In this study, we aimed to investigate the prognostic impact of splenectomy on post-operative complications and the long-term oncologic results of cytoreductive surgery with hyperthermic intraperitoneal chemotherapy for treating peritoneal metastasis. All consecutive patients who underwent cytoreductive surgery with hyperthermic intraperitoneal chemotherapy for peritoneal metastasis were analysed between 2007 and 2020. In addition to clinicopathological features, peritoneal cancer index, completeness of cytoreduction, (neo-)adjuvant chemotherapy, operative time, need for surgical intensive care unit, use of albumin, erythrocyte suspension and fresh frozen plasma, infectious and non-infectious post-operative complications were evaluated. The data of 660 patients were analysed, and splenectomy was performed in 92 patients (13.9%). The 3- and 5-year-survival rates were significantly lower in the splenectomy group (42.9% and 25.6% vs. 50.2% and 34.0%; p=.038). High peritoneal cancer index score (OR, 2.34; 95%CI, 1.40–3.90; p =.001) and the number of resected organs (OR, 4.34; 95%CI, 2.58–7.29; p =.000) were significant risk factors in the splenectomy group. Introduction: Various potential prognostic histopathologic factors for colorectal carcinoma liver metastasis have been proposed. However, there is still no consensus on pathological reporting of colorectal carcinoma liver metastasis resection materials. The aim of this study was to investigate the relation between selected tumoral and parenchymal histopathologic features and prognostic factors for better characterization and prognostic prediction of the patients with colorectal carcinoma liver metastasis. Methods: Hematoxylin-eosin stained slides from 100 patients who underwent hepatic resection were evaluated. Pathologic characteristics; including number of tumor nodules, largest tumor size, status of surgical margin, tumor distance to closest margin, tumor necrosis, the presence of tumor capsule, tumor differentiation, perineural and lymphovascular invasion, micrometastasis, tumor budding, peritumoral lymphocytic infiltrate and parenchymal features including steatosis, steatohepatitis, lobular inflammation, confluent necrosis, hepatocyte ballooning, portal inflammation were assessed. For 49 patients who were treated with preoperative chemotherapy, tumor regression grade and chemotherapy-related parenchymal changes such as sinusoidal damage, venous obstruction, nodular regenerative hyperplasia, steatosis and steatohepatitis were also evaluated. Results: The presence of lymphovascular invasion (p < 0.001), micrometastasis (p=0.004), absent or mild peritumoral lymphocytic infiltration (p =0.013), high tumor budding score (p=0.033) and moderate/poor differentiation (p=0.022) were significantly associated with shorter overall survival. Lymphovascular invasion (p < 0.001) was an independent predictor of mortality in multivariate analysis. Conclusions: We conclude that tumor differentiation, lymphovascular invasion, micrometastasis, peritumoral lymphocytic reaction and tumor budding score are potential prognostic histopathological features and candidates for inclusion in pathology reports of colorectal carcinoma liver metastasis resections.

Purpose: We aimed to determine the long-term oncologic outcome for patients with rectal cancer treated with preoperative Chemoradiotherapy (CRT) followed by total mesorectal excision (TME). In addition, we attempted to identify prognostic factors and the oncologic significance of a pathological complete response (pCR) on the patient's oncologic outcome. Materials and Methods: Between February 1993 and January 2019, a prospectively maintained database of 357 consecutive patients with locally advanced (cT3-4 or any N1-2) primary rectal adenocarcinoma who received preoperative CRT followed by curative TME at our institute was analyzed. Results: With a median follow-up of 72 (23–295) months, local recurrence occurred in 21 (5.9%) patients. The 5-, 10-, and 15-year locoregional free survival rates were 92.7%, 91.3%, and 91.3%, respectively. The median time to pelvic recurrence was 28 (12–103) months. Pathologic complete response was reported in 41 (11.6%) patients, and no patient with pCR developed local recurrence. Advanced pathologic T stage, pathologic nodal stage, and circumferential resection margin involvement were significantly poor prognostic factors for these patients. Conclusion: Neoadjuvant radiochemotherapy for rectal cancer significantly improved the down-staging and sphincter preservation rates of low-lying rectal carcinomas. However, some patients will continue to recur after 5 years of follow-up.

The objective of the study was to evaluate the morbidity-mortality results in terms of immunscore factors and to predict the outcomes of urgent re-laparotomized patients treated with cytoreductive surgery (CRS) and hyperthermic intraperitoneal chemotherapy (HIPEC).Prospectively maintained database of 661 patients treated with potentially curative intent of CRS and HIPEC through the years of 2007 and 2020 was evaluated. URL was done for 28 (4.2%) patients as unplanned re-explorative surgery; 22 (78.6%) of them was female. The median age was 57 year (ranging, 24-76 years). There were 22 (78.6%) elderly patients over 65 years old. All standard clini-co-pathological characteristics, re-operative findings, and the morbidity-mortality results were analyzed. The well-known immunoscores such as neutrophil-lymphocyte ratio (NLR), neutrophil-thrombocyte ratio (NTR), and CRP-albumin ratio (CAR) were determined.The main indication for URL was small bowel anastomotic leak (n=13, 46.4%). The abdominal wall disruption (n=5, 17.9%) was the second indication. The frequent localization of injured organ was again small bowel. The 28.6% of patients (n=8) were re-op-erated in early postoperative period (in 7 days), while as the rest of them (n=20, 71.4%) in 90 days. There was only one repeat-URL patient in this series. Many of the URL patients (n=16, 57.1%) had more than one co-morbidities. Delving into the overall group, there were Clavien-Dindo (C-D) Grade I-II complications in 104 (16.4%) patients and C-D Grade III-IV in 88 (13.9%) patients, whereas in URL patient group, C-D Grade III-IV complications were seen in 22 (78.6%). In this prospective cohort, the overall mortality rate was 3.2% (n=20) in patients who were not re-explored. Six (21.4%) patients were lost in URL patients, which the main reason for fail-ure-to-rescue was sepsis due to entero-enteral anastomotic leak. In four of them, multiple co-morbidities were affected the post-URL period of complex cancer care. Pre-URL median NLR, NTR, and CAR values were 9.12 (ranging, 1.72-37.5), 0.03 (ranging, 0.01-0.12), and 41.4 (ranging, 4.2-181.3), respectively. NLR and CAR values (4.71 and 28.8) estimated before pre-CRS were also significantly high (p=0.01 and p0.01) in patients who were going to be operated for URL. These immunoscores values did not show any association in between pre-CRS and pre-URL mortal patients.The crucial decision-making factors at work were complex and complicated in 'unplanned' URL. The overall mor-bidity-mortality results seemingly depends on the severity and extent of peritoneal metastatic disease. Medically-unfit URL patients with high-risk factors should be selected to a vigilant monitoring and clinical care. Timely surgical intervention and intense management strategy are utmost important issues to lower morbi-mortality results in patients treated with URL.

Objective. Endometrial cancer is one of the most common malignancies in the world. There is no effective screening method used for predicting the clinical stage of the disease in the preoperative period and to plan the surgical procedure. The aim of this study is to compare the serum levels of CA-125 and HE-4 with MRI, to evaluate their ability to predict the depth of myometrial invasion in patients diagnosed with endometrial cancer, and to compare the specificity between these two tests. Materials and Methods. This prospective study was conducted at Dokuz Eylul University Faculty of Medicine, Department of Obstetrics and Gynecology, between February 2019 and February 2020. A total of 47 cases were included in the study. The cases were evaluated with MRI preoperatively to determine the extent of the disease. Preoperative serum CA-125 and HE4 levels were measured by ELISA method and compared, and the superiority of the tests to each other in determining the depth of myometrial invasion was evaluated. Results. In patients with endometrioid cancer, preoperative CA-125 and HE-4 levels were found to be higher if the myometrial invasion is >50% and lymphovascular space invasion is positive, and the difference was statistically significant. A cut-off of 7,05 U/ml for CA-125 could detect the deeper myometrial invasion with a sensitivity of 83% and a specificity of 52%, respectively. A cut-off of 94,25 pmol/L for HE-4 could detect deeper myometrial invasion with a sensitivity of 75% and a specificity of 56%, respectively. The sensitivity of MRI in determining the depth of myometrial invasion was 87,1%, and the specificity was 86,7%. Conclusions. MRI, HE-4 and CA-125 were found to be valuable tests in predicting the extent of the disease and planning the surgical treatment. All three methods successfully predicted the myometrial invasion depth of the disease. The success rates of the tests in predicting the depth of myometrial invasion were MRI, HE-4 and CA-125, respectively. HE-4 and CA-125 could not reach the sensitivity and specificity of MRI in determining the depth of myometrial invasion at any cut-off value. However, it was proved in this study that they can play important roles as simpler and more cost-effective tests in determining the extent of the disease, able to contribute to surgical planning during preoperative evaluation.

Aim:Peritoneal Surface Disease Severity score (PSDSS) has been used in the evaluation of patients who are scheduled for cytoreductive surgery (CRS) and hyperthermic intraperitoneal chemotherapy (HIPEC) for peritoneal metastasis of colorectal origin (PMCO). The aim of this study was to evaluate the validity of PSDSS in predicting prognosis in patients who underwent CRS + HIPEC for PMCO at our center.Method:Demographic data, operation data, histopathological features, perioperative morbidity and mortality, and oncologic follow-up data were obtained retrospectively from the database and analyzed. Peritoneal carcinomatosis index (PCI) values were calculated from preoperative computed tomographies. PSDSS was calculated by evaluating clinical symptoms, PCI and histology of primary tumor. Two PSDSS groups were formed: PSDSS 1 and 2 groups as low PSDSS group, and PSDSS 3 and 4 groups as high PSDSS group.Results:Sixty-one patients, including 36 females (59%) and 25 males (41%), with PMCO who underwent CRS and HIPEC were included in the study. Forty-four patients were enrolled to the low PSDSS group and 17 patients were enrolled to the high PSDSS group. Three patients (3.2%) died during the perioperative period. Twenty-one patients (34.4%) had perioperative complications. The mean follow-up was 35.0±23.2 months. During the follow-up period, 36 patients (59%) had recurrence and 44 patients (72.1%) died. The mean survival was 46.5±5.5 months, and 1-,3- and 5-year survival rates were 85%, 47% and 21%, respectively. There was no correlation between low and high PSDSS groups in terms of morbidity and recurrence (p=0.486 and p=0.385, respectively). Mortality was more frequent in high PSDSS group (94% vs. 63%; p=0.024). The mean survival of patients in the low PSDSS group was significantly longer than in the high PSDSS group (57.2±6.7 months vs. 16.5±2.6 months; p=0.001).Conclusion:The findings of this study demonstrated the validity of PSDSS in predicting prognosis in patients with PMCO who were scheduled for CRS and HIPEC.

Summary: Background: The repair of difficult abdominal wall defects (AWDs) continues to be a crucial and demanding issue for surgeons. This study aimed to present the risk factors and the long-term results of usage of an expanded-polytetrafluoroethylene (e-PTFE) synthetic mesh for the AWR of difficult abdominal wall defects. Methods: This study included 156 adult patients who underwent difficult AWR with e-PTFE mesh for incisional hernia, ventral hernia, and created AWDs of various etiopathologies. The association between the risk factors and the postoperative complications of AWR was analyzed, and overall long-term outcomes of e-PTFE repair were assessed. Results: The median follow-up duration was 119.1 (ranging from 2 to 206) months. In 70 (44.8%) patients, there were major co-morbidities. A surgical site infection developed in 17 (10.9%) patients. Of these, only 2 (1.3%) patients had e-PTFE mesh infection. Seven (4.4%) patients experienced recurrence. Recalcitrant seroma formation occurred in 8 (36.3%) patients. Conclusion: E-PTFE synthetic mesh usage for difficult abdominal wall hernias can help the hernia surgeon obtain safe and durable long-term results of sound repair. Background The aim of this study was to evaluate the prognostic value of preoperative sarcopenia with regard to postoperative morbidity and long-term survival in patients with peritoneal metastasis from colorectal cancer treated with cytoreductive surgery (CRS) and hyperthermic intraperitoneal chemotherapy (HIPEC). Methods A longitudinal cohort study was conducted on patients with peritoneal metastases of colorectal origin treated with CRS-HIPEC between 2008 and 2018. Data on patient demographics, body mass index, operative characteristics, perioperative morbidity and survivorship status and oncological follow-up were obtained from the hospital registry. Sarcopenia was assessed using preoperative computed tomography (CT) findings. Results Sixty-five patients [mean (SD) age: 54.4 (13.4) years, 64.6% females] were included in the study. Sarcopenia was evident in 30.8% of patients, while mortality rate was 66.2% with median survival time of 33.6 months. Presence of sarcopenia was associated with older age (59.6 (9.2) vs. 52.1 (14.4) years, p = 0.038), higher likelihood of morbidity (70.0% vs. 35.6%, p = 0.015) and mortality (90.0% vs. 55.6%, p = 0.010) and shorter survival time (17.7 vs. 37.9 months, p = 0.005). Cox regression analysis revealed that the presence of sarcopenia (HR 2.245, 95% CI 0.996-5.067, p = 0.050) was a significant predictor of increased likelihood of mortality. Conclusions Preoperative sarcopenia is an independent prognostic factor of postoperative morbidity and shorter survival in CRC peritoneal metastasis patients treated with CRS-HIPEC. Our findings support the importance of preoperative screening for sarcopenia as part of preoperative risk assessment for better selection of CRS-HIPEC candidates or treatment modifications in CRC patients with peritoneal metastasis.

Introduction Living donor liver transplantation (LDLT) is performed with increasing frequency worldwide due to the shortage of donated organs. It is a life-saving procedure for the recipient, but, on the other hand, a major surgical procedure for healthy donors and it may cause morbidity and even mortality. Patients and Methods This research was completed at Dokuz Eylul University Faculty of Medicine Hospital General Surgery Department Liver Transplant Unit and included 280 cases (4 with simultaneous liver and kidney transplants from living donors) who underwent donor right hepatectomy for LDLT from June 2000 to June 2016. We analyzed the data of patients retrospectively. Results Of 280 donor right hepatectomies for LDLT, 181 were male (M; 64.6%) and 99 were female (F; 35.4%) (M/F: 1.82). Mean donor age was 31.2 ± 0.9 years (range, 18–56). Mean donor monitoring duration was 45 ± 2.4 months (range, 3–192 months). Mean body mass index (BMI) was 24.28 ± 2.96 kg/m2 (range, 18.1–32.42 kg/m2). In our study 72 cases (25.7%) developed postoperative complications. There were 17 Clavien grade 3A, 1 grade 3B, and 5 grade 4A complications and also 1 death due to pulmonary embolism. Conclusion Together with the increase in living donor surgery, the morbidity and mortality of these cases are becoming controversial. Full donor safety is only possible with appropriate donor choice requiring very detailed studies, a problem-free hepatectomy process, and close postoperative donor monitoring.

Introduction In living donor liver transplantation (LDLT), hepatic arterial continuity is crucial to avoid biliary leakage, biliary stricture, cholangitis, and graft and patient loss. Sometimes there exist factors making anastomosis difficult or even impossible. In these cases, a vascular graft may be needed to bridge the two arteries for revascularization. Method Medical records of 297 patients who underwent LDLT between June 2000 and July 2016 at the Hepatopancreatobiliary Surgery and Liver Transplantation Unit of Dokuz Eylul University Hospital were reviewed retrospectively. Twenty-eight (9%) patients younger than the age of 18 were excluded from the study. The remaining 269 patients were included in the study. We analyzed data of patients who developed hepatic arterial complications during or after LDLT and underwent revascularization using autologous interposed inferior mesenteric artery (IMA) grafts. Results In 8 (2.9%) of the 269 patients who underwent LDLT and were included in the study, autologous interposed IMA grafts were used for the hepatic artery revascularization. All of the patients were males. Their mean age was 42 (range, 25–57). The mean duration of follow-up was 83.25 months (range, 3–144 months). One patient developed intraoperative hepatic arterial thrombosis (HAT) after autologus IMA reconstruction and this patient needed retransplantation. No arterial complications developed in the other 7 patients. Conclusion Autologous interposed IMA graft could be used as an alternative vascular graft in hepatic artery revascularization to provide tension-free hepatic arterial continuity.

Aim The aim was to compare the pathological complete response (pCR) rate at 8 compared to 12 weeks' interval between completion of neoadjuvant chemoradiotherapy (CRT) and surgery in patients with locally advanced rectal cancer. Method This was a randomized trial which included a total of 330 patients from two institutions. Patients with locally advanced (T3-4N0M0, TxN+M0) rectal cancer were randomized into 8- and 12-week interval groups. All the patients received long-course CRT (45 Gy in 1.8 Gy fractions and concomitant oral capecitabine or 5-fluorouracil infusion). Surgery was performed at either 8 or 12 weeks after CRT. The primary end-point was pCR. Secondary end-points were sphincter preservation, postoperative morbidity and mortality. Results Two-hundred and fifty-two patients (n = 125 in the 8-week group, n = 127 in the 12-week group) were included. Demographic and clinical characteristics were similar between groups. The overall pCR rate was 17.9% (n = 45): 12% (n = 15) in the 8-week group and 23.6% (n = 30) in the 12-week group (P = 0.021). Sphincter-preserving surgery was performed in 107 (85.6%) patients which was significantly higher than the 94 (74%) patients in the 12-week group (P = 0.016). Postoperative mortality was seen in three (1.2%) patients overall and was not different between groups (1.6% in 8 weeks vs 0.8% in 12 weeks, P = 0.494). Groups were similar in anastomotic leak (10.8% in 8 weeks vs 4.5% in 12 weeks, P = 0.088) and morbidity (30.4% in 8 weeks and 20.1% in 12 weeks, P = 0.083). Conclusion Extending the interval between CRT and surgery from 8 to 12 weeks resulted in a 2-fold increase in pCR rate without any difference in mortality and morbidity.

The frequency of abdominal computed tomography examinations is increasing, leading to a significant level of patient dose. This study aims to quantify and evaluate the effects of automatic tube current modulation (ATCM) technique on patient dose and image quality in contrast-enhanced biphasic abdominal examinations.Two different scan protocols, based on constant tube current and ATCM technique, were used on 64 patients who visited our radiology department periodically. For three patient groups with different patient size, results from two protocols were compared with respect to patient dose and image quality. Dosimetric evaluations were based on the Computed Tomography Dose Index, dose length product, and effective dose. For the comparison of image qualities between two protocols, Noise Index (NI) and Contrast to Noise Ratio (CNR) values were determined for each image. Additionally, the quality of each image was evaluated subjectively by an experienced radiologist, and the results were compared between the two protocols.Dose reductions of 31% and 21% were achieved by the ATCM protocol in the arterial and portal phases, respectively. On the other hand, NI exhibited an increase between 9% and 46% for liver, fat and aorta. CNR values were observed to decrease between 5% and 19%. All images were evaluated by a radiologist, and no obstacle limiting a reliable diagnostic evaluation was found in any image obtained by either technique.These results showed that the ATCM technique reduces patient dose significantly while maintaining a certain level of image quality.Abdominal bilgisayarlı tomografi incelemelerinin sıklığı artmakta ve önemli miktarda hasta dozuna yol açmaktadır. Bu çalışma, kontrastlı, çift fazlı abdomen incelemelerinde otomatik tüp akım modülasyon tekniğinin hasta dozu ve görüntü kalitesi üzerine etkilerini ölçmeyi ve değerlendirmeyi amaçlamaktadır.Radyoloji anabilim dalını periyodik olarak ziyaret eden 64 hastaya sabit tüp akımı ve otomatik tüp akımı modülasyonu tekniğine dayanan iki farklı tarama protokolü uygulandı. Farklı hasta boyutlarına sahip üç hasta grubu için, iki protokolden elde edilen sonuçlar hasta dozu ve görüntü kalitesi açısından karşılaştırıldı. Dozimetrik değerlendirmeler, Bilgisayarlı Tomografi Doz İndeksi, doz uzunluk çarpımı ve etkin doza dayandırıldı. İki protokol arasındaki görüntü kalitesinin karşılaştırılması amacıyla her görüntü için Gürültü İndeksi (NI) ve Kontrast Gürültü Oranı (CNR) değerleri belirlendi. Ek olarak, her görüntü deneyimli bir radyolog tarafından öznel olarak değerlendirildi ve sonuçlar iki protokol arasında karşılaştırıldı.Otomatik tüp akım modülasyon protokolü ile arteriyel ve portal fazlarda sırasıyla %31 ve %21 doz düşüşleri sağlandı. Öte yandan, NI karaciğer, yağ ve aort için %9 ile %46 arasında bir artış gösterdi. CNR değerlerinin ise %5 ile %19 arasında azaldığı gözlendi. Tüm görüntüler bir radyolog tarafından değerlendirildi ve herhangi bir teknikle elde edilen görüntülerde güvenilir bir tanısal değerlendirmeyi sınırlayan bir engel bulunmadı.Bu sonuçlar, otomatik tüp akım modülasyon tekniğinin, belirli bir görüntü kalitesi seviyesini korurken hastanın dozunu önemli ölçüde azalttığını göstermiştir.

Aim There is a well-known risk of the emergence of hepatic failure in living donor transplant cases on whom are performed a right donor hepatectomy (RDH). There are different prevalence ratios in literature on this phenomenon. In our study, we aim to depict the prevalence of hepatic failure and risk factors in our cases regarding the most recent description criteria related to hepatic failure. Patients and Methods We included right liver donor hepatectomy cases who fit the donor evaluation algorithm at the Dokuz Eylul University Liver Transplantation Unit between the period of June 2000 and September 2017. The patients were evaluated regarding preoperative data. Liver failure was defined according to the International Study Group of Liver Surgery (ISGLS) criteria. We also included statistical analysis of risk factors that are potentially related to liver failure. Results We included a total of 276 patients. In 27 (9.7%) patients, we observed posthepatectomy liver failure (PHLF). In 26 (9.4%) patients, we observed Grade A liver failure; in 1 (0.3%) patient, we observed Grade B liver failure. We did not observe any Grade C hepatic failure. In patients with hepatic failure, we observed a significantly longer period of hospitalization (P = .007). Old age (odds ratio = 1.065, 95% confidence interval, 1.135–29.108, P = .035) and preoperatory red blood cell (RBC) transfusion (odds ratio = 5.749, 95% confidence interval, 1.019–1.113, P = .005) were shown as independent risk factors for PHLF. Conclusion Posthepatectomy liver failure is a vital complication of RDH. The risk can be decreased by careful selection of donor candidates. Elderly donor candidates and intraoperative RBC are independent risk factors for PHLF.

Aims: The objective of this study is to identify the diagnostic performance of three-dimensional transvaginal ultrasonography (3D-US) and magnetic resonance imaging (MRI) in detecting myometrial, lower uterine segment and/or cervical invasion in endometrial cancer patients.Materials and methods: In this prospective study, 40 patients diagnosed with endometrial cancer were performed 3D-US and MRI, preoperatively. Deep myometrial, lower uterine segment and cervical invasion were evaluated subjectively and results were compared with the final histology as a gold standard.Results: Diagnostic accuracy of 3D-US for detecting deep myometrial, lower uterine segment and cervical invasion were 87.5%, 80% and 85%, respectively. The same results for MRI were 75%, 65% and 70%, respectively. For deep myometrial, lower uterine segment and/or cervical invasion in endometrial cancer, 3D-US had higher sensitivity, specificity, negative and positive predictive value and accuracy than MRI. The combination of these two imaging techniques had an increased sensitivity of detecting all parameters related with tumoral invasion but decreased specificity and the accuracy.Conclusion: 3D-US had better performance in detecting myometrial, lower uterine segment and/or cervical invasion than MRI in endometrial cancer patients. Combination of these techniques was not preferred according to this study.

Parastomal hernia is a frequent complication of an abdominal wall stoma. Surgical repairs have high complication and recurrence rates. Several different techniques have been suggested to prevent parastomal hernia during stoma creation. The aim of the present case–control study was to evaluate the efficacy of modified Stapled Mesh stomA Reinforcement Technique (SMART) for prevention of parastomal hernia compared with conventional colostomy formation in patients who underwent open or laparoscopic rectal resection and end colostomy for cancer. Between January 2014 and May 2016, all consecutive patients who underwent open or laparoscopic resection and end colostomy for primary or recurrent rectal cancer were identified from a prospectively collected database. Since January 2014, one surgeon in our team has routinely offered modified SMART procedure to all patients who are candidates for permanent terminal colostomy. In the SMART group patients, while creating an end colostomy, we placed a standard polypropylene mesh in the retromuscular position, fixed and cut the mesh by firing a 31- or 33-mm-diameter circular stapler and constructed the stoma. In the control group, a stoma was created conventionally by a longitudinal or transverse incision of the rectus abdominis sheath sufficiently large for the colon to pass through. Twenty-nine patients underwent parastomal hernia prophylaxis with modified SMART and 38 patients underwent end-colostomy formation without prophylaxis (control group). Groups were similar in terms of age, sex and underlying conditions predisposing to herniation. Median follow-up time is 27 (range 12–41) months. Nineteen patients (28.4%) developed parastomal herniation. In the SMART group, 4 patients (13.8%) developed parastomal herniation which is significantly lower than the control group in which 15 patients (39.5%) developed parastomal herniation (p = 0.029). We did not observe mesh infection, stenosis, erosion or fistulation in the SMART group. One patient in the control group underwent surgical correction of stoma stricture, another patient underwent surgery for stoma prolapse and four patients underwent surgery for parastomal herniation. New systemic reviews and meta-analysis support parastomal hernia prevention with the use of a prophylactic mesh. Until more evidence is available, prophylactic mesh should be routinely offered to all patients undergoing permanent stoma formation. SMART is easy to use, safe and effective for paracolostomy hernia prophylaxis.

Background Diagnosis of peritoneal carcinomatosis (PC) may be missed by preoperative imaging. We are presenting our experience with incidentally detected PC of colorectal origin treated with cytoreductive surgery (CRS) and intraperitoneal chemotherapy (IPC) at the same operation. Methods Between January 2010 and September 2016, 19 patients underwent CRS and IPC due to incidentally detected PC of colorectal origin. Data were analyzed from a prospectively collected database. Results The median age was 59 (29–78). In three patients, PC was diagnosed during emergency surgery. The primary tumor was located in the rectum (three patients; one with recurrent disease), left colon (9 patients), and right colon (7 patients). All patients underwent CRS and IPC, and one patient operated laparoscopically. Median peritoneal cancer index (PCI) was 5 (range, 3–14), and complete cytoreduction (CC-0) was achieved in 14 patients. After CRS, 8 patients received early postoperative intraperitoneal chemotherapy (EPIC), 7 patients received hyperthermic intraperitoneal chemotherapy (HIPEC), and 4 patients received both HIPEC and EPIC. The median hospital stay was 9 (6–29) days. Postoperative complications occurred in 6 patients. There was no postoperative mortality. Median follow-up was 40.2 (12–94) months. Five-year overall survival was 63.2%. The predictive value of magnetic resonance-based pelvic measurements and tumor volume on the surgical difficulty and oncologic outcome were analyzed.Results125 patients were included. The independent risk factors related to the circumferential resection margin status were the pT stage [odds ratio (OR) 3.64, confidence interval (CI) 1.409-7.327] and tumor volume after neoadjuvant chemoradiotherapy (OR 1.59, CI 1.018-2.767). The operative time (p=0.014, OR 1.453) and pelvic depth (p=0.023, OR 1.116) were independent predictive factors for anastomotic leak. The median follow-up was 72 (2-113) months. Local recurrence was seen in 17 (14.1%) patients. Anastomotic leak (OR 1.799, CI 0.978-3.277), the circumferential resection margin status (OR 3.217, CI 1.262-7.870) and the relative tumor volume rate (OR 1.260, CI 1.004-1.912) were independent prognosticators of local recurrence. The 5-year overall survival was 66.7%. The circumferential resection margin status (hazard ratio: 4.739, CI 2.276-9.317), pN stage (OR 3.267, CI 1.195-8.930) and relative tumor volume rate (OR 2.628, CI 1.042-6.631) were independent prognostic factors for the overall survival.ConclusionsRelative dimensions of the tumor in the pelvis influence the local recurrence and overall survival rates. Magnetic resonance-based measurements can predict the difficulty of surgery and allow surgeons to consider the appropriate surgical approach.

Lung is primary capillary filter of venous drainage for many organs. As a result of this, it is the most common metastasis site of extrapulmonary organ tumours. Cystic and cavitary forms of lung metastases are unusual. In evaluation of patients with these lesions benign and malignant lesions should be distinguished from each other. In this case report, we aimed to present computed tomography findings of an unusual cystic form of lung metastasis that is originated from rectum adenocarcinoma.

Objectives: The aim of the study was to evaluate the frequency of anatomic variations of the hepatic duct bifurcation using magnetic resonance cholangiopancreatography (MRCP). Methods: A total of 1,160 consecutive patients, referred to our department for MRCP due to suspected pancreatobiliary disease or before liver transplantation, were reviewed retrospectively. A total of 149 patients with less than optimal results due to imaging limitations or secondary differentiations of bile duct anatomy were excluded from the study. The final study population was composed of 1,011 cases. Results: Of the 1,160 patients, 149 were excluded from the analysis. Typical biliary anatomy was observed in 79.4% of cases, but female potential living liver donors more frequently presented an anatomic variation. Typical anatomy was present in 75.7% of the females and 85.3% of the males (p < 0.05). Out of the remaining 1,011 patients, 208 (20.57%) were diagnosed with different levels of various anatomic variations of the intra- and extrahepatic biliary ducts. Of the 208 cases with diagnosed variations, 204 (98.07%) and 4 (1.92%) turned out to have 1 and 2 different variations, respectively. The trifurcation variant was observed in 81 cases (8.01%), while 73 subjects (7.23%) had an aberrant right biliary duct draining into the common hepatic duct. A right dorsocaudal branch draining into the left hepatic duct was present in 42 cases (4.15%). Four cases (0.4%) had 2 different variations and 8 (0.8%) had uncommon anatomic variations. Conclusions: Typical intrahepatic biliary anatomy is present in about 80% of the inhabitants of the Aegean region of Turkey, but anatomic variants seem to be more frequent in females as compared to males. Trifurcation was the most common anatomic variation in our study population. The presence of an aberrant right hepatic duct emptying into the common hepatic duct was the second most common observation amongst our findings.

Fibromas are classified in a spectrum from fibromas to fibrosarcomas according to the number of mitosis they include. Malignant fibrosarcomas which have aggressive pattern show higher mitotic activity and nuclear atypia. Cellular fibromas with less than 4 mitotic figures under 10 high power fields (HPF) are benign. "Mitotically active cellular fibromas" that are classified between the cellular fibromas and fibrosarcomas, have >= 4 mitotic figures in 10 HPF but do not have nuclear atypia. A very few cases of mitotically active cellular fibromas have been reported in the literature. In this report, we present the case of mitotically active cellular fibroma in a patient who applied to our clinic with the complaint of pelvic mass.

Objective: To assess the efficacy of high-resolution magnetic resonance imaging (HRMRI) for preoperative local staging in patients with rectal cancer who did not receive preoperative radiochemotherapy. Methods: In this retrospective study, 30 patients with biopsy proved primary rectal cancer were evaluated by HRMRI. Two observers independently scored the tumour and lymph node stages, and circumferential resection margin (CRM) involvement. The sensitivity, specificity, the negative predictive value and the positive predictive value of HRMRI findings were calculated within the 95% confidence interval. The area under the curve was measured for each result. Agreement between two observers was assessed by means of the Kappa test. Results: In T staging the accuracy rate of HRMRI was 47-67%, overstaging was 10-21%, and understaging was 13-43%. In the prediction of extramural invasion with HRMRI, the sensitivity was 79-89%, the specificity was 72-100%, the PPV was 85-100%, the NPV was 73-86%, and the area under the curve was 0.81-0.89. In the prediction of lymph node metastasis, the sensitivity was 58-58%, the specificity was 50-55%, the PPV was 43-46%, and the NPV was 64-66%. The area under the curve was 0.54-0.57. When the cut off value was selected as 1 mm, the sensitivity of HRMRI was 38-42%, the specificity was 73-82%, the PPV was 33-42%, and NPV was 79-81% in the prediction of the CRM involvement. The correlation between the two observers was moderate for tumour staging, substantial for lymph node staging and predicting of CRM involvement. Conclusion: Preoperative HRMRI provides good predictive data for extramural invasion but poor prediction of lymph node status and CRM involvement. Conflict of interest:None declared.

Vesicouterine fistula is a rare type of urogenital fistulas. It is most commonly observed after cesarean section (C/S) due to iatrogenic reasons. In this article, a case of a vesicouterine fistula which developed after C/S operation is presented. This was the patient’s second C/S and this time placenta previa totalis was the primary pathology. Since it is a rare complication, we found it interesting, and, in this article, this clinical problem was discussed with details about diagnosis and treatment in light of the literature.

Introduction. We discuss a rare gastrointestinal stromal tumor (GIST) case detected at the 10th postpartum week and we want to pay attention to the challenges and improvements in the diagnosis, surgery, chemotherapy, and follow-up of this rare tumor accompanied with the review of the current literature. Case Presentation. A 32-year-old multiparous woman presented with abdominal swelling 10 weeks after her second vaginal birth. Abdominal examination revealed a mass starting from the pelvic level and extending to the right upper quadrant. Radiological examinations showed a solid, multiloculated, and hypervascular mass starting from the pelvis and extending to the transverse colon. En bloc mass with a 20 cm jejunal segment resection and a left pelvic side wall peritonectomy with omentectomy was performed. The pathologic examination revealed a high-risk GIST which originated from the jejunum and disseminated to the peritoneum. The patient has been given imatinib 400 mg/day since then. She did not reveal any progression during the 15-month follow-up postoperatively. Conclusion. GIST tumors are rare and there is not sufficient information in the literature regarding its management. In this patient having high risk GIST and GIST sarcomatosis we successfully treated the patient by surgery and adjuvant imatinib chemotherapy.

We aimed to evaluate the relationship between gastrectomy and the volume of liver segments II and III in patients with gastric cancer.Computed tomography images of 54 patients who underwent curative gastrectomy for gastric adenocarcinoma were retrospectively evaluated by two blinded observers. Volumes of the total liver and segments II and III were measured. The difference between preoperative and postoperative volume measurements was compared.Total liver volumes measured by both observers in the preoperative and postoperative scans were similar (P0.05). High correlation was found between both observers (preoperative r=0.99; postoperative r=0.98). Total liver volumes showed a mean reduction of 13.4% after gastrectomy (P = 0.977). The mean volume of segments II and III showed similar decrease in measurements of both observers (38.4% vs. 36.4%, P = 0.363); the correlation between the observers were high (preoperative r=0.97, P0.001; postoperative r=0.99, P0.001). Volume decrease in the rest of the liver was not different between the observers (8.2% vs. 9.1%, P = 0.388). Time had poor correlation with volume change of segments II and III and the total liver for each observer (observer 1, rseg2/3=0.32, rtotal=0.13; observer 2, rseg2/3=0.37, rtotal=0.16).Segments II and III of the liver showed significant atrophy compared with the rest of the liver and the total liver after gastrectomy. Volume reduction had poor correlation with time.

Aims Solid-pseudopapillary neoplasm (SPN) is an uncommon malignant tumor of the pancreas with a favorable prognosis unlike other pancreatic neoplasms. We investigated the clinicopathological features of 20 patients with SPN in details. Methods The patients diagnosed as SPN in Dokuz Eylul University Hospital between January 2005 and March 2016 were reviewed in terms of clinical and histopathological data. Results Mean age of the patients was 33. Three of our cases were male and 4 were children. Some patients had synchronuous malignancies. Nine patients were diagnosed by fine needle aspiration cytology (FNAC). One of our tumors had 2 × 1 mm of pancreas endocrine neoplasm in addition to SPN. One case had foci of atypical and multinucleated giant cells. All cases were positive for vimentin and CD10 antibodies and most were positive for PR and β-catenin. The mean follow-up duration was 40 mo (range 2–110 mo). Only one case showed liver metastasis. Conclusion Herein we present a series of 20 patients with 3 male and 4 pediatric cases, almost half of which were diagnosed with FNAC findings, and most of which are clinically being followed with one patient showing progression. Our series includes rare examples like collision tumor of SPN and pancreas endocrine neoplasm, SPN with multinucleated giant cells. Also cases with no surgical treatment and no progression, as well as cases with synchronous malignancies are presented. We believe that FNAC findings of any pancreatic mass should be investigated in detail for the designation of a therapy plan especially for the patients with high operation risks. The findings in our series also show that extensive necrosis, angioinvasion, perineurial invasion and larger tumor size might be predictive for worse prognosis and these patients should be more closely followed up.

Transanal single-port access microsurgery (TSPAM) is an emerging and feasible minimally invasive method for the transanal excision of large sessile adenomas and early-stage carcinomas of the rectum. Here we present our TSPAM experience on rectal adenomas (high-grade rectal adenomas in 5 cases and carcinoma in situ in 1 case). TSPAM is an innovative method that can be an affordable and disseminated alternative to transanal endoscopic microsurgery for the local excision of the rectal lesions.

To investigate the effect of intraoperative ultrasonographic (US) findings on the decision for the type of nephrectomy to be performed in patients who had renal tumors that were preoperatively evaluated by using magnetic resonance (MR) imaging, with pathologic results as the reference standard.The institutional review board approved the study protocol, and informed consent was obtained. Between June 2008 and September 2009, 44 patients (25 men, 19 women; mean age, 56.6 years; range, 28-76 years) with 46 renal tumors were prospectively assessed by using intraoperative US examinations to demonstrate tumor relationship with the nontumoral intact parenchyma. Findings at preoperative MR examinations were retrospectively evaluated by two radiologists to determine the type of surgery that would be recommended. The reference standard was results of pathologist's review of gross specimens and postoperative reports. The observers assigned their decisions as follows: score group 1, radical nephrectomy should be (should have been) performed; score group 2, partial nephrectomy can be (could have been) attempted; and score group 3, partial nephrectomy should be (should have been) performed.Radical nephrectomy was performed in 36 lesions. In all cases, the intraoperative US observer and the pathologist were concordant in the decision that radical nephrectomy versus partial nephrectomy could or should have been performed. MR observers 1 and 2 overcalled the need for radical nephrectomy in seven and four cases, respectively. Compared with pathologic results, the overall correlation of intraoperative US was 0.991, and the correlation for MR observer 1 was 0.786 and that for MR observer 2 was 0.731.Intraoperative US can be suggested as a valuable examination method in patients with tumors at a central location with suspicious renal sinus extension demonstrated by using MR imaging. The close cooperation of urologist and radiologist in renal tumor work-up could reduce performance of unnecessary radical nephrectomy.

A 31-year-old female presented with acute left flank pain; she had a C/S at the postpartum day 24. Ureteral stone was suspected but ultrasound examination was normal. Then Doppler ultrasound revealed a trombus in left renal vein and inferior vena cava. Contrast enhanced MDCT scan showed swelled and nonfunctional left kidney, a trombus including distal part of left ovarian vein, left renal vein, and inferior vena cava. We started anticoagulation treatment. Further examination revealed diagnosis of chronic myeloproliferative disease. The trombus was completely recanalized at 3-month followup.

The ability to predict response to chemoradiotherapy before the treatment may allow protecting poorly responding patients from the side effects of neoadjuvant treatment. Several molecular markers have been proposed to radio and chemosensitivity of rectal cancer. In this study, from pre-irradiation tumor biopsies, a novel and promising candidate factor survivin, and p53 and Ki-67 were assessed as predictors of response to preoperative chemoradiotherapy.Expression of each marker was evaluated by immunohistochemistry on pretreatment biopsies from 37 patients having rectal cancer treated with preoperative chemoradiotherapy and curative surgery. Treatment response was assessed histopathologically in the resected surgical specimen.There was no correlation between expression of p53, Ki-67, and survivin with response to preoperative chemoradiotherapy and prognosis.Our data suggest that these molecular markers are not helpful to identify patients who would have benefit from neoadjuvant treatment of rectal cancer. Further investigations are necessary to select patients for preoperative treatment based on analysis of the preoperative biopsies.

With the advent of technological improvements, computed tomography (CT) became one of the important diagnostic tools in the evaluation of local characteristics, preoperative staging, and prognostic factors of colon cancers (1). CT is recommended by EURECCA consensus group for staging of colon cancers (2). Extramural invasion (EMI) is an important factor affecting the prognosis in patients with colon cancer (3). Preoperative CT can detect EMI in colon cancers with high sensitivity (4). In addition, CT-based T staging can be used to stratify patients into good and poor prognosis (4, 5). Correlation between local recurrence and circumferential resection margin involvement in rectal cancer suggests the importance of retroperitoneal surgical margin (RSM) involvement in retroperitoneal ascending and descending colon tumors. RSM involvement is defined as less than 1 mm distance between RSM and primary adenocarcinoma or metastatic retroperitoneal lymph node in descending and ascending colon cancers (6). Studies suggest that RSM positivity may be a predictor and an independent prognostic indicator showing local recurrence in colon cancers (7). Classical colon cancer treatment is based on histopathologic prognostic factors in the resected specimen (1). However, at the present time, with the development of more effective chemotherapeutic agents and higher accuracy in preoperative staging, neoadjuvant treatments are preferred in patients with high-risk colon cancer (2, 8). Preoperative radiologic assessment of EMI and RSM positivity can decrease the local recurrence risk through timely recommendation of neoadjuvant chemotherapy which would lead to regression of metastatic lymph nodes, retroperitoneal extension, and tumor burden (6–8). Since severe adverse effects can be observed, neoadjuvant treatment should be administered to patients who would benefit the most from it. The aim of this study was to evaluate preoperative T and N staging and RSM involvement in colon cancer using multidetector CT (MDCT) and compare them with histopathology results.

AimWe aimed to investigate the value of PET-CT in therapy response and the correlation of quantitative PET parameters with histopathologic results in patients with locally advanced rectal cancer (LARC) before and after neoadjuvant chemoradiotherapy. We also analyzed the correlation of PET-CT parameters between Ki-67 and glucose transporter 1 (GLUT1).Patients and methodsA total of 29 patients diagnosed with LARC who had undergone a biopsy between 2009 and 2012 were included in our study.
