Resume Introduction L’hospitalisation sous contrainte est une mesure psychiatrique lourde et complexe. En Seine-Saint-Denis (faible densite medicale), l’evaluation du patient en decompensation psychiatrique incombe aux medecins generalistes mandates par le Samu. Leur ressenti est le non-aboutissement de la procedure, une fois le patient aux urgences. L’objectif etait d’evaluer le devenir des patients a la suite d’une demande de soins psychiatriques a la demande d’un tiers emanant de ces medecins generalistes. Methodes Nous avons realise une etude retrospective a partir des demandes de soins psychiatriques a la demande d’un tiers au Samu 93, durant l’annee 2016. Les caracteristiques de l’appel et du patient ainsi que les decisions du regulateur et du medecin generaliste etaient recueillies. Le devenir etait recherche pour les patients adresses aux urgences pour soins psychiatriques a la demande d’un tiers. Resultats Parmi les 7541 appels pour decompensation, 539 concernaient une demande de soins psychiatriques a la demande d’un tiers. Ces appels survenaient durant les heures non ouvrees dans 55 % des cas. Un medecin generaliste etait mandate dans deux tiers des cas et a organise une demande de soins psychiatriques a la demande d’un tiers pour 240/304 (79 %) patients. Les patients etaient des hommes dans 56 % des cas avec une moyenne d’âge de 40 (± 16) annees. Le psychiatre a confirme l’hospitalisation sous contrainte pour 132 (61 %) patients. Ce taux n’est pas different de celui de 65 %, rapporte dans la litterature (Z-test, p = 0,26). Une hospitalisation libre etait realisee pour 37 (17 %) autres patients. Discussion Le taux de soins psychiatriques a la demande d’un tiers realise a la suite de la demande des medecins generalistes mandates par le Samu etait comparable a celui suite aux demandes des medecins traitant, validant leur intervention dans ce contexte critique.

Background The individual factors associated to Frequent Users (FUs) in Emergency Departments are well known. However, the characteristics of their geographical distribution and how territorial specificities are associated and intertwined with ED use are limited. Investigating healthcare use and territorial factors would help targeting local health policies. We aim at describing the geographical distribution of ED’s FUs within the Paris region. Methods We performed a retrospective analysis of all ED visits in the Paris region in 2015. Data were collected from the universal health insurance’s claims database. Frequent Users (FUs) were defined as having visited ≥3 times any ED of the region over the period. We assessed the FUs rate in each geographical unit (GU) and assessed correlations between FUs rate and socio-demographics and economic characteristics of GUs. We also performed a multidimensional analysis and a principal component analysis to identify a typology of territories to describe and target the FUs phenomenon. Results FUs accounted for 278,687 (11.7%) of the 2,382,802 patients who visited the ED, living in 232 GUs. In the region, median FUs rate in each GU was 11.0% [interquartile range: 9.5–12.5]. High FUs rate was correlated to the territorial markers of social deprivation. Three different categories of GU were identified with different profiles of healthcare providers densities. Conclusion FUs rate varies between territories and is correlated to territorial markers of social deprivation. Targeted public policies should focus on disadvantaged territories.

Air travel-related pulmonary embolism is rarely associated with other embolic complications, such as stroke, ST-segment elevation acute coronary syndrome and other systemic events we presented in our case report. Un retour d’expérience sur la direction médicale de crise (DMC) pendant la première phase de l’épidémie de Covid-19 a été effectué à l’Assistance publique–Hôpitaux de Paris (AP–HP), le plus important centre hospitalier universitaire européen. L’AP–HP s’est dotée d’un directeur médical de crise (DMC) AP–HP, de six DMC de groupes hospitaliers (GH) et d’un DMC pour chacun des 39 sites hospitaliers. Le pilotage s’est appuyé sur des réunions quotidiennes de crise AP–HP et de GH, des groupes de travail disciplinaires et des tableaux de bord quotidiens fiabilisés qui ont permis d’optimiser les actions. Des actions innovantes ont été mises en place : cellules de régulation des entrées et des sorties de réanimation, suivi des patients infectés à domicile, traçage des contacts, transferts interrégionaux de patients de réanimation. Les éléments clés de la réussite ont été les relations entre direction générale et DMC, l’articulation entre l’échelon central (AP–HP) et celui des GH, la mobilisation de tous les acteurs vers un objectif unique identifié et la mobilisation de l’ensemble des soignants, y compris les étudiants. Parmi les pistes d’amélioration soulignées, il convient de citer la généralisation des DMC hors AP–HP, conformément à la réglementation, le développement de la connaissance du mode de fonctionnement de crise, l’anticipation de la formation à la gestion de crise, la réalisation d’une information quotidienne de l’ensemble des acteurs des actions menées dans une crise de longue durée et la participation des représentants des usagers. La gestion de la recherche en temps de crise reste à inventer au niveau national, voire européen.

International audience; We aimed to both quantitatively and qualitatively describe interventional research performed in emergency medical communication centres. We conducted a systematic review of articles published in MEDLINE, Cochrane Central Register of Controlled Trials and Web of Science. Studies evaluating therapeutic or organizational interventions directed by call centres in the context of emergencies were included. Studies of call centre management for general practice or nonhealthcare agencies were excluded. We assessed general characteristics and methodological information for each study. Quality was evaluated by the Cochrane Risk of Bias tool or the Newcastle-Ottawa Scale. Among 3896 articles screened, we retained 59; 41 studies were randomized controlled trials (69%) and 18 (31%) were before-after studies; 41 (69%) took place in a single centre. For 33 (56%), 22 (37%) and 4 (7%) studies, the models used were simulation training, patient-based or experimental, respectively. The main topic was cardiac arrests (n = 45, 76%), with outcome measures of cardiopulmonary resuscitation quality and dispatch assistance. Among randomized controlled trials, risk of bias was unclear or high for selective reporting for 37 (90%) studies, low for blinding of outcomes for 34 (83%) and low for incomplete outcomes for 31 (76%). Regarding before-after studies, quality was high in 9 (50%) studies. Few interventional studies have been performed in call centres. Studies mainly involved simulation and focussed on cardiac arrest. The quality of studies needs improvement to allow for a better recognition and understanding of emergency medical call control.

Introduction : L’impact sanitaire des vagues de froid et de chaleur est majeur. Neanmoins, l’impact respectif des temperatures extremes, hautes et basses, demeure controverse.Methode : Le nombre quotidien de (1) dossiers de regulation medicale (DRM) geres par le SAMU 93 – indicateur de la demande en soins primaires, (2) interventions des equipes SMUR – indicateur de severite des patients et (3) patients decedes ont ete releves. Les temperatures minimales et maximales quotidiennes ont ete relevees de 2010 a 2018.L’analyse a porte sur les 10 journees plus chaudes et les plus froides de chaque annee (2 × 70 journees), et sur les 30 journees les plus chaudes et les plus froides de la periode (2 × 30 journees).Resultats : Sur 2 702 jours, 1 513 070 DRM, 89 478 interventions SMUR et 7 350 deces ont ete analyses. To determine the efficacy of emergency medical center physicians' use of a protocol to guide their management of telephone consultations for fever and gastroenteritis.Cluster randomized controlled trial. Participating centers were randomized to use the telephone protocol or provide usual telephone assistance. Six emergency centers in France included calls from patients needing advice on fever or gastroenteritis. Centers assigned to the protocol followed specific guidelines on managing the call and giving advice on treatment. Primary endpoints were the number of in-person visits and hospital admissions required within 15 days of the call. Secondary endpoints were patient satisfaction and costs.A total of 2498 calls were included. Use of the assigned protocol while attending 1234 calls was associated with a relative risk for hospitalization or an unscheduled in-person visit for care of 0.70 (95% CI, 0.58-0.85) versus usual practice. Ambulance use, admission to an intensive care unit, mortality, morbidity, and symptom improvement did not differ significantly between centers using the protocol and those following usual practice. Ninety percent of the patients were satisfied. The cost of care was €91 in centers applying the protocol and €150 in the other centers (P .01).Use of the protocol was associated with fewer unscheduled in-person visits for care and fewer hospital admissions. The protocol is safe and less costly than the centers' usual approaches to giving telephone advice.Determinar la eficacia de un protocolo de asesoramiento médico telefónico formalizado (AMTF), realizado por un médico para consultas, para fiebre o gastroenteritis en centros de comunicación médica de emergencia.Ensayo clínico por conglomerado, controlado. Los pacientes fueron aleatorizados al grupo AMTF o al grupo de atención habitual. Participaron 6 centros de comunicación médica de emergencia franceses. Se incluyeron pacientes que solicitaban asistencia telefónica por fiebre o gastroenteritis. El grupo ATMF realizó recomendaciones protocolizadas sobre el manejo terapéutico. Se valoró el número de consultas presenciales o ingreso hospitalario durante los 15 días siguientes a la consulta. También se evaluó la satisfacción del paciente y el coste económico.Se incluyeron 2.498 llamadas. El grupo ATMF (n = 1.234) tuvo un riesgo relativo de 0,70 (CI 95% 0,58 a 0,85) de requerir un ingreso hospitalario o de realizar una consulta no programada durante el seguimiento. No hubo diferencias entre los dos grupos en cuanto al uso de ambulancia, el ingreso en cuidados intensivos, la mortalidad o morbilidad y la mejoría de los síntomas. La satisfacción de los pacientes fue del 90%. El coste total fue de 91 euros en el grupo de la ATMF y de 150 euros en el grupo de atención habitual (p 0,01).El grupo ATMF se asoció con una disminución de las consultas presenciales no programadas o del ingreso en el hospital. Este procedimiento es seguro y comporta un menor coste que la atención que se realiza habitualmente en la actualidad.

La survenue d’un arrêt cardiaque intra-hospitalier, quiconstitue la forme la plus extrême de l’urgence vitaleintra-hospitalière (UVIH), constitue un évènementindésirable majeur qui concerne environ 1 à 5 patientspour 1000 admissions. Le pronostic de ces victimesest étroitement dépendant de la mise en œuvre d’unechaîne de survie intra-hospitalière efficace, dont l’élémentle plus important est le déclenchement immédiat dessecours. Chaque minute compte : tout retard engendrénotamment par une incertitude ou une hésitation surle système d’alerte diminue les chances de survie etaugmente les risques de séquelles.

A higher survival rate was observed in Sudden Cardiac Arrest (SCA) occurring during sports activities, although the underlying mechanisms remain unclear. We tested the hypothesis that better initial management, rather than sports per se, may account for the observed better outcomes during sports activities.Data was taken between May 2011 and March 2016 from a prospective ongoing registry that includes all SCA in Paris and suburbs (6.7 million inhabitants). Sports-related SCA (i.e. SCA occurring during sport activities or within one hour of cessation of the activity) were identified.Over the study period, 13,400 SCA occurred, of which 154 were sports-related (median age: 51.2 years, 96.1% males). At discharge, sports activity was associated with an 8-times higher survival rate (39.7% vs. 5.1%, P 0.001). Logistic regression showed that after considering potential confounders, including age, gender, SCA location, witness presence, time to response, and initial shockable rhythm, occurrence of SCA during sports was associated with a higher survival rate (OR 1.77, 95% CI 1.14-2.74, P = 0.01). However, after further adjustment for initial basic life support, i.e. bystander CPR and AED use, there was no association between sports setting and survival at hospital discharge (OR 1.43, 95% CI 0.91-2.23, P = 0.12).Sports-related SCA is a rare event, with an 8-times higher survival rate compared to non-sports-related SCA. Better initial management, including bystander CPR and AED use, rather than sports per se, mainly accounts this difference. This highlights the major importance of population education to basic life support in improving SCA outcome.

Objectives Life threatening complications can occur at any stage of cirrhosis progression. There are few studies on the prognosis of cirrhotic patients managed in an Emergency Department (ED) although management of patients will occur in the ED. The objective of our study was to determine the risk factors for mortality in cirrhotic patients who visited to the ED. Methods All cirrhotic patients attending ED in three different university hospitals of Assistance Publique - Hopitaux de Paris between January 2014 and June 2015 were identified by a retrospective analysis of digital records and included in the study. The primary end-point was 30-day mortality in all cirrhotic patients who visited the ED. Results A total of 609 ED visits were analyzed among 224 patients: 115 (51%) presented a cirrhosis of alcoholic origin, 43 (19%) were caused by Hepatitis C, 28 (13%) of mixed origin (viral and alcoholic), 17 (8%) were caused by Hepatitis B and 21 (9%) of other origins. Fifty-five (25%) of these patients died within 30 days of their initial presentation to the ED. In multivariate analysis, the age (Odds Ratio: 1.04 [1.01–1.07]), cirrhosis associated with hepatocellular carcinoma (OR: 3.07 [1.37–6.91]), serum creatinine at admission (OR: 1.01 [1.01–1.02]), serum bilirubin at admission (OR: 1.01 [1.01–1.02]) and health impairment (OR: 2.57 [1.28–5.16]) were associated with mortality. Conclusions The mortality rate of cirrhotic patients attending an ED was high. The prognosis of cirrhotic patients admitted to the ED depended on the severity of the liver and other organ dysfunction. The presence of a hepatocellular carcinoma on admission was also a risk factor for death.

Background The terrorist attacks in Paris and in Saint-Denis in November 2015 were unprecedented events involving various human and material resources. These events question the role of nurse students in prehospital teams. Purpose To investigate nursing students’ preference about whether they wished to participate in the prehospital care during a terrorist attack. Methods This cross-sectional study was conducted with student nurses, from two nursing schools in the Greater Paris area. They completed an anonymous survey assessing the desire to be called to help the mobile intensive care units (MICU) or another ward; whether their presence should be mandatory, and the feelings associated with their experience. The responses were collected with a visual analogue scale and could range from 1 (yes, very much) to 10 (no, not at all). A Chi-square test was performed for qualitative variables and a Mann–Whitney test for quantitative variables. Findings Among 225 students, 205 (91%) responded, 133 (65%) were women. When on duty, 169 (82%) would have preferred to accompany the MICU team, compared with 31 (15%) who would have preferred not to go. Overall, 146 students (71%) considered that this presence should be optional. Only gender was significantly associated with the choice to accompany the MICU team (W = 87% vs. M = 13%; p = .002). Students expressed a moderate feeling of frustration and fear. Discussion Students would prefer to assist the MICU team responding to the scene of a terrorist attack but feel this choice should be optional. A discussion in nursing schools and universities should be considered for the implementation of a “systematic” procedure to ensure the student's willingness to participate in such interventions.

Objective Cardiac arrest (CA) is considered a major public health issue. Few studies have focused on geographic variations in incidence and socioeconomic characteristics. The aim of this study is to identify clusters of municipalities with high or low CA incidence, and find socioeconomic factors associated with them. Patients and methods CA data from three Parisian counties, representing 123 municipalities, were extracted from the French CA registry. Socioeconomic data for each municipality were collected from the French national institute of statistics. We used a statistical approach combining Bayesian methods to study geographical CA incidence variations, and scan statistics, to identify CA incidence clusters of municipalities. Finally, we compared clusters of municipalities in terms of socioeconomic factors. Results Strong geographical variations were found among 123 municipalities: 34 presented a significantly increased risk of incidence and 37 presented a significantly low risk. Scan statistics identified seven significant spatial clusters of CA incidence, including three clusters with low incidence (the relative risk varied from 0.23 to 0.54) and four clusters with high incidence (the relative risk varied from 1.43 to 2). Clusters of municipalities with a high CA incidence are characterized by a lower socioeconomic status than the others (low and normal CA incidence clusters). Analysis showed a statistically significant relationship between social deprivation factors and high incidence. Conclusion This study shows strong geographical variations in CA incidence and a statistically significant relationship between over-incidence and social deprivation variables.

Background: In out-of-hospital cardiac arrest (OHCA), geographic disparities in outcomes may reflect baseline variations in patients’ characteristics but may also result from differences in the number of ambulances providing basic life support (BLS) and advanced life support (ALS). We aimed at assessing the association between allocated ambulance resources and outcomes in OHCA patients in a large urban community. Methods: From May 2011 to January 2016, we analyzed a prospectively collected Utstein database for all OHCA adults. Cases were geocoded according to 19 neighborhoods and the number of BLS (firefighters performing cardiopulmonary resuscitation and applying automated external defibrillator) and ALS ambulances (medicalized team providing advanced care such as drugs and endotracheal intubation) was collected. We assessed the respective associations of Utstein parameters, socioeconomic characteristics, and ambulance resources of these neighborhoods using a mixed-effect model with successful return of spontaneous circulation as the primary end point and survival at hospital discharge as a secondary end point. Results: During the study period, 8754 nontraumatic OHCA occurred in the Greater Paris area. Overall return of spontaneous circulation rate was 3675 of 8754 (41.9%) and survival rate at hospital discharge was 788 of 8754 (9%), ranging from 33% to 51.1% and from 4.4% to 14.5% respectively, according to neighborhoods ( P P for trend P 1.5 per neighborhood and aOR, 1.21 [1.04–1.41]; P =0.01 for BLS ambulances >4 per neighborhood). Regarding survival at discharge, only the number of ALS ambulances >1.5 per neighborhood was significant (aOR, 1.30 [1.06–1.59] P =0.01). Conclusions: In this large urban population-based study of out-of-hospital cardiac arrests patients, we observed that allocated resources of emergency medical service are associated with outcome, suggesting that improving healthcare organization may attenuate disparities in prognosis.

Chest compressions are the cornerstone of cardiopulmonary resuscitation. The recent International Liaison Committee on Resuscitation guidelines recommend increasing the rate and the depth of chest compressions, to 100-120/min and 5-6 cm, based on theoretical arguments and observational studies. We hypothesized that an increased chest compressions rate could decrease chest compressions depth.Operators were asked to perform continuous chest compressions on a mannequin. Chest compressions rate and depth were collected. The ratio of chest compressions compliance to the guidelines, that is rate 100-120/min and depth 5-6 cm, was calculated.In total 951 sequences of chest compressions were studied. Median chest compressions rate: 114 (104-130)/min. Median chest compressions depth: 4.5 (3.4-5.3) cm. Correlation between rate and depth: low (R2 = 0.12). Chest compressions in compliance with the recommended rate: 434 (46%). Rate120/min in 285 (30%) cases and100/min in 223 (23%) cases. Chest compressions in compliance with the recommended depth: 286 (30%). Depth6 cm in 50 (5%) cases and5 cm in 621 (65%) cases. Finally, chest compressions were in compliance with the guidelines for both rate and depth in 141 (15%) cases. The ratio of chest compressions in compliance with the recommended depth significantly decreased with the increase of the rate: 40% for a rate100/min, 32% for a rate in the target (100-120/min) and 18% for a rate100/min (P 0.0001).The ratio of chest compressions in compliance with the recommended rate and depth was as low as 15%. The rate of chest compressions in compliance with the recommended depth significantly decreased when the chest compressions rate increased. To reach both recommended rate and depth seems illusive.

IntroductionIn cardiac arrest (CA), time is directly predictive of patient prognosis. The increase in mortality resulting from delayed cardiopulmonary resuscitation has been quantified minute by minute. Times reported in such a situation could reflect a bias referred to as “digit preference”. This phenomenon leads to privilege certain numerical values (like 2, 5, or 10) over others (like 13). We investigated this bias in times reported during cardiac arrest management in a national register. MethodsWe analyzed data from the French National Electronic Registry of Cardiac Arrests. All the data, including the twelve times corresponding to the main steps of the management of cardiac arrest are prospectively reported by the emergency physician who managed the patients in prehospital settings. The frequency of times as multiples of 15 (0, 15, 30, and 45) was our primary end-point.Results 47,211 times related to 6,131 cardiac arrests were analyzed. The most overrepresented numbers were: 0, with 3,737 occurrences (8% vs 2% expected, pConclusionProspectively collected time were considerably influenced by digit preference phenomenon. Studies that are not based on automatic recording of times as well as studies that have not evaluated and considered this bias should be interpretated with caution.

Background In out-of-hospital cardiac arrest (OHCA), bystander initiated cardiopulmonary resuscitation (CPR) increases the chance of return of spontaneous circulation and survival with a favourable neurological status. Socioeconomic disparities have been highlighted in OHCA field. In areas with the lowest average socioeconomic status, OHCA incidence increased, and bystander CPR decreased. Evaluations were performed on restricted geographical area, and European evaluation is lacking. We aimed to analyse, at a national level, the impact of area-level social deprivation on the initiation of CPR in case of a witnessed OHCA. Methods We included all witnessed OHCA cases with age over 18 years from July 2011 to July 2018 form the OHCA French national registry. We excluded OHCA occurred in front of rescue teams or in nursing home, and patients with incomplete address or partial geocoding. We collected data from context, bystander and patient. The area-level social deprivation was estimated by the French version of the European Deprivation Index (in quintile) associated with the place where OHCA occurred. We assessed the associations between Utstein variables and social deprivation level using a mixed-effect logit model with bystander-initiated CPR. Results We included 23,979 witnessed OHCA of which 12,299 (51%) had a bystander-initiated CPR. More than one third of the OHCA (8,326 (35%)) occurred in an area from the highest quintile of social deprivation. The higher the area-level deprivation, the less the proportion of bystander-initiated CPR (56% in Quintile 1 versus 48% in Quintile 5). The In the multivariable analysis, bystander less often began CPR in areas with the highest deprivation level, compared to those with the lowest deprivation level (OR=0.69, IC95%: 0.63-0.75). Conclusions The level of social deprivation of the area where OHCA occurred was associated with bystander-initiated CPR. It decreased in the more deprived areas although these areas also concentrate more younger patients.

Education is at the cornerstone of management of insulin-treated diabetic patient. Previous studies reported that the education of diabetic patients and their entourage was failing, hypothesizing that this lack of education was explained by a lack of knowledge on the disease and its complications and the underestimation of their respective severity. The "Educated Why" suggests that diabetic patients treated with insulin know that hypoglycemia and to a lesser extent diabetes itself are serious or very serious diseases.

International audience; Aim: Early-onset pneumonia (EOP) is a common in-hospital complication in survivors of out-of-hospital cardiac arrest. In this substudy of the CAAM trial, we aimed to compare whether bag mask ventilation (BMV) compared to endotracheal intubation (ETI) performed during cardiopulmonary resuscitation increases the risk of developing EOP.Methods: Adult patients from the CAAM trial that survived beyond 12 h of hospitalization were included. Information about in-hospital management and outcome of study subjects was systematically collected. Our primary aim was to compare the incidence of EOP in the BMV and ETI group using a series of bivariate analysis adjusting for one variable at a time and a logistic regression controlled for survival beyond 96 h, age, gender, catecholamine administration, no flow time, and initial shockable rhythm.Results: Of 627 patients from the CAAM trial that survived to hospital admission, 409 patients were hospitalized beyond 12 h and thus included (202 randomized to BMV and 20 7 randomized to ETI). Patients in the BMV group had a significantly longer period of unsecured airway during prehospital cardiopulmonary resuscitation (BMV (median): 33 min; ETI (median): 17 min, p < 0.0001). No significant difference in the development of EOP according to airway management was identified on univariate analysis (BMV: 53%, ETI: 53%, Odds Ratio 1.0 [0.7-1.5], p = 1.0). We found no difference in the development of EOP according to airway management in the series of bivariate analyses or in the multivariable regression analysis either.Conclusion: In this substudy of the CAAM trial, development of early-onset pneumonia in out-of-hospital cardiac arrest survivors did not depend on airway management technique during CPR.

Background The aim of our retrospective study was to evaluate the earliest COVID19-related signal to anticipate requirements of intensive care unit (ICU) beds. Although the number of ICU beds is crucial during the COVID-19 epidemic, there is no recognized early indicator to anticipate it. Methods In the Ile-de-France region, from February 20 to May 5, 2020, emergency medical service (EMS) calls and the response provided (ambulances) together the percentage of positive reverse transcriptase polymerase chain reaction (RT-PCR) tests, general practitioner (GP) and emergency department (ED) visits, and hospital admissions of COVID-19 patients were recorded daily and compared to the number of ICU patients. Correlation curve analysis was performed to determine the best correlation coefficient, depending on the number of days the indicator has been shifted. Primary endpoint was the number of ICU patients. Results EMS calls, percentage of positive RT-PCR tests, ambulances used, ED and GP visits of COVID-19 patients were strongly associated with COVID-19 ICU patients with an anticipation delay of 23, 15, 14, 13, and 12 days respectively. Hospitalization did not anticipate ICU bed requirement. Conclusion The daily number of COVID19-related telephone calls received by the EMS and corresponding dispatch ambulances, and the proportion of positive RT-PCR tests were the earliest indicators of the number of COVID19 patients requiring ICU care during the epidemic crisis, rapidly followed by ED and GP visits. This information may help health authorities to anticipate a future epidemic, including a second wave of COVID19 or decide additional social measures.

BackgroundFor over 35 years the ‘Breast Cancer Awareness Month’ (BCAM) has worked to increase public information about breast cancer and to raise funds for research. The aim of this study was to analyze how the interest generated by this campaign evolved over time, in the US and other countries.MethodsWe investigated the Google Trends® according to previously recommended methods. The study was based on Internet searches using the term ‘’breast cancer’’ worldwide and in the United-States. Google Trends® gives the relative search volume of every request as an index on a scale from 0 to 100, (100 corresponding to the maximum number of requests during the studied period). We compared the relative searches index using the term ‘’breast cancer’’ during October, the breast cancer awareness month, and the rest of the of the year avec a 10 years period.Results120 months were analyzed including 10 October months. Median index for worldwide searches on ‘’breast cancer’’ during the 110 months was 35 (34-39). It didn’t significantly change during the study period (R2=0,3). Median index of the 10 peaks was 83 (64-92) (p2=0,88) during the study period. It reached 100 in 2012 and 64 in 2020. In the United-States, median index for searches during the 110 months was 28 (26-32). It didn’t significantly change during the study period (R2=0,3). Median index of the 10 peaks was 83 (63-92) (p2=0,73) during the study period. It reached 100 in 2012 and 64 in 2020.ConclusionsThis internet activity study shows that the impact of the ‘Breast Cancer Awareness Month’ seems to be slowly decreasing. The number of Internet searches generated by the campaign has been reduced by almost 40% in both the United-States and worldwide.

Background Epinephrine effectiveness and safety are still questioned. It is well known that the effect of epinephrine varies depending on patients’ rhythm and time to injection. Objective We aimed to assess the association between epinephrine use during out-of-hospital cardiac arrest (OHCA) care and patient 30-day (D30) survival. Methods Between 2011 and 2017, 27,008 OHCA patients were included from the French OHCA registry. We adjusted populations using a time-dependent propensity score matching. Analyses were stratified according to patient's first rhythm. After matching, 2837 pairs of patients with a shockable rhythm were created and 20,950 with a nonshockable rhythm. Results Whatever the patient's rhythm (shockable or nonshockable), epinephrine use was associated with less D30 survival (odds ratio [OR] 0.508; 95% confidence interval [CI] 0.440–0.586] and OR 0.645; 95% CI 0.549–0.759, respectively). In shockable rhythms, on all outcomes, epinephrine use was deleterious. In nonshockable rhythms, no difference was observed regarding return of spontaneous circulation and survival at hospital admission. However, epinephrine use was associated with worse neurological prognosis (OR 0.646; 95% CI 0.549–0.759). Conclusions In shockable and nonshockable rhythms, epinephrine does not seem to have any benefit on D30 survival. These results underscore the need to perform further studies to define the optimal conditions for using epinephrine in patients with OHCA.

Introduction Worldwide, the COVID-19 epidemic has put health systems to the test. The excess mortality is partly due to the influx of patients requiring hospitalization and intensive care. We propose that the chronology of epidemic spread gives a window of time in which hospitals can act to prevent reaching capacity. Methods The out-of-hospital SAMU Emergency Medical System in an entry point into the French health care system. We recorded the number of patients managed, of patients transferred to emergency departments (ED), and of mobile intensive care units (MICUs) dispatched. Each criterion was compared to the mean of the same criterion over the previous 5 years. The alert threshold which indicated a public health crisis was defined as a 20% increase compared to the 5-year mean. Results The reference period, from January 2015 to December 2019, included 3 381 611 calls, and 1 137 856 patients. The study period, from 17 February to 28 March 2020, included 166 888 calls, and 56 708 patients. The daily numbers of patients managed crossed the threshold on February 25, and increased until the end of the study period. The daily number of patients transferred to ED crossed the threshold on March 16, and increased until the end of the period. The daily number of MICUs dispatched crossed the threshold on March 15, and increased until the end of the period. Conclusion The COVID-19 epidemic reached our department in three consecutive waves which overwhelmed the health care system. The first wave preceded by 30 days the massive arrival of critical patients. Health care systems must take advantage of this delay to prepare for the third wave.

Clinical features of COVID-19 have been mostly described in hospitalized patients with and without ICU admission. Yet, up to 80% of patients are managed in an outpatient setting. This population is poorly documented. In France, health authorities recommend outpatient management of patients presenting mild-to-moderate COVID-19 symptoms. The aim of this study was to describe their clinical characteristics. The study took place in an emergency medical dispatching center located in the Greater Paris region. Patients included in this survey met confirmed COVID-19 infection criteria according to the WHO definition. We investigated clinical features and classified symptoms as general, digestive, ear–nose–throat, thoracic symptoms, and eye disease. Patients were included between March 24 and April 6 2020. 1487 patients included: 700 (47%) males and 752 (51%) females, with a median age of 44 (32–57) years. In addition to dry cough and fever reported in more than 90% of cases, the most common symptoms were general symptoms: body aches/myalgia (N = 845; 57%), headache (N = 824; 55%), and asthenia (N = 886; 60%); shortness of breath (N = 479; 32%) and ear–nose–throat symptoms such as anosmia (N = 415; 28%) and ageusia (N = 422; 28%). Chest pain was reported in 320 (21%) cases and hemoptysis in 41 (3%) cases. The main difference between male and female patients was an increased prevalence of ear–nose–throat symptoms as well as diarrhea, chest pains, and headaches in female patients. General symptoms and ear–nose–throat symptoms were predominant in COVID-19 patients presenting mild-to-moderate symptoms. Shortness of breath and chest pain were remarkably frequent. BACKGROUND The internet has changed the world. Meanwhile, internet searches themselves are now also being used a scientific tool. Recently, a famous European fashion brand (Desigual®) published as publicity, the photograph of a young model with vitiligo. OBJECTIVE To study the impact of the public display of such a rare disease on related internet searches. METHODS To do this, we investigated the Google Trends according to previously recommended methods. We compared the number of internet searches using the word ‘’vitiligo’’ and the model’s name, before and after the publication of this picture. Google Trends gives the frequency of such requests on a scale from 0 to 100, 100 corresponding to the maximum number of requests during the period of interest. A six year period was studied; three years before and three years after the campaign (i.e. 2013 September 1st). RESULTS The median frequency of searches for ‘’vitiligo’’ according to Google Trends, worldwide, was 52 (46-61) during the reference period. After the campaign, the median number of internet requests significantly increased to 70 (59-79) (p CONCLUSIONS This advertising campaign featuring a model with vitiligo seems to have been associated with a significant increase in internet searches for this condition. Such campaigns could indeed help to incidentally ‘’popularize’’ such skin disorders, with better public understanding contributing to an improvement in patients’ quality of life. Background: Our purpose was to describe the care pathway of patients hospitalized for acute heart failure (AHF) and investigate whether a management involving a cardiology department had an impact on in-hospital mortality. Methods: Between June 2014 and October 2018, we included patients hospitalized for AHF in 24 French hospitals. Characteristics of the episode, patient’s care pathway and outcomes were recorded on a specific assessment tool. The primary outcome was the association between patient care pathway and in-hospital mortality. The independent association between admission to a cardiology ward and in-hospital mortality was assessed through a multivariate regression model and propensity score matching. Results: A total of 3677 patients, mean age of 78, were included. The in-hospital mortality rate was 8% (n = 287) and was associated on multivariate regression with advanced age, presence of sepsis, of cardiogenic shock, high New York Heart Association (NYHA) score and increased plasma creatinine level on admission. High blood pressure and admission to a cardiology department appeared as protective factors. After propensity score matching, hospitalization in a cardiology department remained a protective factor of in-hospital mortality (OR = 0.61 [0.44–0.84], p = 0.002). Conclusion: A hospital course of care involving a cardiology department was associated with an increase in hospital survival in AHF patients. These finding may highlight the importance of collaboration between cardiologists and other in-hospitals specialties, such as emergency physicians, in order to find the best in-hospital pathway for patients with AHF.Clinical Trial NCT03903198. The health impact of cold and hot waves is major. Nevertheless, the respective impact of extreme high and low temperatures remains controversial.The daily number of (1)#160;patients managed by SAMU#160;93 dispatching centre#8211; primary care requirement indicator, (2)#160;Mobile Intensive Care Unit (MICU) interventions#8211; patient severity indicator and (3)#160;number of deaths have been recorded. Daily minimum and maximum temperatures were recorded from 2010 to 2018. The analysis covered the 10#160;warmest and coldest days of each year (2#160;#215;#160;70#160;days), and the 30#160;warmest and coldest days of the total period (2#160;#215;#160;30 days).Over 2,702#160;days, 1,513,070#160;patients, 89,478#160;MICU interventions and 7,350#160;deaths were analysed. BACKGROUND Oligo-analgesia is common in the emergency department (ED). This study aimed at reporting, when initiated by triage nurse, the superior efficacy of inhaled methoxyflurane plus standard of care (m-SoC) analgesia versus placebo plus SoC (p-SoC) for moderate-to-severe trauma-related pain in the hospital ED. METHODS A randomised, double-blind, placebo-controlled trial was conducted at eight EDs. Adults with pain score ≥4 (11-point numerical rate scale, NRS) at admission were randomised to receive one or two inhalers containing m-SoC or p-SoC. Primary outcome measure was time until pain relief ≤30 mm, assessed on the 100-mm Visual Analogic Scale (VAS). RESULTS A total of 351 patients were analysed (178 m-SoC; 173 p-SoC). Median pain prior to first inhalation was 66 mm, 75% had severe pain (NRS 6-10). Resume Introduction La pollution atmospherique constitue un probleme de sante publique de pregnance croissante. Les regions fortement urbanisees comme d’Ile-de-France semblent particulierement exposees. Pour autant l’impact sanitaire global est peu documente. Objectif Etudier l’impact de la degradation de la qualite de l’air sur la demande de soins primaires. Methode Site : regulation medicale du SAMU 93-Centre 15. Bassin de population : 1,6 millions d’habitants. Donnees analysees : nombre quotidien de dossiers de regulation medicale (DRM) et indice quotidien de qualite de l’air (IQA) au moyen de la base Airparif® de janvier 2014 a fevrier 2017. L’IQA est classe en cinq niveaux. Le niveau 4 correspond au seuil d’information et de recommandations pour reduire certaines sources d’emissions polluantes et le niveau 5 au seuil d’alerte instaurant des mesures de restriction ou de suspension des activites concourant a la pollution dont la circulation routiere. Resultats L’analyse a porte sur 1134 jours consecutifs et un total de 639 576 DRM. Nombre median quotidien de DRM : 564 (507–643). IQA ≥ 4 pendant 56 (5 %) jours et IQA ≥ 5 pendant 4 (0,4 %) jours. Le nombre de DRM etait tres etroitement correle a l’IQA (R2 = 0,91) ; la mediane quotidienne variait de 502 (494–621) pour un IQA de niveau 1 a 650 (540–704) pour un IQA ≥ 4. Conclusion La degradation de la qualite de l’air etait significativement correlee a la demande de soins primaires. L’alerte environnementale constitue aussi une alerte sanitaire. L’impact etait majeur (DRM +30 %) en considerant tous les pathologies, tous les polluants a une echelle departementale.

Introduction : Le taux d’occupation des lignes téléphoniques d’un Samu-Centre 15 conditionne la fluidité de la gestion des appels et donc sa capacité de répondre à l’urgence. La disponibilité des assistants de régulation médicale (ARM) et des médecins régulateurs est déterminante. Nous nous sommes interrogés sur l’impact du nombre d’ARM sur la prise en charge des appels d’un Samu-Centre 15. Méthodes : Le taux d’appels raccrochés (par l’appelant) après 15 secondes a été retenu comme critère de jugement principal. Il a été analysé en fonction de l’effectif « réel » d’ARM. L’effectif théorique d’ARM a été pris comme référence. Deux analyses ont été réalisées, la première portant sur 672 périodes d’un quart d’heure de la semaine 51 de l’année 2016 (en période d’épidémie de grippe) et la seconde sur 3 624 périodes d’une heure sur les cinq premiers mois de l’année 2017. Résultats : Sur la première période, le nombre médian d’appels raccrochés après 15 secondes était de 1 (0–3) par quart d’heure. Il variait de 0 (0–1) à 3 (1–5) selon l’effectif d’ARM auquel il était parfaitement inversement corrélé (p < 0,001). Sur la seconde période, le nombre médian d’appels raccrochés était de 4 (1–8) par heure. Il variait de 2 (1–6) à 10 (6–16) selon l’effectif d’ARM auquel il était parfaitement inversement corrélé (p < 0,001). Conclusion : Le taux d’appels raccrochés après 15 secondes était parfaitement corrélé à l’effectif (plus précisément au sous-effectif) d’ARM. Sur une année, le nombre estimé d’appels perdus ainsi était proche de 50 000 ! Prendre en considération l’effectif des ARM est une nécessité absolue.

BACKGROUND The terrorist attacks in Paris and Saint Denis on November 13, 2015 were an unprecedented traumatic event in France. It was an especially distressing ordeal for the healthcare personnel involved in the care of the victims. The aim of this study was to estimate the effect of direct participation in the rescue on posttraumatic stress disorder (PTSD) symptoms among these workers. METHODS Less than a month later, 613 healthcare providers (professionals and paraprofessionals) from three hospitals in the Paris suburbs were asked to complete an anonymous questionnaire. A multivariable Poisson model estimated the effect of participating onsite in the rescue (exposure variable) on the number of PTSD symptoms measured by the Trauma Screening Questionnaire (TSQ; outcome variable), adjusted for covariates. RESULTS Two hundred thirty-three providers completed the assessment (38% response rate), 130 participated directly in the rescue (56%). The quality of procedural analgesia and sedation among trauma patients has not been studied much in the prehospital setting. The main objective of this study was to characterize the quality of procedural analgesia sedation practices in prehospital settings in trauma patients.This was an open-label observational prospective multicenter study (January 01, 2012-December 31, 2013). We included all consecutive trauma victims undergoing a potentially painful procedure on the accident scene. The primary endpoint was the procedural pain intensity.Data for 210 patients aged 11 to 98 years were analyzed. The most common lesions were limb fractures or dislocations. The most common procedures were limb realignment and splinting. Overall, 25 different drug combinations [with paracetamol [acetaminophen], non-steroidal anti-inflammatory drugs, nefopam, opioids, loco-regional anesthesia, Equimolar Mixture of Oxygen/Nitrous Oxide (EMONO), sedative drugs] were used by the emergency medical services (EMS). One hundred seventeen patients (55%) received either one or two sedative drugs (among ketamine, propofol, and midazolam), 171 patients (81%) received morphine that was combined with a sedative drug in 54% of cases. During the procedure, 95 patients, 45% [95% Confidence Interval (CI) 39-52] experienced intense to severe pain. Among patients who received sedative drugs, 27% (32/117) had intense to severe pain vs. 68% (63/93) in patients who did not, that is, 40% difference [95% CI 33.8-47.0]. Seventeen patients (8%) experienced 18 adverse events of which 6 were respiratory adverse events. A deep sedation occurred in 17 patients. No center had any specific protocols for procedural sedation analgesia.Procedural sedation-analgesia was inadequate in almost half of the trauma patients in the out-of-hospital setting. The reasons of these failures were probably multiple. The non-administration of a sedative drug despite an indication or non-adapted doses, in the context of a lack of specific protocols, was certainly one of them.
