1. A Whispering Gallery Mode based sensor platform for single enzyme real time conformational changes

Abstract

Understanding the conformational changes that occur in an enzyme during turnover is crucial for understanding biomolecular processes and for new rational drug discovery. Many single-molecule techniques have been developed in the past three decades, such as Förster resonance energy transfer, although powerful, these techniques have their limitations, for example, limited temporal resolution, or necessity for fluorescent labelling. In this study a new class of highly sensitive optical devices based on the whispering gallery mode sensor have been investigated and employed to track the conformational changes of a kinase enzyme in real time. A thermostable Geobacillus stearothermophilus phosphoglycerate kinase enzyme has been used as model enzyme to prove that the optoplasmonic whispering gallery mode sensor can detect the conformational changes involved in enzyme turnover. The enzyme was expressed and purified to a high degree of purity that was suitable for crystallographic studies as well as biochemical investigation. The purified enzyme has been used for kinetics and biophysical enzymatic characterisation. A reproducible protocol has been established to immobilise the enzyme onto gold nanoparticles in a defined orientation to track the conformational changes occurring on the sensor. The enzyme has been immobilised onto the gold nanoparticles using two different methods: through the his-tags introduced onto the recombinant enzyme and directly onto the gold through a surface cysteine residue introduced by site-directed mutagenesis. The kinase enzyme was found to retain activity using both methods. To rationalise the observed conformational changes occurring during enzymatic turnover, the crystal structures of the native enzyme and its complexes with the substrate, and with substrate together with non-hydrolysable ATP, have been determined to high resolution, between 1.2 Å and 2 Å. Finally, for the first-time a real-time visualisation of the movement of the phosphoglycerate kinase during enzymatic turnover has been recorded. Repeating signals from the sensor were registered and only observed when both the substrate 3-phosphoglycerate and ATP are introduced with the enzyme into the sensor chamber. No signal is observed with the 3-phosphoglycerate alone or ATP alone or when 3-phosphoglycerate and a non-hydrolysable ATP analogue were used with the enzyme. The sensor system presented in this thesis shows potential for future fast, real-time, rapid throughput, lab-on-chip measurements for studying single enzymes.

Abstract

Background: Previous studies on oxaliplatin and fluoropyrimidines as adjuvant therapy in older patients with stage III colon cancer (CC) produced conflicting results. Patients and methods: We assessed the impact of age on time to tumour recurrence (TTR), disease-free survival (DFS), cancer-specific survival (CSS), and overall survival (OS) in 2360 patients with stage III CC (1667 aged <70 years and 693 ≥ 70 years) randomised to receive 3 or 6 months of FOLFOX or CAPOX within the frame of the phase III, TOSCA study. Results: Older patients compared with younger ones presented more frequently an Eastern Cooperative Oncology Group performance status equal to 1 (10.5% vs 3.3%, p < 0.001), a greater number of right-sided tumours (40.9% vs 26.6%, p < 0.001), and were at higher clinical risk (37.2% vs 33.2%, p = 0.062). The treatments were almost identical in the two cohorts (p = 0.965). We found a greater proportion of dose reductions (46.7% vs 41.4%, p = 0.018), treatment interruptions (26.1% vs 19.3%, p < 0.001) and a higher proportion of recurrences (24.2% vs 20.3%, p = 0.033) in the older patients. The multivariable analysis of the TTR did not indicate a statistically significant effect of age (hazard ratio [HR]: 1.19; 95% confidence interval [CI]: 0.98–1.44; p = 0.082). The HR comparing older with younger patients was 1.34 (95% CI: 1.12–1.59; p = 0.001) for DFS, 1.58 (95% CI: 1.26–1.99; p < 0.001) for OS, and 1.28 (95% CI: 0.96–1.70; p = 0.089) for CSS. Conclusions: Worse prognostic factors and reduced treatment compliance have a negative impact on the efficacy of oxaliplatin-based adjuvant therapy in older patients.

Abstract

Gastrointestinal stromal tumors (GISTs), tumors characterized by c-KIT mutations, are the most frequent mesenchymal tumors of the digestive tract. The stomach is the most commonly involved site. Localization, size and mitotic rate are reliable predictors of survival and the two milestones of GISTs treatment are surgery and imatinib. This article is aimed to report the data of an audit, carried out on the morphological and clinical aspects of the disease and to review the present knowledge on GISTs. A total of 172 patients with GISTs (M : F=1 : 1; mean age 65 years) were recruited. The stomach was the most frequently involved site. In 50% of the cases the tumor was smaller than 5 cm, whereas major symptoms were observed in 43% of the cases. Predictors of progressive disease were present only in a small percentage of cases but the disease was in the metastatic phase in over 25% of the cases at diagnosis. Familial aggregation was rare but a consistent share of the patients (21%) had other synchronous or metachronous cancers. The most frequent mutations were in-frame deletions and point mutations of c-KIT exon 11. This report confirms in part the available data on GIST in a consecutive series of patients recruited in Italy and shows that only large collaborative multicenter studies provide data sound enough to enable making reasonable clinical and therapeutic choices, and suggests that, as a measure of secondary prevention, a diagnostic definition should be obtained in all submucosal lesions of the GI tract and that GIST patients should be screened for second tumors.

Abstract

BACKGROUND. The aim of the current randomized Phase II study was to investigate the efficacy and safety of capecitabine combined with irinotecan as first-line treatment in metastatic colorectal carcinoma (CRC). METHODS. A total of 140 patients received capecitabine at a dose of 1250 mg/m2 twice daily on Days 2-15 and irinotecan at a dose of either 300 mg/m2 on Day 1 (Arm A) or 150 mg/m2 on Days 1 and 8 (Arm B) every 3 weeks. During the course of the study, enrollment was continued using lower doses of capecitabine (1000 mg/m2 twice daily) and irinotecan (Arm A: 240 mg/m2; Arm B: 120 mg/m2) to improve the safety profile of the combinations. RESULTS. Efficacy was evaluable in 134 patients (68 in Arm A, 66 in Arm B). Objective responses were observed in 46% of the patients (8% complete response [CR]), including 47% in Arm A (9% CR; 38% partial response [PR]) and 44% in Arm B (8% CR; 36% PR). The median progression-free survival was 8.3 months in Arm A and 7.6 months in Arm B. Among the first 52 patients treated with the higher doses, the most frequent Grade 3-4 adverse event was diarrhea (27%). The lower doses adopted in the subsequent 88 patients led to better diarrhea control, particularly in Arm A, and significant reductions in the incidence of all-grade hand-foot syndrome and abdominal pain. CONCLUSIONS. The capecitabine and irinotecan combination was a highly active first-line therapy in metastatic CRC. An acceptable safety profile was observed after dose reduction, particularly when irinotecan was administered on 1 day.

Abstract

Carcinoma, or cancer, of the vestibule (entrance) of the nose is very infrequently found and is often diagnosed incorrectly or late, when it is already advanced. The tumor often grows so that most of it is hidden beneath the skin surface and only a small portion of it is visible. It usually appears as a crusty lesion that is initially treated with ointments; this delays reaching the correct diagnosis and beginning treatment. While the usual form of treatment is radiotherapy, this report shows the value of treatment with chemotherapeutic agents administered through an artery prior to radiotherapy. The authors report on 12 patients treated during the years 1975 to 1987 who had biopsy-proven cases of cancer of the nasal vestibule. The group consisted of 11 men and 1 woman. The average age of the patients was 65 years, with a range from 50 to 82 years. Ten of the patients were smokers and five normally drank at least 1 liter of wine daily. All patients had a painful ulcer at the site of the tumor, which was their principle symptom. Most small tumors were treated by simple resection (surgical removal) while those which were more advanced were treated by chemotherapy followed by radiotherapy. The chemotherapeutic agent was delivered by a catheter placed in the artery which served the area of the tumor. The drug was slowly delivered over a treatment period of several day in several cycles. It was concluded that the treatment approach used appears effective at reaching the best possible rate of cure with the maximum of function retained.
