Extracellular vesicles (EV) function as messengers between endothelial cells (EC) and vascular smooth muscle cells (VSMC). Since chronic kidney disease (CKD) increases the risk for vascular calcifications, we investigated whether EV derived from uraemic milieu‐stimulated EC and derived from uraemic rats impact the osteogenic transdifferentiation/calcification of VSMC. For that purpose, human EC were treated with urea and indoxyl sulphate or left untreated. Experimental uraemia in rats was induced by adenine feeding. ‘Uraemic’ and control EV (EVUR; EVCTRL) were isolated from supernatants and plasma by using an exosome isolation reagent. Rat VSMC were treated with a pro‐calcifying medium (CM) with or without EV supplementation. Gene expressions, miRNA contents and protein expressions were determined by qPCR and Western blots, respectively. Calcifications were determined by colorimetric assays. Delivery of miRNA inhibitors/mimics to EV and siRNA to VSMC was achieved via transfection. EVCTRL and EVUR differed in size and miRNA contents. Contrary to EVCTRL, EC‐ and plasma‐derived EVUR significantly increased the pro‐calcifying effects of CM, including altered gene expressions of osterix, runx2, osteocalcin and SM22α. Further, EVUR enhanced the protein expression of the phosphate transporter PiT‐1 in VSMC and induced a phosphorylation of AKT and ERK. Knock down of PiT‐1 and individual inhibition of AKT and ERK signalling in VSMC blocked the pro‐calcifying effects of EVUR. Similar effects were achieved by inhibition of miR‐221/‐222 and mimicking of miR‐143/‐145 in EVUR. In conclusion, EVUR might represent an additional puzzle piece of the complex pathophysiology of vascular calcifications in CKD.

Chronic kidney disease (CKD) is associated with excessive mortality from cardiovascular disease (CVD). Endothelial dysfunction, an early manifestation of CVD, is consistently observed in CKD patients and might be linked to structural defects of the microcirculation including microvascular rarefaction. However, patterns of microvascular rarefaction in CKD and their relation to functional deficits in perfusion and oxygen delivery are currently unknown. In this in-vivo microscopy study of the cremaster muscle microcirculation in BALB/c mice with moderate to severe uremia, we show in two experimental models (adenine feeding or subtotal nephrectomy), that serum urea levels associate incrementally with a distinct microangiopathy. Structural changes were characterized by a heterogeneous pattern of focal microvascular rarefaction with loss of coherent microvascular networks resulting in large avascular areas. Corresponding microvascular dysfunction was evident by significantly diminished blood flow velocity, vascular tone, and oxygen uptake. Microvascular rarefaction in the cremaster muscle paralleled rarefaction in the myocardium, which was accompanied by a decrease in transcription levels not only of the transcriptional regulator HIF-1α, but also of its target genes Angpt-2, TIE-1 and TIE-2, Flkt-1 and MMP-9, indicating an impaired hypoxia-driven angiogenesis. Thus, experimental uremia in mice associates with systemic microvascular disease with rarefaction, tissue hypoxia and dysfunctional angiogenesis.

Zielsetzung der Arbeiten war es, die Bedeutung von extrazellulärer Matrix, Kaliumkanälen sowie eines Lindera obtusiloba Extraktes bzw. daraus isoliertem (+)-Episesamin für eine mögliche Beeinflussung von Fibrose, Inflammation und Neoplasie zu untersuchen. Die Arbeiten zeigten, dass Kollagene und Kollagenfragmente, als Hauptbestandteil fibrotischer Matrix, inaktive Proformen von Matrix-Metalloproteinasen (ProMMPs) speichern und zelluläre Prozesse wie Proliferation und Differenzierung beeinflussen. Die tripelhelikale Kollagensequenz „(Glycin-Prolin-Hydroxyprolin)n“ - (GPO)n - wurde als Konsensusbindungsstelle für ProMMPs identifiziert. Mit dem daraus abgeleiteten synthetischen Kollagenpeptid (GPO)10 konnte die Kollagenbindung der Gelatinasen ProMMP-2 und -9 geschwächt bzw. gebundene Gelatinasen freigesetzt und im Fall von ProMMP-2 aktiviert werden. Dies führte in vitro zu verstärkter Proliferation und Invasion von hepatischen Sternzellen (HSZ) und Tumorzellen aufgrund erhöhter Verfügbarkeit von aktiviertem MMP-2. Neben der Leberfibrose werden MMP-2 und -9 auch im Darm exprimiert und sind dort an der Aufrechterhaltung der strukturellen Integrität des Darmepithels beteiligt. Im akuten Modell der murinen DSS-Colitis zeigte (GPO)10 neben anti- inflammatorischen Effekten auch protektive Effekte gegenüber der strukturellen Zerstörung des Darmepithels, wahrscheinlich durch Freisetzung und Aktivierung von Epithel-protektiver MMP-2 sowie durch Freisetzung, aber keiner Aktivierung von Epithel-schädigender MMP-9. Mit der tripelhelikalen Domäne der alpha 2-Kette von Kollagen VI wurde ein weiteres Matrixfragment identifiziert, welches die Matrixbindung und enzymatische Aktivität vor allem der Kollagenasen hemmte, die Gelatinase ProMMP-2 dagegen aktivierte und vor Autodegradation schützte. Aus pathophysiologischer Sicht wird bei der Fibrose die lokale, Matrix-abbauende Wirkung der MMPs durch überschüssiges Kollagen neutralisiert, so dass Kollagene akkumulieren. Durch gezielte Beeinflussung dieser lokalen MMP-Matrix-Interaktion, z.B. durch Freisetzung von MMPs mit o.g. MMP-spezifisch wirksamen Peptiden, ist ein lokal wirkender, fibrolytischer Stimulus denkbar. Neben der Matrix befassten sich komplementäre Studien auf Zellebene mit der Expression und den Auswirkungen einer Inhibition von Kalzium-aktivierten Kaliumkanälen mit mittlerer Leitfähigkeit (KCa3.1) auf die Fibrose. Erstmals wurde eine KCa3.1-Expression vor allem in aktivierten HSZ sowie in humanen, fibrotischen Lebergewebsproben gezeigt. In quieszenten HSZ und gesundem Lebergewebe dagegen wurden KCa3.1 nicht bzw. nur schwach exprimiert. Eine KCa3.1-Inhibition mit dem spezifischen Inhibitor TRAM-34 hemmte zum einen die HSZ-Proliferation durch Induktion eines Zellzyklusarrestes und zum anderen die TGF-β1-Signaltransduktion in HSZ, einem wichtigen, fibrogenen Signalweg. Im Rattenmodell der durch Gallengangsligation induzierten Leberfibrose reduzierte TRAM-34 den Fibrose-fördernden portalen Druck, was Effekte von KCa3.1 auf die Kontraktion der Gefäßzellen impliziert. TRAM-34 zeigte auch anti-neoplastisches Potential in humanen Zelllinien des HCC, u.a. durch Hemmung der Aktivierung des Transkriptionsfaktors NF-ĸB, welcher beim HCC durch spontane Aktivierung als Überlebensfaktor gilt, und durch verminderte ERα-Expression. TRAM-34 ist physiologisch gut verträglich, was besonders bei eingeschränkter Leberfunktion, z.B. bei vorliegender Zirrhose als Vorerkrankung des HCC, von Bedeutung sein kann. Jüngere eigene Arbeiten weisen zudem auf eine Beteiligung von KCa3.1 bei der Kalzifizierung glatter Gefäßmuskelzellen hin, welche durch TRAM-34 gehemmt wurde. Weitere Studien auf Zellebene befassten sich mit der Wirkung pflanzlicher Wirkstoffe auf Fibrose und Neoplasie. Im in vitro-Modell der Leberfibrose mit HSZ hemmte das Lindera obtusiloba Extrakt die Proliferation sowie die TGF-β1-induzierte Expression fibrogener Marker wie Kollagen I und inhibierte der Bildung reaktiver Sauerstoffradikale. In humanen HCC-Zelllinien zeigte das Extrakt anti-mitogene Effekte, einhergehend mit der Induktion von Apoptose sowie der Hemmung der IGF-1-Signaltransduktion und von NF-ĸB. Mit (+)-Episesamin wurde ein pharmakologisch wirksamer Inhaltsstoff aus dem Extrakt isoliert und dessen absolute Konfiguration erstmals beschrieben. Ähnlich wie das Extrakt, zeigte (+)-Episesamin durch Hemmung von Proliferation und dem TGF-β1-Signalweg ebenfalls anti-fibrotische Effekte in HSZ. Zum anderen auch anti-neoplastische Effekte in HCC-Zelllinien durch MMP-Inhibition sowie durch Hemmung von Proliferation, NF-ĸB und der VEGF-Sekretion. Anti-inflammatorische Effekte von (+)-Episesamin wurden in Tumorzellen und Makrophagen durch Hemmung TNF-- und LPS-induzierter IL-6-Sekretion sowie durch Hemmung der TNF-- und PDGF- induzierten Aktivierung glatter Gefäßmuskelzellen gezeigt. Wie in Abbildung 5 schematisch dargestellt, beschreiben die hier vorgestellten Untersuchungen zur Pathogenese von Fibrose und Karzinogenese zusammengefasst potentielle Möglichkeiten ihrer Beeinflussung auf Matrix- und Zellebene. Die Ergebnisse stellen eine Rationale für weitere Untersuchungen in spezifischen Tiermodellen dar, wobei auch die Kombination von Matrix- (z.B. (GPO)10) und zellspezifischen Ansätzen (z.B. TRAM-34) denkbar ist., Aim of this work was to identify potential therapeutic tools to impact fibrosis, inflammation and neoplasia. Effects of extracellular matrix (ECM) components, ion channel inhibitors and an extract from Lindera obtusiloba were studied. We showed that collagens and collagen fragments store inactive proforms of matrix-metalloproteinases (proMMPs). The triplehelical collagen sequence Gly-Pro-Hyp (GPO) was identified as consensus binding motif. The related collagen peptide (GPO)10 interfered with the collagen binding of the gelatinases proMMP-2 and -9 and induced enzymatically activation of proMMP-2. In vitro, (GPO)10 induced proliferation and invasion of hepatic stellate cells (HSC) and tumor cells, respectively, due to a higher amount of active MMP-2. In an acute murine model of DSS-colitis, (GPO)10 protected the epithelial barrier from DSS-induced disruption and showed anti-inflammatory activity. This might be due to a release and activation of matrix bound epithelial- protective (pro)MMP-2 and due to a release of epithelial-damaging (pro)MMP-9 (without activation of proMMP-9). Another identified ecm fragment was the triplehelical domain of the alpha 2-chain of collagen VI. The alpha 2-chain interfered with the collagen binding and activity of the collagenases MMP-1, MMP-8 and MMP-13. In addition, the alpha 2-chain activated the gelatinase proMMP-2 and protected it from auto-degradion. In summary, the results indicate that the local matrix degrading activity of MMPs is neutralized by excess amounts of collagens. A targeted modulation of this local MMP-ECM interaction e.g. by (GPO)10, might induce a fibrolytic stimulus. Besides the ECM, expressions of intermediate conductance calcium-activated potassium channels (KCa3.1) and effects of the KCa3.1-inhibitor TRAM-34 on fibrosis were studied. We showed that fibrotic tissues and TGF-beta1-activated HSC exhibited higher KCa3.1-expressions than normal tissue and untreated cells. KCa3.1 inhibition with TRAM-34 reduced HSC proliferation by induction of cell cycle arrest and reduced TGF-beta1-induced gene expression of collagen I, alpha- smooth muscle actin and TGF-beta1 itself. Furthermore, TRAM-34 blocked TGF- beta1-induced activation of TGF-b signalling in HSC. In vivo, TRAM-34 reduced the thromboxane agonist-induced portal perfusion pressure. We conclude that inhibition of KCa3.1 with TRAM-34 downregulates fibrosis-associated gene expression in vitro, and reduces portal perfusion pressure in vivo. Thus, KCa3.1 may represent novel targets for the treatment of liver fibrosis. TRAM-34 also showed anti-neoplastic effects in human hepatocellular carcinoma cell lines by inhibition of the transcription factor NF-ĸB and by reduced expression of the estrogen receptor alpha. Further studies investigated effects of natural drugs on fibrosis and neoplasia. An extract from Lindera obtusiloba (LOE) showed protective effects in in vitro models of liver fibrosis and HCC in vitro. (+)-Episesamin was identified as an active drug in LOE. (+)-Episesamin showed the same protective effects on fibrosis and neoplasia as LOE. In summary, the results suggest further studies in specific animal models.

Antifibrotische Therapieansätze zielen sowohl auf die Verringerung der EZM-Synthese als auch den verstärkten EZM-Abbau, z.B. durch Matrix-Metalloproteinasen (MMPs). Kollagene als Hauptbestandteile der EZM modulieren indirekt durch die Speicherung von MMPs und Wachstumsfaktoren (WF) zelluläre Prozesse wie Proliferation, Migration und Differenzierung. Die gezielte Beeinflussung dieser Matrix-Bindung von MMPs oder WF stellt einen rationalen Ansatzpunkt für neue lokal wirkende, therapeutische Strategien dar. Voraussetzung dafür ist die Kenntnis des Bindungsverhaltens der biologisch wirksamen Moleküle an Kollagene aus der EZM. Ziel dieser Arbeit war daher die Bestimmung der Bindungsstärken der Gelatinasen und Kollagenasen an Kollagene bzw. Kollagenfragmente sowie die mögliche Modulation dieser Bindungen. Bindungsstudien wurden unter Einsatz der Surface Plasmon Resonance-Methode durchgeführt. MMP-Kollagen-Wechselwirkungen wurden zusätzlich in situ auf Gewebeschnitten der Leber, in enzymatischen Aktivitätsassays sowie in vitro mit hepatischen Sternzellen untersucht. Die Dissoziationskonstanten (KD-Werte) der Bindungen der Gelatinasen proMMP-2 und -9 an tripelhelikales Kollagen I (KI) lagen im Bereich von KD ~ 10-7 M. Die Bindung der aktivierten MMPs war um den Faktor 4 schwächer. Die proMMP-2-Bindung an das synthetische, tripelhelikale Kollagenmimetikum (GPO)10 war ca. 100fach stärker als die proMMP-2-Bindung an KI. (GPO)10 schwächte die proMMP-2-Kollagenbindung kompetitiv, was zu Freisetzung zuvor Kollagen-gebundener MMPs führte. (GPO)10 wirkte zusätzlich als Aktivator von proMMP-2. Die u.a. durch MMP-2 stimulierbare Proliferation hepatischer Sternzellen wurde durch (GPO)10 gesteigert. Die Kollagenasen proMMP-1, -3, -8 und proMMP-13 banden mit KD- Werten im Bereich von KD ~ 10-8 M an das nicht-Kollagenasesubstrat Kollagen VI (KVI). Die isolierten KVI-Einzelketten waren darüber hinaus als Kollagenase- Inhibitoren wirksam. Aus den Ergebnissen ergeben sich folgende Schlussfolgerungen: nicht-Substrat Kollagene wie die in der Leberfibrose verstärkt hochregulierten Kollagene I und VI wirken als Speicher für inaktive proMMPs der Gelatinasen bzw. Kollagenasen. Das synthetische Kollagenmimetikum (GPO)10 kann kompetitiv Kollagen-gebundene Proformen der Gelatinasen freisetzen und führt zu damit einhergehender proMMP-2 aktivierung. KVI-Ketten wirken als Kollagenase Inhibitoren. Die gezeigte Kollagenspeicherung von z.B. proMMP-2 impliziert einen Therapieansatz auf Basis lokaler proMMP-2-Freisetzung aus der Lebermatrix und gleichzeitiger proMMP-2-Aktivierung. Dies könnte einen Abbau fibrotischer Kollagenmatrix induzieren und damit die Fibroseresolution fördern. Der Einsatz der als Kollagenase-Inhibitoren wirksamen KVI-Ketten bzw. von strukturähnlichen, synthetischen Kollagenmimetika ist bei MMP-assoziierten, pathologischen Prozessen wie Tumormetastasierung denkbar., Liver fibrosis is characterized by accumulated extracellular matrix (ECM). Anti-fibrotic therapy therefore aimed at decreasing matrix synthesis and increasing ECM-degradation by matrix-metalloproteinases (MMPs). Since collagens which are the main ECM-components serve as storage site for MMPs and growth factors they therefore indirectly modulate cellular proliferation, migration and differentiation. A targeted modulation of this MMP/collagen interaction offers a rational approach for a local fibrolytic therapy. Evaluation of binding structures and domains together with binding strength and kinetics is pivotal. Aim of this work was to elucidate the binding strength of gelatinases and collagenases to non-substrate collagens, collagen derived fragments and collagen analogs. Kinetic parameters were obtained using the surface-plasmon-resonance method (Biacore). MMP/collagen interactions were then studied in situ using human cirrhotic liver slides and in enzymatic activity assays. Finally cellular effects using hepatic stellate cells were investigated. The dissociation-constants (Kd-values) of the gelatinases proMMP-2 and -9 bound to triple helical collagen type I (CI) were in the range of Kd ~ 10-7 M. Activated enzymes showed a 4fold weaker binding to CI. The binding strength of proMMP-2 to the synthetic triple helical collagen analog (GPO)10 was about 100fold stronger than binding to CI. (GPO)10 was shown to be a potent competitor of the proMMP-2-CI binding. The observed (GPO)10-induced activation of proMMP-2 lead to a strongly reduced binding to CI. MMP-2 mediated proliferation of hepatic stellate cells was induced after treatment with (GPO)10 implicating endogenous proMMP-2 activation by the synthetic collagen analog. The collagenases proMMP-1,-3,-8 and proMMP-13 bound to the non-substrate collagen type VI (CVI) with a Kd ~ 10-8 M. In enzymatic activity assays the isolated single chains of CVI functioned as collagenase-inhibitors. Based on these results we conclude that non-substrate collagens CI and CVI which are highly upregulated during liver fibrosis act as storage site for inactive proMMPs. The synthetic triple helical (GPO)10 could be used to competitively release collagen-bound proMMP-2 and -9 which is accompanied by proMMP-2 activation. In contrast, CVI single chains serve as collagenase inhibitors. The use of such synthetic collagen analogs such as (GPO)10 could become the basis of a local therapeutic antifibrotic strategy by releasing and activating collagen-stored progelatinases thereby initiating degradation of fibrotic neomatrix and fibrosis resolution. Since the CVI single chains act as collagenase inhibitors peptides or structural analogs should be explored as MMP-inhibitors in MMP-associated pathological processes like tumor metastasis.
