Abstract

BackgroundAssociations between dairy intake and body mass index (BMI) have been inconsistently observed in epidemiological studies, and the causal relationship remains ill defined.MethodsWe performed Mendelian randomization (MR) analysis using an established dairy intake-associated genetic polymorphism located upstream of the lactase gene (LCT-13910 C/T, rs4988235) as an instrumental variable (IV). Linear regression models were fitted to analyze associations between (a) dairy intake and BMI, (b) rs4988235 and dairy intake, and (c) rs4988235 and BMI in each study. The causal effect of dairy intake on BMI was quantified by IV estimators among 184802 participants from 25 studies.ResultsHigher dairy intake was associated with higher BMI (β = 0.03 kg/m2 per serving/day; 95% CI, 0.00-0.06; P = 0.04), whereas the LCT genotype with 1 or 2 T allele was significantly associated with 0.20 (95% CI, 0.14-0.25) serving/day higher dairy intake (P = 3.15 × 10-12) and 0.12 (95% CI, 0.06-0.17) kg/m2 higher BMI (P = 2.11 × 10-5). MR analysis showed that the genetically determined higher dairy intake was significantly associated with higher BMI (β = 0.60 kg/m2 per serving/day; 95% CI, 0.27-0.92; P = 3.0 × 10-4).ConclusionsThe present study provides strong evidence to support a causal effect of higher dairy intake on increased BMI among adults.

Abstract

Autophagy defects accelerate aging, while stimulation of autophagy decelerates aging. Acyl‐coenzyme A binding protein (ACBP), which is encoded by a diazepam‐binding inhibitor (DBI), acts as an extracellular feedback regulator of autophagy. As shown here, knockout of the gene coding for the yeast orthologue of ACBP/DBI (ACB1) improves chronological aging, and this effect is reversed by knockout of essential autophagy genes (ATG5, ATG7) but less so by knockout of an essential mitophagy gene (ATG32). In humans, ACBP/DBI levels independently correlate with body mass index (BMI) as well as with chronological age. In still‐healthy individuals, we find that high ACBP/DBI levels correlate with future cardiovascular events (such as heart surgery, myocardial infarction, and stroke), an association that is independent of BMI and chronological age, suggesting that ACBP/DBI is indeed a biomarker of "biological" aging. Concurringly, ACBP/DBI plasma concentrations correlate with established cardiovascular risk factors (fasting glucose levels, systolic blood pressure, total free cholesterol, triglycerides), but are inversely correlated with atheroprotective high‐density lipoprotein (HDL). In mice, neutralization of ACBP/DBI through a monoclonal antibody attenuates anthracycline‐induced cardiotoxicity, which is a model of accelerated heart aging. In conclusion, plasma elevation of ACBP/DBI constitutes a novel biomarker of chronological aging and facets of biological aging with a prognostic value in cardiovascular disease. Récemment, l’ANSES a rendu un avis prudent sur la consommation des phytostérols/stanols dans le but de réduire le risque cardiovasculaire. Parmi les principales raisons justifiant la prudence de l’ANSES figurent notamment, d’une part la variabilité inter-individuelle de la réponse à la consommation de phytostérols/stanols, et, d’autre part, les relations entre phytostérolémie plasmatique et risque cardiovasculaire. Ces deux points font l’objet de la présente revue. Il existe une variabilité de la réponse individuelle à l’apport de stérols/stanols végétaux, avec une absence de réponse (baisse LDLC) chez 20 à 42 % des sujets. Parmi les sujets non-répondeurs, certains augmentent le LDLC. La diminution du cholestérol LDLC est proportionnelle à sa concentration initiale en valeur absolue, et constante en pourcentage. La réponse est corrélée négativement au taux de synthèse du cholestérol et positivement au niveau d’absorption. L’effet des facteurs génétiques sur la réponse reste encore à démontrer. Il existe un manque de puissance statistique générale dans toutes les études génétiques publiées à ce jour. Concernant la relation entre phytostérolémie et le risque cardiovasculaire, de nombreux résultats contradictoires ont été publiés. Une méta-analyse montre globalement une absence de relation, mais l’hétérogénéité significative entre ces études ne permet pas de conclure. D’autre part, des études génétiques montrent que les variants qui augmentent la phytostérolémie sont associés à une augmentation du risque. La phytostérolémie est liée à l’absorption du cholestérol. L’absorption pourrait être le facteur causal de la relation entre phytostérolémie et risque cardiovasculaire, mais il n’est pas exclu que les phytostérols circulants aient aussi un effet délétère. L’ANSES estime que des études cliniques sont nécessaires pour conclure sur l’impact cardiovasculaire de la consommation des produits contenant des phytostérols/stanols.

L’obésité a une part génétique démontrée par des études de corrélations familiales, comme les études d’adoptés ou de jumeaux élevés séparément. Une étude de jumeaux suralimentés a démontré l’importance des gènes dans la réponse à la nutrition, et donc de l’interaction gènes\\nutrition. Les modèles animaux et des études métaboliques ont fourni de nombreuses pistes de gènes dits « candidats ». Les gènes candidats pour l’obésité et ses complications peuvent intervenir dans la prise alimentaire, la dépense énergétique, la biologie des adipocytes, le métabolisme lipidique. Des mutations des gènes de la voie leptine‐mélanocortines ont été impliquées dans les cas exceptionnels d’obésité monogénique chez l’homme. Cependant, dans le cas général, l’obésité commune est un phénotype complexe qui résulte d’interactions entre l’environnement (nutrition) et des facteurs génétiques (polygéniques) de prédisposition. Les interactions avec la nutrition peuvent être révélées par une alimentation pléthorique mais aussi par la réponse au régime restrictif, et certains polymorphismes génétiques modifient la réponse au régime hypocalorique. Puisque le séquençage du génome humain est presque terminé et va fournir une liste exhaustive des polymorphismes génétiques, les variants génétiques devront être testés dans des modèles d’interaction (études longitudinales prospectives, études d’interventions nutritionnelles). Ces études serviront à trouver de nouvelles voies physiopathologiques et thérapeutiques, plutôt qu’à estimer un risque individuel.

Abstract

Objective: Patients with diabetes have an increased risk for lower-limb amputation (LLA), but biomarkers to assess risk of LLA are lacking. Adrenomedullin (ADM) is a vasodilator peptide that also plays a role in fluid and electrolyte homeostasis in the kidney, increasing natriuresis and diuresis. ADM was shown to be associated with cardiovascular and renal events in diabetes, but it was not investigated in terms of LLA risk. We investigated the hypothesis that ADM is associated with LLA in people with type 2 diabetes.Research Design and Methods: We studied 4,375 participants in the DIABHYCAR and SURDIAGENE cohorts (men, 68%; mean 66 years of age; mean duration of diabetes 12 years; and median follow-up 5.3 years). Plasma midregional proadrenomedullin (MR-proADM; a surrogate for ADM) was measured by immunofluorescence. Five single nucleotide polymorphisms (SNPs) in the ADM gene region were genotyped.Results: LLA requirement during follow-up by increasing tertiles of plasma MR-proADM distribution was 1.0% (tertile 1 [T1]), 2.3% (T2), and 4.4% (T3) (P < 0.0001). In Cox multivariate analysis, the adjusted hazard ratio (95% CI) for LLA was 4.40 (2.30-8.88) (P < 0.0001) for T3 versus T1. Moreover, MR-proADM significantly improved indices for risk stratification of LLA. Four SNPs were associated with plasma MR-proADM concentration at baseline and with LLA during follow-up. Alleles associated with higher MR-proADM were associated with increased LLA risk.Conclusions: We observed associations of plasma MR-proADM with LLA and of ADM SNPs with plasma MR-proADM and with LLA in people with type 2 diabetes. Abstract

Background High adiponectin levels are associated with diabetic nephropathy. Nevertheless, it is not known whether plasma adiponectin is associated with renal function decline in the general population. We evaluated whether adiponectin concentrations were associated with changes in renal function in a community cohort, the Data from an Epidemiological Study on the Insulin Resistance Syndrome (DESIR) study. Methods Plasma adiponectin concentrations were measured in a random sample of 3284 people from the DESIR study, a 9-year prospective cohort from the general population. Data were analysed for three endpoints during follow-up: incidence of Stage 3 chronic kidney disease (CKD); the Kidney Disease: Improving Global Outcomes (KDIGO) criterion 'certain drop in eGFR' and rapid kidney function decline [estimated glomerular filtration rate (eGFR) slope steeper than −3 mL/min/1.73 m2/year]. Results After exclusion of participants with an eGFR <60 mL/min/1.73 m2 at baseline and those with type 2 diabetes or impaired fasting glycaemia at any time during follow-up (remaining n  = 2174), there was a 113% higher risk for a rapid decline in kidney function in participants with adiponectin above the third tertile (T3) versus below the first tertile (T1) (P trend = 0.004) and a 53% higher risk for kidney function decline as defined by the KDIGO criterion (P trend = 0.04). In a cross-sectional analysis, adiponectin was positively associated with urinary albumin:creatinine ratio at baseline (P  = 0.009). Conclusions In a healthy cohort from the general population, higher levels of plasma adiponectin were associated with decreased renal function at baseline and at follow-up.
