Abstract

Laut aktuellem Forschungsstand steht Cannabismissbrauch mit erhöhten Erkrankungsraten an psychotischen Störungen, einem früheren Erkrankungsbeginn sowie häufigeren Rezidiven in Verbindung. Adoleszente mit Cannabisabusus weisen im Vergleich zu deren Cannabis-abstinenten Altersgenossen eine bis zu sechsfach höhere Anfälligkeit für psychotische Störungen auf, weshalb präventive Maßnahmen in dieser Gruppe besonders sinnvoll erscheinen. Im hier dargestellten Fall wird die Rolle von Cannabis als Risikofaktor für die Entwicklung psychotischer Symptome und für eine potenzielle Aggravierung des Krankheitsverlaufs am Beispiel eines jungen Patienten mit einem religiösen Wahn diskutiert. A great number of studies have consistently pointed to the association between cannabis abuse and higher rates of psychotic disorders, earlier age of psychosis onset, and an increase in the frequency of relapses. As adolescent cannabis users show a significantly increased risk for psychotic disorders in comparison to nonusers, preventive measures would be particularly important in this age group. In this case report, we will discuss cannabis use as a risk factor for the development of psychotic symptoms and for the potential aggravation of the course of psychotic disorders by providing an illustrative example of a young patient with a religious delusion.

21. Case report: Hyperactive delirium after a single dose of zolpidem administered additionally to psychopharmacotherapy including clozapine.

Abstract

The non-benzodiazepine hypnotic zolpidem is frequently administered as a short term psychopharmacotherapy for insomnia. Although it is well-established in a broad clinical routine and often well-tolerated, severe delirium and complex sleep behavior were reported in rare cases. Hereby, it remains unclear whether zolpidem's potential for delirium might be enhanced when combined with further psychopharmacotherapeutics. The present case report portrays a young male Caucasian inpatient with schizoaffective disorder, who was admitted due to severe hyperactive delirium after a single dose of zolpidem 10mg that was administered in addition to already established psychopharmacotherapy including clozapine 200 mg/day, aripiprazole 15 mg/day and cariprazine 4.5 mg/day. In detail, disorientation, agitation, confabulations, bizarre behavior, and anterograde amnesia occurred shortly after ingestion of zolpidem and gained in intensity within a couple of hours. Once zolpidem was discontinued, the abovementioned symptoms subsided completely and did not reoccur. Since a clear temporal association could be drawn between the intake of zolpidem and the onset of hyperactive delirium, the present clinical experience should serve as a cautionary note for combining potent sedative-hypnotics and substances with anticholinergic properties, even in young adults in a good general condition. Moreover, our case argues for the necessity of further research into the pathomechanism of the interaction potential of non-benzodiazepines as zolpidem, especially with substances exerting anticholinergic properties, which are known for their potential to precipitate delirium. Abstract

Earth is a complex dynamical system exhibiting continuous change in its atmosphere, ocean,and surface elements. Nearly all (99.97%) of the energy driving these systems is linked to the Sun. Measurements of reflected sunlight contain a unique signature of wavelength-specific scattering and absorption interactions occurring between incoming solar energy and atmospheric (molecules, aerosols,clouds) and surface features Clouds can affect significantly both shortwave and long wave radiation, depending on altitude/vertical structure, thermodynamic phase, and optical properties. Low, warm, and optically thick clouds predominantly have a cooling effect, while high, cold, optically thin clouds can cause warming by absorbing warmer radiation emitted from the surface and lower atmosphere.When the net difference between outgoing and incoming solar radiation is matched by the net infrared radiation emitted to space, the Earth's climate is in radiative balance. While radiative forcing components (GHGs, aerosols - direct and indirect) contribute to a net radiative imbalance, climate sensitivity is ultimately determined by the contribution of various system feed backs. The role of cloud feedback in a warming climate is currently the largest inter-model uncertainty in climate sensitivity and therefore in climate prediction [Bony and Dufresne 2005]. A comprehensive understanding of current cloud propertiesand dynamic/microphysical processes requires a global perspective from satellites.

24. Predictors of hospital readmission for patients diagnosed with delirium: An electronic health record data analysis.

Abstract

Introduction: Delirium is an acute and fluctuating change in attention and cognition that increases the risk of functional decline, institutionalisation and death in hospitalised patients. After delirium, patients have a significantly higher risk of readmission to hospital. Our aim was to investigate factors associated with hospital readmission in people with delirium. Methods: We carried out an observational retrospective cohort study using linked mental health care and hospitalisation records from South London. Logistic regression models were used to predict the odds of 30‐day readmission and Cox proportional hazard models to calculate readmission risks when not restricting follow‐up time. Results: Of 2814 patients (mean age 78.9 years SD ±11.8) discharged from hospital after an episode of delirium, 823 (29.3%) were readmitted within 30 days. Depressed mood (odds ratio (OR) 1.34 (95% confidence interval (CI) 1.08–1.66)), moderate‐to‐severe physical health problems (OR 1.67 (95% CI 1.18–2.2.36)) and a history of serious circulatory disease (OR 1.29 (95% CI 1.07–1.55)) were associated with higher odds of hospital readmission, whereas a diagnosis of delirium superimposed on dementia (OR 0.67 (95% CI 0.53–0.84)) and problematic alcohol/substance (OR 0.54 (95% CI 0.33–0.89)) use were associated with lower odds. Cox proportionate hazard models showed similar results. Conclusion: Almost one‐third of patients with delirium were readmitted within a short period of time, a more detailed understanding of the underlying risk factors could help prevent readmissions. Our findings indicate that the aetiology (as alcohol‐related delirium), the recognition that delirium occurred in the context of dementia, as well as potentially modifiable factors, as depressed mood affect readmission risk, and should be assessed in clinical settings. Abstract

Background: Sex-related effects on the evolution and phenotype of major depressive disorder (MDD) were reported previously. Methods: This European multicenter cross-sectional study compared sociodemographic, clinical, and treatment patterns between males and females in a real-world sample of 1410 in- and outpatients with current MDD. Results: Male MDD patients (33.1%) were rather inpatients, suffered from moderate to high suicidality levels, received noradrenergic and specific serotonergic antidepressants (ADs) as first-line AD treatment, generally higher mean AD daily doses, and showed a trend towards a more frequent administration of add-on treatments. Female MDD patients (66.9%) were rather outpatients, experienced lower suicidality levels, comorbid thyroid dysfunction, migraine, asthma, and a trend towards earlier disease onset. Conclusions: The identified divergencies may contribute to the concept of male and female depressive syndromes and serve as predictors of disease severity and course, as they reflect phenomena that were repeatedly related to treatment-resistant depression (TRD). Especially the greater necessity of inpatient treatment and more complex psychopharmacotherapy in men may reflect increased therapeutic efforts undertaken to treat suicidality and to avoid TRD. Abstract

The Moderate Resolution Imaging Spectroradiometer (MODIS) is one of five instruments aboard the Terra Earth Observing System (EOS) platform launched in December 1999. After achieving final orbit, MODIS began earth observations in late February 2000 and has been acquiring data since that time. The instrument is also being flown on the Aqua spacecraft, launched in May 2002. A comprehensive set of remote sensing algorithms for cloud detection and the retrieval of cloud physical and optical properties have been developed by members of the MODIS atmosphere science team. The archived products from these algorithms have applications in climate change studies, climate modeling, numerical weather prediction, as well as fundamental atmospheric research. In addition to an extensive cloud mask, products include cloud-top properties (temperature, pressure, effective emissivity), cloud thermodynamic phase, cloud optical and microphysical parameters (optical thickness, effective particle radius, water path), as well as derived statistics. We will describe the various algorithms being used for the remote sensing of cloud properties from MODIS data with an emphasis on the pixel-level retrievals (referred to as Level-2 products), with 1-km or 5-km spatial resolution at nadir. An example of each Level-2 cloud product from a common data granule (5 min of data) off the coast of South America will be discussed. Future efforts will also be mentioned. Relevant points related to the global gridded statistics products (Level-3) are highlighted though additional details are given in an accompanying paper in this issue.
