5. Age-related changes in the optics of human eye with accommodation

Abstract

PURPOSE: The mechanism of accommodation together with its age-related changes is complex and still not fully understood. The objective of this PhD thesis is to extend this knowledge and evaluate the accommodative performance with some options for presbyopia correction available at present. METHODS: The latest technology was used to assess the accommodative function both from a structural and optical point of view. Corneal and ciliary muscle changes with accommodation were evaluated in the first two experimental chapters. Chapter 3 combined imaging data of the ciliary muscle with information of the optical response to different accommodative demands in an age divided cohort, whose accommodative response was analysed in a static and dynamic way in Chapter 4. Fluctuations in accommodation and dissaccommodation and time constants were measured in natural conditions. Additional dynamic (Chapter 4) and static (Chapter 5) assessment of the accommodative performance was done for young subjects fitted with multifocal contact lenses (MFCLs). RESULTS and CONCLUSION: Cornea is stable during accommodation while the ciliary muscle thickness varies at different accommodative demands but not as a consequence of ageing. Microfluctuations in accommodation show highly intra-subject variability, but a trend to play a role in accommodation control can be observed. In general, a bigger magnitude of fluctuations is found when accommodation is more accurate. Ageing has a significant effect on the accommodative fluctuations when a small stimulus (2.5 D) is presented but not on time constants. Subjects in the third decade of life present the highest magnitude of accommodative fluctuations as their accommodative system is more stressed. Multifocal contact lenses fitted in young subjects do not affect the accommodative dynamics. Some improvements have been seen when different generations of multifocal contact lenses are assessed. The applicability of this research to the improvement of solutions capable of restoring the accommodative ability in older eyes, widens the future scope of this research.

Abstract

Introduction: Localized bullous pemphigoid (LBP) is an infrequent bullous pemphigoid (BP) variant restricted to a body region. According to the most compelling evidence, LBP occurs in patients with pre-existent serum antibodies against the basement membrane zone, which occasionally acquire the capacity to induce disease after the influence of different local factors acting as triggers. Methods: We hereby present a multicenter cohort of 7 patients with LBP developed after local triggers: radiotherapy, thermal burns, surgery, rosacea, edema and a paretic leg. In addition, we conducted a review of the literature, and we propose a set of diagnostic criteria for LBP, also based on our case series and the 2022 BP guidelines from the European Academy of Dermatology and Venereology. Results: During follow-up, three of the patients from our series evolved to a generalized BP, with only one requiring hospitalization. Our literature search retrieved 47 articles including a total of 108 patients with LBP, with a 63% with a potential local precipitating factor previous to their diagnosis. LBP mostly affected older females, and a subsequent generalized progression occurred in 16.7% of the cases. The most frequently involved areas were the lower limbs. Radiation therapy and surgery were responsible for the inducement of nearly 2 in 3 cases of LBP. We observed a significantly higher risk of generalization in cases where the trigger led to the developing of LBP earlier (p=0.016). Our statistical analysis did not detect any other prognosis factor for generalization when assessing direct immunofluorescence, histological and serological results, or other patient related factors. Conclusion: LBP should be suspected in patients with recurrent localized bullous eruptions. The presence of a trauma history in the same anatomic area is reported in most cases. Abstract

BACKGROUND: To date there are yet no available approved therapies for Geographic Atrophy (GA) secondary to age-related macular degeneration (AMD). METHODS: Single site, non-randomized safety and efficacy study presenting the preliminary results in a cohort of five late stage AMD (GA) patients successfully implanted with the Argus II Retinal Prosthesis System (Second Sight Medical Products Inc., Sylmar, CA, USA). Extensive fundus imaging including retinal photographs from which the GA area was measured. A combination of custom and traditional tests designed for very low vision subjects assessed visual function in study subjects. A Functional Low-Vision Observer Rated Assessment was carried out to evaluate the impact of the system on the subject's daily life. In addition, a study to evaluate structural characteristics of the visual cortex of the brain was performed in one subject using magnetic resonance imaging. RESULTS: Seven device-related adverse events were reported, four of which were classed as serious adverse events. Retinal detachment was reported in three patients and was successfully treated within 12 months of onset. Testing showed an improvement in visual function in three of five patients with the system turned on. Magnetic resonance imaging assessed in one patient after implantation indicates a selective increase in cortical myelin and thickness in visual brain regions 1 year post implantation. CONCLUSIONS: Epiretinal prostheses can successfully be implanted in those affected by GA secondary to late-stage AMD and can elicit visual percepts by electrical stimulation of residual neuroretinal elements and improve basic visual function in those affected.

Abstract

Purpose: To report on the short-term visual and refractive outcomes of laser-assisted in situ keratomileusis (LASIK) for the correction of myopia carried out with a VisuMax femtosecond laser (for flap creation) and a MEL 90 excimer laser (for ablation) (Carl Zeiss Meditec, Jena, Germany). Methods: A retrospective non-comparative case series study was carried out analysing 382 myopic LASIK procedures (191 patients) treated at a single site (Optegra Eye Hospital, London, UK) by a single surgeon using the VisuMax femtosecond laser for flap creation and a MEL 90 excimer laser (Carl Zeiss Meditec). A triple-A ablation profile was selected at 500 Hz for all treatments and the inbuilt nomogram was selected. Attempted spherical equivalent refraction and cylinder ranged from -0.50 D to -11.00 D and 0.00-5.00 D, respectively. Outcomes were analysed at the last postoperative visit using internationally agreed methods for reporting refractive surgery outcomes. Results: There were no operative complications. Postoperative assessments were typically carried out during the first 3 months after surgery. Visually insignificant postoperative complications, such as dry eye, were noted in 84 eyes (22%). A total of 353 eyes (92%) achieved 6/6 or better unaided distance visual acuity (UDVA) postoperatively compared with 370 eyes (97%) presenting with 6/6 corrected distance visual acuity (CDVA) preoperatively. In all, 359 eyes (94%) had a UDVA postoperatively within one line of preoperative CDVA. No eyes lost two or more lines of CDVA. Postoperative mean spherical equivalent refraction was within +0.50 D in 349 eyes (91%) and within +1.00 D in 376 eyes (98%). Conclusions: LASIK with the VisuMax femtosecond laser and MEL 90 excimer laser using an aspherically optimised ablation profile that also focuses on minimal tissue removal (triple-A ablation profile) at 500 Hz pulse rate is a safe, predictable and efficacious method for the correction of myopia up to -11.00 D and cylinder up to 5.00 D. Abstract

Adalimumab is the only approved biological therapy for hidradenitis suppurativa (HS). The last published recommendations support the use of other off-label biologic therapies. We report on a multicentric retrospective review of patients with HS treated with an ustekinumab dosing schedule of intravenous infusion adjusted by weight, followed by a subcutaneous maintenance dose of 90 mg every 8 weeks, as recently approved for Crohn's disease. The minimal follow-up period required for inclusion was 16 weeks. A total of 14 patients from six hospitals were included. In 50% of the treated patients, therapeutic outcomes, measured by means of the Hidradenitis Suppurativa Clinical Response (HiSCR) and decrease of Dermatology Life Quality Index (DLQI) and visual analog scale (VAS) of pain, were reached at week 16. In 71.42% of patients DLQI and VAS of pain improved, irrespective of achievement of HiSCR. Two patients abandoned treatment due to lack of efficacy or patient preferences. No ustekinumab-related adverse effects were reported. The results are limited by the retrospective nature of the study, the short follow-up period, and the small patient number. This therapeutic regime proved to be safe and showed moderate efficacy in treating HS with failure to previous biologic therapy. Ideally, the efficacy of ustekinumab in HS should be tested in randomized and controlled clinical trials. [ABSTRACT FROM AUTHOR]
