La capacité d'offrir des soins du cancer de qualité dépend en grande partie de l'accessibilité des services à ceux qui en ont besoin. Dans l'état actuel des choses, on constate des disparités en matière d'accès aux services de cancérologie au Canada, ce qui constitue un problème sur le plan de l'éthique. Le présent article fait ressortir les points de vue éthiques et stratégiques liés à l'équité dans l'accès aux soins du cancer au Canada. The ability to provide quality cancer care largely depends on how accessible the services are to those in need. In the current state, disparities exist in access to Canadian cancer services, and this poses an ethical challenge. This article highlights ethical and strategic considerations related to equity in access to Canadian cancer care. Lipoma arborescens is rare and usually found in the knee. We report two cases in the bicipital bursa of the elbow that showed characteristic findings on magnetic resonance imaging with contrast enhancement.

A review of 43 patients with soft-tissue tumours identified from a tumour registry showed a 23% rate of incorrect initial diagnosis by ultrasound imaging. Seven patients, five with malignant tumours, suffered a delay in diagnosis (ranging up to 6 months) as a result. The most common error was to mistake a solid tumour for a haematoma. Caution is recommended in the interpretation of ultrasound of soft-tissue masses, particularly with regard to the erroneous diagnosis of haematoma.

Specific physical and chemical agents found in the workplace and in the general environment are responsible for characteristic pathologic processes within the nervous system. It has been shown that many neurotoxic agents produce a dose-related spectrum of impairment ranging from mild slowing of nerve conducting velocity or prolongation in reaction time to neuropathy and frank encephalopathy. Clinical manifestations are determined by the agent involved, by the dose of exposure, the vulnerability of the cellular target, the ability of the organism to metabolize and excrete the agent, and the ability to repair damage. An occupational history, including evaluation of evidence of specific agents and job history, is a critical component in the clinical management of individuals with suspect neurotoxic disease. Environmentally-induced disorders can be prevented by appropriate environmental controls. Prevention of neurotoxic disease is a complex process requiring continuous involvement of public health agencies and strong scientific research.

Recent evidence from epidemiologic studies conducted in several areas of the United States shows an association of excess risk of asthmatic attacks with elevated levels of suspended sulfates within specific temperature ranges. These findings are disscussed in the context of experimental animal studies which tend to support these observations.

The indoor environment has frequently been ignored as a significant source of exposure to air pollutants. To date there are a number of documented examples of levels of indoor air pollutants greatly exceeding those levels which commonly occur in the outdoor environment. Among these instances are airborne buildup of polynuclear aromatics and cadmium from cigarette smoke, lead from burning candles, and vinyl chloride from use of aerosols containing this substance as a propellant. These examples suggest that there may be additional sources of indoor air pollutants, particularly halogenated hydrocarbons from aerosol products, which have heretofore not been generally recognized as important. The present paper endeavors to review those instances where halogenated hydrocarbons in the indoor air environment may build up to concentrations of potential public health concern. These considerations may be especially relevant in future years as increasing efforts are being made to insulate buildings more efficiently as a means to conserve energy. The available data strongly suggest that halogenated hydrocarbons are an important class of air pollutants in the indoor environment and that their presence in the outdoor environment should also be carefully examined. In this regard, halogenated hydrocarbons in the outdoor environment may also contaminate indoor air spaces.
