Abstract

BACKGROUND AND PURPOSE: Currently, human papillomavirus (HPV) positivity represents a strong prognostic factor for both reduced risk of relapse and improved survival in patients with oropharyngeal squamous cell carcinoma (OPSCC). However, a subset of HPV-positive OPSCC patients still experience poor outcomes. Furthermore, HPV-negative OPSCC patients, who have an even higher risk of relapse, are still lacking suitable prognostic biomarkers for clinical outcome. Here, we evaluated the prognostic value of LINE-1 methylation level in OPSCC patients and further addressed the relationship between LINE-1 methylation status and p53 protein expression as well as genome-wide/gene-specific DNA methylation. RESULTS: In this study, DNA was extracted from 163 formalin-fixed paraffin-embedded tissue samples retrospectively collected from stage III-IVB OPSCC patients managed with curative intent with up-front treatment. Quantitative methylation-specific PCR revealed that LINE-1 hypomethylation was directly associated with poor prognosis (5-year overall survival-OS: 28.1% for LINE-1 methylation < 35% vs. 69.1% for ≥ 55%; p < 0.0001). When LINE-1 methylation was dichotomized as < 55% versus ≥ 55%, interaction with HPV16 emerged: compared with hypermethylated HPV16-positive patients, subjects with hypomethylated HPV16-negative OPSCC reported an adjusted higher risk of death (HR 4.83, 95% CI 2.24-10.38) and progression (HR 4.54, 95% CI 2.18-9.48). Tumor protein p53 (TP53) gene is often mutated and overexpressed in HPV-negative OPSCC. Since p53 has been reported to repress LINE-1 promoter, we then analyzed the association between p53 protein expression and LINE-1 methylation levels. Abstract

Deep learning models based on medical images play an increasingly important role for cancer outcome prediction. The standard approach involves usage of convolutional neural networks (CNNs) to automatically extract relevant features from the patient’s image and perform a binary classification of the occurrence of a given clinical endpoint. In this work, a 2D-CNN and a 3D-CNN for the binary classification of distant metastasis (DM) occurrence in head and neck cancer patients were extended to perform time-to-event analysis. The newly built CNNs incorporate censoring information and output DM-free probability curves as a function of time for every patient. In total, 1037 patients were used to build and assess the performance of the time-to-event model. Training and validation was based on 294 patients also used in a previous benchmark classification study while for testing 743 patients from three independent cohorts were used. The best network could reproduce the good results from 3-fold cross validation [Harrell’s concordance indices (HCIs) of 0.78, 0.74 and 0.80] in two out of three testing cohorts (HCIs of 0.88, 0.67 and 0.77). Additionally, the capability of the models for patient stratification into high and low-risk groups was investigated, the CNNs being able to significantly stratify all three testing cohorts. Results suggest that image-based deep learning models show good reliability for DM time-to-event analysis and could be used for treatment personalisation.

Abstract

Geopolymers have been recently explored as sorbents for wastewater treatment, thanks to their mechanical and chemical stability and to their low-energy manufacturing process. One specific application could be the removal of ammonium (NH₄+) through exchange with Na+ ions. Additive manufacturing (AM) represents an especially interesting option for fabrication, as it allows to tailor the size, distribution, shape, and interconnectivity of pores, and therefore the access to charge-bearing sites. The present study provides a proof of concept for NH₄+ removal from wastewater using porous geopolymer components fabricated via direct ink writing (DIW) AM approach. A metakaolin-based ink was employed for the fabrication of a log-pile structure with 45° rotation between layers, producing continuous yet tortuous macropores which are responsible for the high permeability of the sorbents. The ink consolidates in an amorphous, mesoporous network, with the mesopores acting as preferential sites for ion exchange. The printed sorbents were characterized for their physicochemical and mechanical properties and the NH₄+ removal capacity in continuous-flow column experiments by using a model effluent. The lattices present high permeability and high cation exchange capacity and maintained a high amount of active ions after four cycles, allowing to reuse them multiple times.

Abstract

In this work, we compared the main characteristics of highly porous geopolymer components for water treatment applications manufactured by 3D printing, direct foaming, or granulation. Furthermore, different approaches to impregnate the materials with Ag or Cu were evaluated to obtain filters with disinfecting or catalytic properties. The results revealed that all of the investigated manufacturing methods enabled the fabrication of components that possessed mesoporosity, suitable mechanical strength, and water permeability, even though their morphologies were completely different. Total porosity and compressive strength values were 28 vol% and 16 MPa for 3D-printed, 70–79 vol% and 1 MPa for direct-foamed, and 27 vol% and 10 MPa for granule samples. Both the filter preparation and the metal impregnation method affected the amount, oxidation state, and stability of Ag and Cu in the filters. However, it was possible to prepare filters with low metal leaching between a pH of 3–7, so that the released Ag and Cu concentrations were within drinking water standards.

Abstract

BACKGROUND: Inclusion of new biomarkers to improve a personalized treatment approach for oropharyngeal squamous cell carcinoma (OPSCC) is urgently needed. Hypomethylation of the Long interspersed nucleotide element-1 (LINE-1) repetitive elements, a widely accepted surrogate of overall genomic DNA methylation content, was found to be associated with a poor prognosis in several cancers. At present, no studies have investigated the influence of LINE-1 methylation levels on OPSCC relapse. The main goal of this study was the evaluation of the prognostic value of LINE-1 methylation status in predicting early tumor relapse in locally advanced OPSCC. METHODS: We retrospectively reviewed a cohort of 77 patients with stage III-IVB OPSCC. Methylation of LINE-1 repetitive sequences was evaluated by real-time quantitative methylation-specific PCR in formalin-fixed paraffin-embedded tissues. The prognostic relevance of LINE-1 methylation was assessed by comparing patients who relapsed within 2 years from the end of treatment (cases) with those who did not (controls). Results were validated in an independent cohort of 33 patients with OPSCC. RESULTS: With respect to early OPSCC relapse, the mean LINE-1 methylation level was significantly lower in relapsed cases than in control group (p < 0.01). Interestingly, LINE-1 methylation was lower in relapsed cases than in controls in both HPV16-negative and HPV16-positive OPSCC patients, even if statistical significance was reached only for the former group (p = 0.01). LINE-1 methylation levels were also significantly reduced in relapsed cases with respect to the controls in OPSCC current smokers (p = 0.02). Consistently, in HPV16-negative current smokers, OPSCC relapse was significantly associated with decreased levels of LINE-1 methylation (p = 0.02). Abstract

Carcinoma, or cancer, of the vestibule (entrance) of the nose is very infrequently found and is often diagnosed incorrectly or late, when it is already advanced. The tumor often grows so that most of it is hidden beneath the skin surface and only a small portion of it is visible. It usually appears as a crusty lesion that is initially treated with ointments; this delays reaching the correct diagnosis and beginning treatment. While the usual form of treatment is radiotherapy, this report shows the value of treatment with chemotherapeutic agents administered through an artery prior to radiotherapy. The authors report on 12 patients treated during the years 1975 to 1987 who had biopsy-proven cases of cancer of the nasal vestibule. The group consisted of 11 men and 1 woman. The average age of the patients was 65 years, with a range from 50 to 82 years. Ten of the patients were smokers and five normally drank at least 1 liter of wine daily. All patients had a painful ulcer at the site of the tumor, which was their principle symptom. Most small tumors were treated by simple resection (surgical removal) while those which were more advanced were treated by chemotherapy followed by radiotherapy. The chemotherapeutic agent was delivered by a catheter placed in the artery which served the area of the tumor. The drug was slowly delivered over a treatment period of several day in several cycles. It was concluded that the treatment approach used appears effective at reaching the best possible rate of cure with the maximum of function retained.
