Objectives The aim of this clinical observational study was to assess the efficacy of L-PRF as a hemostatic agent in patients under treatment with vitamin K antagonists (VKAs) or direct oral anticoagulants (DOACs). Materials and methods Patients under oral anticoagulant therapy (VKA or DOACs) who needed a single simple tooth extraction were enrolled. L-PRF plug was positioned inside the alveolus and secured with non-absorbable sutures. Surgical time, pain-VAS, paracetamol intake, intra-operative, post-operative biological complications, and bleeding events have been registered. Results A total of 112 patients (59 patients for DOAC and 53 for VKA group) were enrolled. Post-operative bleeding was recorded in nine patients (17%) for VKA group and nine patients (15.3%) for DOACs group. None of the patients needed a medical support for managing of bleeding. Seven days after surgery, no cases of post-extractive complications occurred. Conclusions The use of L-PRF resulted in limited mild late post-operative bleedings without the need of medical intervention. Clinical relevance The use of L-PRF can be adopted for an uneventful post-operative curse in anticoagulated patients without chasing their therapy for single tooth extraction.

A insuficiência renal crônica é uma doença progressiva caracterizada por uma destruição gradual dos néfrons e uma conseqüente redução da função renal. A doença renal em estágio terminal (DRT) requer terapia de substituição renal como diálise peritoneal, hemodiálise ou transplante. Os pacientes afetados pela DRT ou em hemodiálise correm risco de desenvolver uma série de comorbidades, incluindo hipertensão, anemia, risco de sangramento, suscetibilidade à infecção, efeitos colaterais dos medicamentos e manifestações orais associadas à própria doença e ao tratamento em hemodiálise. Nesse contexto, as doenças bucais representam uma causa potencial e evitável de maus resultados de saúde em pessoas com DRT devido à sua relação com infecção, inflamação e desnutrição.

The aim of this study is to evaluate if the risk of neurological injury to the inferior alveolar nerve (IAN) and the lingual nerve (LN), following the extraction of lower third molars are influenced by the anesthetic modality (local anesthesia (LA) vs. general anesthesia (GA)). A systematic search was performed through the PubMed, Scopus, Cochrane Library, an Web of Science databases; furthermore, a manual search was performed by analyzing the references of full-text articles. From a total of 309 studies (collected after the removal of duplicates), 6 studies were selected. Of these, 4 reported a correlation between GA and nerve damage, while the other 2 did not show an obvious as- sociation. The level of bias in the studies was also calculated. Only 2 studies showed a medium risk of bias, while 4 studies showed a high risk of bias; no study showed a low risk of bias. Four of the 6 studies high- lighted a higher incidence of IAN and LN injury, following the extractions performed under GA. Although no scientific evidence is yet available, due to the scarcity and the limited quality of the studies in the literature, considering the risk–benefit ratio, LA should be the first choice in lower third molar surgery.

Background: Neurological alterations are one of the main complications occurring after the third molar extractions. The aim of this prospective multicenter cohort study was to find out Cone Beam Computed Tomography (CBCT) features and distribution of neurological complications in patients undergoing lower third molar surgery and to determine the radiological and patient-related factors that could be correlated to the occurrence of inferior alveolar and lingual nerves injury. Material and Methods: 378 patients who underwent lower third molar extraction from March 2018 to March 2019 were included. Clinical and radiological data were collected. CBCT features were recorded following Maglione et al. classification. Symptoms and characteristics of patients who experienced neurological alterations were evaluated. Results: 193 patients needed a second-level radiological exam (CBCT). In these patients, the most common feature was Maglione class 3: a higher frequency of apical or buccal mandibular canals in direct contact with the tooth was observed. 3.17% of the patients developed a neurological complication. Maglione class 4, increased age, and operative time were all positively correlated with neurological alterations. Conclusions: while the buccal or apical position of the mandibular canal was the more common findings, the lingual position was found to have a higher correlation with a negative outcome. Age and operative time were also found to be risk factors for developing nerve injury in the considered population.

Objectives To evaluate the level of knowledge of pregnant women and puerpers about oral health and prevention during and after gestation. Methods One hundred women aged 18–49 years (mean age 33±6 years) were included in this cross-sectional study. An anonymous questionnaire with 24 items related to oral health has been administered during or just after pregnancy. Firstly, answers have been analyzed on the full population and then subdividing the sample on the base of age ranges (G1: 18–25 years, G2: 26–35 years and G3: >35 years) and number of pregnancies (FP: first pregnancy; SP: second or more pregnancies). Parametric tests have been chosen for the statistical analysis; in particular, Anova test for independent samples was used to evaluate differences of baseline demographic characteristics among subgroups G1, G2, G3 while chi-square test was used for FP and SP subgroups. Anova test was also used to intercept differences on answers given to the questionnaire among G1, G2 and G3 group; for FP and SP group was used t-test. Results Level of information and knowledge of the full sample was medium-low and no significant differences have been observed between groups regarding awareness of the own level of the oral hygiene and knowledge of oral care. Conclusions Results of this survey underline the high necessity of educational programs regarding oral care in pregnant and puerpers women. A strict collaboration between medical figures (dentist, oral hygienist, gynecologist and obstetric) is strongly encouraged to spread the concept of prevention.

Lack of adequate amount of keratinized gingiva around dental or implants is generally treated with coronally advanced flap in combination with connective tissue graft. The procedures of harvesting the soft tissue grafting are usually associated with a certain degree of morbidity; for this reason xenogenic collagen matrix was proposed to be used as an option to reduce morbidity. This collagen matrix quickly stabilizes the blood clot and promotes rapid vascularization. Moreover, this product promotes root coverage, reduction of recession and regeneration of keratinized gingiva both in width and thickness. Recently, xenogenic collagen matrix was also proposed as a biological material able to regenerate keratinized gingiva around implants. In this review, the role of xenogenic collagen matrix (Mucograft®) has been critically analyzed to evaluate its effectiveness and predictability in keratinized tissue augmentation around implants supporting prosthetic restorations. Most of the studies showed that xenogenic collagen matrix was effective in increasing the thickness of the peri-implant mucosa and in the gaining of keratinized gingiva with comparable or slightly lower results than autologous connective tissue grafts. From the aesthetics point of view, the gold standard appeared to be the autologous connective tissue graft. Histologic analysis showed a good integration of the collagen membrane that matures into a healthy tissue. Mucograft® seems an effective alternative to the autologous connective tissue graft with regard to the gain of keratinized tissue and the increase in thickness of peri-implant soft tissues, with less post-operative morbidity and reduced operative times.

Objective This systematic review had the purpose to collect existing data concerning the influence of food hardness on mastication in adults. Design The review was listed with PROSPERO (CRD42017069760) and was directed following with PRISMA and CRD (Centre for Reviews and Dissemination, University of York) statement. A database search of articles issued from 1998 up to December 2018 was carried-out using four databases: Pubmed, Cochrane Library, Web of Science and Scopus. The lists of references of the articles selected for the review were read to identify any other relevant studies. The included publications were analysed for level of evidence, study design, sample characteristics, test of mastication, primary outcomes and key results. A quality assessment of the articles included in the review was performed. Results 1686 articles were found through database searching. The studies that complied with all the inclusion criteria and were considered for the conclusive analysis were 38 and their methodological quality was scored as moderate/low. The findings of the analysed articles were consistent, despite the presence of different methodologies and the lack of a complete control of the bias. They revealed that the majority of the chewing parameters, which were gathered in four groups: 1) number of cycles, 2) sequence duration, 3) muscle activity and 4) coordination and amplitudes and shape of mandibular displacements, increased in the transition from soft to hard food. Conclusions Hard-diet in adults has an impact on the masticatory function increasing almost all the physiological masticatory parameters, muscle coordination and changes of masticatory side.

Objective: The aim of this systematic review was to evaluate the accuracy and the diagnostic reliability of kinesiography and magnetic resonance imaging (MRI) in diagnosis of patients presenting temporomandibular disorders. Methods: A literature survey carried out through PubMed, SCOPUS, LILIACS, and the Cochrane Library from the inceptions to the last access on August 18 2016 was performed to locate randomized clinical trials, controlled trials, prospective cohort studies, or retrospective studies (with or without a control group), that examined the diagnostic reliability of recording devices of mandibular movements in comparison to MRI. Results: From the results, it was found that a significant correlation between these electronic devices and MR images could not be detected in case of disc displacement. Discussion: The scientific evidence does not support the usefulness in clinical practice of the jaw-tracking devices to diagnose temporomandibular disorders because their diagnostic reliability is poor.

Obesity is a systemic disease, associated with an increased risk of cardiovascular disorders, type 2 diabetes, cancer, asthma, and osteoarthritis. Overweight and obesity have been suggested to be associated with periodontitis as published in studies and narrative summaries. Obesity and periodontal diseases are very prevalent in the world, and both can lead to severe chronic health conditions and impair people’s life quality. Knowledge of how immune mechanisms and inflammatory responses are regulated is critical for understanding the pathogenesis of complex diseases, such as periodontitis. In conditions of overweight, it has been demonstrated that approximately 70–80% of individuals present an adipose tissue turnover that is both structurally and functionally causing of the systemic inflammatory reaction. The objective of this review is to explore the influence of lipoinflammation. The effects of lipoinflammation and obesity on development of periodontal disease are reported together with the exploration of the mechanisms of interaction between these two diseases.

Complications after tooth extraction may occur because of several factors correlated to the patient&rsquo, s medical history, surgical site or type of intervention. The aim of this retrospective cohort study was to evaluate type and frequency of complications after exodontic surgery, its correlation with antibiotic administration and between patient&rsquo, s related systemic factors. From June 2015 until February 2016 1701 exodontic interventions, for a total of 2322 extracted teeth, were carried out at the Unit of Oral Surgery in Trieste. Descriptive statistic, and backward multiple logistic regressions were performed to identify the variables associated with the presence of post-operative alveolitis or any other post-operative complication. The presence of coagulopathy and smoking habit were related to high risk of post-operative alveolitis (OR = 5.51, p = 0.035 and OR = 2.5, p = 0.029, respectively). Tooth fracture was found to be correlated with higher probability of post-operative alveolitis (p = 0.001) and concomitant chemotherapy put at a higher risk post-operative complications, including alveolitis (OR = 29.5, p = 0.018). According to the present results, antibiotic consumption did not seem to reduce the incidence of post-operative infective complications (alveolitis). A careful analysis of medical history, the adequate surgical technique, and the correct instructions in post-surgical behavior, prevent the insurgence of intra and post-operative complications.

Purpose. Recent advances show that ultrasonic implant site osteotomy is related to a decreased trauma and a better postoperative healing of the surgical site when compared to traditional drilling techniques. The micrometric bone cutting control and the operative advantages related to the piezoelectric approach are also characterized by a learning curve for the clinician in surgical practice and an increased operative duration of the procedure. The aim of this investigation is to compare the operative time, the postoperative pain, and the amount of painkillers taken by the patient during the healing period. Methods. A total of 65 patients were treated at the Unit of Oral Surgery (Department of Medical Sciences, Surgery and Health, University of Trieste, Italy) using a split mouth model: 75 drill-inserted implants (G1) and 75 piezoelectric device-inserted implants (G2) were placed. The Visual Analogue Scale (VAS) was performed to evaluate the postoperative pain at 15 days from surgery. The operative time and frequency of intake of painkillers were measured. Results. The G1 and G2 groups showed a significant difference with a higher use of painkillers observed for G1. The G2 patients showed a lower level of pain (VAS) at all experimental times between 8 hours to 7 days (p<0.01) postsurgery. At 15 days, the pain levels were similar for both groups. No differences were found in site preparation duration between the study groups. Conclusions. The evidence supports the application of the piezoelectric approach compared to the drill’s osteotomy as a useful technique for implant site preparation. This trial is registered with NCT03978923.

Objectives: The aim of this prospective comparative clinical study was to evaluate the effect of oral anticoagulants on peri- and post-operative bleeding during simple single tooth extractions, comparing patients in treatment with vitamin K antagonists (VKAs) and patients assuming direct oral anticoagulants (DOACs). Materials and methods: Patients under oral anticoagulant therapy needing dental extraction were eligible for entering the study; patients were enrolled following inclusion and exclusion criteria and divided into VKAs and DOAC group according to the anticoagulation therapy. Included patients underwent a simple single dental extraction with elevators and forceps with a maximum surgical time of 15 minutes, without anticoagulation therapy discontinuation. All participants were assessed pre-operatively, during surgery, 30 min minutes and 7 days after surgery. Biological complications were registered and post-extraction bleeding was clinically defined according to Iwabuchi classification. Parametric and non-parametric tests were used to evaluate the variables between the groups. Results: Sixty-five patients per group were enrolled and 130 teeth were extracted. The two groups were comparable for pre-, peri-, and post-operative variables. Only 1 patient of DOAC group and 2 patients for VKA group needed medical evaluation for post-extractive bleeding. No statistically significant difference resulted in post-operative bleeding events between the groups (p = 0.425). Conclusions: DOAC and VKA patients showed the same incidence of bleeding complications after simple single tooth extraction. Bleeding events were not statistically significant and not clinically relevant. Clinical relevance: Patients assuming DOACs can be treated similarly to patients in VKAs therapy with INR index between 2 and 3. Non-ceasing of DOAC therapy seems to be appropriate for simple single dental extractions. © 2018, Springer-Verlag GmbH Germany, part of Springer Nature.

During the past, a more comprehensive knowledge of mechanisms implicated in bone resorption processes has driven researchers to develop a compound library of many small molecules that specifically interfere with the genesis of osteoclast precursors cells. Natural compounds that suppress osteoclast commitment may have therapeutic value in treating pathologies associated with bone resorption like osteoporosis, rheumatoid arthritis, bone metastasis, and periodontal disease. The present review is focused on the current knowledge on the polyphenols derived from plants that could be efficacious in suppressing osteoclast differentiation and bone resorption.

Objective: To investigate the relationship between the presence of ectopic calcification in the elongation of the styloid process (SP) and its possible clinical manifestation (Eagle syndrome) in a population of kidney-transplant patients previously treated with hemodialysis. Methods: Digital orthopantomography of 92 kidney-transplanted patients and 68 control subjects were analyzed to measure the length of SPs. Calcium, phosphate, alkaline phosphatase, and parathyroid hormone (PTH) blood levels were also available for comparison. Results: The mean length of SPs was significantly different between groups, as were phosphate and PTH values. Discussion: Renal transplant patients who have been treated with hemodialysis present elongated SPs, presumably due to alterations in phosphate balance and PTH levels. Thus, in cases of orofacial pain in patients with a history of altered bone metabolism, particularly when due to renal insufficiency, Eagle syndrome should be suspected as the main cause of symptoms.

BACKGROUND: The longitudinal aspect of dental caries has not been previously reported for the Italian population. The primary object of the present study is to collect information of the prevalence of dental decay among the schoolchildren of primary school of Gradisca d'Isonzo (GO) and to analyze the tendency of caries among the students followed since the first year of school. METHODS: Subsequent examinations hold from 2011 to 2015 has been conducted by two calibrated examiners. Oral hygiene instruction and motivation followed the visits. According to WHO principals DMFT and dmft were recorded. The children in the survey were divided into 5 groups according to their ages (6, 7, 8 and 9 years), and these groups were considered separately. Descriptive and statistical analysis of the data was conducted. RESULTS: More than 400 pupils were recruited, resulting in almost 900 examinations during 5 years. Overall, the %dft ≥1 children range from 18.9% (10 years, 2013) to 53.5% (8 years, 2011) across the different age groups. Overall, the %DFT≥1 children range from 8.3% (6 years, 2011) to 44.1% (10 years, 2012) across the different age groups. CONCLUSIONS: Within the limits of the present study, the WHO goals are still not met, among the population in exam. Our results show a trend of decay diminution that enhances in the cohort of 10-aged children suggesting the importance of dental education. Furthermore, the lack of pediatric initiatives of oral hygiene may be overcome by a national intensive educational program, supported by further scientific evidence.

Chronic renal failure is a progressive disease characterized by a gradual destruction of nephrons and a consequent reduction of kidney function. End-stage renal disease (ESRD) necessitates renal replacement therapy as peritoneal dialysis, hemodialysis, or transplantation. Patients affected by ESRD or in hemodialysis are at risk for developing a number of comorbidities including hypertension, anemia, risk of bleeding, susceptibility to infection, medication side effects, and oral manifestations associated with the disease itself and with hemodialysis treatment. In this context, oral diseases represent a potential and preventable cause of poor health outcomes in people with ESRD due to their relation to infection, inflammation, and malnutrition. The aim of this article was to review ESRD and hemodialysis-associated manifestations and to describe the dental operative protocols for patients awaiting kidney transplantation in light of the most recent literature.

Vascular malformations are rare in the orbito-malar region being 64 cases only described since 1950. After ablation, reconstruction is crucial to avoid facial deformities. A 76-year-old male case complaining of a painful left orbital swelling is reported. TC and MRI showed the intraosseous vasal malformation imaging pattern. A segmental orbital ostectomy with immediate PEEK implant reconstruction was performed. Histopathology and immunopathology confirmed the nature of the lesion. At 1 month, the patient was satisfied about the appearance. Medical history and digital imaging patterns allow to avoid the risks of severe hemorrhage from a bony biopsy which can be life-threatening; one-block excision is the treatment of choice, reconstruction is challenging for facial aesthetics. Computer assisted reconstruction, allows to produce an alloplastic customized implant, mirroring the unaffected side into the defect, reducing operation time and overall morbidity. Histopathology and immunohistochemistry define clearly the nature of the lesion.

Background The aim of this study was to compare orthopantomography (OPG) and computed tomography (CT) accuracy in predicting inferior alveolar nerve (IAN) injury after lower wisdom teeth extraction. Methods Twenty-three patients with completely impacted lower third molars were enrolled in this study and OPG and CT exams were obtained before surgical intervention. Finally, a total of 26 lower third molars were extracted and cases of paresthesia following a IAN lesion have been registered. Then, OPG and CT images of the same patients have been randomly submitted to four experienced oral surgeons (with a seniority of at least 5 years) who were required to predict the possibility of a IAN injury by only evaluating the radiological exams. The images were anonymous and have been submitted to the surgeons without an order avoiding to influencing the answers. The agreement between the predicted and the real outcomes was statistically evaluated using the positive predictive value (PPV), the negative predictive value (NPV), Sensitivity, Specificity and the Spearman's rank correlation coefficient. Results A perfect agreement between OPG-based prevision and the real development of paresthesia due to IAN injury has been observed. This agreement was only moderate when considering CT images. Conclusions OPG is a first-level diagnostic exam that provides enough information for predicting IAN lesions after impacted lower third molars surgery. CT is a second-level radiological exam that provides more information on roots morphology and on the amount of the contiguity between roots and mandibular canal resulting useful in performing a minimally invasive surgery.

Aim. To determine whether a relationship exists between the linear measurements of overjet and overbite and the interincisal space delimited by the morphology of the upper and lower incisors. Method and Materials. 30 subjects (age range from 14.1 to 34.8 years, with a median age of 23.5 years and sex ratio F/M: 5/10) with overjet and overbite equal to 2 mm were selected from a group of 381 individuals with a full and well-aligned dentition, no previous dental treatment, and no signs or symptoms indicative of temporomandibular disorder. Computed Tomography images of vinyl polysiloxane impressions of the 30 subjects’ anterior teeth were acquired. The interincisal space was defined as Immediate Overjet Angle and was calculated on the Computed Tomography images. Results. Although the 30 subjects presented overlapping measures of overjet and overbite, the values of the Immediate Overjet Angles were different in a range of a minimum value of 12° and a maximum value of 54°. Conclusion. This study reveals that (1) only 30 (7.9%) of the 381 individuals considered have values of overjet and overbite equal to 2 mm and (2) the Immediate Overjet Angle values of the 30 subjects are not related to the values of overjet and overbite.

Introduction: Symmetric transverse expansion is the main outcome of the early treatment in subjects with unilateral functional posterior crossbite. The aim of this study was to analyse mesial rotation and mesialization of upper first molars as sagittal parameters to be corrected in the treatment of these patients during the mixed dentition. Methods: Digital dental cast measurements (rotation and mesialization) were performed in a sample of 48 subjects with unilateral posterior crossbite (UPXB; 19 males and 29 females, mean age 10.2 ± 1.2 years) and in a control group of 35 subjects with normal Class I occlusion (17 males and 18 females, mean age 9.9 ± 1.3 years). An independent sample t-test, the Mann–Whitney test, Fisher’s exact test, and Pearson correlation were used for statistical comparison. Results: The amount of upper molar rotation was significantly greater in the experimental group when compared with the control group. A clinically significant ‘upper molar rotation’ (UMR) was present in 66.7 per cent of the subjects with UPXB versus 5.7 per cent of the control group. The UMR group presented also a significant mesialization of upper first molars when compared with the control group. In the experimental group, there was a significant difference between rotation and mesialization in the right and left side and a correlation has been found between these two variables and the amount of Class II molar relationship at the crossbite side. Limitations: This is an epidemiological case–control study and the discussed effects of an early correction of the asymmetric upper molars’ migration are only speculations based on an association relationship. Conclusions: The findings of this study show an asymmetric upper first molars’ migration (rotation and mesialization) in unilateral functional posterior crossbite versus a control group. An early evaluation and correction of the molars’ migration during the mixed dentition should be considered in order to obtain a correct inter-occlusal sagittal molar relationship, space for an adequate eruption of permanent teeth, and perhaps reduce the need of a following fixed appliance treatment in the permanent dentition.

Background The choice of the anaesthetic modality is one of the primary steps during planning of third molar surgery. The aim of the present study was to compare the risk of developing neurological injures of the inferior alveolar nerve (IAN) and lingual nerve (LN) in patients treated for wisdom teeth removal under general anaesthesia (GA) with a group treated under local anaesthesia (LA). Material and Methods This is an observational retrospective, unicentric study; between September 2013 and September 2014, 534 patients underwent third molar surgery, 194 (36,3%) under GA and 340 (63,7%) under LA by the same oral surgeon. Differences in the incidence of IAN and LN injures between groups have been statistically analyzed with Fisher exact test and estimated odd ratio for development of such complications has been calculated. Results In GA patients the incidence of IAN and LN injures was 4.6% and 2.1%, respectively while in the LA group it was and 0.3% and 0%, respectively. Background: Neurological involvement is a serious complication associated to the surgical removal of impacted mandibular third molars and the radiological investigation is the first mandatory step to assess the risk of a possible post-operative injury to the inferior alveolar nerve (IAN). The aim of this study was to introduce a new radiological classification that could be normally used in clinical practice to assess the relationship between an impacted third molar and mandibular canal on cone beam CT (CBCT) images. Material and Methods: CBCT images of 80 patients (133 mandibular third molars) were independently studied by three members of the surgical team to draw a classification that could describe all the possible relationships between third molar and IAN on the cross-sectional images. Subsequently, the study population was subdivided according to this classification. The SPSS software, version 15.0 (SPSS® Inc., Chicago, Illinois, USA) was used for the statistical analysis. Results: Eight different classes were proposed (classes 0-7) and six of them (classes 1-6) were subdivided in two subtypes (subtypes A-B). The distribution of classes showed a prevalence of buccal or apical course of the mandibular canal followed by lingual position and inter-radicular one. No differences have resulted in terms of anatomic relationship between males and females apart from a higher risk of real contact without corticalization of the canal when the IAN had a lingual course for female group. Younger patients showed an increased rate of direct contact with a reduced calibre of the canal and/or without corticalization. Conclusions: The use of this classification could be a valid support in clinical practice to obtain a common language among operators in order to define the possible relationships between an impacted third molar and the mandibular canal on CBCT images

Background Eagle syndrome (ES) is a rare disorder that can be responsible for orofacial pain. Objective To describe the treatment of an elderly patient affected by ES and temporomandibular disorders (TMD). Materials and methods A patient complained of constant pain of the right temporomandibular joint (TMJ) and of the sensation of having a foreign body in the throat. Based on the patient's medical history and symptoms, a TMJs internal derangement and concomitant ES were suspected. A magnetic resonance and a computerised tomography confirmed the clinical diagnosis. A conservative treatment was initially performed to re-establish a functional occlusion. Results The rehabilitative treatment alleviated the pain almost totally. A slight residual uncomfortable sensation of the presence of a foreign body in the throat persisted after the oral rehabilitation but without any influence on the quality of life. Conclusion In elderly patients complaining a chronic orofacial pain, the possibility of a concomitant TMD and ES has to be considered to correctly identify the source of pain. A conservative approach to identify weather TMD is the main source of pain is preferable, avoiding unnecessary invasive treatments.

Infective endocarditis (IE) in high-risk patients is a potentially severe complication which justifies the administration of antibiotics before invasive dental treatment. This literature review presents the current guidelines for antibiotic prophylaxis and discusses the controversial aspects related to the antibiotic administration for prevention of IE.According to the guidelines of the American Heart Association, individuals who are at risk to develop IE following an invasive dental procedure still benefit from antibiotic prophylaxis. In contrast, the guidelines of the National Institute for Health and Clinical Excellence in England and Wales have recommended that prophylactic antibiotic treatment should no longer be performed in any at-risk patient. Bacteraemia following daily routines such as eating and toothbrushing may be a greater risk factor for the development of IE than the transient bacteraemia that follows an invasive dental procedure. However, a single administration of a penicillin derivate 30 to 60 minutes pre-operatively still represents the main prophylactic strategy to prevent bacteraemia.Presently, there is not enough evidence that supports and defines the administration of antibiotics to prevent IE. The authors suggest performing a risk-benefit evaluation in light of the available guidelines before a decision is made about administration.

Objective: Immunosuppressive drugs may induce an increase of the gingival connective tissue in the extracellular matrix. The aim of this study was to assess the effectiveness of nonsurgical periodontal treatment in reducing gingival overgrowth (GO) in transplant patients taking cyclosporin A (CsA) or tacrolimus (Tcr). Method and Materials: An observational cohort study employing 68 transplant patients with diagnosis of GO, 51 taking CsA and 17 in therapy with Tcr, was performed at the Periodontal Unit of the School of Dental Sciences (University of Trieste, Italy). Percentages of plaque index (PI), bleeding on probing (BoP), sites with probing depth (PD) > 3 mm, and hypertrophy index (HI) were registered at baseline, 90 days, 180 days, and at 1 year after nonsurgical periodontal therapy. Furthermore, HI at baseline and after 1 year was investigated by multiple linear regression. Results: Both groups have significantly improved their clinical parameters: CsA group: PIbaseline = 41.67%; PIyear = 33%; BoPbaseline = 13.88%; BoPyear = 6.94%; PD > 3 mmbaseline = 18.6%; PD > 3 mmyear = 12.96%; HIbaseline = 22%; HIyear = 10%; Tcr group: PIbaseline = 40.73%; PIyear = 38.54%; BoPbaseline = 20.78%; BoPyear = 12.5%; PD > 3 mmbaseline = 21.53%; PD > 3 mmyear = 13.19%; HIbaseline = 12%; HIyear = 6.5%. Age showed a statistical negative correlation with HI at baseline (P < .05), while PD > 3 mm was positively correlated to the baseline HI (P < .001). Only HI at baseline showed a statistically significant negative relation with HI at 1 year (P < .001). Conclusion: After nonsurgical periodontal therapy no patients needed additional periodontal surgery. Nonsurgical periodontal treatment itself represents an efficacious therapy in transplant patients treated with CsA and Tcr.

Objectives The objective of this epidemiologic study was to estimate the healing time of acute dental abscesses and to evaluate the main variables involved in the healing process itself. Subjects and Methods Among a sample of over 24 000 patients visited at the emergency dental unit, 688 subjects were diagnosed with dental abscess and enrolled in the study. Case histories of all patients were collected to investigate the clinical course and healing time of dental abscess according to anamnestic and diagnostic data and therapeutic management. A multiple logistic regression model was performed to evaluate the association of each variable with the healing time required for dental abscess. Results Variables associated with increased healing time were spring seasonality at admission, pyretic state, trismus, involvement of multiple anatomic spaces, and spontaneous drainage. Moreover, administration of some, but not all, classes of antibiotics was also associated with an increased healing time. Conclusions The knowledge of variables involved in healing time for dental abscess is crucial in the optimization of managing such infections in terms of cost–benefit ratio. This would represent a valuable way to ensure a shortened and more effective healing.

Introduction Complications associated to group-A streptococcal pharyingitis include non-suppurative complications such as acute rheumatic fever and glomerulonephritis and suppurative complications such as peritonsillar or retropharyngeal abscess, sinusitis, mastoiditis, otitis media, meningitis, brain abscess, or thrombosis of the intracranial venous sinuses. Case presentation We described a case of a 15-year-old patient with a history of acute pharyngodinia early followed by improvise fever and a progressive formation of a diffuse orbital edema, corneal hyperaemia, diplopia and severe decrease of visual acuity. The patient was surgically treated with functional endoscopic sinus surgery (FESS) after the response of a maxillofacial computed tomography scans that showed a pansinusitis complicated by a left orbital cellulites. Numerous colonies of Streptococcus pyogenes were found in the samples of pus and an antibiotic therapy with meropenem was initiated on the basis of the sensitivity test to antibiotics. The patient was finally discharged with diagnosis of left orbital cellulites with periorbital abscess, endophtalmitis and acute pansinusitis as a consequence of streptococcal pharyngitis. Conclusion The case highlights the possible unusual complication of a group-A streptococcal pharyingitis in a immunocompetent child and the needing of a prompt surgical and medical approach toward the maxillofacial complications associated to the infection.

Objectives Wegener's granulomatosis (WG) is an uncommon multisystemic vasculitis characterized by a pathological triade, consisting of necrotizing granulomatous lesions of the respiratory tract, systemic vasculitis involving small arteries and veins, and necrotizing glomerulonephritis. We report a case of WG in a patient who initially presented with ulcerative gingivitis. Materials and methods A 59-year-old woman was referred to the Department of Oral Pathology and Medicine, School of Dental Sciences, University of Trieste, for an uncommon form of gingivitis with ulcerative and necrotic lesions. On the basis of the clinical, serologic, and histologic findings, the condition was provisionally diagnosed as an orofacial granulomatous condition. During follow-up, bilateral serous otitis media and acute ulceronecrotic rhinitis were also diagnosed by an otorhinolaryngologist. Systemic steroid therapy (prednisolone 25 mg for 10 days) was prescribed. It produced partial remission of the ear and nasal symptoms, as well as good improvement of the oral lesions. Results Considering the clinical, histological, laboratory, and instrumental findings (maxillofacial CT, chest radiography), the final diagnosis was WG. Conclusions Lesions involving the ear, nose, and oral cavity can be an early manifestation of WG. The mean survival of untreated patients is very low. Therefore, otorhinolaryngologists and dentists should both be aware of this condition as prompt referral and early diagnosis can be of crucial importance to the treatment outcome.

The purpose of this research was to compare the reliability of two different methods for cranial midline localization through cephalometric analysis of mandibular condyle asymmetries. A retrospective cohort study was performed analyzing consecutively the SMV radiograms of 47 patients undergoing oral surgery before orthodontic treatment at the Dental School, University of Trieste (Italy) from 2003 to 2008. Two different cephalometric analyses were used to identify the basicranium midline (Tracing 1: initial landmarks = craniostat ear rods; Tracing 2: initial landmarks = spinosum foramina), and the left/right symmetry ratio (SR) for four parameters (condylar length, condylar angle, intra-condylar hemidistance, extra-condylar hemidistance) was calculated. The main result showed that no significant statistical difference between the SRs of the intra-condylar and extra-condylar hemidistance obtained with the same tracing was found ( 𝑡 -test; 𝑃 = N S ; C.I. 95%). Conversely, the difference between the SRs obtained with the two different tracings was statistically significant ( 𝑡 -test; 𝑃 0 . 0 0 0 ; C.I. 95%). In conclusion, if the analysis of condylar asymmetries is performed in growing subjects, utilization of anatomic references such as the neurovascular foramina seems to guarantee a lower error compared to non-fixed references such as ear rods.

Objective To evaluate postextractive neurological complications after third molar extraction under general anesthesia and to identify correlations between the surgical procedure, the third molar-related pathology, and neurological involvement. Study design The clinical records of 183 patients were analyzed for a total of 408 third molars extracted at the Dental Clinic of Trieste (Italy). Individual effects of clinical data on the presence of paresthesia were evaluated by a logistic regression model. Results Neurological involvement was observed in 13 patients (6.1%). No permanent inferior alveolar nerve damage was found (0%) and only 1 patient presented a permanent lesion of the ipsilateral lingual nerve (0.3%). Pell and Gregory classification and surgical difficulty were not associated with the incidence or gravity of neurological lesions (P = NS). Among the pathologies associated with third molars, only the variable “abscess” presented a significant correlation with paresthesia (OR 6.86; 95% CI 1.21-38.8; P = .029). Conclusion The percentage of nerve injuries agrees with the literature data, inclusion class, and surgical technique seem not to influence paresthesia risk. Further studies are necessary to evaluate the role of infectious pathologies as a cofactor in the development of neurological lesions after oral surgery.

Descending Necrotizing Mediastinitis is a rare complication that can be secondary to dental infections or surgical procedures involving the oral region. Despite a prompt pharmacological therapy and surgical intervention, a delayed diagnosis is still responsible for a too high mortality rate (about 40%). We present a review of the current literature on Descending Necrotizing Mediastinitis, focusing on the evolution of its management from Pearse to today. In particular, recent patents focused on novel compositions and methods to prevent and treat oral infections.

Implant relocation is a new surgical technique for correcting the alignment of malpositioned implants by mobilizing them with the surrounding bone until the desired position is achieved. In this case report, a 25-year-old woman was treated for the malposition of an implant in the maxillary left canine site. The use of a piezoelectric scalpel permits narrow, precise, and safe osteotomies, thus preventing involvement of the soft tissue and producing better healing potential compared to burs or saws. The results suggest that inadequately axially inclined implants can be successfully reconfigured using segmental piezoelectric osteotomies.

Eagle’s syndrome, an uncommon sequela of an elongated styloid process, can manifest itself as a sensation of a foreign body in the throat and a retrogoniac or anterolateral neck pain often referred to the TMJ and the ear. When treating patients affected by temporomandibular disorders (TMD), complaining of atypical orofacial pain, and with a concomitant radiographic finding of a bilaterally elongated styloid, the possible coexistence of Eagle’s syndrome should always be considered. The aim of this review is to present an overview of the epidemiology, pathophysiology, diagnosis, and therapy for Eagle’s syndrome and to help the clinician in differential diagnosis with other conditions that can provide similar signs and symptoms.

The aim of this systematic review was to evaluate the clinical effectiveness of the surgical technique of coronectomy for third molars extraction in close proximity with the inferior alveolar nerve. A literature survey carried out through PubMed, SCOPUS and the Cochrane Library from inceptions to the last access in January 31, 2014, was performed to intercept randomised clinical trials, controlled clinical trials, prospective cohort studies or retrospective studies (with or without control group) that examined the clinical outcomes after coronectomy. The following variable were evaluated: inferior alveolar nerve injury, lingual nerve injury, postoperative adverse effects, pulp disease, root migration and rate of reoperation. Ten articles qualified for the final analysis. The successful coronectomies varied from a minimum of 61.7% to a maximum of 100%. Coronectomy was associated with a low incidence of complications in terms of inferior alveolar nerve injury (0%-9.5%), lingual nerve injury (0%-2%), postoperative pain (1.1%-41.9%) and swelling (4.6%), dry socket infection (2%-12%), infection rate (1%-9.5%) and pulp disease (0.9%). Migration of the retained roots seems to be a frequent occurrence (2%-85.3%). Coronectomy appears to be a safe procedure at least in the short term, with a reduced incidence of postoperative complications. Therefore, a coronectomy can be indicated for teeth that are very close to the inferior alveolar nerve. If a second operation is needed for the remnant roots, they can be removed with a low risk of paresthesia, because the roots are generally receded from the mandubular nerve.

Background The aim of this paper is to contribute to the discussion on how to approach patients taking new orally administered anticoagulants (NOAs) dabigatran etexilate (a direct thrombin inhibitor), rivaroxaban and apixaban (factor Xa inhibitors), before, during and after dental treatment in light of the more recent knowledges. Discussion In dentistry and oral surgery, the major concerns in treatment of patients taking direct thrombin inhibitors and factor Xa inhibitors is the risk of haemorrhage and the absence of a specific reversal agent. The degree of renal function, the complexity of the surgical procedure and the patient’s risk of bleeding due to other concomitant causes, are the most important factors to consider during surgical dental treatment of patients taking NOAs. For patients requiring simple dental extraction or minor oral surgery procedures, interruption of NOA is not generally necessary, while an higher control of bleeding and discontinuation of the drug (at least 24 h) should be requested before invasive surgical procedures, depending on renal functionality. Summary The clinician has to consider that the number of patients taking NOAs is rapidly increasing. Since available data are not sufficient to establish an evidence-based dental management, the dentist must use caution and attention when treating patients taking dabigatran, rivaroxaban and apixaban.
