Abstract

This article discusses the differences between checkpoint inhibitor-induced bullous pemphigoid (irBP) and spontaneous bullous pemphigoid (BP). The study compared patients with irBP to patients with BP and found that irBP patients were younger, had a shorter duration of disease, and had a better therapeutic response. However, irBP patients required permanent discontinuation of ICI therapy in 78% of cases. The article provides clinical characteristics and treatment information for both irBP and BP patients. The study also identified differentially expressed genes in irBP compared to BP and normal skin tissue, suggesting potential targets for irBP treatment. The JAK-STAT signaling pathway was found to be enriched in both irBP and BP, indicating that JAK inhibitors could be potential treatment options. Abstract

This article explores the use of line-field confocal optical coherence tomography (LC-OCT) as a non-invasive method for diagnosing nail disorders. The study examined the characteristics of healthy nails and nails affected by different disorders, such as leukonychia, subungual haemorrhage, psoriasis, lichen planus, longitudinal melanonychia, subungual melanoma, and onychomycosis. LC-OCT was found to be effective in identifying these disorders by capturing images of the nail plate and surrounding skin. The findings of this study offer valuable insights for accurate and early diagnosis of nail disorders, potentially reducing the need for invasive biopsies. The article also discusses the potential of confocal laser scanning microscopy (LC-OCT) to improve the diagnosis of nail diseases, prevent unnecessary biopsies, and enhance patients' quality of life. However, further research is needed to validate these findings and evaluate the sensitivity and specificity of LC-OCT. [Extracted from the article]

Abstract

The interleukin (IL)-1 family of cytokines is a central regulator of a myriad of immunological responses. It comprises several cytokines, including those belonging to the IL-1, IL-36 and IL-18 subfamilies, as well as IL-33. The IL-1 family primarily plays a role in orchestrating innate immune responses, but is also involved in adaptive immunity. Increased interest in the IL-1 family occurred following the discovery that dysregulation of IL-1 signalling underlies the pathogenesis of several monogenic autoinflammatory diseases, characterized by sterile inflammation involving the skin and other organs. This also provided increased understanding of the role of innate immunity and the IL-1 family in polygenic autoinflammatory skin conditions, such as neutrophilic dermatoses, as well as in some of the most common chronic inflammatory skin diseases, such as psoriasis and hidradenitis suppurativa. Several therapeutic agents have been developed to inhibit the IL-1 family members and their signalling pathways. These have shown therapeutic efficacy in several chronic inflammatory skin disorders. The aim of this review is to thoroughly describe the consequences of pathological dysregulation of the IL-1, IL-33, IL-36 and IL-18 pathways in dermatological conditions and to provide a forward-looking update on therapeutic strategies targeting signalling by IL-1 family cytokines.

Abstract

Optical coherence tomography (OCT) has been able to establish itself in recent years not only in academic-scientific, but also in everyday dermatological practice. Its focus lies on epithelial tumors of the skin, which can be diagnosed intuitively and within a few seconds. Thus, basal cell carcinomas, actinic keratoses, and different stages of field cancerization can be diagnosed and monitored for response to therapy or possible recurrence. This often helps to avoid invasive sample extraction. Recently, the field of OCT and its latest advancement, dynamic OCT (D-OCT), has been expanded to include non-oncologic dermatological diseases. This encompasses inflammatory dermatoses and the analysis of physiological skin parameters such as hydration. Thanks to automated vascular imaging and the measurement of objective parameters such as epidermal thickness, blood flow at depth, optical attenuation coefficient, and skin roughness, more and more characteristics of the skin can be studied in a noninvasive and standardized way. New potential areas of application are eczema, contact allergic dermatitis, psoriasis, rosacea, telangiectasia, acute and chronic wounds, melasma and nevus flammeus but also melanocytic lesions.Die optische Kohärenztomographie (OCT) konnte sich in den letzten Jahren nicht nur im akademisch-wissenschaftlichen, sondern auch im dermatologischen Alltag etablieren. Hierbei liegt ihr Hauptfokus auf epithelialen Hauttumoren, deren Diagnostik intuitiv und innerhalb weniger Sekunden gelingt. Dadurch können Basalzellkarzinome, aktinische Keratosen und unterschiedliche Stufen der Feldkanzerisierung diagnostiziert und auf Therapieansprechen oder ein eventuelles Rezidiv überwacht werden. Dies ermöglicht häufig einen Verzicht auf eine invasive Probeentnahme. Seit Kurzem wurde das Einsatzgebiet der OCT und ihrer neuesten Weiterentwicklung, der dynamischen OCT (D-OCT), um weitere nichtonkologische dermatologische Krankheitsbilder, vor allem entzündliche Dermatosen, erweitert. Analysiert werden können ebenso physiologische Hautparameter, wie die Hydratisierung. Dank der automatischen Gefäßdarstellung und der Messung objektiver Parameter, wie Epidermisdicke, Blutfluss in unterschiedlichen Tiefen, Abschwächungskoeffizient und Rauheit, können immer mehr Charakteristika der Haut nichtinvasiv und standardisiert untersucht werden. Neue potenzielle Einsatzgebiete sind Ekzeme, kontaktallergische Dermatitis, Psoriasis, Rosazea, Teleangiektasien, akute wie chronische Wunden, Melasma und Naevus flammeus, aber auch melanozytäre Läsionen.

25. The effectiveness of an extra-curricular lecture for STI prevention and sexual education.

Abstract

While the incidence of infections with the human immunodeficiency virus largely remained unchanged in Germany, an increase of other sexually transmitted infections (STIs) was observed. The aim was to analyse the effectiveness of our sexual education lecture for students in improving the awareness, knowledge and prevention of STIs. We conducted a cross-sectional survey after students had attended our extra-curricular lecture at the Department of Dermatology of the Ludwig-Maximilians-University of Munich, Germany (LMU). We compared the data with a previously performed study in which the same survey was carried out before the lecture had started. A total of 5866 questionnaires were included in the analysis. After attending the lecture significantly more students were aware of STIs (syphilis: 36.8% (before) vs. 63.5% (after); chlamydia: 30.5% vs. 49.3%; gonorrhoea: 22.4% vs. 38.2%; human papillomaviruses (HPV): 17.7% vs. 30.2%), the transmission pathways of STIs (oral: 36.6% vs. 82.6%; vaginal: 81.8% vs. 97.3%; anal: 42.8% vs. 94.0%; penile: 68.7% vs. 92.1%), knew that the HPV vaccination is directed against a virus (36.8% vs. 56.9%) and were interested in receiving a vaccination (57.7% vs. 78.8%). This study demonstrates the positive educative effects of our lecture for awareness and improved knowledge of STIs. To satisfy the need for a comprehensive sexual education, a combination of school and health facility-based programmes should be implemented as one single lecture cannot convey the entire information about STIs. Abstract

Background Following the first investigational study on the use of extracorporeal photopheresis for the treatment of cutaneous T‐cell lymphoma published in 1983, this technology has received continued use and further recognition for additional earlier as well as refractory forms. After the publication of the first guidelines for this technology in the JEADV in 2014, this technology has maintained additional promise in the treatment of other severe and refractory conditions in a multidisciplinary setting. It has confirmed recognition in well‐known documented conditions such as graft‐vs.‐host disease after allogeneic bone marrow transplantation, systemic sclerosis, solid organ transplant rejection including lung, heart and liver and to a lesser extent inflammatory bowel disease. Materials and methods In order to further provide recognized expert practical guidelines for the use of this technology for all indications, the European Dermatology Forum (EDF) again proceeded to address these questions in the hands of the recognized experts within and outside the field of dermatology. This was done using the recognized and approved guidelines of EDF for this task. All authors had the opportunity to review each contribution as it was added. Results and conclusion These updated 2020 guidelines provide at present the most comprehensive available expert recommendations for the use of extracorporeal photopheresis based on the available published literature and expert consensus opinion. The guidelines were divided into two parts: PART I covers Cutaneous T‐cell lymphoma, chronic graft‐vs.‐host disease and acute graft‐vs.‐host disease, while PART II will cover scleroderma, solid organ transplantation, Crohn’s disease, use of ECP in paediatric patients, atopic dermatitis, type 1 diabetes, pemphigus, epidermolysis bullosa acquisita and erosive oral lichen planus.
