Background: Smoking and unhealthy diet are important risk factors for cardiovascular and metabolic diseases, contributing to public health crises., Objective: To evaluate the consumption of natural/minimally processed and ultra-processed foods by Brazilian adults (18-59 years old) according to smoking status., Design and Setting: Cross-sectional study of a representative population sample from 26 state capitals and the Federal District (Brazil-2018)., Methods: Data were obtained from Vigitel-Surveillance System for Risk and Protection Factors for Chronic Diseases by Telephone Survey. Participants were categorized as smokers, ex-smokers, and never smokers. Multinomial logistic regression was used for analyses., Results: Of the 30,800 adults evaluated, 9.4% (95%CI: 8.7-10.2) were smokers and 16.5% (95%CI: 15.8-17.3) were ex-smokers. Smokers were less likely to consume fruit and natural juice, and more likely to consume soda or artificial juice (≥ 5 days/week) than ex-smokers and never smokers. Regarding the daily frequency of consumption, smokers were observed to be less likely to eat fruit more than 1 time/day and more likely to drink ≥ 3 cups/cans of soda/day. Compared to never smokers, smokers had a 42% higher chance of consuming ≥ 3 glasses of natural juice/day. On the day before the interview, fruit, milk, tubers, squash, and okra consumption were lower among smokers than non-smokers. Smokers were more likely to report consuming soft drinks, fruit juice, sauces, ready-made dishes, margarine, and sausages., Conclusion: Smokers had lower fruit consumption, and higher consumption of natural juices and ultra-processed foods. We highlight the need for strategies that encourage healthy eating and smoking cessation.

The scope of this article was to evaluate the influence of multimorbidity and associated effects on the activities in the day-to-day lives of community-dwelling elderly individuals. It involved a cohort study with data from the FIBRA Study, the baseline (2008-2009) and follow-up (2016-2017). The basic activities in daily living (ADL) were evaluated using Katz's index, and the chronic diseases were classified as: (1) multimorbidity and multimorbidity patterns; (2) cardiopulmonary; (3) vascular-metabolic; and (4) mental-musculoskeletal. The chi-square test and Poisson regression data were used for analysis. A total of 861 older adults with no functional dependency at baseline were analyzed. Elderly individuals with multimorbidity (RR = 1.58; 95%CI: 1.19-2.10) and classified according to cardiopulmonary (RR = 2.43; 95%CI: 1.77-3.33), vascular-metabolic (RR = 1.50; 95%CI: 1.19-1.89) and mental-musculoskeletal (RR = 1.30; 95%CI: 1.03-1.65) had a higher risk of presenting functional decline in ADL in the follow-up compared to those who didn't have the same disease patterns. Multimorbidity and its patterns increased the risk of functional disability in older adults over the nine-year period.

Information about the prevalence of specific chronic diseases and the ownership of a health plan can help size and monitor care demands. This study aimed to estimate and compare the prevalence of chronic diseases among the elderly, according to the possession of a health plan in 2013 and 2019. This is a population-based cross-sectional study with data from elderly people (age ≥ 60 years) from the Brazilian National Health Survey (2013: n = 11,177; 2019: n = 22,728). The prevalence of self-reported chronic diseases and adjusted prevalence ratios were estimated, according to health plan ownership (medical and/or dental) and by year. In the period, increased prevalence was observed for hypertension (PR = 1.11; 95%CI: 1.06-1.16), diabetes (PR = 1.12; 95%CI: 1.01-1.24), heart disease (PR = 1.21; 95%CI: 1.05-1.39), stroke (PR = 1.27; 95%CI: 1.04-1.54), back pain (PR = 1.14; 95%CI: 1.05-1.23), hypercholesterolemia (PR = 1.09; 95%CI: 1.01-1.18), and depression (PR = 1.23; 95%CI: 1.05-1.43) among those without a health plan. In 2019, arthritis/rheumatism (PR = 1.21; 95%CI: 1,03-1,43), hypercholesterolemia (PR = 1.13; 95%CI: 1.01-1.26), and depression (PR = 1.26; 95%CI: 1.03-1.53) increased among elderly patients with a health plan. The findings showed differences in the prevalence of chronic diseases according to health plan ownership and an increase for some diseases in the period. Health promotion policies with an emphasis on reducing modifiable risk factors need to be maintained and intensified. Particularly for the elderly population, the importance of expanding actions focused on case tracking and early diagnosis, prevention and control of complications that favor equity in care is highlighted.

This article aims to estimate the prevalence of chronic diseases/conditions in octogenarians according to sex, age groups and private health insurance, and its relationship with difficulty in performing usual activities. Cross-sectional population-based study with elderly data (n = 6,098) from the National Health Survey (PNS) 2019. Prevalences and 95% confidence intervals were estimated. The prevalences were: hypertension 61.7%, chronic back problem 30.0%, hypercholesterolemia 22.0%, diabetes 20.3%; arthritis/rheumatism 19.4%, heart disease 19.3%, depression 9.4%, cancer 8.9%, cerebrovascular accident (CA) 7.5%, asthma 4.9%, lung disease (LD) 4.2% and kidney failure (KF) 3.0%. Hypertension, chronic back problem, hypercholesterolemia, major arthritis/rheumatism and depression in women, and cancer in men. Major heart disease, hypercholesterolemia, arthritis/rheumatism, KF, cancer and depression in those with private health insurance. Restriction of usual activities 14.8%, more frequent in cardiac patients, with chronic back problem, arthritis/rheumatism, KF, depression, CA, cancer and LD. There were higher prevalences in women and in those who have health insurance. Difficulties in usual activities related to diseases demand the expansion of care for the older adults.

Background: A healthy diet is a protection factor against type 2 diabetes and plays an important role in the treatment of the disease, as well as associated comorbidities., Objective: Characterize the eating habits of older adults (≥ 65 years) with and without diabetes residing in capital cities and the Federal District of Brazil., Methods: A cross-sectional study was conducted using data from the Surveillance of Risk and Protection Factors for Chronic Diseases Through a Telephone Survey (Vigitel, 2016). The prevalence of diabetes mellitus was estimated according to sociodemographic variables, physical inactivity level, self-rated health status and body mass index. Dietary habits were assessed based on the frequency (weekly and daily) of consumption of healthy and unhealthy foods and the replacement of food by snacks. Differences were determined using Pearson's chi-square test (Rao-Scott), with the significance level set at 5%., Results: A total of 13,649 older adults were interviewed. The prevalence of self-reported diabetes was 27.2% (95% CI: 25.5; 29.0). Compared to non-diabetics, diabetic individuals had a higher consumption of raw vegetables (32.1% vs. 26.5%/3-4 days/week) and lower consumption of chicken (3.8% vs. 6.4%/hardly ever/never), fruit juice (24.0% vs. 29.6%) and sweets (6.8% vs. 16.2%) ≥ 5 days/week. Compared to non-diabetics, diabetic individuals consumed more skim milk (51.5% vs. 44.6%) and diet soda (60.0% vs. 17.3%) ≥ 5 days/week, raw vegetables (9.1% vs. 2.5%/at dinner) and sweets (37.7% vs. 20.5%/twice/day) 3-4 days/week., Conclusion: The observed differences emphasize the need for healthy eating interventions for all older adults, as well as specific counseling for those with diabetes.

Objective: To estimate the prevalence of multimorbidity in long-lived Brazilian individuals (age ≥80 years) and to associated it with the use of health services., Methods: Cross-sectional population-based study with data from the 2019 National Survey of Health (n=6,098). Frequencies of use of services were estimated for older people with multimorbidity and according to sex, health insurance ownership, and self-rated health. The prevalence rates, crude and adjusted prevalence ratios, and the respective 95% confidence intervals were calculated., Results: The average age of the older adults was 85 years and about 62% were women; the prevalence of multimorbidity was 57.1%, higher in women, in those who have health insurance, and who reside in the southern region of the country (p<0.05). In the oldest old with multimorbidity, the use of services in the last 15 days reached 64.6%, and more than 70% were hospitalized in the last year or did not carry out activities in the previous two weeks for health reasons. Differences were observed for the indicators of service use in relation to sex, health insurance ownership, and self-rated health, according to multimorbidity., Conclusion: Indicators for the use of health services were higher in older individuals who have two or more chronic diseases, regardless of sociodemographic conditions and self-rated health, showing the impact of multimorbidity per se in determining the use of services among the oldest old.

This article aims to estimate the risk of death according to sociodemographic characteristics, chronic diseases, frailty, functional capacity, and social participation in older people as well as determine the median time of death in relation to health status and social participation. A retrospective longitudinal study was conducted with older people (≥65 years) in 2008-09 and 2016-17 in the city of Campinas and the subdistrict of Ermelino Matarazzo in the city of São Paulo. Face-to-face interviews were conducted at community centers and the participants' homes. The cumulative incidence of death was estimated and associations with the predictor variables were analyzed using Poisson multiple regression. The Kaplan-Meier method and the log-rank test were also used. Among the 741 individuals located at follow-up, 192 had deceased. The incidence of death was higher among those who reported having heart disease and those who were dependent on others regarding the performance of instrumental activities of daily living. The incidence of death was lower among women, individuals in the highest income stratum, and those who performed three or more activities related to social inclusion. No differences in median survival times were found. Predictors of mortality can contribute to broadening knowledge on the singularities of the aging process.

This study aimed to estimate the prevalence of non-vaccination and the reasons for nonadherence to the influenza vaccine among older Brazilians according to sociodemographic characteristics. A cross-sectional study was conducted with data from older people (≥ 60 years of age; n = 23,815) who participated in the 2013 National Health Survey. Frequencies of non-vaccination and the main reasons for nonadherence were calculated with respective 95% confidence intervals. The prevalence of non-vaccination was 26.9% (approximately 7,106,730 older people). The reason rarely gets the flu was the most cited among the men (28.2%), the 60-to-69-year-old age group (29.6%), individuals with higher education (41.9%), and those with health insurance (32.3%). Fear of a reaction was the most cited reason in the northeastern region (25.4%), among women (29.3%), longer-lived individuals (≥70 years; 28.7%), and those who did not know how to read/write (26.7%). A total of 12.1% reported not believing in the vaccine's protection, and 5.5% did not know that it was necessary to take vaccine. The proportions of the main reasons for non-vaccination varied by sociodemographic characteristics. This study's findings highlight the need to increase older people's knowledge regarding influenza and influenza vaccines. Healthcare providers should be encouraged to counsel older people-especially those in subgroups with lower adherence, such as residents in the Northeast region, those aged 60-69 years, those who do not know how to read/write, those without a spouse/companion, and those without health insurance-regarding the different aspects of the vaccine and formally indicate it for groups at risk., Competing Interests: The authors have declared that no competing interests exist.

The aims of the present study were to estimate the frequency of change in self-rated health (SRH) among community-dwelling older adults, between two measures taken at a 9-year interval; and determine factors associated with a decline and an improvement in SRH, in relation to aspects of physical/emotional health and subjective wellbeing. Data were derived from a community-based study on frailty among Brazilian elderly. Associations were investigated using Pearson's chi-square test and relative risk ratios were estimated using multinomial logistic regression analysis. 39.3% of participants did not change their SRH at both assessment times, 21.7% rated it as worse and 39.0% rated it as better. The relative risk ratio of an improvement in SRH for individuals with disability in basic activities of daily living (ADLs) was lower than for individuals with independence in basic ADLs (IRR=0.22; IC95%: 0.08-0.63). Understanding the complex interactions between self-rated health and the dimensions that influence the improvement of health perception may shed light on key determinants of the wellbeing among older adults., (© 2021. This study aimed to assess characteristics of healthcare networks in four large Brazilian cities (Campinas, Fortaleza, Porto Alegre, and São Paulo), in the provision of mental healthcare. The following outcomes were used: (i) place of identification of the mental health problem; (ii) mental healthcare in primary care; (iii) pharmaceutical care in mental health; and (iv) social rehabilitation. This is a mixed-methods study with a concurrent and sequential approach, conducted with 10 administrators and 1,642 users of Centers for Psychosocial Care (CAPS, in Portuguese) in the four cities. The study showed the persistence of high-complexity services such as hospitals as the site for initial identification of mental health problems in Campinas (40% of users) and Fortaleza (37%); low proportion of mental health treatment in primary care (Fortaleza, 23%); differences between cities in psychotropic medication prescription in primary care (Porto Alegre, 68%; São Paulo, 64%; Campinas, 39%; Fortaleza, 31%) and in shortages of prescribed medication (higher in Fortaleza, 58%; lower in Campinas, 28%); and overall frailty in enabling return to work (lower in São Paulo, 17%; higher in Campinas, 39%), with better overall results regarding religion and leisure activities (higher in São Paulo, 53% and 56%, respectively). The study contributes to the discussion of the Brazilian scenario of mental healthcare, with evidence of persistent inequalities in the national context, pointing to gaps in some mental healthcare network configurations with the potential for better performance and longitudinal follow-up.

Objective: To report the main results of studies on prejudice, stereotyping, and age-based discrimination (ageism) in the context of the COVID-19 pandemic., Methods: This is an integrative review of the literature on ageism in the context of the COVID-19 pandemic, conducted between May and June 2020, with data collected from the following databases: Medical Literature Analysis and Retrieval System Online (MEDLINE/PubMed), Web of Science (Thompson Reuters), Scopus (Elsevier Science), Literatura Latino-Americana e do Caribe em Ciências da Saúde (Lilacs) and Scientific Eletronic Library Online (SciELO)., Results: Twenty-one publications addressing ageism during the pandemics, its origins, consequences, and ethical and political implications were analyzed. All publications were theoretical with a critical/reflexive approach, being 90,5% opinion articles (n = 19) and 9,5% research (n = 2). The main findings indicate criticisms regarding resources allocation and intensive care based exclusively on age. The results also highlight the impacts of social isolation, the use of technologies and social media, and intergenerational relationships within the COVID-19 scenario., Conclusion: According to most publications, although ageism has always been present, it became more evident during the COVID-19 pandemic as a form of discrimination against older adults. "Ageist" discourses may exert a negative influence in older adults' lives, causing severe social and psychological impacts.

Objective: To estimate the prevalence of use of oral medications for the treatment of diabetes, as well as the distribution of sources for obtaining according to sociodemographic variables, in the Brazilian states' capitals and in the Federal District, and their evolution from 2012 to 2018., Methods: Cross-sectional and population-based study with individuals aged ≥ 20 years who reported a medical diagnosis of diabetes, interviewed through Vigitel from 2012 to 2018. We estimated the prevalence of use and the distribution of sources for obtaining according to sociodemographic variables (95%CI). We checked differences among proportions using the Pearson's χ2 test (Rao-Scott), with a significance level of 5%., Results: There was an increase in the prevalence of use of oral medications for the treatment of diabetes from 77.4 to 85.2% between 2012 and 2018, and a decrease in obtaining in the Health Unit Pharmacies of the Unified Health System (SUS), while there was an increase in obtaining in Popular Pharmacies., Conclusions: In Brazil, SUS remained the main source for obtaining oral antidiabetic drugs, financing more than 70% of them in the country, considering the Health Unit Pharmacies and Popular Pharmacies, thereby showing the importance of public Pharmaceutical Policies in guaranteeing the access to medications by the Brazilian population, as well as in reducing inequities in the country. Nevertheless, the migration of obtaining by users from SUS Health Units to Popular Pharmacies suggests the weakening the responsibility of Primary Health Care in the provision oral antidiabetic drugs, thereby undermining the bond and the longitudinal care.

The aim of the present study was to estimate the prevalence of psychotropic drug utilization among adults and seniors and determine associated factors, therapeutic classes and sources of acquisition. Data from the Brazilian National Survey on Access, Use and Promotion of the Rational Use of Medicines (2013-2014) were analyzed. The prevalence of use of at least one psychotropic drug in the overall sample (adults and seniors) was 8.7%. In the adjusted analyses, positive associations were found between the use of psychotropic drugs and the female sex, poorer self-rated health and chronic diseases (p < 0.05). The most frequently used therapeutic classes were antidepressants (55.3%) by the adults and anxiolytics (59.3%) by the seniors. Approximately 23.0% of psychotropic drugs were obtained exclusively from pharmacies of the public healthcare system and 77.0% were acquired from other sources. The findings reveal a low proportion of attaining psychotropic drugs through the Brazilian public healthcare system as well as the need for public policies that encourage the rational use of prescriptions and treatments to promote a better quality of life and ensure the population's right to health.

The aim of this study was to estimate the prevalence of non-vaccination against influenza among Brazilian older adults with systemic arterial hypertension and determine the main reasons for non-adherence. A cross-sectional study was conducted using data from older adults (≥ 60 years of age) with hypertension who participated in the 2013 National Health Survey and reported not having been vaccinated against flu over the previous 12 months (n = 1,295). The analyses were performed using the Stata 14.0 software. The data were weighted because of the sampling design. An estimated 3,026,080 older adults with hypertension had not received a flu vaccine over the 12 months prior to the survey (22.6%). No significant associations were found with sex, age group or schooling. The prevalence of unvaccinated older adults was lower in the southern and southeastern regions of Brazil than in the northern and northeastern regions, even after adjusting for age. The prevalence was higher among individuals without private health insurance. The main reasons for non-vaccination were fear of a reaction, rarely having the flu and not believing in the protection of the vaccine. The present findings underscore the need for healthcare professionals to explain to the population the benefits of the vaccine for preventing severe influenza (protective effect and possible reactions) and for secondary prevention of cardiovascular events. Increasing the prevalence of vaccination in older adults with hypertension and other cardiovascular diseases is of fundamental importance within the realm of public health as a strategy for reducing occurrences of complications and deaths associated with influenza.

In view of the epidemiological relevance of periodontal disease and chronic noncommunicable diseases, the study aimed to evaluate the relationship between them through subclinical indicators of systemic risk in a population group with healthy habits, including alcohol and tobacco abstinence. A complete periodontal examination of six sites per tooth was performed in a sample of 420 participants from the Advento study (Sao Paulo), submitted to anthropometric and laboratory evaluation. Periodontitis was defined and classified based on the Community Periodontal Index score 3 (periodontal pocket = 4-5 mm) and score 4 (periodontal pocket ≥ 6 mm). The prevalence of mild/moderate and severe periodontitis was 20% and 8.2%, respectively. Both categories of periodontal disease had significantly higher levels of triglycerides, C-reactive protein, calcium score, and calcium percentile, whereas blood glucose after tolerance test was significantly higher among people with severe periodontitis and HDL-c levels were lower (p < 0.05). Young adults with severe periodontitis had significantly higher prevalence of obesity, pre-diabetes, hypertension, and metabolic syndrome. Besides these conditions, the older adults with severe periodontitis had significantly higher prevalence of dyslipidemia and subclinical atherosclerosis. The group with periodontitis had also a higher coronary heart disease risk based on the PROCAM score (p < 0.05). The results indicated associations of periodontitis with several systemic indicators for chronic noncommunicable diseases, and highlighted the need for multiprofessional measures in the whole care of patients.

Objective: The prevalence of hypertension in Brazil and worldwide has been increasing in recent decades, and drug therapy is one of the strategies used to control this condition. The objective of this study was to estimate the prevalence of use and identify the sources for obtaining antihypertensive drugs in Brazil, according to sociodemographic variables, comparing three periods: 2011, 2014 and 2017., Methods: Data from individuals aged ≥20 years who reported a medical diagnosis of hypertension, interviewed by Vigitel in 2011, 2014 and 2017 were used. Frequency and prevalence of drug use in addition to the sources for obtaining medication were estimated by sociodemographic variables, with 95% confidence intervals. The differences between proportions were verified by Pearson's chi-square test (Rao-Scott), with a significance level of 5%., Results: The prevalence of antihypertensive drug use remained stable (80%). Regarding the sources for obtaining these medicines, there was variation in the period, indicating a decrease in usage through the Brazilian Unified Health System (SUS) (44.2% in 2011; 30.5% in 2017). This decrease was accompanied with increase in PFPB (16.1% in 2011; 29.9% in 2017). The prevalence of other sources for obtaining medicine (private pharmacies/drugstores) showed stability in the period., Conclusions: The prevalence of medication use remained high and there was a change in the pattern of use according to sources, demonstrating migration between SUS pharmacies to the PFPB, and suggesting a reduction in the availability of medicines from public pharmacies universally, and for free.

Objective To estimate the prevalence of use of drugs to treat gastrointestinal disorders, according to demographic, socioeconomic, and health characteristics of the Brazilian population. Methods This is a population-based survey that interviewed individuals residing in cities of the five regions in Brazil. The study sample was composed of 32,348 individuals aged 20 or more years. The profile of use of drugs for gastrointestinal disorders was evaluated considering the variables sex, age, healthcare plan, region, and number of chronic diseases. We also analyzed the frequency of individuals who declared using other drugs, besides those already employed for treatment of gastrointestinal disorders. Additionally, the estimated frequencies of the drug classes used were determined. Results The prevalence of use of drugs for gastrointestinal disorders in Brazil was 6.9% (95% confidence interval - 6.4-7.6), higher in females, among persons aged over 60 years, in those who had a private healthcare insurance, and presented with two or more chronic diseases. It was noted that 42.9% of the aged who used drugs for gastrointestinal disorders were also on polypharmacy. As to the classes of drugs, 82% corresponded to drugs for the food tract and metabolism, particularly proton pumps inhibitors. Conclusion The use of drugs for treatment of gastrointestinal disorders was significant among women and elderly. In this age group, consumption may be linked to gastric protection due to polypharmacy. This study is an unprecedented opportunity to observe the self-reported consumption profile of these drugs in Brazil and, therefore, could subsidize strategies to promote their rational use.

Objective to identify cut-off points for waist circumference (WC) and waist-to-height ratio (WHtR) with better sensitivity, specificity and accuracy to distinguish overweight elderly people by sex, using three body mass index criteria as anthropometric references. Methods this was a cross-sectional study that analyzed weight, height and WC measurements of 3,477 elderly people (2008-2009); receiver operating characteristic (ROC) curves were used to define the cut-off points. Results WC cut-off points were 86.5cm, 88.7cm and 91.5cm in women, and 93.0cm, 96.0cm and 97.5cm in men, according to World Health Organization/WHO, Nutrition Screening Initiative/NSI and Pan American Health Organization/PAHO criteria, respectively; WHtR cut-off points corresponded to 0.57, 0.58, 0.59 in the female sex, and 0.56, 0.58 and 0.59 in the male sex; highest absolute overall accuracy values: PAHO (men) and WHO (women). Conclusion WC and WHtR cut-off points were similar, regardless of the BMI criterion adopted.

Background: Smoking and an inadequate diet are behavioral risk factors that contribute to the majority of deaths and disabilities caused by noncommunicable diseases., Objectives: To estimate the prevalence of the co-occurrence of smoking and inadequate diet and identify associated factors in adults., Methods: A cross-sectional population-based study was conducted with a sample of 28,950 Brazilian adults (18 to 59 years old). Data were obtained from Sistema de Vigilância por Inquérito Telefônico (Vigitel [Brazilian Health Surveillance Telephone Survey]) in 2014. Independent associations were investigated using Poisson hierarchical regression analysis with 5% significance level., Results: The prevalence of the co-occurrence of smoking and unhealthy eating was 8.6% (95% CI: 7.9-9.3) and was higher among individuals residing in the southern region of the country than in those living in the central western region (PR = 1.50; 95% CI: 1.18-1.89), those with no private health insurance (PR = 1.14; 95% CI: 1.03-1.25), those who drank alcohol abusively (binge drinkers) (PR = 3.22; 95% CI: 2.70-3.85) and those who self-rated their health as fair (PR = 1.65; 95% CI: 1.36-1.99) or poor/very poor (PR = 1.70; 95% CI: 1.18-2.44). The prevalence of both factors was lower among individuals residing in the northeastern region of the country, women, individuals with brown skin color, those with a spouse, the more educated ones and those with overweight or obesity., Conclusion: The more vulnerable segments to the co-occurrence of the risk factors studied were men residing in the southern region of the country, individuals with a lower socioeconomic status and those who reported binge drinking. Interventions addressing multiple behavioral risk factors adapted to specific contexts could have a greater impact on the Brazilian population.

Objective: To estimate the co-occurrence of the major risk factors for chronic diseases in adults (18-59 years old) and older people (≥ 60 years old) living in Brazilian state capitals and the Federal District., Methods: Cross-sectional study with population-based data from 35,448 adults and 18,726 older people collected in the Sistema de Vigilância de Fatores de Risco e Proteção para Doenças Crônicas por Inquérito Telefônico (System of Surveillance of Risk and Protective Factors for Chronic Diseases by Telephone Survey - Vigitel) in 2015. The prevalence of each of the five risk factors (smoking, overweight, physical inactivity, alcohol and unhealthy diet) was estimated, as well as their co-occurrence for the different possible combinations, according to socioeconomic and health self-assessment variables. The independent associations were verified via multinomial logistic regression to obtain the estimates of the odds ratio (OR) and corresponding 95% confidence intervals., Results: At least two risk factors were present in 38.5% of the adults and 37.0% of the older participants. The male adults and older participants who did not have private health insurance and classified their health as average or poor/very poor were more likely to have two or more concurrent risk behaviors. The greater chance of co-occurrence of smoking and alcohol abuse in adults (adjusted OR = 3.52) and older people (adjusted OR = 2.94) stands out., Conclusions: The subgroups with increased risk of developing multiple unhealthy behaviors and the most prevalent behaviors were identified. These findings are expected to contribute to the better targeting of health promotion and preventive care. It is worth noting that, for the adoption of healthy lifestyle habits, macro-social and inter-sectoral policies are more effective.

The aim of the present study was to analyze the mortality trend due to ischemic heart disease (IHD) among older adults, identify changes in the trend and determine the correlation with influenza vaccine coverage (2000 to 2012) in the state of São Paulo between 1980 and 2012. An ecological time series study was conducted involving secondary data from Brazilian information systems. Linear and polynomial regression models as well as joinpoint regression were used to estimate the trends. Pearson's correlation coefficient was used to evaluate the correlation between age-standardized mortality coefficients and vaccine coverage. A decreasing tendency in mortality due to IHD occurred in both sexes, higher mortality rates were found for males and greater reductions were found in the period after the vaccination campaigns. However, no statistically significant changes occurred in the year coinciding with or near the onset of the campaigns. In the overall sample, no evidence of a linear correlation was found between the mortality coefficients and vaccination coverage. Other factors directly associated with morbidity and mortality due to ischemic heart disease may have influenced the trend.

The aim of the present study was to evaluate knowledge, attitudes and practices regarding tuberculosis (TB) among transvestites and transsexual women. A cross-sectional study was conducted with a convenience sample (n = 124; 58 transvestites and 66 transsexuals) in the city of São Paulo, Brazil, in 2014 and involved the administration of the Knowledge, Attitudes and Practices questionnaire. Absolute and relative frequencies were calculated for all variables and the comparisons of percentage distributions between groups were performed using Pearson's Chi-square test, Fisher's exact test or its generalization, with a 5% significance level. Most participants were young and non-white. Transvestites had lower levels of schooling, reported more passages through the prison system and declared themselves to be sex workers more frequently. Little more than half of the participants were aware that treatment for TB was free of charge. Knowledge on TB was modest, permeated with misunderstandings regarding signs/symptoms, transmission and prevention, which influence attitudes and practices in relation to the disease. The findings demonstrate that health education actions do not achieve their goals in the control of tuberculosis.

The aim of this study was to estimate the prevalence of underweight among older adults according to socio-demographic characteristics in different regions of Brazil as well as determine associated contextual and individual factors. Cross-sectional population-based study with older adults (≥ 65 years) interviewed by telephone survey in 2014. The body mass index was calculated based on weight and height. Associations were determined using Pearson's chi-square test, considering a 5% significance level. Adjusted prevalence ratios were estimated using multilevel Poisson regression. Mean age was 73.3 years and the prevalence of underweight was 15.6% (95%CI: 14.1-17.1%). Higher prevalence rates of underweight were found among women, individuals aged ≥ 80 years, smokers and those who reported the regular consumption of beans. The prevalence rate of underweight was lower among those who reported abusive alcohol intake and those with a medical diagnosis of hypertension. The northern region of the country had the highest prevalence of underweight after adjusting for associated individual factors. The findings demonstrate the subgroups with higher prevalence rates of underweight that demand greater attention from the health services in terms of recovering of an adequate nutritional status.

This study aimed to estimate the prevalence of influenza vaccination in elderly Brazilians with specific chronic diseases. The sample included individuals 60 years or older (n = 23,815) participating in the National Health Survey (PNS) in 2013. The study estimated the prevalence rates for influenza vaccination and the respective 95% confidence intervals (95%CI). The associations were verified with the chi-square test (Rao-Scott) with 5% significance. Prevalence of influenza vaccination in the elderly was 73.1% (95%CI: 72.0-74.1), and there was no significant difference in prevalence rates between men and women (p = 0.237). Statistically significant differences were observed in prevalence rates for elderly that reported arterial hypertension, 75% (p < 0.001), diabetes mellitus, 76.5% (p = 0.009), cardiac disease, 79.2% (p < 0.001), and lung disease or chronic obstructive pulmonary disease (COPD), 87% (p = 0.001). The results showed low prevalence of vaccination in elderly with some specific chronic diseases for whom there is formal recommendation to receive the vaccine, suggesting the need for a vaccination campaign to extrapolate the age cutoff (elderly) and better targeting to these specific subgroups.

Objective: to analyze the distribution of leptospirosis according to sociodemographic, epidemiologic, and clinical characteristics, assistance time, rainfall and spatial distribution in Campinas/SP in the period from 2007 to 2014., Method: This is an ecological study using information from the confirmed cases. A descriptive analysis was carried out according to the selected variables. The relationship between the cases and rainfall was verified through the Spearman's correlation coefficient. The cases/deaths were georeferenced per areas classified according to socioenvironmental deprivation indexes in Campinas., Results: From the 264 cases, 76.1% occurred in men, 58,4% in the 20 to 49 years old age range. Approximately 55% were hospitalized, with a prevalence of clinical-laboratorial diagnoses of 89.4%, and the lethality was 10.6%. The urban area concentrated 74.2% of the cases, and 48,9% occurred in the household environment. The main risk factor was evidence of the presence of rodents at the site. There was strong correlation between the incidence of cases and rainfall (p < 0.05) in 2010 to 2012. A higher concentration of the cases was observed in the Center-Southwest region. Spatial distribution of cases/deaths showed clusters in regions classified at lower socioeconomic levels., Conclusions: The patterns of occurrence of leptospirosis in Campinas showed similarity with other Brazilian municipalities. The identification of areas with higher incidence of cases/deaths contributes to the adoption of strategies for intervention and prioritization of resources aiming at the reduction of the risk of infection and at early treatment for those affected.

This study describes the trend of the coefficients of mortality due to cerebrovascular diseases (CbVD) among the elderly in São Paulo State, Brazil, from 1980 to 2012, before and after influenza vaccination campaigns, and identifies change points. It is an ecological, time-series study carried out with death data from the Health Ministry's Mortality Information System and population data from the Brazilian Institute of Geography and Statistics. We used linear, polynomial and joinpoint regression models in the data analysis. Between 1980 and 2012, there were 480,955 deaths due to CbVD. The mean mortality coefficients decreased for both sexes in all age groups we analyzed, with the greatest reduction in the older ages and male sex. We observed a significant reduction in the mortality trend in 1998 for male sex in the age group 60-69 years (annual percent change - APC = -3%, 95%CI: -4.3; -1.6) and for all elderly (APC = -3.8%, 95%CI: -4.4; -3.1). Considering the period as a whole, we did not observe change points for the age group 70-79 (average annual percent change - AAPC = -3.3%, 95%CI: -3.5; -3.1) and, for male sex, for the group ≥ 80 years (AAPC = -2.9%, 95%CI: -3.1; -2.6). For all elderly, the mean percentage reduction was of 3.1% per year (AAPC = -3.1%, 95%CI: -3.5; -2.7). Results show a reduction in the mortality due to CbVD in the period, with different percentage variations in coefficient reduction. The study's findings add information to the debate regarding possible effects of vaccination campaigns in reducing mortality due to CbVD among the elderly.

Mental disorders are present in four of the ten main causes of incapacity across the world. This article aims to analyze the prevalence of Common Mental Disorders (CMD) in incarcerated women and associated factors. A cross-sectional study was conducted with a population of 1,013 women incarcerated in a female prison. The prevalence of CMD was assessed by the SRQ-20. The hierarchical logistic regression was the method of analysis used to search for independent associations between sociodemographic, lifestyle, morbidity and violence variables with CMD and strength of association. The prevalence of CMD was 66.7%. The following variables were independently and positively associated with CMD: lack of income, hypertension, tranquilizers, physical inactivity, smoking, scabies/pediculosis, psychological violence in the year before being arrested, and having witnessed psychological violence in the family in childhood/adolescence. Interdisciplinary activities among health, justice and education institutions can contribute to a qualified assessment of women before admission into the prison system. This can enable an approach that does not exacerbate or trigger the onset of CMD, contributing to the improvement in living conditions and for better health and recovery strategies.

The simultaneous occurrence of specific diseases in older adults is explored little in the literature. The aim of this study was to estimate the prevalence of concomitant hypertension and diabetes in Brazilian older adults and determine associated contextual and individual factors. A population-based cross-sectional study was conducted with 10,991 older adults (≥ 60 years of age) interviewed during the 2012 Vigitel Survey. Statistical analysis involved multi-level Poisson regression. Mean age was 69.4. The prevalence of the concomitant occurrence of hypertension and diabetes was 16.2%, with differences found among the capital cities. Higher prevalence rates were found in cities located in the southern, southeastern and central-western regions of the country as well as among black and brown older adults, those with up to eight years of schooling, non-smokers, ex-smokers and those with excess weight. The effect of geographic region was demonstrated by the 23.5% increase in the prevalence of the concomitant diseases after adjusting for individual variables. The present findings enabled gauging the effect of context (region of residence) on the prevalence of the main diseases related to an increased mortality rate in the older population.

Objectives: To assess the consumption of ultra-processed foods among inmates in a women's prison in the State of São Paulo, Brazil., Methods: We conducted a cross-sectional study in 2012/2013 with 1,013 female inmates. A structured interview questionnaire was used to collect socio-demographic, self-reported morbidity and healthy lifestyles and health-related behaviors data. The inmates' usual diet was assessed with a food frequency questionnaire. Foods were classified as either natural or minimally processed, processed or ultra-processed., Results: Inmates age ranged from 18 to 65 years, 51.7% were black, 80.3% had children, 69.5% smoked, 47% were overweight/obese and half of them had high blood triglycerides. The prevalence of daily consumption of natural or minimally processed foods (rice, beans and cassava flour) was 87.7%. The prevalence of daily consumption of green leafy and other vegetables was 63.7%. Almost two-thirds reported consuming milk and fried chicken frequently. A high prevalence of daily consumption of ultra-processed foods was observed in this study. Hot dog bread and sweet bread with margarine were consumed by 86.5% of the interviewees on a daily basis; sugar sweetened beverages by 68.4%, and biscuits as well as candies by 77.1%., Conclusions: The women's diet in the prison is of low quality, and may lead to metabolic alterations, obesity and other comorbidities. It is recommended that the prison's Food Evaluation Committee considers improvements to the quality of the prisoners' diets.

The aim of this study was to assess the prevalence of being underweight among the elderly according to demographic and socioeconomic characteristics, health-related behaviors, diseases and health status. This was a cross-sectional multi-center study with 3,478 community-dwelling elders (≥ 65 years). The dependent variable was the prevalence of being underweight, classified by Body Mass Index < 22kg/m2. Adjusted prevalence rate ratios were estimated using multivariable Poisson regression. The mean age was 72.9 years and 12.0% of the elderly were underweight (CI95%:10.9-13.1). Aged elderly ≥ 80 years, former and current smokers, those who reported appetite loss and those classified as pre-frail or frail (PR=1.41; CI95%:1.09-1.82) presented a higher prevalence of being underweight. Individuals who received medical diagnosis of hypertension, diabetes and rheumatism had the lowest underweight prevalence observed. The results highlight the importance of nutritional status assessment and monitoring among the elderly, with emphasis on the most vulnerable subgroups, particularly the frail elderly, taking into account the health consequences of low weight.

The objective was to estimate the prevalence of influenza vaccination in adults and elderly with chronic respiratory diseases (CRDs). The sample included individuals 20 to 59 years of age (n = 23,329) and ≥ 60 years (n = 9,019) that participated in the Brazilian National Survey on Access, Utilization, and Promotion of Rational Use of Medicines (PNAUM) in 2013-2014. We estimated influenza vaccination prevalence rates and respective 95% confidence intervals (95%CI). Associations were verified with the Rao-Scott chi-square test, with significance set at 5%. For all adults and elderly with CRD, prevalence of vaccination was 42.1% (95%CI: 37.2-47.1), with a statistically difference between the two age groups (p < 0.001). The adult population showed significant differences according to marital status (p < 0.05), and the elderly population showed higher vaccination prevalence in the South of Brazil compared to the Northeast (p < 0.05). For the sample as a whole, chronic bronchitis was the most frequently reported disease (43.5%). Considering each specific disease, the percentage of vaccinated adults was low, varying from 25% (other respiratory diseases) to 42% (chronic bronchitis), without a statistically significant difference (p = 0.330). Individuals with respiratory diseases and the elderly in general (≥ 60 years) are priority groups for influenza vaccination; in general, in all the subgroups, prevalence rates were below the target set by the Brazilian Ministry of Health. The recommendation that the vaccine should be applied by a health professional may explain this low adherence to vaccination by the elderly.

Objective: to estimate the prevalence of use of psychotropic medicines and associated factors in adults and elderly, and to identify the main classes used., Methods: cross-sectional population-based study, conducted in Campinas-SP, Brazil (January/2008-April/2009); chi-square Rao-Scott test and Poisson multiple regression were applied., Results: the study included 2,472 individuals and the prevalence of use of at least one psychotropic medication in the three previous days was 6.8% (95%CI 5.5;8.1); in the adjusted analyses, there were positive associations between use and female sex (PR=1.48; 95%CI 1.01;2.18), worse health perception (PR=2.10; 95%CI 1.13;3.89), common mental disorder (PR=1.66; 95%CI 1.09;2.51) and emotional problems (PR=8.04; 95%CI 4.87;13.02); black/brown-skinned individuals presented significantly lower use (PR=0.58; 95%CI 0.39;0.88); antidepressants (52.6%), anxiolytics (28.1%) and antipsychotics (17.0%) were the most used., Conclusion: inequalities regarding sex and ethnicity/skin color were observed; the findings contribute to the evaluation of the rational use of these drugs.

This study shows a descriptive data analysis related to the procurement of medicines for primary care in Brazilian municipalities, as recorded in the National database of Pharmaceutical Service Actions and Services for the period July 2013 - June 2014, by geographic region and population size. Nine hundred and sixty municipalities were analyzed, of which 27% invested monetary value equal to or greater than the minimum statutory recommendations and 43% of these are located in the Southeast. The North region has the highest number of municipalities with less investment. Municipalities with a population over 500,000 inhabitants used, on average, lower resources to provide more items to users. The average number of items purchased was 86 and represents 25% of the National List of Essential Medicines (Rename); 64% had a decentralized resource management and the most commonly used procurement method was "tender". The most purchased drugs are in line with the most prevalent primary care diseases. Findings showed that most municipalities invests below statutory recommendations and are located mainly in the North, Northeast and Midwest. It was not possible to establish a trend between population and amount invested per capita/year.

Objective: To analyze the temporal trends in pneumonia mortality rates (standardized by age, using the 2010 population of Brazil as the standard) in all Brazilian geographical regions between 1996 and 2012., Methods: This was an ecological time-series study examining secondary data from the Mortality Database maintained by the Information Technology Department of the Brazilian Unified Health Care System. Polynomial and joinpoint regression models, and corresponding 95% CIs, were used for trend analysis., Results: The pneumonia mortality rates in the South, Southeast, and Central-West showed a decreasing behavior until 2000, followed by increases, whereas, in the North and Northeast, they showed increasing trends virtually throughout the period studied. There was variation in annual percent change in pneumonia mortality rates in all regions except the North. The Central-West had the greatest decrease in annual percent change between 1996 and 2000, followed by an increase of the same magnitude until 2005. The 80 years and over age group was the one most influencing the trend behavior of pneumonia mortality rates in all regions., Conclusions: In general, pneumonia mortality trends reversed, with an important increase occurring in the years after 2000.

Objective: The objective of this study is to identify factors associated with mortality, with emphasis on gender and age differences., Methods: This is a cross-sectional study, which uses data from the FIBRA-2008-2009 network in Campinas, State of São Paulo, Brazil, with information on non-institutionalized residents of the urban area and the Mortality Information System. The dependent variable has been death, in 2013. The associations have been tested by odds ratio (OR) and their 95% confidence intervals, and the analyses have been conducted using the program Stata 12.0., Results: Average age has been 72.3 years, 69.3% have been women, and 8.9% have died. We have found greater OR for mortality in individuals aged ≥ 75 years, classified as pre-frail or frail, and in those who have reported heart disease., Conclusions: In this study, the analysis of specific subgroups has allowed us to better understand the relationship between the factors associated with death in the elderly. With the exception of age, strategies based on primary and secondary care, focused on priority groups, can have a positive impact on the reduction of mortality among the elderly., Objetivo: Identificar fatores associados à mortalidade, com ênfase nas diferenças de gênero e idade., Métodos: Trata-se de estudo transversal, utilizando dados provenientes da rede FIBRA-2008-2009 em Campinas, SP, com informações de pessoas não institucionalizadas residentes na área urbana e pelo Sistema de Informações de Mortalidade. A variável dependente foi a ocorrência de óbito, verificada em 2013. As associações foram testadas pelas razões de chances (OR) e respectivos intervalos de confiança de 95%, e as análises conduzidas no programa Stata 12.0., Resultados: A média de idade foi 72,3 anos, 69,3% eram mulheres e 8,9% foram a óbito. Encontrou-se maior OR para mortalidade nos indivíduos com idade ≥ 75 anos, nos classificados como pré-frágil ou frágil e naqueles que referiram doença do coração., Conclusões: Neste estudo, a análise para os subgrupos específicos permitiu melhor compreender a relação entre fatores que se associam ao óbito no idoso. Com exceção da idade, estratégias baseadas no cuidado específico de atenção primária e secundária, direcionadas a grupos prioritários, podem ter um impacto positivo na redução da mortalidade entre os idosos.

Objective: To characterize the profile of transvestites and transgender women participating in a survey regarding tuberculosis and HIV/Aids in the city of São Paulo., Method: This cross-sectional study employed the Knowledge, Attitudes and Practices (KAP) questionnaire adapted for data collection. Sociodemographic aspects as well as aspects related to history of imprisonment, process of gender transition, use of condoms, previous treatment of sexually transmitted diseases and tuberculosis and knowledge regarding HIV/Aids were addressed., Results: The study included 58 transvestites and 66 transgender women. There were differences between the two groups regarding schooling (P = 0.008), prostitution (P < 0.001), imprisonment (P < 0.001), silicone injections (P = 0.005), and hormone therapy without medical guidance (P = 0.004). All trans women (100%) and 80% of transvestites mentioned treatment of syphilis; 25.9% of transvestites reported tuberculosis/HIV co-infection. The investigation regarding knowledge of HIV/AIDS revealed inconsistent use of condoms and mistaken beliefs related to modes of transmission., Conclusion: The present results show that transvestites and trans women were vulnerable to tuberculosis and HIV/AIDS. The differences observed between the two groups suggest the need for different approaches to transvestites and transgender women in future studies.
