We have used near-infrared spectroscopy to measure the hemoglobin saturation at rest and during exercise on patients affected by peripheral vascular disease (PVD). The instrument used in our study is a frequency-domain tissue oximeter which employs intensity modulated (110 MHz) laser diodes. We examined 9 subjects, 3 of which were controls and 6 were patients affected by stage II PVD. The optical probe was located on the calf muscle of the subjects. The measurement protocol consisted of: (1) baseline (∼5 min); (2) stationary bicycle exercise (∼5 min); (3) recovery (∼15 min). The change in hemoglobin saturation during exercise (ΔY) and the recovery time after exercise (trec) were significantly greater in the PVD patients (ΔY = -21±3%, trec= 5.9±3.8 min) than in the control subjects (ΔY = 2±3%, trec = 0.6±0.1 min).

Patients with COVID-19 may experience multiple conditions (e.g., fever, hyperventilation, anorexia, gastroenteritis, acid-base disorder) that may cause electrolyte imbalances. Hypokalemia is a concerning electrolyte disorder that may increase the susceptibility to various kinds of arrhythmia. This study aimed to estimate prevalence, risk factors and outcome of hypokalemia in a cohort of non-critically ill patients. A retrospective analysis was conducted on 290 hospitalized patients with confirmed COVID-19 infection at the tertiary teaching hospital of Modena, Italy.Hypokalemia (Hypokalemia is a frequent disorder in COVID-19 patients and urinary potassium loss may be the main cause of hypokalemia. The disorder was mild in the majority of the patients and was unrelated to poor outcomes. Nevertheless, hypokalemic patients required potassium supplements to dampen the risk of arrhythmias.

BackgroundPatients colonized with carbapenem-resistant Enterobacteriaceae (CRE) are at higher risk of developing CRE infection after liver transplantation (LT), with associated high morbidity and mortality. Prediction model for CRE infection after LT among carriers could be useful to target preventive strategies.MethodsMultinational multicenter cohort study of consecutive adult patients underwent LT and colonized with CRE before or after LT, from January 2010 to December 2017. Risk factors for CRE infection were analyzed by univariate analysis and by Fine-Gray subdistribution hazard model, with death as competing event. A nomogram to predict 30- and 60-day CRE infection risk was created.ResultsA total of 840 LT recipients found to be colonized with CRE before (n = 203) or after (n = 637) LT were enrolled. CRE infection was diagnosed in 250 (29.7%) patients within 19 (interquartile range [IQR], 9–42) days after LT. Pre- and post-LT colonization, multisite post-LT colonization, prolonged mechanical ventilation, acute renal injury, and surgical reintervention were retained in the prediction model. Median 30- and 60-day predicted risk was 15% (IQR, 11–24) and 21% (IQR, 15–33), respectively. Discrimination and prediction accuracy for CRE infection was acceptable on derivation (area under the curve [AUC], 74.6; Brier index, 16.3) and bootstrapped validation dataset (AUC, 73.9; Brier index, 16.6). Decision-curve analysis suggested net benefit of model-directed intervention over default strategies (treat all, treat none) when CRE infection probability exceeded 10%. - Abstract
Statins have been suggested to exert anticancer properties by modulating angiogenesis, fibrosis, inflammation, and the tumor microenvironment, generating interest in their clinical use for chronic liver diseases (CLD) and hepatocellular carcinoma (HCC) chemoprevention. However, the effects of statin therapy in patients treated with immune checkpoint inhibitors for CLD-associated HCC remain unknown. This study primarily aimed to assess the potential effect of statins on overall survival (OS) mand progression-free survival (PFS) in patients with advanced HCC treated with atezolizumab and bevacizumab (A+B). The ARTE dataset, a retro-prospectively maintained database, includes 305 consecutive patients with unresectable HCC treated with A+B, enrolled from 12 tertiary care centers in Italy. From the original cohort, a 1:1 propensity score matching was performed to balance potential confounding factors between 63 patients on statin therapy and those who were not. The primary outcomes were OS and PFS, while secondary outcomes included all-cause mortality, liver-related death, treatment interruption, and incidence of liver decompensation events. Among the matched population of 126 patients, 75% had liver cirrhosis, with metabolic disfunction-associated steatotic liver disease (MASLD) being the most common etiology. Ninety-seven patients (32%) had diabetes. No significant differences were found between statin users and non-users for OS, PFS, or liver-related death. Additionally, the log-rank test revealed no significant difference between the groups in terms of treatment interruption due to liver decompensation events (p=0.28). Statin use did not show any impact on OS, PFS, or reduction in mortality or treatment interruption due to liver-related decompensation events in patients with advanced HCC treated with A+B. - Abstract
Atezolizumab/bevacizumab (atezo/bev) is a standard treatment for unresectable hepatocellular carcinoma (HCC). Bevacizumab may increase the risk of bleeding, causing concerns of variceal bleeding in patients with cirrhosis. Assess the predictors of esofago-gastric varices, high-risk varices (according to the Baveno criteria), and variceal bleeding in a population receiving atezo/bev for HCC. Analysis of prospectively collected data from 15 Italian centers included in the ARTE database (March 2022-June2024). Logistic regressions were run for predictors of varices amongst patients who had received an upper-gastrointestinal endoscopy (UGE) <6 months before starting treatment. Competing-risk analyses were performed to assess the predictors of variceal bleeding. Among 317 patients treated with atezo/bev included in the ARTE database, 256 had a recently performed UGE and were considered for this study. The main characteristcs of the study population were: median age 70 years, 83% male, 58% viral etiology, 88% Child-Pugh A, 52% ALBIgrade 1, 34.3% neoplastic portal vein thrombosis (nPVT), 59.9% Barcelona Clinic Liver Cancer-C stage. At treatment start, 27.3% of patients were receiving non-selective beta-blockers, and 5.8% had received a prior elastica band ligation. The prevalence of any-type and high-risk varices was 32.0 and 8.6%, respectively. Independent predictors of varices were: platelet count < 150.000/mmc (OR 3.69, 95%CI 2.06-6.61), ALBI grade >1 (OR 1.95, 95%CI 1.09-3.48), and nPVT (OR 1.78, 95%CI 1.01-3.18). High risk varices were independently associated platelet count < 150.000/mmc (OR 5.81, 95%CI 1.91-17.67) and ALBI grade >1 (OR 2.44, 95% CI 1.02-5.77). Nine patients had variceal bleeding during the follow-up (G3: n=6; G4: n=2; G5: n=1), accounting for a 3.5% 12-month cumulative incidence. Amongst patients with varices, high-risk varices were the only factor associated with bleeding (sHR 4.06, 95% CI 1.14-14.46) at the competing risk analysis. In these patients the 12-month risk was 12.7%. The risk of variceal bleeding was low, but non negligible in the subgroup of patients with high-risk varices at baseline. UGE should be performed in all patients before starting treatment: patients with platelet count < 150.000/mmc, ALBI grade > 1 or neoplastic portal vein thrombosis are at increased risk of varices. Background During the last decade carbapenem-resistant Acinetobacter baumannii (CRAB) became hyper-endemic in hospitals due to difficult to control spreading. Our aim is to identify risk factors for nosocomial rectal CRAB colonization in an endemic hospital. Methods A retrospective matched case–control study (ratio 1:2) with a prospective inclusion of cases and concurrent selection of controls was conducted from January 2017 to December 2018 in a tertiary-care hospital. Universal active surveillance for CRAB was implemented. Univariate and multivariate logistic regression was carried out using a stepwise selection method to compare prognostic factors between cases and controls. A sub-analysis was carried out according to the type of department. Results Forty-five cases with nosocomial rectal CRAB colonization and 90 controls were included. One hundred and two (75%) patients were hospitalized in medical departments. At multivariable analysis significant risk factors associated with CRAB colonization were: use of permanent devices (OR 10.15, 95% CI 2.27–45.39; P = 0.002), mechanical ventilation (OR 40.01, 95% CI 4.05–395.1; P = 0.002), urinary catheters (OR 4.9, 95% CI 1.52–16.19; P = 0.008), McCabe score (OR 5.45, 95% CI 1.87–15.89; P = 0.002), length of stay (OR 1.03, 95% CI 1.01–1.05; P = 0.002), carbapenem use (OR 5.39, 95% CI 1.14–25.44; P = 0.033). The sub-analysis showed that patients admitted to different departments had different risk factors. In geriatric department a fatal disease and a longer hospital stay represented significant risk factors both in univariate and multivariate analysis, while in internal medicine department the use of permanent devices, current antibiotic therapy and antibiotic polytherapy represented significant risk factors for CRAB at the univariate analysis, also confirmed in multivariate analysis. Conclusions Our data suggest that active surveillance for rectal CRAB colonization should be addressed to patients with an unfavourable prognosis, longer hospitalizations and carriers of multiple devices. To counter CRAB spreading in endemic settings, clinicians must limit the use of carbapenems, and reinforce interventions aimed at proper use of devices.

- Abstract
Background: Limited and wide-ranging data are available on the recurrent Clostridioides difficile infection (rCDI) incidence rate. Methods: We performed a cohort study with the aim to assess the incidence of and risk factors for rCDI. Adult patients with a first CDI, hospitalized in 15 Italian hospitals, were prospectively included and followed-up for 30 d after the end of antimicrobial treatment for their first CDI. A case-control study was performed to identify risk factors associated with 30-day onset rCDI. Results: Three hundred nine patients with a first CDI were included in the study; 32% of the CDI episodes (99/309) were severe/complicated; complete follow-up was available for 288 patients (19 died during the first CDI episode, and 2 were lost during follow-up). At the end of the study, the crude all-cause mortality rate was 10.7% (33 deaths/309 patients). Two hundred seventy-one patients completed the follow-up; rCDI occurred in 21% of patients (56/271) with an incidence rate of 72/10,000 patient-days. Logistic regression analysis identified exposure to cephalosporin as an independent risk factor associated with rCDI (RR: 1.7; 95% CI: 1.1-2.7, p = 0.03). Conclusion: Our study confirms the relevance of rCDI in terms of morbidity and mortality and provides a reliable estimation of its incidence.

The epidemiology of BSIs is changing due to the increase in multidrug resistance, and it is unclear whether the presence of multidrug-resistant (MDR) organisms, per se, is an independent risk factor for mortality. Our objectives were, first, to describe the epidemiology and outcome of BSIs and, second, to determine the risk factors associated with mortality among patients with BSI. METHODS: This research used a single-center retrospective observational study design. Patients were identified through microbiological reports. Data on medical history, clinical condition, bacteria, antimicrobial therapy, and mortality were collected. The primary outcome was crude mortality at 30 days. The relationships between mortality and demographic, clinical, and microbiological variables were analyzed by multivariate analysis. RESULTS: A total of 1049 inpatients were included. MDR bacteria were isolated in 27.83% of patients, where 2.14% corresponded to an extremely drug-resistant (XDR) isolate. The crude mortality rates at days 7, 30, and 90 were 12.11%, 25.17%, and 36.13%, respectively. Pitt score >2, lung and abdomen as site of infection, and XDR Pseudomonas aeruginosa were independent risk factors for 7-, 30-, and 90-day mortality. Charlson score >4, carbapenem-resistant Klebsiella pneumoniae, and XDR Acinetobacter baumannii were independent risk factors for 30- and 90-day mortality. Infection by XDR gram-negative bacteria, Charlson score >4, and immunosuppression were independent risk factors for mortality in patients who were stable at the time of BSI. CONCLUSIONS: BSI is an event with an extreme impact on mortality. Patients with severe clinical condition are at higher risk of death. The presence of XDR gram-negative bacteria in blood is strongly and independently associated with patient death.

International audience; Ultrasonic Computed Tomography (USCT) is an imaging technique that has proven effective for soft-tissue characterization. The classical tomography procedures are adapted to broadband data acquired in scattering configurations under Born approximation for homogeneous media and far-field conditions, while the heterogeneous objects are probed by spherical waves in near-field conditions. An elliptical Fourier transform theory is then derived to solve the near-field inverse problem in case of harmonic ellipsoidal waves. More recently, the use of USCT has been envisaged for bone imaging. In this field, the large variations of impedance distribution (high impedance contrast) require that the modelling of wave propagation be integrated into the reconstruction scheme. In that case, iterative inversion schemes are proposed. These various reconstruction procedures are validated against experiments and numerical simulations .

Item does not contain fulltext BACKGROUND: Bococizumab is a humanized monoclonal antibody that inhibits proprotein convertase subtilisin-kexin type 9 (PCSK9) and reduces levels of low-density lipoprotein (LDL) cholesterol. We sought to evaluate the efficacy of bococizumab in patients at high cardiovascular risk. METHODS: In two parallel, multinational trials with different entry criteria for LDL cholesterol levels, we randomly assigned the 27,438 patients in the combined trials to receive bococizumab (at a dose of 150 mg) subcutaneously every 2 weeks or placebo. The primary end point was nonfatal myocardial infarction, nonfatal stroke, hospitalization for unstable angina requiring urgent revascularization, or cardiovascular death; 93% of the patients were receiving statin therapy at baseline. The trials were stopped early after the sponsor elected to discontinue the development of bococizumab owing in part to the development of high rates of antidrug antibodies, as seen in data from other studies in the program. The median follow-up was 10 months. RESULTS: At 14 weeks, patients in the combined trials had a mean change from baseline in LDL cholesterol levels of -56.0% in the bococizumab group and +2.9% in the placebo group, for a between-group difference of -59.0 percentage points (P/=70 mg per deciliter [1.8 mmol per liter] and the median follow-up was 7 months), major cardiovascular events occurred in 173 patients each in the bococizumab group and the placebo group (hazard ratio, 0.99; 95% confidence interval [CI], 0.80 to 1.22; P=0.94). In the higher-risk, longer-duration trial (in which the patients had a baseline LDL cholesterol level of >/=100 mg per deciliter [2.6 mmol per liter] and the median follow-up was 12 months), major cardiovascular events occurred in 179 and 224 patients, respectively (hazard ratio, 0.79; 95% CI, 0.65 to 0.97; P=0.02).
