Abstract

BACKGROUND:Programmed cell death ligand-1 (PD-L1) expression on tumor cells (TCs) is associated with improved survival in patients with head and neck squamous cell carcinoma (HNSCC) treated with immunotherapy, although its role as a prognostic factor is controversial. This study investigates whether tumoral expression of PD-L1 is a prognostic marker in patients with recurrent and/or metastatic (R/M) HNSCC treated with standard chemotherapy. METHODS:This retrospective, multicenter, noninterventional study assessed PD-L1 expression on archival R/M HNSCC tissue samples using the VENTANA PD-L1 (SP263) Assay. PD-L1 high was defined as PD-L1 staining of ≥ 25% TC, with exploratory scoring at TC ≥ 10% and TC ≥ 50%. The primary objective of this study was to estimate the prognostic value of PD-L1 status in terms of overall survival (OS) in patients with R/M HNSCC. RESULTS:412 patients (median age, 62.0 years; 79.9% male; 88.2% Caucasian) were included from 19 sites in seven countries. 132 patients (32.0%) had TC ≥ 25% PD-L1 expression; 199 patients (48.3%) and 85 patients (20.6%) had TC ≥ 10% and ≥ 50%, respectively. Bradycardia, atrial tachyarrhythmia, heart failure, residual shunts, and pulmonary hypertension (PH) are significant problems after congenital heart disease surgery. We performed His‐bundle pacing (HBP) for drug‐resistant PH with bradycardia in a woman post‐total anomalous pulmonary venous return repair complicated by bradycardia, hypertension, PH, residual atrial shunts, and heart failure. Significant improvement in PH post‐HBP was achieved without exacerbation of left ventricular function. We successfully closed the atrial shunts 12 months after HBP. [ABSTRACT FROM AUTHOR]

Abstract

ScAlMgO4 (SAM) substrates have attracted significant attention as template substrates for fabricating bulk GaN crystals. Radio‐frequency plasma‐assisted molecular beam epitaxy (RF‐MBE) is preferred for the growth of the first GaN layer. However, the metastable zincblende (ZB) phase is mixed on the SAM substrate with a smaller terrace width, coupled with lower growth temperatures in MBE. Herein, the time evolution of GaN growth on SAM substrates is investigated to elucidate the mechanism underlying ZB mixing. ZB‐GaN is generated as stacking faults during the coalescence of the initially grown wurtzite‐ (WZ‐) GaN islands on adjacent SAM terraces, owing to the extraordinarily large step height close to three bilayers of WZ‐GaN. Therefore, high‐temperature growth on a SAM substrate with wider terraces suppresses the generation of the ZB phase in the initial growth stage. ZB‐GaN also decreases with an increase in the growth time. Based on these results, a 940 nm‐thick GaN layer is grown at 750 °C on the SAM substrate with a terrace width of 820 nm. A pure WZ‐GaN layer from the GaN/SAM interface with a flat surface with a root mean square value of below 1 nm and a dislocation density of 7.3 × 109 cm−2 is obtained. [ABSTRACT FROM AUTHOR]

Abstract

Semiconductor evaluation methods frequently require sample processing and carry the risk of defects and property changes. Therefore, nondestructive and noncontact electrical property measurement techniques are necessary. Terahertz time‐domain spectroscopic ellipsometry (THz‐TDSE) can simultaneously estimate the electrical properties and film thickness of a sample. However, whether this method can simultaneously determine electrical properties and film thickness for semiconductor thin films of 1 μm or less remains unclear. This study uses THz‐TDSE to evaluate the electrical properties and film thickness of a GaN thin film (≈0.8 μm thick) on a ScAlMgO4 substrate. Using the conventional analysis based on Fresnel's formula (FR model), which is used in the field of optics, uniquely determining the electrical properties and film thickness is not possible owing to multiple optimal solutions. Therefore, a model combining the characteristic impedance model used in the radio‐wave region with the FR model is employed in the analysis. The obtained values of electrical properties of the GaN thin film evaluated using THz‐TDSE are consistent with those using Hall effect measurement. Abstract

ScAlMgO4 (SAM) has attracted considerable attention as a substrate for growing InGaN template layers owing to its lattice matching with In0.17Ga0.83N. Furthermore, radio‐frequency plasma‐assisted molecular beam epitaxy (RF‐MBE) can be utilized to grow (In)GaN directly on SAM substrates because the growth is conducted at relatively low temperatures without reactive gases. This prevents the diffusion and desorption of the constituent elements of the SAM substrate. In this study, the substrate‐temperature dependence of the direct growth of InGaN on SAM substrates is investigated through RF‐MBE. In droplets are formed below 600 °C, whereas, the In content of the InGaN crystal decreases rapidly, and the surface roughness increases at ≈650 °C. These results are attributed to the acceleration of In desorption at higher temperatures. The improvement in crystal coherency moderates above 600 °C. Therefore, a nearly lattice‐matched In0.18Ga0.82N film is grown at an optimal temperature of 600 °C by adjusting the group‐III metal fluxes. The resulting film is continuous and relatively flat, with no droplets. The dislocation density with screw component is one order of magnitude lower than that of the lattice‐mismatched InGaN. The dislocation density of the InGaN film is comparable to that of a film grown by metal‐organic vapor‐phase epitaxy. [ABSTRACT FROM AUTHOR]

Abstract

Background and Aim: Effective treatment of lesions that develop in the irradiated area of head and neck squamous cell carcinoma is a major concern. This study aimed to clarify the efficacy and safety of endoscopic resection for such lesions. Methods: Among consecutive patients who underwent endoscopic resection for histologically proven head and neck squamous cell carcinoma between January 2014 and December 2021, those who received definitive radiotherapy/chemoradiotherapy before endoscopic resection were included in this single‐center, retrospective study. Short‐ and long‐term outcomes were evaluated. Results: Among 422 patients who underwent endoscopic resection for 615 lesions, 43 patients with 57 lesions were eligible. All 57 lesions were treated with endoscopic submucosal dissection and en bloc resection was achieved in all lesions. Grade 3 of Common Toxicity Criteria for Adverse Events v5.0 occurred in eight (19%) patients (dysphagia, seven; stricture, three; aspiration pneumonia, two; and pharyngeal necrosis, one [some cases overlapped]), but no grade ≥ 4 events occurred. Enteral nutrition by gastrostomy was temporarily required in two patients owing to dysphagia and laryngeal necrosis. During the median follow‐up of 40 (interquartile range, 29.5–61) months after endoscopic submucosal dissection for the lesions developed in the irradiated area, local recurrence and metachronous lesions developed in two (5%) and nine (21%) patients, respectively. However, total laryngectomies and tracheostomies were avoided in all patients. The 3‐year overall and disease‐specific survivals were 81% (95% confidence interval, 64%–91%) and 94% (95% confidence interval, 79%–99%), respectively. Abstract

Aim: C‐reactive protein (CRP) is both an inflammatory and prognostic marker in various cancers. This study aimed to elucidate the characteristics of CRP and the prognostic factors in patients who were administered with atezolizumab plus bevacizumab (ATZ + BEV) for unresectable hepatocellular carcinoma (HCC). Methods: A total of 213 patients who received ATZ + BEV for HCC from November 2020 to March 2023 at 15 hospitals were enrolled in this retrospective study. The prognosis was analyzed by subdividing the patients based on baseline characteristics, radiologic response, and treatment lines. Accuracy of survival prediction was assessed using CRP, alpha fetoprotein (AFP), C‐reactive protein and alpha fetoprotein in immunotherapy (CRAFITY), and Glasgow Prognostic Score. Results: Compared with patients with baseline CRP <1 mg/dL, those with baseline CRP ≥1 mg/dL (n = 45) had a significantly higher baseline albumin–bilirubin score and AFP levels, significantly lower disease control rate (62.2%), and significantly shorter median overall survival (hazards ratios 2.292; 95% confidence interval 1.313–5.107; log‐rank test, p < 0.001). Multivariate analysis identified CRP ≥1 mg/dL, AFP ≥100 ng/mL, and modified albumin–bilirubin grade as the significant prognostic factors. The baseline CRP, AFP, CRAFITY, and Glasgow Prognostic Score demonstrated higher discrimination for 1‐year survival prediction after first‐line ATZ + BEV administration, compared with beyond second line, with area under the receiver operating characteristic curves of 0.759, 0.761, 0.805, and 0.717, respectively. Conclusions: CRP was a significant biomarker in patients treated with ATZ + BEV for HCC. Elevated CRP levels may indicate aggressive cancer progression and potential resistance to ATZ + BEV therapy.
