Objetivo: realizar a adaptação transcultural da Collett-Lester Fear of Death Scale (CL-FODS) – Escala de Medo da Morte de Collett-Lester (EMMCL). Métodos: Trata-se de um estudo metodológico e descritivo. A escala de Medo da Morte foi originalmente elaborada nos Estados Unidos da América e validada em diversos contextos culturais. A versão original da CL-FODS está constituída por 28 itens agrupados em quatro dimensões. Essa escala é destinada a avaliação do medo da morte e do morrer. Resultados: Para se adquirir a primeira versão, foi realizada a sua tradução para a língua portuguesa, separadamente, por três peritos em inglês. A primeira versão foi submetida ao corpo de cinco juízes para a “Avaliação das Equivalências Semântica e Idiomática” da qual se obteve a segunda versão, que foi submetida ao segundo grupo de cinco juízes para a “Avaliação das Equivalências Conceitual e Cultural” (terceira versão). A seguir desenvolveu-se o Grupo Focal, no qual a EMMCL foi analisada por representantes da comunidade, de ambos os gêneros, diferentes faixas etárias e graus de escolaridade, estabelecendo-se a quarta versão, que foi submetida à back-translation e enviada ao autor da versão original da escala. Após os ajustes sugeridos por ele, alcançou-se a versão final da EMMCL. Conclusão: De acordo com as etapas metodológicas desenvolvidas, considera-se o presente instrumento devidamente adaptado à cultura brasileira.

Recent studies evaluated the disease-modifying properties of lithium in mild cognitive impairment and dementia. Although potentially effective for these purposes, chronic lithium use in regard to safety in the elderly needs to be better explored.To evaluate the effect of long-term lithium treatment at subtherapeutic doses on renal function in older adults. Secondary aims were to evaluate the clinical safety and tolerability of this treatment and its effects on thyroid, immune, and glycemic functions.Between February 2007 and October 2011, a 2-year randomized, double-blind, placebo-controlled trial followed by a single-blinded phase for an additional 2 years. Sixty-one patients with mild cognitive impairment (Mayo Clinic criteria) were randomized to receive lithium or placebo. Renal function was estimated by the abbreviated Modification of Diet in Renal Disease (aMDRD) and the Chronic Kidney Disease-Epidemiology study (CKD-EPI) equations. Leukocytes, serum thyroid-stimulating hormone (TSH) and free thyroxine (T₄), and serum glucose and insulin were determined. Tolerability was evaluated at 3-month intervals through systematic clinical examinations and by the UKU Side Effect Rating Scale.Analysis of longitudinal regression indicated that no significant changes in renal function were detected by the aMDRD (P = .453) and CKD-EPI (P = .213) equations after 4 years of lithium treatment. Significant increases in the number of neutrophils (P = .038), serum TSH (P = .034), and body weight (P = .015) were observed in the lithium group. The lithium group presented more overall adverse events (P = .045), particularly interfering in daily activities (P.001). In addition, those patients had a higher incidence of diabetes mellitus (P = .037) and arrhythmia (P = .028).Chronic use of lithium at low doses did not affect renal function and was clinically safe. However, some other potentially relevant adverse events were observed and others could not be ruled out due to limitations of the study design.ClinicalTrials.gov identifier: NCT01055392.

Objetivo: Investigar a compreensão de enfermeiros assistenciais a respeito das finalidades dos cuidados paliativos direcionados ao paciente com HIV/AIDS. Método: Estudo exploratório, com abordagem qualitativa. A amostra consistiu de treze enfermeiros. Utilizou-se um instrumento contendo questões pertinentes ao objetivo proposto, cujos dados foram analisados mediante a Técnica de Análise de Conteúdo. O estudo foi aprovado pelo Comitê de Ética em Pesquisa da Universidade Federal da Paraíba, sob o n°04171512.7.0000.5183. Resultados: Elaborou-se a seguinte categoria: “Finalidades dos cuidados paliativos: alívio da dor e sofrimento e melhorar a qualidade de vida do paciente e da família”. Conclusão: Espera-se que este estudo possa contribuir para subsidiar novas investigações acerca da temática. Por outro lado, convém mencionar que a pesquisa apresenta algumas limitações, como o número reduzido de participantes, o que impede a generalização dos resultados. Objective: To describe what has been published in Brazilian scientific literature regarding pediatric palliative care. Data sources: Bibliographic review with a descriptive approach. In LILACS and SciELO databases, the descriptors "palliative care", "child", "pediatrics", "terminal illness" and "death" were sought, from January 2002 to December 2011. The eight selected articles were analyzed according to year of publication, type of study, data collected, target population, pathology, professionals involved, types of care and main findings. Data synthesis: Regarding the year of publication, there was an increase in the number of publications related to pediatric palliative care. Regarding the type of study, four articles were literature reviews and four were qualitative researches. Data was collected mainly by semi-structured interviews. The participants of the majority of the studies were children's relatives and health professionals. The main pathology addressed was cancer and the nurses were the most frequently cited professionals. The types of care provided were related to physical aspects, general care and psychological, social and spiritual aspects (less emphasis). The main findings were: little emphasis on the children's needs, the importance of including the family in the care provided and the lack of preparation of the health team. Conclusions: Despite the difficulties and the challenges in establishing pediatric palliative care, many articles brought important considerations for the development of this practice in the country. Objetivo: Describir lo que se viene publicando en la producción científica brasileña respecto a la asistencia al niño en cuidados paliativos. Fuentes de datos: Estudio de revisión bibliográfica con acercamiento descriptivo. En las bases de datos LILACS y SciELO se buscaron los descriptores "cuidados paliativos", "niño", "pediatría", "terminalidad" y "muerte", seleccionándose artículos de enero de 2002 a diciembre de 2011. Los ocho artículos seleccionados fueron analizados conforme al año de publicación, el tipo de estudio, la forma de recolección de datos, el público meta, la patología, los profesionales implicados en el cuidado, los tipos de cuidado ofrecido y los principales resultados. Síntesis de los datos: Respecto al año de publicación, se percibió un aumento en el número de publicaciones referentes al área de los cuidados paliativos pediátricos. Respecto al tipo de estudio, cuatro artículos eran de revisión bibliográfica y cuatro eran investigaciones cualitativas. La principal forma de recolección de datos fue la entrevista semiestructurada. Los participantes de la mayoría de las investigaciones fueron los familiares del niño y los profesionales de salud. La principal patología tratada fue el cáncer y los enfermeros fueron los profesionales más citados. Los tipos de cuidado ofrecido se relacionaron a los aspectos físicos, a los cuidados generales y a los aspectos psicológicos, sociales y espirituales (menor énfasis). Los principales resultados encontrados fueron: poca énfasis en las necesidades de los niños, importancia de incluir a los familiares en el cuidado prestado y falta de preparación del equipo de salud. Conclusiones: Aun siendo relatadas dificultades y desafíos para el establecimiento de los cuidados paliativos pediátricos, muchos artículos señalaron cuestionamientos y reflexiones importantes para el desarrollo de la práctica. Non-communicable diseases, including cancer, are overtaking infectious disease as the leading health-care threat in middle-income and low-income countries. Latin American and Caribbean countries are struggling to respond to increasing morbidity and death from advanced disease. Health ministries and health-care systems in these countries face many challenges caring for patients with advanced cancer: inadequate funding; inequitable distribution of resources and services; inadequate numbers, training, and distribution of health-care personnel and equipment; lack of adequate care for many populations based on socioeconomic, geographic, ethnic, and other factors; and current systems geared toward the needs of wealthy, urban minorities at a cost to the entire population. This burgeoning cancer problem threatens to cause widespread suffering and economic peril to the countries of Latin America. Prompt and deliberate actions must be taken to avoid this scenario. Increasing efforts towards prevention of cancer and avoidance of advanced, stage IV disease will reduce suffering and mortality and will make overall cancer care more affordable. We hope the findings of our Commission and our recommendations will inspire Latin American stakeholders to redouble their efforts to address this increasing cancer burden and to prevent it from worsening and threatening their societies.

Muitos são os estudos realizados sobre a morte, entretanto, não existem na realidade brasileira instrumentos disponíveis sobre esse fenômeno. Este estudo teve como objetivo realizar a adaptação transcultural do Multidimensional Orientation Toward Dying and Death Inventory (MODDI-F) - Inventário de Orientação Multidimensional em Relação ao Morrer e à Morte (IMMOR); originalmente elaborado na Alemanha e traduzido para o inglês, está constituído por 47 itens divididos em oito domínios. Essa escala avalia as reações e atitudes das pessoas em relação ao medo da morte e do morrer. Para se adquirir a primeira versão, foi realizada a sua tradução para a língua portuguesa, separadamente, por três peritos em inglês. A primeira versão foi submetida ao corpo de cinco juízes para a "Avaliação das Equivalências Semântica e Idiomática" da qual se obteve a segunda versão, que foi submetida ao segundo grupo de cinco juízes para avaliação conceitual e cultural (3ª versão). Many studies have been conducted about death; however, there are no instruments available in Brazil to evaluate this phenomenon. The purpose of this study was to perform the cross-cultural adaptation of the Multidimensional Orientation Toward Dying and Death Inventory (MODDI-F), originally designed in Germany and translated into English, constituted by 47 items divided in eight domains. This scale evaluates people's reactions and attitudes in relation to fear of death and dying. To produce the first version, it was translated into Portuguese, separately, by three experts in English. The first version was submitted to five judges for the "Semantic and Idiomatic Equivalences Evaluation", which produced the second version. The second version was submitted to the second group of five judges for a conceptual and cultural evaluation (third version). Then the Focal Group was developed, in which the instrument, which was called IMMOR in Portuguese, was analyzed by community representatives, of both sexes, different ages and levels of schooling, establishing the fourth version, which was submitted to a back-translation and sent to the author. After the implementation of the changes he suggested, we reached the final version. According to the methodological steps that were developed, it is possible to state that the present instrument is adapted to the Brazilian culture.

A questão do término da vida é fonte de reflexões desde os primórdios da civilização e demanda diligências para a tentativa de seu enquadramento social ao longo da história do pensamento humano. Com o desenvolvimento e aprimoramento da medicina, podese modificar, na maioria das vezes, a história natural das doenças. É possível prolongar a vida e adiar o processo do morrer. Isso engendrou um novo protótipo médico em que há necessidade de se conviver e cuidar de pacientes gravemente enfermos, situação muitas vezes acompanhada de árduo sofrimento. A sociedade atribui ao médico a função de ser o responsável por debelar e vencer a morte. No contexto oncológico, essas questões surgem de forma salutar, uma vez que, em diversas situações, não há possibilidade de se oferecer uma terapêutica curativa aos enfermos. O objetivo do presente artigo é debater as relações que norteiam a temática proposta, baseando-se em uma revisão de literatura. Busca-se, assim, uma perspectiva que figura como um caminho argumentativo que conduz evidências a esse debate.The issue of life-ending has been a source of considerations since the dawn of civilization, and calls for great circumspection when one attempts to fit it socially throughout the history of human thinking. The development and improvement of Medicine might modify, in most cases, the natural history of disease. We have managed to prolong life and the process of dying. This has created a new medical prototype that needs to care for terminally-ill patients, a situation often accompanied by severe suffering. Society attributes to the physician the role of being responsible for conquering and overcoming death. In the oncology context, these questions are well addressed, as in many situations there is no possibility to offer a curative treatment to the patients. The objective of the present study was to discuss the relations that guide the proposed theme, based on a medical literature review. Therefore, a perspective is sought as an argumentative alternative that brings evidence to the proposed debate.

The following article starts by presenting an socio-anthropological reflection about the relation between health and disease, demonstrating through these indicators that death and grief which is what we want to point out, can be interpreted through multiple boosters and epistemological bases. Here we choose Anthropology and Sociology. By the exposition of the text analysis inside the social anthropological approach we’d like to reinforce the need to look at this in a broach multidisciplinary To do so, we decided to report/ describe an experience in operative group in one of the Social Assistance Center, where the subject “death” emerged as a factor that induces the memories of traumatic experience, related to losses and grief. The observation of these collective phenomena instigated us to reflect about death as a social construction still linked to negative aspects and of challenging overcoming (mourning), outside of the welfare work scope, necessarily. O presente artigo inicia apresentando uma reflexão sócioantropológica sobre a relação saúde e doença, demonstrando a partir desses indicadores que a morte e luto, os quais queremos destacar, podem ser lidos através de múltiplos impulsionadores e bases epistemológicas. Aqui escolhemos a Antropologia e a Sociologia. Através da exposição de análises de textos dentro da abordagem sócio-antropológica gostaríamos de reforçar a necessidade desse olhar multidisciplinar, destacando seu caráter enriquecedor e clareador para o estudo da morte e do luto. Para tanto, nos propomos a narrar uma vivência em um grupo operativo numa unidade de Centro de Referência em Assistência Social (CRAS), onde o assunto “morte” surgiu como fator disparador de experiências passadas, relacionadas a perdas e luto. A observação desse fenômeno coletivo nos instigou a refletir sobre a morte como uma construção social ainda atrelada a aspectos negativos e de superação (luto) desafiadora, fora do âmbito de trabalho em saúde, necessariamente.

O delirium é uma das síndromes mais fascinantes na medicina, apesar de pobremente compreendida. Apesar de sua ocorrência freqüente e de haver crescente informação sobre o diagnóstico, fenomenologia, epidemiologia e etiologias, estudos sobre os mecanismos que mediam a fisiopatologia são, freqüentemente, ausentes. O desenvolvimento de sofisticadas metodologias de imagem cerebral tem permitido ir além das considerações diagnósticas e investigar a neurobiologia dos sintomas específicos observados no delirium. Esses avanços na neuropsiquiatria e na neuroimagem têm revelado diferenças entre as regiões cerebrais, incluindo os hemisférios. O delirium é uma síndrome que pode ocorrer como o resultado de múltipla e complexa interação entre sistemas de neurotransmissores e processos patológicos. Os neurotransmissores, acetilcolina e serotonina, podem ter participação importante no delirium devido a condições clínicas comuns, bem como no delirium pós-cirúrgico. Outros neurotransmissores (dopamina e GABA) e fatores neurobiológicos tais como citocinas, hormônios e radicais livres precisarão de mais estudos para definir as suas participações na gênese do delirium. Futuros estudos, centralizados na fisiopatologia do delirium, poderão levar a melhores estratégias de prevenção e tratamento.Delirium is one of the most fascinating and poorly understood syndromes in medicine. Despite its frequent occurrence and growing information on diagnosis, phenomenology, epidemiology, and aetiologies, studies on mediating pathophysiological mechanisms in delirium are largely lacking. The emergence of sophisticated brain imaging methodologies has made it possible to move beyond diagnostic consideration and investigate the neurobiology of specific symptom clusters observed in delirium. Advances in neuropsychiatry and neuroimaging have revealed differences between brain regions, including the hemispheres. Delirium is a syndrome that may occur as the result of multiple complex interacting neurotransmitter systems and pathologic processes. The neurotransmitters acetylcholine and serotonin may play particularly important roles in common medical and surgical delirium. Others neurotransmitters such as dopamine and GABA and neurobiologic factors such as cytokines, cortisol abnormalities, and oxygen free radicals will require further study to define their role in delirium. Distinct neuropathologic processes leading to delirium are beginning to be defined. Further situation-specific studies of delirium pathophysiology should lead more effective prevention and treatment strategies.

Objective: To describe the clinical features as well as morbidity and mortality of elderly patients submitted to coronary artery bypass graft (CABG). Design: Randomized, single-center, observational study. Methods: A prospective study was carried out in a tertiary referral teaching hospital (Heart Institute-Brazil). A total of 220 inpatients submitted to elective CABG were evaluated and data collected from the pre-, intra- and postoperative periods. Instruments: The patients were assessed according to the Geriatric Depression Scale. A diagnosis of delirium was determined according to DSM-IV criteria. Results: Of these 220 patients 64.5% were males and 35.5% females. The mean age was 71 years. The most important preoperative findings were a higher incidence of comorbidities such as hypertension (73%), diabetes mellitus (35%), dyslipidemia (27%), and previous myocardial infarction (41%).The majority of patients were found to have three or more vessels obstructed at angiography (74%). The major intraoperative findings were that a high number of the patients received more than 3 grafts (74%). A large proportion (72%) of patients needed a blood transfusion and the overall incidence of clinical complications was 33%. The overall postoperative mortality rate for this observational study was 5%. A large number of patients presented complications (56%) such as hypotension (22%), atrial fibrillation (22%), and delirium (34%). Conclusions: Although elderly patients have a somewhat increased, however acceptable, operative mortality rate, we believe that this patient group can benefit from CABG.

Objective: To investigate the risk factors for delirium in the elderly during the post-operative period of coronary artery bypass graft surgery (CABG).Methods: A total of 220 inpatients submitted to CABG were evaluated prior to and after surgery. In order to investigate the possible risk factors, data were collected from pre-intra- and post-operative periods.Instruments: The patients were assessed using the Mini-mental State Examination and to the Geriatric Depression Scale. The diagnosis of delirium was determined according to DSM-IV criteria.Results: Delirium was detected in 74 (33.6%) patients. Increasing age, blood urea level, cardio-thoracic index, hypertension, smoking habits, blood replacement during bypass, atrial fibrillation (AF), pneumonia and blood balance in the post-operative period were the risk factors for delirium selected by the logistic regression analysis. No specific factor associated with the CABG (perfusion pressure, number of grafts) was correlated with an increased risk for delirium post-operatively. The length of stay was twice as long in the delirious group (pConclusions: Delirium in the elderly after CABG is a multifactorial disorder. Nine factors taken together can identify patients at great risk for delirium. No specific factor associated with bypass procedure could be identified as a risk factor for delirium. The control of the risk factors should bring a decrease in delirium morbidity and mortality.

The attitude towards death and dying depends on the culture. In prehistoric times grave artifacts suggest a belief in the continuation of life. This belief in an afterlife has continued through different cultures and societies to the present day. The fear of death seems to have grown in parallel with those religions which have promised judgment at the time of death. In our modern Western secular society death is regarded as a medical failure, the rituals which used to attend it have largely been abandoned, and life is prolonged so that death has lost all dignity. It is now beginning to be recognised that dying may not be a simple switching off, but a process leading to death and the gradual dissolution of consciousness. This dissolution seems to involve experiences for the dying which are spiritual and important for them. A number of these phenomena raise the possibility that consciousness may not be limited to the brain, but extend beyond it. Fortunately, palliative care is now taught in medical schools, and treatment of the dying is now recognised to be as important as treatment for the living. This article looks at the history of death, the significance of the dying process for consciousness research, and the education needed for carers of the dying.

OBJECTIVE: To evaluate the effects of a multidisciplinary rehabilitation program on cognition, quality of life, and neuropsychiatric symptoms in patients with mild Alzheimer's disease. METHOD: The present study was a single-blind, controlled study that was conducted at a university-based day-hospital memory facility. The study included 25 Alzheimer's patients and their caregivers and involved a 12-week stimulation and psychoeducational program. The comparison group consisted of 16 Alzheimer's patients in waiting lists for future intervention. INTERVENTION: Group sessions were provided by a multiprofessional team and included memory training, computer-assisted cognitive stimulation, expressive activities (painting, verbal expression, writing), physiotherapy, and physical training. Treatment was administered twice a week during 6.5-h gatherings. MEASUREMENTS: The assessment battery comprised the following tests: Mini-Mental State Examination, Short Cognitive Test, Quality of Life in Alzheimer's disease, Neuropsychiatric Inventory, and Geriatric Depression Scale. Test scores were evaluated at baseline and the end of the study by raters who were blinded to the group assignments. RESULTS: Measurements of global cognitive function and performance on attention tasks indicated that patients in the experimental group remained stable, whereas controls displayed mild but significant worsening. The intervention was associated with reduced depression symptoms for patients and caregivers and decreased neuropsychiatric symptoms in Alzheimer's subjects. The treatment was also beneficial for the patients' quality of life. CONCLUSION: This multimodal rehabilitation program was associated with cognitive stability and significant improvements in the quality of life for Alzheimer's patients. We also observed a significant decrease in depressive symptoms and caregiver burden. These results support the notion that structured nonpharmacological interventions can yield adjunct and clinically relevant benefits in dementia treatment.

Quality of life is seldom explored in evaluations of therapeutic interventions in Alzheimer's disease. Objective: To verify whether participation in a cognitive and functional rehabilitation program improves quality of life (QOL) among Alzheimer's disease (AD) patients. Methods: 19 AD patients participated in this study, 12 of whom attended 24 multi-professional intervention sessions - the experimental group - whereas the remaining 7 comprised the control group. The following tools were used to assess changes: a) Mini-Mental State Examination (MMSE); b) Geriatric Depression Scale (GDS); c) Quality of Life in AD evaluation scale (QOL-AD); d) Open question on QOL. Results: Participation had no positive impact on quantitative clinical variables (MMSE, GDS, QOL-AD). The answers to the open question, examined using the Collective Subject Discourse (CSD) method, suggested that QOL improved after the intervention. Conclusion: Combining pharmacological treatment with psychosocial intervention may prove to be an effective strategy to enhance the QOL of AD patients. Resumo Qualidade de vida não é frequentemente explorada na avaliação dos resultados de intervenções terapêuticas em DA. Objetivo: Verificar se a participação em um programa de reabilitação cognitiva e funcional melhora a qualidade de vida (QV) de pacientes com doença de Alzheimer. Metodos: 19 pacientes com DA participaram deste estudo, 12 participaram de 24 sessões de intervenção multiprofissional - grupo experimental - enquanto os 7 restantes compuseram o grupo controle. Os seguintes instrumentos foram utilizados para avaliar mudanças: a) Mini-Exame do Estado Mental (MEEM); b) Escala de Depressão Geriátrica (EDG); c) Escala de Avaliação de Qualidade de Vida na Doença de Alzheimer (QdV-DA); d) questão aberta sobre QV. Resultados: A participação não mostrou efeitos positivos em variáveis clínicas quantitativas (MEEM, EDG, QdV-DA). Os relatos na questão aberta, analisados através da metodologia do discurso do sujeito coletivo (DSC), sugeriram que a QV melhora após a intervenção. Conclusão: Aliar o tratamento medicamentoso às intervenções psicossociais pode ser uma boa estratégia para a melhora da QV dos portadores de DA.

O delirium é uma das síndromes mais fascinantes na medicina, apesar de pobremente compreendida. Apesar de sua ocorrência freqüente e de haver crescente informação sobre o diagnóstico, fenomenologia, epidemiologia e etiologias, estudos sobre os mecanismos que mediam a fisiopatologia são, freqüentemente, ausentes. O desenvolvimento de sofisticadas metodologias de imagem cerebral tem permitido ir além das considerações diagnósticas e investigar a neurobiologia dos sintomas específicos observados no delirium. Esses avanços na neuropsiquiatria e na neuroimagem têm revelado diferenças entre as regiões cerebrais, incluindo os hemisférios. O delirium é uma síndrome que pode ocorrer como o resultado de múltipla e complexa interação entre sistemas de neurotransmissores e processos patológicos. Os neurotransmissores, acetilcolina e serotonina, podem ter participação importante no delirium devido a condições clínicas comuns, bem como no delirium pós-cirúrgico. Outros neurotransmissores (dopamina e GABA) e fatores neurobiológicos tais como citocinas, hormônios e radicais livres precisarão de mais estudos para definir as suas participações na gênese do delirium. Futuros estudos, centralizados na fisiopatologia do delirium, poderão levar a melhores estratégias de prevenção e tratamento. Delirium is one of the most fascinating and poorly understood syndromes in medicine. Despite its frequent occurrence and growing information on diagnosis, phenomenology, epidemiology, and aetiologies, studies on mediating pathophysiological mechanisms in delirium are largely lacking. The emergence of sophisticated brain imaging methodologies has made it possible to move beyond diagnostic consideration and investigate the neurobiology of specific symptom clusters observed in delirium. Advances in neuropsychiatry and neuroimaging have revealed differences between brain regions, including the hemispheres. Delirium is a syndrome that may occur as the result of multiple complex interacting neurotransmitter systems and pathologic processes. The neurotransmitters acetylcholine and serotonin may play particularly important roles in common medical and surgical delirium. Others neurotransmitters such as dopamine and GABA and neurobiologic factors such as cytokines, cortisol abnormalities, and oxygen free radicals will require further study to define their role in delirium. Distinct neuropathologic processes leading to delirium are beginning to be defined. Further situation-specific studies of delirium pathophysiology should lead more effective prevention and treatment strategies.

Objective: To describe the incidence, risk factors and clinical features (subtypes) of delirium during the postoperative period after hip fracture surgery in elderly patients. Design: Prospective study. Methods: Thirty-four consecutive patients (9 men and 25 women) were included in this study between June 16 to July 14, 2003. All patients underwent surgery for a fractured neck of femur and were pre- and postoperatively cared for at a combined geriatric/orthopedic ward. Assessment: The diagnosis of delirium was based on the criteria of the DSM-IV and the Confusion Assessment Method Scale. Subtypes of delirium were classified according to the criteria proposed by Lipowski: hyperactive-hyperalert (or agitated), hypoactive-hypoalert (somnolent) and mixed delirium. Results: Fifty-five percent (n = 19) of the 34 patients developed delirium after surgery. The development of delirium was associated with the medication midazolam taken perioperatively. Nine (47%) of the delirious patients had a hyperactive type of delirium; 5 (26%) developed a hypoactive delirium, and 5 (26%) had a mixed type. We did not find any association among subtypes of delirium and clinical features. Conclusions: Delirium is a common complication in the postoperative period of elderly patients treated for hip fractures. The use of midazolam in the perioperative period increased the risk of developing postoperative delirium. Thehyperactive type of delirium was the most common subtype of delirium.

Summary The issue of life-ending has been a source of considerations since the dawn of civilization, and calls for great circumspection when one attempts to fit it socially throughout the history of human thinking. The development and improvement of Medicine might modify, in most cases, the natural history of disease. We have managed to prolong life and the process of dying. This has created a new medical prototype that needs to care for terminally-ill patients, a situation often accompanied by severe suffering. Society attributes to the physician the role of being responsible for conquering and overcoming death. In the oncology context, these questions are well addressed, as in many situations there is no possibility to offer a curative treatment to the patients. The objective of the present study was to discuss the relations that guide the proposed theme, based on a medical literature review. Therefore, a perspective is sought as an argumentative alternative that brings evidence to the proposed debate.

INTRODUÇÃO: A osteoartrose de joelho (OAJ) é uma das patologias que mais afetam o aparelho locomotor e interferem de forma considerável nas atividades da vida diária, acometendo os indivíduos em faixa etária variável e em diversos graus e níveis de dor articular. As propostas terapêuticas são bastante diversificadas para a osteoartrose (OA), a terapia física, a TENS está para colaborar no tratamento dos pacientes com osteoartrose de joelho. OBJETIVOS: Devido à importância da articulação do joelho nas atividades locomotoras e sua relação com as atividades da vida diária. Os pacientes com osteoartrose de joelho sofrem não somente pelo problema localizado, existe uma alteração funcional global que pode levar a processos dolorosos em outras articulações. Para uma eficaz orientação e avaliação do paciente primeiro é necessário que este faça a descrição clara de sua dor para que o profissional perceba como que esse estímulo doloroso prejudica sua vida. O objetivo do presente estudo foi avaliar o efeito da estimulação elétrica nervosa transcutânea (sigla em inglês TENS) nível sensório para tratamento de dor em pacientes com diagnóstico de osteoartrose de joelho, utilizando escalas de avaliação de dor, escala de avaliação funcional. MÉTODOS: Foram estudados 10 pacientes com diagnóstico de osteoartrose de joelho, utilizando uma modalidade fisioterapêutica, a eletroterapia, especificamente a (TENS), com parâmetros de 80 Hz e 140s, com um total de 10 sessões, 30 minutos para cada sessão, o período total de tratamento compreendeu 4 semanas, nesse total de 10 sessões, cada paciente foi entrevistado com um questionário na 1ª sessão, na 5ª e na 10ª sessão. Os formulários utilizados foram Questionário da Dor McGill Melzack, escala Analógica visual da dor, escala numérica com expressão facial de sofrimento, escala de atividades da vida diária da knee Outcome Survey - RESULTADOS: O resultado com a TENS nível sensório para efeito de analgesia mostra que as diferenças foram estatisticamente significativas (p
