importantly, ion release studies confirmed these glass nanospheres biodegrade into an aqueous medium with degradation products that can be easily metabolised in the body. To the knowledge of the author, this is the first report of sol-gel synthesis and electrospraying to prepare glass nanospheres at low processing temperature and the first use of such a system for both diagnostic and therapeutic purposes.

Abstract

Background: Machine learning (ML) methods are increasingly used in addition to conventional statistical modelling (CSM) for predicting readmission and mortality in patients with myocardial infarction (MI). However, the two approaches have not been systematically compared across studies of prognosis in patients with MI. Methods: Following PRISMA guidelines, we systematically reviewed the literature via Medline, EPub, Cochrane Central, Embase, Inspec, ACM Digital Library, and Web of Science. Eligible studies included primary research articles published from January 2000 to March 2020, comparing ML and CSM for prognostication after MI. Results: Of 7,348 articles, 112 underwent full-text review, with the final set composed of 24 articles representing 374,365 patients. ML methods included artificial neural networks (n = 12 studies), random forests (n = 11), decision trees (n = 8), support vector machines (n = 8), and Bayesian techniques (n = 7). CSM included logistic regression (n = 19 studies), existing CSM-derived risk scores (n = 12), and Cox regression (n = 2). Thirteen of 19 studies examining mortality reported higher C-indexes with the use of ML compared with CSM. One study examined readmissions at 2 different time points, with C-indexes that were higher for ML than CSM. Across all studies, a total of 29 comparisons were performed, but the majority (n = 26, 90%) found small (< 0.05) absolute differences in the C-index between ML and CSM. With the use of a modified CHARMS checklist, sources of bias were identifiable in the majority of studies, and only 2 were externally validated. Conclusion: Although ML algorithms tended to have higher C-indexes than CSM for predicting death or readmission after MI, these studies exhibited threats to internal validity and were often unvalidated. Further comparisons are needed, with adherence to clinical quality standards for prognosis research. (Trial registration: PROSPERO CRD42019134896)

Abstract

Automatic Voltage Regulator (AVR) is employed to stabilize the output voltage of generators at electric power plants. However, reliable performance of AVR depends on professional tuning of its PID controller parameters. Therefore, different optimization algorithms are used to determine those parameters. The objective of the optimization is defined as minimizing the characteristics of transient step response such as settling time, rise time, overshoot, and steady state error. Then, to verify the optimization results, a simulator is built experimentally for AVR and PID system which can also be used for other studies on AVR systems. Experimental results are compared with those of MATLAB and Pspice software. Close agreement between the simulation and experimental results confirms the success of the optimization.
