The authors report a case of a 61 years old man with incomplete duodenal diaphragm. According to literature this affection is very rare in grown up persons: 38 adults only were reported until 1977. The most supported pathogenetic hypothesis is that the duodenal membrane is secondary to an embryologic defect. Endoscopy is essential for a correct diagnosis before surgical operation. In fact especially in adults X-rays hardly distinguish intraluminal diverticulum from membraneous stenosis. The recommended operation is duodenum resection with membrane removal.
