GİRİŞ: Sinovyal sarkom mezenşimal ve epitelyal diferansiasyon göstermekte ve tümyumuşak doku sarkomlarının %5-10 ‘unu oluşturmaktadır. Sıklıkla olguların %65’ineyakını alt ekstremite yerleşimlidir. Toraksın primer sinoviyal sarkomu ise oldukçanadir görülmekte olup akciğer, plevra veya göğüs duvarı tutulumu izlenebilir. Ayrıcaliteratürde boyun, intraartikular, dil, larenks, mediasitinum, kalp, karın duvarı,vasküler, retroperiton yerleşimli olgular bildirilmiştir. Biz de nadir görülen primerakciğer sinoviyal srakomlu olgumuzu sunmaktayız.OLGU: 60 yaş erkek hasta, 1 aydır öksürük, minör-major hemoptizi ve kilo kaybıkliniği mevcutmuş. Bilgisayarlı tomografide(BT)’sinde sol parahiler bölgede, 6 cmboyutunda, sol üst lob bronşunu oblitere eden kitlesel lezyon izlenmiş (SUVmax:4,7).Hemoptizi sebebiyle yapılan rijid bronkoskopide sol ana bronş tümör ile tamamentıkalı olarak izlenmiş. Tanı amaçlı tru-cut biyopsi yapılmış ve patoloji raporu iğsihücreli malign tümör ön planda sinovyal sarkom olarak sonuçlanmış. Pet/bt’sinde ektutulumu olmayan kitleden malignite tanısı olan hastaya intraperikardiyal solpnömonektomi uygulandı. Raporda; mevcut tümörün en büyük boyutu 6 cm, mitotikhızı (2 milimetre karelik alanda 10'un üzerinde) ve histolojik derecesi yüksek olduğubelirtilmişti. Tümör mediastinal plevra ve hiler yağlı dokuya, myokardkomşuluğundaki fibröz dokuya invaze ve lenfovasküler aralık invazyonu mevcuttu,sinir invazyonu yoktu. Bronş ve damar cerrahi sınırları salim, in-situ ya da invazivkarsinoma rastlanmadı. Disseke edilen 5, 6, 7, 8, 9 ve 10.nolu lenf düğümleri reaktifözellikte olduğu ancak materyal üzerinden diseke edilen lenf düğümlerinin birindesarkom metastazı izlenmişti (T4N1M0).Hastaya adjuvan 8 kür KT ve 10 seans RT uygulandı. Takiplerinde postop 11. aydamultilober pnömoni sebebiyle hastane yatışı olan ve yatışı sırasında akut koronersendrom gelişen hasta, kardiak arrest sebebiyle kaybedilmiş.SONUÇ: Akciğerin primer sinovyal sarkomları oldukça nadirdir ve akciğerkanserlerinin %0,1 ila %0,5'ini oluşturur. Göğüs ağrısı ve dispne en sık tanımlananbaşvuru semptomlarıdır. Toraks görüntülemelerinde mediastinal şifte sebepolabilecek büyük kitleler olarak tespit edilir. Tümör boyutu nedeniyle tam cerrahirezeksiyon genellikle mümkün değildir. Neoajuvan ve adjuvan kemo-radyoterapi iletedavi şansı mevcuttur. Beş yıllık sağkalım 5 cmtümör, yüksek mitoz sayısı, nörovasküler invazyon ve sitogenetik SYT-SSX1 varyantvarlığıdır.

The determination of primary focus in metastasis is crucial. CDX2 and SATB2 for colorectal, TTF-1 and Napsin-A for lung, GCDFP-15 and GATA-3 for breast, and PSA, PSAP, and NKX3.1 for prostate carcinomas are the most useful site-specific biomarkers. Most metastatic carcinomas in the liver are adenocarcinomas. The most common primary sites are the colon-rectum, breast, lung, stomach, and pancreas. A panel including CK7, CK20, CDX2, GATA-3, TTF-1, and PAX-8 in women and CK7, CK20, CDX2, TTF-1, and NKX3.1 in men seems reasonable for metastatic adenocarcinoma in the liver if the tissue is limited. If available, adding SATB2, Napsin-A, and PSA/PSAP to the panelincreases the diagnostic value. In other types of carcinomas, morphological features are more important to decide on a panel which may contain CK7, CK20, p40, GATA-3, chromogranin A, synaptophysin, RCCMa/PAX-8, SF-1, arginase-1, HepPar1, etc. Breast, colon, and prostatecarcinomas often metastasize to the lung, whereas the most common sources are the prostate, breast, lung, and kidney in the bone. Similar panels may be useful in detection of the primary in most cases. The differential diagnosis of primary site in lymph nodes varies according to the region. Although there is no specific marker for primary focus in squamous cell carcinomas, determination of EBV and HPV, especially in cervical lymph node metastases, is significant for the nasopharynx or oropharynx primary.

ObjectiveDue to the limited intrinsic healing and repair capacityof the articular cartilage, most treatment methodscannot achieve reliable regeneration of normalhyaline cartilage, resulting in early developmentof osteoarthritis. The purpose of this study was todetermine the effects of human amniotic fluid andmembrane on chondral defects.Material and MethodsSixty-four knees of 32 immature New Zealand rabbitswere included in the study. Full thickness chondraldefects were created in the weight-bearing surface ofthe medial condyles of the rabbits. The rabbits weredivided randomly into four groups: no adjunct treatmentwas given in group 1, 0.3 ml human amniotic fluid(HAF) alone in group 2, human amniotic membrane(HAM) alone in group 3 and both of 0.3 ml HAF andHAM in group 4 was administered. The condyles werehistopathologically evaluated at 4th and 12th weekusing the modified O’Driscoll Grading Scale.ResultsThere were no significant differences in the qualityof the regenerated tissue within and between groups(p>0.05). The mean results of groups at the 12th weekwere worse than results at the 4th week; however,the difference was statistically significant for only thesham group (group 1) and the combined therapygroup (group 4) (p=0.007 and p=0.014, respectively).ConclusionHAF alone, HAM alone, and combined administrationof both biomaterials neither affected chondral defecthealing nor had any differences between eachother. Nevertheless, we believe that some earlyregeneration due to an intrinsic repair mechanism ispossible in immature rabbits as this study showedbetter results at 4th week than those at 12th week,although they are prone to degenerative processes inlong-term follow-up. We suggest that a larger samplesize in an experimental study would probably displaya statistically significant difference when investigatingeffects of HAF, HAM, or both., AmaçArtiküler kıkırdağın kısıtlı intrensek iyileşmesi ve tamirkapasitesinin az olması nedeniyle çoğu tedavimetodları ile normal hyalen kıkırdak rejenerasyonusağlanamamakta ve sonuç olarak osteoartroz gelişebilmektedir.Bu çalışmada insan amniyotik sıvısı vemembranının tavşanlardaki kondral defektlerdeki etkileriniincelemek amaçlanmıştır.Gereç ve Yöntem32 immatür albino Yeni Zelanda tavşanının 64 dizi buçalışmaya dahil edildi. Tam kat kıkırdak defektleri tavşanlarınmedial kondillerinin yük taşıma yüzeylerindecerrahi olarak oluşturuldu. Tavşanlar randomize olarakdört gruba bölündü: ek tedavi verilmeyenler Grup1, 0,3 ml insan amniyotik sıvısı (İAS) verilenler Grup2, sadece insan amniyotik membranı (İAM) verilenlerGrup 3, hem 0,3 ml İAS hem İAM verilenler Grup4’ü oluşturdu. Kondiller histopatolojik olarak 4. ve 12.haftalarda modifiye O’Driscoll evreleme skalası iledeğerlendirildi. Sonuçlar Mann-Whitney U ve ANOVAtestleri ile istatistiksel olarak analiz edildi.BulgularGruplar arasında rejenere dokunun kalitesi açısındanistatistiksel olarak anlamlı fark bulunmadı (p>0.05).Grupların ortalama sonuçları 12. haftada, 4. hafta sonuçlarındandaha kötü olarak saptandı; buna rağmenbu farkın, sadece Grup 1 (sham grubu) ve Grup 4 açısındananlamlı olduğu görüldü (İAS+İAM) (sırasıyla,p=0,007 ve p=0,014).SonuçSadece İAS, sadece İAM ve her iki biyomateryalinkombine edildiği tedavi yöntemlerinin hiçbirinde kıkırdakdefekt iyileşmesi daha iyi ve kaliteli olmadığıve birbirlerine de bir üstünlüğü olmadığı görüldü. Buçalışmada gösterilmiş olan 4. hafta sonuçlarının 12.hafta sonuçlarından daha iyi olmasının çalışmadakullanılan immatür tavşanlardaki intrensek tamir mekanizmasındakierken rejenerasyon kapasitesindenkaynaklandığını düşünmekteyiz.

In recent years, demand for products produced with goat milk has been increasing. On the other hand, compared to cow's milk, component and flavour differences in goat milk can cause significant differences in the properties of products produced with goat's milk and affect the acceptability of the product. In this study, it was aimed to compare the antioxidant capacity, rheological and sensory properties of kefir produced with goat milk with those properties of kefir produced of cow milk. In this context, cow milk, goat milk and 1: 1 ratio of cow and goat milk kefir were produced, physico-chemical, rheological and sensory analyses were performed. It was determined that the rheological properties of all kefir samples exhibit pseudoplastic behaviour and are compatible with the power-law model. The apparent viscosity of the kefir produced by the goat milk was lower than cow milk kefir. It was determined that the total antioxidant capacity of goat milk kefir was higher than cow's milk kefir and a and b values were lower in colour. On the other hand, in goat milk kefir, panellists perceived lower consistency and kefir flavour, also its overall acceptability score was lower. The addition of 50% cow's milk to goat's milk influenced the condition of goat's milk. The sensory properties of the mixture milk kefir were similar to the sensory properties of the cow's milk kefir. Although the mixture kefir positive results, it has been concluded that further studies are needed to obtain better consistency and flavour in kefir production from goat milk.
