紀要委員会企画, Special Articles, 2022 年４月１日より保健師助産師看護師学校養成所指定規則の一部を改正した省令（厚生労働省，2019）が適応される。本規則の改正では、地域在宅看護の創設、実習単位が見直され、看護師に求められる社会的役割、及び疾病構造の変化に伴う療養生活の場の多様性に合わせた内容が示された。それに伴い、本学看護学部では本学の理念である生命の尊厳と隣人愛の精神を基盤として、2019 年よりカリキュラム改革を推進し検討を継続した。中長期ビジョン検討会、カリキュラム骨子作成委員会を経て、カリキュラム改革委員会が30 回を超える協議を重ね、新たなカリキュラムを申請するに至った。本稿では、新たなカリキュラムに至った経緯とカリキュラム編成の概要について報告する。

報告, Reports, 本学看護学部における急性期看護学実習は、2021 年８月から９月の新型コロナウイルス感染症（Corona Virus Disease；COVID-19）の感染拡大により学内実習となった。本学内実習は、模擬電子カルテMedi-EYE（Medi-LX）上の患者の看護過程を展開しながら、立案した看護計画を演習において実践し評価することで、周術期看護を学修するものである。学生は、模擬電子カルテに日々追加される情報を基に、患者の回復過程や予測される合併症、必要な看護を具体的に考え、模擬電子カルテの操作や演習などの体験を通し、周術期にある患者に必要な看護を学修できていた。今後は、学生のレディネスや臨床状況を反映した模擬電子カルテの情報整理・演習内容の検討、学修効果の評価が課題である。

The purpose of this article is to develop a virtual reality (VR) instructional environment that allows users to visually experience scattered radiation intensity distribution during computed tomography (CT) is presented. Scattered radiation generated during a CT scan of the head was measured with an air dosimeter to create a dose profile. A color map showing the spread and intensity of scattered radiation was created from the dose profile and used to generate VR content. Scattered radiation was visually recognized using the developed VR educational environment. From the results obtained, it is clear that with the developed immersive teaching material, it is possible to visually recognize the characteristics of scattered radiation in head CT examinations. Users can look back on their own radiation protection methods through virtual experiences with the developed immersive teaching materials.

報告, Reports, 2020 年の新型コロナウイルス感染症（以下、COVID-19）の世界的感染拡大により、生活のあらゆる場面における情報通信技術（Information and Communication Technology；以下、ICT）の重要性が飛躍的に高まり、看護基礎教育においては、ICT を生かした看護実践能力を育む新たな教育手法の開発とその知見の蓄積が喫緊の課題となった。 このような学修環境の変化を契機に、2020 年度より本学の成人看護学領域は、専用クラウド上の映像教材（以下、コンテンツ）に個人が所有する端末でアクセスすることで、それらコンテンツを自由に活用できる「映像システム」を取り入れた教育を開始した。 映像システムは、2020 年度開講された全ての成人看護学関連科目において活用され、特にCOVID-19 の感染拡大状況により、その教育手法が大きく影響をうける実習においては、多様な形で用いられていた。今後は、映像システムの活用状況や教育効果に関する評価、さらに組織的な使用環境の整備などが課題である。

報告, Reports, 本学看護学部の成人看護学における3 年次春セメスター必修科目「成人看護援助論演習」では、主体的学修を基盤とする協働学修を取り入れた看護過程演習を実施している。2020 年度においては、新型コロナウイルス感染症（COVID-19）拡大により、面接授業が困難となり、遠隔授業による実施を迫られた。本稿では、短期間で修正した具体的な教育の実践内容およびその過程で生じた課題や対応策等について報告した。看護過程演習では、オンデマンド授業を主体に一部同時配信授業を取り入れ、学習管理システム（Learning Management System）を活用し、学生の提出課題へ教員が個別に対応しながら、学修過程を支援した。新型コロナウイルス感染症の収束の兆しが見えない中（2020 年12 月現在）、今後さらに情報通信技術を活用した学生への効果的・効率的な教育方法の在り方を検討していくことが課題である。

報告, Reports, A 大学看護学部における臨地看護学実習は3 年次10 月秋セメスターから4 年次春セメスターで実施している。2020 年4 月新型コロナウイルス感染症（COVID-19）の拡大により臨地看護学実習は各施設から実習受け入れ中止の連絡があり、急性期看護学実習は遠隔実習を実施することが迫られた。短期間で遠隔実習の内容の検討を行い、事例患者を用いた看護過程の展開、周術期看護関連の文献レポートやがんによる手術体験者の講話を実施した。グループワーク、個別指導等はWeb 会議システムZoom（Zoom Video Communications，Inc．）を、課題の提出等は学習管理システム（Learning Management System）を活用し、3 週間の遠隔実習において概ね実習目標の達成ができた。 今後は遠隔実習用シミュレーション教育の導入、事例の検討、実習施設との更なる協力体制の整備など、遠隔実習における効果的な実習内容の検討が課題である。

報告, Reports, 2018 年度の「公益社団法人ふじのくに地域・大学コンソーシアム・ゼミ学生等地域貢献推進事業」における沼津市との共同事業に「がん看護ゼミナール」として参加し、若年女性の子宮頸がん検診・受診行動を促進する教材作成に取り組んだ。文献検討とターゲット年齢のゼミ生による討議、静岡県内市町が用いている検診関連資料（Web 版および紙媒体）の検討、検診現場見学を実施し、教材に必要な内容や望ましい伝え方と、現状の比較を行った。その結果、現在用いられている資料には検診を動機づける内容が不足していること、検診への抵抗感を軽減する工夫や、多量の文字情報に抵抗感を抱く若年者への対応が必要であることが明らかになった。得られた結果に基づいて作成する教材の内容を整理し、イラストレーターの協力のもと、子宮頸がん検診の必要性を伝え、受診を促し、情報提供により不安を和らげることを目指した啓発用パンフレット作成に至った。

報告, Reports, 近年の医療の高度化・専門化に伴い、看護基礎教育における高度な看護実践能力育成のための効果的教育方法としてシミュレーション教育が注目されている。急性期看護学実習では、学生は不安や緊張の強い中で、手術を受け刻一刻と変化する患者と患者を取り巻く状況を踏まえた看護実践が求められるため、主体的に学修することが困難となる側面が多い。そのため、患者の安全を守りつつ、学生が安心して学ぶことができるシミュレーション教育への期待は大きい。本学の急性期看護学実習では、2019 年度より高機能シミュレーターを用い、臨床現場における手術直後の患者の状況を可能な限り忠実に再現したシミュレーション教育を導入し、その手ごたえを得た。今後は、教員のファシリテーション能力の向上、教育効果の検証、大学のカリキュラムにおける体系的な展開などが課題である。

Purpose We investigated the experiences of Japanese men with sexual dysfunction associated with various prostate cancer treatments. Methods We included 38 Japanese men who underwent the following initial treatments for prostate cancer: radical prostatectomy (n = 10), external beam radiotherapy (n = 12), brachytherapy (n = 5), and androgen deprivation therapy (n = 11). Semi-structured interviews were conducted regarding sexual dysfunction associated with prostate cancer treatment. Data were analyzed using a content analysis method. To obtain a unique experience for each treatment, we confirmed and organized the treatment method from which the code that constituted each category was derived. The category reliability was calculated based on Scott’s formula for the matching rate of the classification by three qualitative researchers. The criterion for good reliability was set at 70%. Results Japanese men with sexual dysfunction associated with prostate cancer treatments experienced the following: a desire to maintain sexual function and conflict in decision-making concerning the initial treatment for prostate cancer; a loss of values related to sexual dysfunction; an uncertainty regarding the consequences of sexual dysfunction; a sense of calm with fewer adverse effects of sexual dysfunction at the early treatment stage; an effort to accept sexual dysfunction; and management of their changed body at the later treatment stages. The concordance rates for the categories were 70% and 78%. Additionally, there were glimpses of experiences common to all treatments and trends in treatment-specific experiences. Conclusion It is necessary to provide care based on the experience of Japanese men with sexual dysfunction after prostate cancer treatment.

Objective: Postoperative patients with gastrointestinal (GI) cancer have multiple adaptation tasks and care needs to improve their quality of life (QOL). Whether their supportive care needs differ according to their physical and psychosocial conditions is unclear. This study investigated patients' (1) physical and psychosocial conditions (QOL, fatigue, anxiety, cognitive plight, and resilience) and (2) responses to an informational booklet describing cancer patients' problems and adaptation tasks, and examined the association between the two factors. Methods: A questionnaire survey was conducted to postoperative patients with GI cancer. Results: The mean age of the 69 respondents was 63 years; 59.4% of the respondents were men. Nine patients who did not read the booklet showed high fatigue and cognitive plight and low QOL. The patients (36.2%) who chose “I vaguely understood the content” showed low scores for resilience and cognitive plight while those (8.5%) who chose “I will deal with my tasks as described in the scenarios” showed high scores for both of these variables. Conclusions: The condition of some patients continued to be highly affected by their cancer. In terms of understanding the contents of the booklet, resilience was significant, and cognitive plight did not necessarily have a negative impact. The provision of information by means of a booklet might not be suitable for patients who are highly affected by their cancer. Patients may need additional support to be able to make good use of the information provided in such a booklet.

Objective The aim of the present study was to collect data on the characteristics, degree, and natural course of urinary complications, as well as self-care for such complications, in patients during and after prostate intensity-modulated radiation therapy (IMRT). Methods Quality of life data were collected retrospectively for all eligible patients who underwent IMRT. In all eligible patients, urinary symptoms were evaluated using questionnaires and face-to-face interview. Participants were asked to respond to a self-administered questionnaire concerning the core lower urinary tract symptom score during the course of IMRT and up to 12 months after the completion of IMRT. Results In all, 29 eligible patients were included in the study. The frequency of urinary symptoms increased over of the course of IMRT, decreased at ≥3 months after completion of IMRT, and disappeared 6 months after IMRT. Responses to the questionnaire revealed a variety of approaches to self-care and adaptations by patients to manage urinary symptoms. Conclusions During and after IMRT for localized prostate cancer, patients often developed more frequent urination and urgency than at the start of IMRT, and recovered 3-6 months after the completion of IMRT. Based on the present study, clinicians and nurses could help convey this information to patients and thus offer better support.
