Abstract

Background: One of the cutaneous reactions associated with tattoos is sarcoidosis and the most frequent presentation is systemic involvement; within this, the manifestation of uveitis and sarcoidal reactions associated with tattoos is little described. Clinical case: A 27-year-old man with a clinical picture of six months of evolution consisting of general malaise, subjective fever, chills, myalgias, arthralgias, cough with hemoptysis, edema in lower limbs, and in whom red eye, diminution of visual acuity, and papules that infiltrated one of the tattoos that were placed, six months earlier, on the right arm and left forearm, were found. Conclusion: Sarcoidosis associated with tattooing is a disease that requires a high index of suspicion, where its infiltration should make the clinician suspect the disease and make an active search for systemic involvement, especially in a patient with a uveal compromise that can lead to blindness. It must keep in mind that, although rare, this triple association is possible (tattoo, systemic sarcoidosis, and panuveitis).

Abstract

Objective: To evaluate the epidemiological, clinical and therapeutic characteristics of pregnant women with systemic lupus erythematosus, as well as their obstetric results, in two reference centers in northwestern Colombia. Methods: Retrospective case series that evaluated the pregnant women seen in the Maternal-Fetal Medicine and Rheumatology Units between 2010 and 2015. Results: 40 patients and 43 pregnancies were included. The average age at diagnosis: 24 ± 7 years; 22 (55 %) were primigravidae. The most common clinical affectation was articular (n = 32, 80 %). Eight of 43 pregnancies (18.6 %) presented lupus activity at the beginning of gestation. The most frequent obstetric complication was preeclampsia in 11/43 (25.6 %) pregnancies. The most common fetal complication was preterm labor (8/43, 18.6 %). Anticardiolipin and anti-DNA antibodies were positive in 10/43 (23.3%) and in 15/38 (39.5 %) pregnancies, respectively. In 13 gestations (31.7 %), there were relapses during the third trimester, and of these, in six cases (46.1 %) there were severe especially nephritis. On admission, prednisone was used in 29/43 (65 %) pregnancies and antimalarial drugs in 32/43 (74.4 %). In the puerperium, two patients had a moderate relapse and two mothers had preeclampsia. No maternal deaths were found and perinatal survival was 86% (37/43 pregnancies). Discussion: In this cohort of patients with systemic lupus erythematosus and pregnancy, nephritis and antiphospholipid antibodies were present in a large part of the patients with obstetric complications, although a low percentage of patients were active before gestation; maternal and fetal complications were frequent.

Abstract

Sarcoidosis is a systemic granulomatous disease of unknown etiology. It affects patients from all the latitudes and ages, being more frequent between the third and the fourth decades of life and with a second peak around the sixth decade of age in the Japanese and Scandinavian population. It’s more common in women, and it has a more severe course in Afro-descendant population. The antigens that start the granulomatous response are unknown, but presumed that be airborne antigens, because of the high frequency of pulmonary involvement. Its clinical presentation is variable, ranging from acute self-limited presentation to chronic disease with progressive organ damage and death. Its diagnosis relies upon the demonstration of noncaseating granulomas in the affected tissue and the exclusion of other conditions including mycobacteria infections.

Those cases represent the principal etilogies of microangiopathy in Lupus such as, microangiopathic haemolytic anaemia and antiphospholipid syndrome with and without castastrofic manifestations. The clinicians must know deeply about this condition to recognize it in a timely manner.

Abstract

Introduction: Systemic sclerosis is a chronic autoimmune disease, characterized by the triad of small vessel vasculopathy, immune system activation and increased deposits of extracellular matrix in the skin and internal organs. In the last few years, pulmonary involvement has gained relevance since the introduction of angiotensin enzyme converter inhibitors with the subsequent decline in scleroderma renal crisis mortality, transforming scleroderma lung disease in the leading cause of mortality. Pulmonary involvement can manifest as hypertension or interstitial lung disease, which usually occurs in patients with generalized scleroderma in the first three years of the disease. Its prognosis is poor without treatment which is aimed at stopping pulmonary function deterioration. Among treatment options, cyclophosphamide has the best evidence, and mycophenolate mofetil, rituximab, and stem cell and lung transplantation are currently under investigation with positive preliminary results. Objective: To describe, according to reports in the literature, epidemiology, pathophysiology, diagnostic methods, and treatment of interstitial lung disease in systemic sclerosis. Methods: Structured, non-systematic literature review, focused on the aforementioned aspects of interest. It included 52 articles.
