Background: Allergic rhinitis (AR) is a highly prevalent disease. We aimed to assess the symptoms that physicians who see patients with AR perceive as the most bothersome in their patients. Methods: We performed a cross-sectional study based on an online questionnaire sent to all members of the Allergic Rhinitis and its Impact on Asthma (ARIA) initiative. The survey included questions on the physicians' perceptions of patients’ AR symptoms as well as of their own AR symptoms. Results: Among 401 respondents, 155 (38.7%) reported having AR. ARIA members reported nasal symptoms to be the most frequent (89.7%) and bothersome (80.0%) symptoms experienced by themselves. Likewise, nasal symptoms were reported by ARIA members as the most frequent (94.8% in members with AR vs 96.0% in members without AR) and bothersome (57.0% in members with AR vs 67.9% in members without AR) in their patients. We found a significant association (p = 0.001) between physicians’ own symptoms and those perceived as the most bothersome in their patients. Conclusion: Physicians perceive nasal symptoms to be the most frequent and the most bothersome symptoms in AR patients. The physicians' personal experiences with AR may influence their perception of patients’ symptoms.

Abstract Introduction The integrated care pathways for atopic dermatitis (AD‐ICPs) aim to bridge the gap between existing AD treatment evidence‐based guidelines and expert opinion based on daily practice by offering a structured multidisciplinary plan for patient management of AD. ICPs have the potential to enhance guideline recommendations by combining interventions and aspects from different guidelines, integrating quality assurance, and describing co‐ordination of care. Most importantly, patients can enter the ICPs at any level depending on AD severity, resources available in their country, and economic factors such as differences in insurance reimbursement systems. Methods The GA2LEN ADCARE network and partners as well as all stakeholders, abbreviated as the AD‐ICPs working group, were involved in the discussion and preparation of the AD ICPs during a series of subgroup workshops and meetings in years 2020 and 2021, after which the document was circulated within all GAL2EN ADCARE centres. Results The AD‐ICPs outline the diagnostic procedures, possible co‐morbidities, different available treatment options including differential approaches for the pediatric population, and the role of the pharmacists and other stakeholders, as well as remaining unmet needs in the management of AD. Conclusion The AD‐ICPs provide a multidisciplinary plan for improved diagnosis, treatment, and patient feedback in AD management, as well as addressing critical unmet needs, including improved access to care, training specialists, implementation of educational programs, assessment on the impact of climate change, and fostering a personalised treatment approach. By focusing on these key areas, the initiative aims to pave the way for a brighter future in the management of AD.

A iniciativa Allergic Rhinitis and Its Impact on Asthma (ARIA) teve início há mais de 20 anos e tem elaborado e disseminado orientações baseadas em evidência, e desenvolvido projetos na área da rinite alérgica. Esta iniciativa está atualmente focada em proporcionar orientações centradas no doente que contribuam para um percurso integrado entre os vários níveis de cuidados e que tirem partido de soluções digitais, tendo sido recomendada a introdução na prática clínica de percursos assistenciais integrados. Neste artigo descrevemos a adaptação para Portugal do documento ARIA Integrated Care Pathways. Após breve revisão sobre a epidemiologia e o impacto da rinite alérgica em Portugal e das atividades realizadas em Portugal no âmbito da iniciativa ARIA, é descrito o conjunto alargado de conhecimento utilizado para o desenvolvimento das recomendações para o tratamento farmacológico da rinite alérgica, recomendações essas baseadas na metodologia GRADE, evidência do mundo real adquirida por tecnologia móvel (mHealth) e resultante de estudos de câmara de exposição alergénica. Em seguida, são resumidos os percursos assistenciais integrados para imunoterapia com alergénios produzidas em 2019. Considera-se a imunoterapia com alergénios um exemplo de medicina de precisão e em que a utilização de tecnologias mHealth permitirá melhorar a estratificação para seleção dos doentes e monitorização da resposta. Estas recomendações foram consideradas como ‘boas práticas’ dos cuidados integrados centrados no doente apoiados por sistemas digitais da DG Santé (Direção Geral de Saúde e de Segurança Alimentar da União Europeia) e representam a estratégia de gestão da mudança da fase 4 do ARIA.

Introdução: O eczema das mãos é uma doença multifactorial onde, muitas vezes, coexistem dermatite atópica, dermatite de contacto irritativa, dermatite de contacto alérgica (mediada principalmente pelas células T) e ainda reacções imediatas, como ocorre na urticária de contacto e/ou dermatite de contacto por proteínas (mediada ou não por IgE). O objectivo do estudo foi a caracterização dos doentes com eczema das mãos que reportaram sintomas imediatos com o manuseamento de produtos diversos. Material e Métodos: Análise retrospectiva dos registos dos doentes do Serviço de Dermatologia com eczema das mãos que realizaram testes epicutâneos e testes cutâneos por picada. Os testes epicutâneos englobaram as séries básica europeia/portuguesa e séries complementares, seleccionadas de acordo com os antecedentes clínicos. Os testes cutâneos por picada foram realizados sempre que os doentes referiam sintomas imediatos com a exposição, e foram correlacionados com IgE específica e total. Resultados: Foram avaliados 52 doentes (49F/3M), média de idades: 33,4 ± 9,9 anos, 25 (48,1%) com antecedentes pessoais de atopia (nove com dermatite atópica). Além do eczema das mãos, considerado ocupacional em 33 doentes (63,5%), a dermatite também envolveu a face em seis doentes (11,5%). Foram observados testes epicutâneos positivos em 31 doentes (59,6%), principalmente ao níquel (14; 45,2%). Verificaram-se testes cutâneos por picada positivos em 28 doentes (53,8%), principalmente para o látex (10; 35,7%), seguido de kiwi (8; 28,6%). Não houve correlação estatisticamente significativa entre os testes cutâneos por picadapositivos e a presença de atopia ou de IgE sérica total elevada. Conclusão: Embora o estudo inclua um número limitado de doentes com eczema das mãos e apenas tenham sido realizados testes cutâneos por picada naqueles com história sugestiva, observaram-se reacções imediatas relevantes numa percentagem muito significativa de doentes (53,8%), principalmente naqueles com antecedentes pessoais de atopia. A IgE específica não foi útil para o diagnóstico, face aos testes cutâneos por picada. Reacções imediatas, não directamente dependentes de células T, podem estar significativamente envolvidos na patogénese do eczema das mãos, pelo que os testes imediatos devem ser mais frequentemente adicionados aos testes epicutâneos no estudo do eczema das mãos.
