Abstract

Virus vectors carrying host-derived sequence inserts induce silencing of the corresponding genes in infected plants. This virus-induced gene silencing (VIGS) is a manifestation of an RNA-mediated defence mechanism that is related to post-transcriptional gene silencing (PTGS) in transgenic plants. Here we describe an infectious cDNA clone of tobacco rattle virus (TRV) that has been modified to facilitate insertion of non-viral sequence and subsequent infection to plants. We show that this vector mediates VIGS of endogenous genes in the absence of virus-induced symptoms. Unlike other RNA virus vectors that have been used previously for VIGS, the TRV construct is able to target host RNAs in the growing points of plants. These features indicate that the TRV vector will have wide application for gene discovery in plants.

Abstract

Asthma control remains a challenge in the UK with hospital admissions reflecting a subset of the population at risk of life threatening asthma. Understanding the uniformity of hospital admissions is the first step towards ensuring most effective clinical resource allocation. We combined Hospital Episode Statistics (HES) data and Quality and Outcomes Framework (QOF) Registry data to analyse the hospital admission rates for asthma patients across the UK region of Wessex (April 2013 to March 2014). HES and QOF data was available for 309 Primary Care Practices and all 9 Clinical Commissioning Groups (CCGs). We compared the average inpatient spend and length of stay for a primary diagnosis of asthma per diagnosed asthma patient from the QOF registry at Practice and CCG level. At CCG level there is a 2.8 fold difference in the inpatient admission rates (by spell) per 1000 of the diagnosed asthmatic population. This represents double the cost per patient to the CCG, from £10.48 to £20.59 per diagnosed asthmatic, a potential saving to the highest admitting CCG of over £160,000. Length of stay varied from 1.8 to 5.0 days per CCG and interestingly, did not appear to correspond to 30 day readmission rates. At GP practice level admission costs for asthmatics ranged widely, with a 17-fold difference in spending per diagnosed asthmatic, from the lowest-spend 10% of practices to the highest-spend 10% of practices (32 practices, mean £3-£53/asthmatic). Whilst there will always be confounding factors in large datasets such as this, these numbers suggest potential areas to target for clinical gain and potential cost savings.

Abstract

COPD causes a large disease and cost burden to the UK population. Key to understanding how to improve health within the COPD community and budget most efficiently, is understanding current practice uniformity in resource allocation. We have combined Hospital Episode Statistics (HES) data and Quality and Outcomes Framework (QOF) Registry data to analyse the hospital resource use for COPD patients across the UK region of Wessex (April 2013 to March 2014). HES and QOF data was available for 320 Primary Care Practices and all 9 Clinical Commissioning Groups (CCGs). We compared the average inpatient spend and length of stay for a primary diagnosis of COPD per diagnosed COPD patient from the QOF registry at Practice and CCG level. Prevalence across the CCGs ranged from 1.2% to 2.0%. We found a difference in inpatient admission rates (per spell) varying from 79 to 148 spells per 1000 of the COPD population in individual practices and CCGs. This represents a 1.9 fold difference in admission rates across the CCGs and a 13-fold difference in mean in-patient spend per COPD patient when comparing the 10% of lowest-spend GP practices with the 10% of highest-spend GP practices. Length of stay varied from 5.3 to 7.7 days and did not uniformly correspond to differences in 30 day readmission rates. Assuming a higher number of COPD admissions reflects poorly controlled disease, this data suggests a range of disease control across the CCGs and individual practices, even allowing for static causes of poor COPD prognosis e.g. deprivation. This provides a basis for further analysis of potential factors contributing to the variation seen within the region.
