2. Refusal of Subsequent Treatment in Patients With EGFR-mutant Non-small-cell Lung Cancer After Response to EGFR-TKIs

Abstract

Background/Aim: During the course of effective and long-term treatment with epidermal growth factor receptor-tyrosine kinase inhibitor (EGFR-TKI), some elderly patients might decline further treatment after EGFR-TKI. We conducted a study to try and understand the reasons for this treatment decision. Patients and Methods: We analyzed the medical records of all patients diagnosed with non-small-cell lung cancer with EGFR mutations between 2016 and 2021. Results: There were 108 patients who received EGFR-TKIs. Of these, 67 patients responded to TKI. These responding patients were divided into two groups according to whether they received subsequent TKI treatment. At their request, 24 patients (group A) did not receive further anticancer treatment following TKI. The other 43 patients (group B) received anticancer therapy following TKI. Progression-free survival in group A patients was significantly longer (median=18 months, range=1-67 months) than in group B patients. The reasons for not wanting subsequent treatment after TKI were older age, reduced general condition, deterioration of physical comorbid disease and dementia. Dementia was the most common reason for patients over 75 years of age. Conclusion: Some elderly patients with well-controlled disease might express their refusal of all subsequent anticancer therapy after TKIs. Medical staff should respond seriously to these requests.

Abstract

Background The Evaluation of pimobendan in dogs with cardiomegaly caused by preclinical myxomatous mitral valve disease (EPIC) study monitored dogs with myxomatous mitral valve disease (MMVD) as they developed congestive heart failure (CHF). Objectives To describe the changes in clinical and radiographic variables occurring as dogs with MMVD and cardiomegaly develop CHF, compared to similar dogs that do not develop CHF. Animals One hundred and thirty-five, and 73 dogs that did or did not develop CHF, respectively. Materials and methods The following variables were evaluated in 2 groups of dogs (dogs that did or did not develop CHF): Heart rate (HR), clinic respiratory rate (RR), home-measured resting respiratory rate (RRR), rectal temperature (RT), body weight (BW), and vertebral heart sum (VHS). Absolute value and rate of change of each variable were calculated for each day a dog was in study. Daily means were calculated and plotted against time. The onset of CHF or last visit before leaving the study were set as reference time points. Results The most extreme values and rate of change occurred in variables immediately before onset of CHF. Vertebral heart sum increased earliest. Heart rate, RR, and RRR also increased. Rectal temperature and BW decreased. Increases in RR and RRR were most extreme and occurred immediately before CHF. Conclusions and Clinical Importance Dogs with MMVD and cardiomegaly experience increases in HR, RR, RRR, and VHS, and decreases in BW and RT as they develop CHF. The variables with highest absolute change and rate of change were RR and RRR. These findings reinforce the value of RR and RRR as indicators of impending or incipient CHF. Abstract

Rheological properties for binary polyolefin blends composed of linear low-density polyethylene (LLDPE) and long-chain branched polypropylene (LCB-PP), in which LCB-PP is the dispersed phase, were studied. Even though LCB-PP was in the dispersed phase, the blend showed marked strain-hardening behavior in the transient elongational viscosity. The deformed LCB-PP droplets acted as “rigid fibers” during the elongational flow owing to its strain-hardening behavior. Consequently, the blend behaved like a fiber-dispersion system and thus exhibited the enhanced elongational viscosity, originated from excess deformation of LLDPE located between fibrous LCB-PP droplets under the elongational flow. Under shear flow, the primary normal stress difference was not enhanced by the addition of LCB-PP. This is reasonable because LCB-PP droplets oriented to the flow direction did not provide excess deformation of a continuous phase greatly.

45. Experimental and numerical study on transient elongational viscosity for PP/LDPE blends

Abstract

The transient uniaxial elongational viscosity for binary blends composed of polypropylene (PP) and low-density polyethylene (LDPE) was evaluated. A strain hardening behavior is detected for the blends, although LDPE is a dispersed phase. This behavior is attributed to LDPE dispersion deformation; the LDPE forms rigid fibers because of strain hardening. Rheological properties are calculated numerically by the Phan–Thien Tanner model by assuming a symmetric geometry with a periodic structure. Based on the simulation, we propose an appropriate LDPE to modify the processability of PP, at which the strain hardening in the elongational viscosity is required., identifier:https://dspace.jaist.ac.jp/dspace/handle/10119/16965
