Russia’s influence in its Near Abroad (called “Bližkoe zarubeže” in Russian) has created a permissive zone, in which the countries within that area can ignore their democratic commitments being aware that their loyalty to Russia can protect them against any possible interference of the US and the EU. The analysis of bilateral relations between Armenia and Russia on one side, and between Russia and Belarus on the other, has led to the identification of the four models of autocracy promotion: spontaneous emulation, hard power efforts (mainly concerning military intervention), rewards (primarily regarding economic assistance) and negative sanctions (or blackmailing). A combination of rewards and punishments has proved to be the most frequent tactic used by the Kremlin, which has also been facilitated by Armenia and Belarus’s weak linkage and leverage with Western democracy promoters. Russia’s “conservative” diplomacy, which has followed a realist approach, has not resulted in a rigorous autocratic promotion policy, but the Belarusian and Armenian cases demonstrate that in these two (non-democratic) regimes (and also in those of Central Asia) the presence of Russia is stronger than in the democratic ones, such as the Baltic States. Russia’s “sanctions” against Belarus have been less than those directed against Armenia, owing mainly to the former country’s proximity to Russia in the cultural arena and the strategic geopolitical location of Belarus in Eastern Europe. Armenia, on the other hand, has also been subject to direct military intervention by Russia, which was however limited to the first Artsakh war

Purposeof Review The purpose of this review is to provide an overview of the burden of schistosomiasis in the European continent. It discusses three subjects: the endemic forms of non-human schistosomiasis in Europe; the introduction of transmission of human schistosomiasis into Europe; and the occurrence of imported cases of human schistosomiasis. Recent Findings Europe is not endemic for human schistosomiasis; nevertheless, it is affected by the disease in multiple ways, although the magnitude of the burden remains elusive because of gaps in surveillance and reporting. Summary Schistosomiasis is a global neglected disease prevalent in tropical and subtropical areas. As of 2022, it is estimated that 251 million people require preventive chemotherapy for schistosomiasis, 90% of whom live in Africa. In Europe, human schistosomiasis is frequently detected in migrants from endemic countries who reach the continent. Additionally, outbreaks due to local transmission can sporadically occur following the introduction of schistosomes in one of the many freshwater bodies in southern Europe where competent snail hosts are found. Finally, human cercarial dermatitis is frequently occurring in Europe, because of the presence of avian schistosomiasis in several countries across the continent. A stronger epidemiological surveillance and reporting system, coupled with more surveys on humans and snails, can contribute to better assess and characterize the burden of schistosomiasis in Europe.

Background and aims Although rectal cancer is predominantly a disease of older patients, current guidelines do not incorporate optimal treatment recommendations for the elderly and address only partially the associated specific challenges encountered in this population. This results in a wide variation and disparity in delivering a standard of care to this subset of patients. As the burden of rectal cancer in the elderly population continues to increase, it is crucial to assess whether current recommendations on treatment strategies for the general population can be adopted for the older adults, with the same beneficial oncological and functional outcomes. This multidisciplinary experts’ consensus aims to refine current rectal cancer-specific guidelines for the elderly population in order to help to maximize rectal cancer therapeutic strategies while minimizing adverse impacts on functional outcomes and quality of life for these patients. Methods The discussion among the steering group of clinical experts and methodologists from the societies’ expert panel involved clinicians practicing in general surgery, colorectal surgery, surgical oncology, geriatric oncology, geriatrics, gastroenterologists, radiologists, oncologists, radiation oncologists, and endoscopists. Research topics and questions were formulated, revised, and unanimously approved by all experts in two subsequent modified Delphi rounds in December 2020–January 2021. The steering committee was divided into nine teams following the main research field of members. Each conducted their literature search and drafted statements and recommendations on their research question. Literature search has been updated up to 2020 and statements and recommendations have been developed according to the GRADE methodology. A modified Delphi methodology was implemented to reach agreement among the experts on all statements and recommendations. Conclusions The 2021 SICG-SIFIPAC-SICE-WSES consensus for the multidisciplinary management of elderly patients with rectal cancer aims to provide updated evidence-based statements and recommendations on each of the following topics: epidemiology, pre-intervention strategies, diagnosis and staging, neoadjuvant chemoradiation, surgery, watch and wait strategy, adjuvant chemotherapy, synchronous liver metastases, and emergency presentation of rectal cancer.

'Zero-dose' refers to a person who does not receive a single dose of any vaccine in the routine national immunization schedule, while 'missed dose' refers to a person who does not complete the schedule. These peopleremain vulnerable to vaccine-preventable diseases, and are often already disadvantaged due to poverty, conflict, and lack of access to basic health services. Globally, more 22.7 million children are estimated to be zero- or missed-dose, of which an estimated 3.1 million (∼14 %) reside in Nigeria.We conducted a scoping review tosynthesize recent literature on risk factors and interventions for zero- and missed-dosechildren in Nigeria. Our search identified 127 papers, including research into risk factors only (n = 66); interventions only (n = 34); both risk factors and interventions (n = 18); and publications that made recommendations only (n = 9). The most frequently reported factors influencing childhood vaccine uptake were maternal factors (n = 77), particularly maternal education (n = 22) and access to ante- and perinatal care (n = 19); heterogeneity between different types of communities - including location, region, wealth, religion, population composition, and other challenges (n = 50); access to vaccination, i.e., proximity of facilities with vaccines and vaccinators (n = 37); and awareness about immunization - including safety, efficacy, importance, and schedules (n = 18).Literature assessing implementation of interventions was more scattered, and heavily skewed towards vaccination campaigns and polio eradication efforts. Major evidence gaps exist in how to deliver effective and sustainable routine childhood immunization. Overall, further work is needed to operationalise the learnings from these studies, e.g. through applying findings to Nigeria's next review of vaccination plans, and using this summary as a basis for further investigation and specific recommendations on effective interventions.

A relativistic jet has been produced in the single well-localised binary neutron star (BNS) merger detected to date in gravitational waves (GWs), and the local rates of BNS mergers and short gamma-ray bursts are of the same order of magnitude. This suggests that jet formation is not a rare outcome for BNS mergers, and we show that this intuition can be turned into a quantitative constraint: at least about $1/3$ of GW-detected BNS mergers, and at least about $1/5$ of all BNS mergers, should produce a successful jet (90\% credible level). Whether a jet is launched depends on the properties of the merger remnant and of the surrounding accretion disc, which in turn are a function of the progenitor binary masses and equation of state (EoS). The incidence of jets in the population therefore carries information about the binary component mass distribution and EoS. Under the assumption that a jet can only be produced by a black hole remnant surrounded by a non-negligible accretion disc, we show how the jet incidence can be used to place a joint constraint on the space of BNS component mass distributions and EoS. The result points to a broad mass distribution, with particularly strong support for masses in the $1.3-1.6\,\mathrm{M_\odot}$ range. The constraints on the EoS are shallow, but we show how they will tighten as the knowledge on the jet incidence improves. One of the most significant effects of an earthquake is the production of a huge amount of waste. An incorrect management of the rubbles causes relevant environmental damages, economic losses and a psychological impact for the population. In this context, the present manuscript assesses different strategies of waste management, estimating the carbon footprint and the economic impact, in order to define the best choices. Overall, different management options can be applied, that have different costs and impacts on the environment: a temporary storage site could be used, rubbles can be treated through different technologies (either simple crushing or advanced refining), and the treatment can be carried out at different distances from the site of the event. The environmental impact assessment evidenced the importance of an in-situ pre-treatment of the rubbles and of an enhanced refining, addressed at the achievement of high quality inert. On the other hand, the economic analysis suggests that the best option is to transport everything to the treatment site, and to carry out a simple treatment of the rubbles. Consequently, our assessment resulted in conflicting conclusions, where an enhanced treatment of the rubbles is positive, from an environmental point of view, but negative, for the increase in the management costs. The economic criteria are currently pushing any decision taken by the emergency managers; however, the environmental load may have a long-term effect with even more significant economic consequences, and it cannot be neglected.

We present realistic expectations for the number and properties of neutron star binary mergers to be detected as multi-messenger sources during the upcoming fourth observing run (O4) of the LIGO-Virgo-KAGRA gravitational wave (GW) detectors, with the aim of providing guidance for the optimization of observing strategies. Our predictions are based on a population synthesis model which includes the GW signal-to-noise ratio, the kilonova (KN) optical and near-infrared light curves, the relativistic jet gamma-ray burst (GRB) prompt emission peak photon flux, and the afterglow light curves in radio, optical and X-rays. Within our assumptions, the rate of GW events to be confidently detected during O4 is $7.7^{+11.9}_{-5.7}$ yr$^{-1}$ (calendar year), 78% of which will produce a KN, and a lower 52% will also produce a relativistic jet. The typical depth of current optical electromagnetic search and follow up strategies is still sufficient to detect most of the KNe in O4, but only for the first night or two. The epidemiology and the outcomes of acute appendicitis in elderly patients are very different from the younger population. Elderly patients with acute appendicitis showed higher mortality, higher perforation rate, lower diagnostic accuracy, longer delay from symptoms onset and admission, higher postoperative complication rate and higher risk of colonic and appendiceal cancer. The aim of the present work was to investigate age-related factors that could influence a different approach, compared to the 2016 WSES Jerusalem guidelines on general population, in terms of diagnosis and management of elderly patient with acute appendicitis. During the XXIX National Congress of the Italian Society of Surgical Pathophysiology (SIFIPAC) held in Cesena (Italy) in May 2019, in collaboration with the Italian Society of Geriatric Surgery (SICG), the World Society of Emergency Surgery (WSES) and the Italian Society of Emergency Medicine (SIMEU), a panel of experts participated to a Consensus Conference where eight panelists presented a number of statements, which were developed for each of the four topics about diagnosis and management of acute appendicitis in elderly patients, formulated according to the GRADE system. The statements were then voted, eventually modified and finally approved by the participants to the Consensus Conference. The current paper is reporting the definitive guidelines statements on each of the following topics: diagnosis, non-operative management, operative management and antibiotic therapy.

Large amounts of waste debris occur in urbanised areas when heavy rain on local geology generates flooding and landslides. Improved understanding of disaster waste management helps to support future strategies. This study aims to find management solutions that are environmentally and economically sustainable, hypothesizing three different options. There are many variables which influence the environmental impact and the operational cost. The distances between the areas of interest and the management site, the extent of a first manual sorting carried out by citizens, the economic load of each involved step. Overall, both the environmental and the economic analysis confirmed the usefulness of the non‐advanced option, which includes a temporary debris storage site for a preliminary shredding. On the other hand, the impact due to a possible biological treatment is not balanced by the advantage of the further volume decrease. The article shows a simple analysis schema, easily adaptable to different geographical context, is useful as supporting tool for the decision makers in flood emergency scenarios.

Summary: Background/Objective: In the late's 90' a new surgical treatment, the stapled trans-anal rectal resection (STARR) was born to treat obstructed defecation syndrome (ODS). In this study we retrospectively analyze a series of 450 cases that underwent STARR in 10 years. Methods: Between January 2001 to December 2011, 450 patients, diagnosed with ODS syndrome caused by rectocele or intussusception, underwent to STARR procedure. The presence of rectocele and/or intussusception was verified by dynamic defecography. The preoperative evaluation was completed with anorectal manometry and colonoscopy. Follow-up visits were scheduled 1 week, 1 month, 3 months, 1 years, 3 years and 5 years after surgery. Results: Mean operative time was 30,2 min. In 408 cases (90.7%) hospital discharge occurred 24 hours after surgery. Among postoperative complications urinary retention was observed in 35 patients (7.8%). Five (1.1%) patients presented an early rectal bleeding and 8 (1.8%) patients presented a late bleeding. In 5 (1.1%) patients a stable pelvic hematoma was found. Six (1.3%) patients presented pelvic sepsis due to subperitoneal perforation. An asymptomatic partial dehiscence of stapler row occurred in 19 patients (4.2%).125 patients (27.8%) reported defecation urgency that completely vanished at 3 months follow-up in 83 patients (66,4%) and in further 42 patients (33,6%) at 6-months. The average preoperative ODS score was 14.1; 3.1 at one year; 4.3 at 3 years and 6.4 after five years. Conclusions: In expert hands, with right indications, STARR procedure is safe with good results in terms of improvement of the ODS score. The geomorphology of the Italian territory causes the incidence of many disasters like earthquakes and floods, with the consequent production of large volumes of waste. The management of such huge flows, produced in a very short time, may have a high impact on the whole emergency response. Moreover, historical data related to disaster waste management are often not easily accessible; on the other hand, the availability of data concerning previous events could support the emergency managers, that have to take a decision in a very short time. In this context, the present paper analyses four relevant recent case studies in Italy, dealing with disaster waste management after geologic and hydrologic natural events. Significant differences have been observed in the quantity and types of generated wastes, and, also, in the management approach. Such differences are mainly associated with the kind of disaster (i.e. earthquake vs. flood), to the geographical location (i.e. internal vs. coastal area), to the urbanisation level (i.e. industrial vs. urban). The study allowed the identification of both strengths and weaknesses of the applied waste management strategies, that represent “lessons to learn” for future scenarios. Even though it deals with Italian case studies, this manuscript may have a high impact also at international level, making available for the first-time emergency waste management data, that are considered an indispensable support for decision makers.

Old World cutaneous leishmaniasis (CL) is one of the most prevalent vector-borne diseases within the World Health Organization's Eastern Mediterranean Region. The conflict in the Syrian Arab Republic generated large population movements and raised concerns about spreading of CL to countries where Syrians have relocated, including Jordan.A review of electronic and paper-based registries of CL cases in Jordan was conducted to assess burden of disease and associated socio-demographic factors.Increasing numbers of CL cases have been reported in Jordan between 2010 and 2016 (from 140 to 281), paralleled by significant increases in incidence rate (from 2.09 per 100 000 person-years, 95% CI (1.77-2.47), to 2.87 per 100 000 person-year, 95% CI (2.55-3.22), p = 0.002) and in the proportion of cases who are Syrian (from 8.6%-55.2%, p 0.001). Syrian refugees have higher odds of presenting with leishmaniasis than Jordanian nationals (adjusted OR 7.1, 95% CI (6.3-8.0), p = 0.000).Presence of large numbers of Syrian refugees within Jordan has so far not contributed to increased risk of developing CL for Jordanians, however surveillance, diagnosis and case management for CL should be reinforced to meet the increased burden. Electronic surveillance can help identify priority populations and areas for interventions.

In line with WHO recommendations on elimination of schistosomiasis, accurate identification of all areas of residual transmission is a key step to design and implement measures aimed at interrupting transmission in low-endemic settings. To this purpose, we assessed the prevalence of active S. mansoni infection in five pilot governorates in the Nile Delta of Egypt by examining schoolchildren (6-15 years) using the Urine-Circulating Cathodic Antigen (Urine-CCA) cassette test; we also carried out the standard Kato-Katz (KK) thick smear, the monitoring and evaluation tool employed by Egypt's national schistosomiasis control programme. BackgroundCutaneous leishmaniasis (CL) has historically been reported from Syria. Since 2011, the country has been affected by a war, which has impacted health and health services. Over the same period, an increase in the number of cases of CL has been reported from several areas across the country and by a number of authors. This study aims to provide the first quantitative evidence of the epidemiological evolution of CL in Syria during the war.Materials and methodsData on number of CL cases for the period 2011-2018 were extracted from three different surveillance systems: the Ministry of Health (MoH) routine surveillance system, the MoH/WHO sentinel-syndromic Early Warning Alert and Response System (EWARS), and surveillance data collected by the international nongovernmental organization (NGO) the MENTOR Initiative. Data were cleaned and merged to generate the best possible estimates on number of CL cases; incidence of CL was also calculated based on data on resident population. Data reported from the years preceding the conflict (2007-2010) were also added to the analysis for comparative purposes.ResultsThe analysis of data from the three available sources over the period considered indicates that number of reported cases progressively grew from prewar levels to reach a peak in 2015, decreased in 2016, remained stable in 2017, and increased again in 2018. Such a trend was mirrored by changes in incidence of infection. Some governorates, which used to report low numbers of CL cases, started recording higher number of cases after the onset of the war.ConclusionThe war coincided with a major rise in reported number of CL cases and incidence of infection, although an increasing trend was already appreciable before its onset.

Background The World Health Organization has recently reemphasized the importance of providing preventive chemotherapy to women of reproductive age in countries endemic for soil-transmitted helminthiasis as they are at heightened risk of associated morbidity. The Demographic and Health Surveys (DHS) Program is responsible for collecting and disseminating accurate, nationally representative data on health and population in developing countries. Our study aims to estimate the number of pregnant women at risk of soil-transmitted helminthiasis that self-reported deworming by antenatal services in endemic countries that conducted Demographic and Health Surveys. Methodology/Principal findings The number of pregnant women living in endemic countries was extrapolated from the United Nations World Population Prospects 2015. National deworming coverage among pregnant women were extracted from Demographic and Health Surveys and applied to total numbers of pregnant women in the country. Sub-national DHS with data on self-reported deworming were available from 49 of the 102 endemic countries. In some regions more than 73% of STH endemic countries had a DHS. The DHS report an average deworming coverage of 23% (CI 19–28), ranging from 2% (CI 1–3) to 35% (CI 29–40) in the different regions, meaning more than 16 million pregnant women were dewormed in countries surveyed by DHS. The deworming rates amongst the 43 million pregnant women in STH endemic countries not surveyed by DHS remains unknown. Conclusions/Significance These estimates will serve to establish baseline numbers of deworming coverage among pregnant women, monitor progress, and urge endemic countries to continue working toward reducing the burden of soil-transmitted helminthiasis. The DHS program should be extended to STH-endemic countries currently not covering the topic of deworming during pregnancy., Author summary Soil-transmitted helminths are intestinal worms that cause significant suffering among the poorest communities in the world. They are transmitted via contaminated water, food or soil, all of which result from poor sanitation. Children and women of reproductive age are at heightened risk of related morbidities such as malnutrition, cognitive impairment and anaemia. Pregnant women are particularly susceptible to severe maternal and neonatal complications. Deworming drugs are cheap, safe, and effective in reducing morbidity related to soil-transmitted helminthiasis. Large scale drug administration campaigns have distributed donated medicines to children in endemic countries, but women of reproductive age are currently not well covered. Yet, demographic surveys show that they are being treated for soil-transmitted helminthiasis through health care services. This study provides estimates for the number of pregnant women at risk of soil-transmitted helminthiasis being dewormed by antenatal services in endemic countries conducting Demographic Health Surveys. These estimates mark the preliminary reference point for deworming coverage among pregnant women in endemic countries, and will thus prove useful for tracking overall progress in the ongoing effort to eliminate neglected tropical diseases.

Summary: Background: Fistula-in-ano is one of the most commonly presenting anorectal diseases. Sphincter sparing treatment options should be considered in patients with complex fistulas. Salvecoll-E gel is a native collagen deantigenated and purified, non-cross-linked equine dermal extract, with an amino acid composition identical to human collagen. Methods: The multicentric trial study was a prospective, single-arm observational clinical study with the objective to assess the efficacy of Salvecoll-E gel for anal fistula repair in 70 patients. All patients had undergone preliminary surgical treatment consisting of positioning of a draining loosing seton that was maintained for a period of 4–6 weeks. After seton removal, a gentle debridement and washing of the fistula track was performed. The scar tissue was removed from the internal orifice. Internal opening was covered by a side-to side mucosal suture. Salvecoll-E was injected through the external opening into the fistula track, the external opening it has been opened. Results: Twelve months after surgery, 55 patients demonstrated a clinically healed fistula (78,5%), 15 patients have a recurrence (21,5%). Most of the recurrences were observed in the first 6 months of treatment (13/15, 86.6%). We don't observe any worsening in CCF score. The results obtained at 1 year certainly seem satisfactory and in line with the best results published in literature using mini-invasive techniques. Conclusion: Salvecoll-E gel is a promising non-invasive technique for conservative treatment of anal fistulas, it's well tolerated by the patients and, in case of recurrence, reinjection or all other known techniques are feasible. Since the 1970s, in the USA, we witnessed a progressive increase of one-day surgical procedures. This attitude soon gained ground in Europe as well. In proctology, this kind of clinical approach has always been limited by the acute sensitivity of the anal- perineal area and by difficulties in attaining a complete sphincter relaxation with local anesthesia. Posterior perineal block seems to be associated with both a good pain control and an effective sphincter relaxation.Between January 2017 and January 2018, we enrolled in our study 33 patients suffering from hemorrhoidal disease. They were all subjected to posterior perineal block. We measured anal resting pressure and squeeze pressure before and after anesthesia. Measurements where taken 5 minutes before and 15 minutes after the administration of local.We registered an average decrease of 39,2% of resting pressure and of 45,4% of squeeze pressure.We may state that perineal posterior block, while reducing striated muscle contractile activity, also causes a relevant reduction of anal basal tone. During surgical procedures done under regional anesthesia, we experienced a good sphincter relaxation, which was comparable, if not equal, to that induced by general anesthesia. In fact, 10 to 15 minutes after performing the block you could observe the elevation of the inferior margin of the exterior sphincter and the concomitant descent of the inferior margin of the internal sphincter (coaxial dislocation).Anorectal manometry, Anesthesia, Local-regional, Perineal block.A partire dagli anni 70 negli Stati Uniti si è assistito ad un progressivo aumento delle procedure chirurgiche in day surgery Questo processo si è diffuso anche nella maggior parte delle strutture sanitarie in Europa. In ambito proctologico le indicazioni sono sempre state limitate dalla particolare sensibilità della zona ano-perineale e da un difficile completo rilassamento dell’apparato sfinteriale dopo anestesia locale. Il blocco perineale posteriore sembra essere associato oltre che ad una buona analgesia ad un efficace rilassamento sfinterico.Da gennaio 2017 a gennaio 2018 sono stati arruolati 33 pazienti affetti da patologia emorroidaria. In tutti i pazienti è stato eseguito un blocco perineale posteriore. Sono state misurate la pressione basale del canale anale e la contrazione massima volontaria prima e dopo l’anestesia. Le due rilevazioni manometriche sono avvenute in posizione litotomica 5 minuti prima e 15 minuti dopo l’esecuzione dell’anestesia.La diminuzione media del tono basale dopo blocco perineale posteriore è stata pari al 39,2% e la riduzione media della contrazione massima è stata del 45,4%.Possiamo affermare che il blocco perineale posteriore determina oltre alla riduzione dell’attività contattile della muscolatura striata, anche un rilevante decremento del tono anale di base. Durante interventi proctologici eseguiti in anestesia locoregionale con il blocco perineale posteriore si è riscontrato notevole rilasciamento della parete sfinteriale dell’ano paragonabile, anche se non uguale, a quello che si verifica durante un’anestesia generale. Infatti dopo circa 10-15 minuti nella quasi totalità dei casi si osserva sia la risalita del margine inferiore dello sfintere esterno che la concomitante discesa del margine inferiore dello sfintere interno (dislocazione coassiale).

Background Gallstone disease is very common afflicting 20 million people in the USA. In Europe, the overall incidence of gallstone disease is 18.8% in women and 9.5% in men. The frequency of gallstones related disease increases by age. The elderly population is increasing worldwide. Aim The present guidelines aims to report the results of the World Society of Emergency Surgery (WSES) and Italian Surgical Society for Elderly (SICG) consensus conference on acute calcolous cholecystitis (ACC) focused on elderly population. Material and methods The 2016 WSES guidelines on ACC were used as baseline; six questions have been used to investigate the particularities in elderly population; the answers have been developed in terms of differences compared to the general population and to statements of the 2016 WSES Guidelines. The Consensus Conference discusses, voted, and modified the statements. International experts contributed in the elaboration of final statements and evaluation of the level of scientific evidences. Results The quality of the studies available decreases when we approach ACC in elderly. Same admission laparoscopic cholecystectomy should be suggested for elderly people with ACC; frailty scores as well as clinical and surgical risk scores could be adopted but no general consensus exist. The role of cholecystostomy is uncertain. Discussion and conclusions The evaluation of pro and cons for surgery or for alternative treatments in elderly suffering of ACC is more complex than in young people; also, the oldest old age is not a contraindication for surgery; however, a larger use of frailty and surgical risk scores could contribute to reach the best clinical judgment by the surgeon. The present guidelines offer the opportunity to share with the scientific community a baseline for future researches and discussion.

The prevalence and intensity of Schistosoma haematobium infection was determined among schoolchildren living in five governorates in Upper Egypt. Between November 2016 and March 2017, urine samples were collected from 30,083 schoolchildren (6-16 years of age) from the governorates of Assiut (n = 7496; 6 districts), Bani Sweif (n = 4493; 7 districts), Fayoum (n = 4597; 6 districts), Menia (n = 7500; 9 districts) and Sohag (n = 5997; 11 districts). All samples were processed using urine filtration to detect and quantify S. haematobium eggs. The overall prevalence was 1.3% (95% Confidence Interval (CI) = 1.1%, 1.4%), but the prevalence varied considerably across districts in the studied governorates (from 0%, Fayoum to 13.4%, Sohag). The prevalence of heavy-intensity infections (≥50 egg/10 ml) varied from 0.05% (95% CI = 0.01-0.1) in Sohag to 0.3% (95% CI = 0.1-0.4) in Menia. No subject with heavy intensity of infection was detected in Fayoum and Bani Sweif governorates. Of the 39 studied districts 97.4% had prevalence of heavy intensity infection of1%, indicating elimination of schistosomiasis haematobia as a public health problem in these districts. Of those studied 72.0% were male. Males were 2.9 times as likely to be infected (1.5% [95% CI: 1.4-1.7]) as females (0.5% [95% CI: 0.3-0.7]); χ2 = 51.2, p 0.0001. Heavy intensity of infection was detected only in males. The prevalence of S. haematobium infection increased steadily with age, and the age group15 years was 7 times as likely to be infected as the younger age group (6-9; 0.8%); χ2 = 44.9, p 0.0001. The national schistosomiasis control programme (NSCP) adopted a new elimination strategy by readjusting thresholds for MDA using praziquantel and targeting all transmission areas. The NSCP, after this major achievement of elimination of schistosomiasis haematobia as a public health problem, is now moving to interruption of its transmission.

Summary: Background: We propose a new open mesh hernia repair procedure for the treatment of inguinal hernias in adults aiming to improve patients' comfort and to reduce the incidence of chronic neuralgia. Methods: From September 2012 to August 2015, 250 consecutive patients were treated with “all in-one” mesh hernioplasty procedure in our Institution. According to the devised technique, a new smaller prosthesis was placed on the floor of the inguinal canal in order to strengthen all areas of weakness from which hernias may originate. The mesh was enveloped by a fibro-cremasteric sheath avoiding contact with neural structures. Follow-up was carried out at 3, 6, 12, 18 and 24 months for evaluation of postoperative pain using Visual Analogue Scale score, need of medication, patients' comfort and short or long-term complications. Results: All patients were discharged within 24 h from surgery. Slight pain was reported by the majority of patients and 47.6% of them did not require pain medication at home. After the 1st postoperative week 96.8% reported no pain and no other symptoms. No relevant limitation of normal activities was reported. There has been no postoperative neuralgia. One recurrence was observed. Conclusions: This new hernioplasty technique respects the anatomy of the inguinal canal, uses a smaller mesh, and seems to avoid neuralgia with maximum comfort for the patients. Intestinal nematodes infections are parasitic diseases caused by helminths (worms) living in the human intestine. Major species involved include: Ascaris lumbricoides (roundworm), Trichuris trichiura (whipworm), Ancylostoma duodenale and Necator americanus (hookworms), Strongyloides stercoralis (threadworm), and Enterobius vermicularis (pinworm). Intestinal nematode infections are among the neglected tropical diseases and are possibly the most widespread and common infections worldwide, constituting a significant public health problem and an obstacle to the social and economic development of affected populations. The article provides the reader with up to date information on their biology; distribution; transmission; epidemiology; pathogenesis, morbidity and burden of disease; diagnosis; therapy; public health control; and global goals set to reduce their health impact.

Recently, WHO has developed a predictive model to evaluate the impact of preventive chemotherapy programs to control the morbidity of soil-transmitted helminths (STHs). To make predictions, this model needs baseline information about the proportion of infections classified as low, moderate and high intensity, for each of the three STH species. However, epidemiological data available are often limited to prevalence estimates.We reanalyzed available data from 19 surveys in 10 countries and parameterized the relationship between prevalence of STH infections and the proportion of moderate and heavy intensity infections.The equations derived allow feeding the WHO model with estimates of the proportion of the different classes of infection intensity when only prevalence data is available.The prediction capacities of the STH model using the equations developed in the present study, should be tested by comparing it with the changes on STH epidemiological data observed in control programs operating for several years.

Infection with soil-transmitted helminths (STH) is a major public health problem in many developing countries, with pregnant women and children particularly at risk. Preventive chemotherapy, which is the intervention currently recommended by the WHO against the main helminth infections including those caused by STHs, aims at reducing morbidity through periodical administration of anthelminthic drugs either alone or in combination. The Expanded Programme on Immunization is one of the most widely implemented health programmes in the world and has well established access to children and women. The present study investigated the cost of the provision of anthelminthic drugs during existing immunisation campaigns. In Lao PDR, use of this integrated approach compared with implementation of the vertical deworming campaign alone allowed a reduction of the individual cost of deworming by 10 times (from US$0.23 in the vertical deworming campaign to US$0.03 in the integrated campaign). When drug cost was excluded, the cost of deworming an individual was US$0.007, implying that deworming 100 children would cost less than US$1 if drug donation was in place. The burden posed on health workers by the integration process was perceived as minimal and manageable. Moreover, delivery of anthelminthic drugs during immunisation campaigns enabled campaign teams to observe drug intake directly, which assured safety. These findings prove that integration is an opportunity to maximise health benefits through the delivery of multiple health products and the attainment of high coverage.

The authors conducted a systematic literature review with the following aims: (i) to investigate how frequently soil-transmitted helminthiasis (STH) infections are endemic where schistosomiasis is present; and (ii) to assess the correlation between the risk level of schistosomiasis and that of STH. Among 155 sites on which data were collected and analyzed, schistosomiasis was present in 130 sites, all of which were also co-endemic for STH, whereas 25 sites were endemic only for STH. Out of 83 sites where at least one biannual round of preventive chemotherapy (PC) for schistosomiasis is recommended, 94% were also eligible for at least a yearly round of PC against STH. And among 21 sites where PC for schistosomiasis is recommended once a year, 81% were also eligible for at least a yearly round of PC for STH. This fact provides managers of control programmes with the operationally important indication that use of available information on endemicity of schistosomiasis is a valid tool to predict the presence of STH in the same geographical area as well as to estimate the need of PC for STH. The implementation of this tool is expected to save financial and human resources and help accelerate the scale-up of PC throughout the world.

SUMMARYSchistosomiasis remains one of the most prevalent parasitic diseases in developing countries. After malaria, schistosomiasis is the most important tropical disease in terms of human morbidity with significant economic and public health consequences. Although schistosomiasis has recently attracted increased focus and funding for control, it has been estimated that less than 20% of the funding needed to control the disease in Africa is currently available. In this article the following issues are discussed: the rationale, development and objectives of the Schistosomiasis Control Initiative (SCI)-supported programmes; the management approaches followed to achieve implementation by each country; mapping, monitoring and evaluation activities with quantifiable impact of control programmes; monitoring for any potential drug resistance; and finally exit strategies within each country. The results have demonstrated that morbidity due to schistosomiasis has been reduced by the control programmes. While challenges remain, the case for the control of schistosomiasis has been strengthened by research by SCI teams and the principle that a national programme using ‘preventive chemotherapy’ can be successfully implemented in sub-Saharan Africa, whenever the resources are available. SCI and partners are now actively striving to raise further funds to expand the coverage of integrated control of neglected tropical diseases (NTDs) in sub-Saharan Africa.

We aimed to map the probability of Schistosoma haematobium infection being >50%, a threshold for annual mass praziquantel distribution. Parasitologic surveys were conducted in Burkina Faso, Mali, and Niger, 2004-2006, and predictions were made by using Bayesian geostatistical models. Clusters with >50% probability of having >50% prevalence were delineated in each country.
