Entre el 15 y el 17 de noviembre Barcelona acogerá el V Congreso Nacional de Farmacéuticos Comunitarios, organizado por SEFAC y que ha sido reconocido de interés sanitario por el Ministerio de Sanidad, Servicios Sociales e Igualdad. La edición de este año tiene como lema Soluciones para una farmacia necesaria, efectiva y segura y no puede ser más oportuna esta idea en el contexto actual en el que se desenvuelve la profesión. Desde hace años vivimos tiempos de incertidumbre y dificultades, jalonados por continuas rebajas en los precios de los medicamentos y agravados por la actual coyuntura económica, lo que se ha traducido en la puesta en marcha de medidas improvisadas, que están generando mucha incertidumbre en un sector que necesita estabilidad. Últimamente estamos sufriendo el establecimiento de nuevos copagos, la desfinanciación de medicamentos, los cambios periódicos en los precios menores y numerosos impagos que arriesgan la viabilidad de muchas farmacias. El Congreso de noviembre quiere ser el punto de encuentro ideal y un punto de inflexión para que los farmacéuticos comunitarios de toda España compartamos nuestras inquietudes y experiencias en la búsqueda de propuestas que nos permitan encarar el futuro con optimismo. Somos conscientes de que en estos momentos el desánimo abunda entre muchos compañeros farmacéuticos, pero no debemos dejar que ese sentimiento nos arrastre, pues son muchas las actividades que el farmacéutico lleva a cabo en la farmacia comunitaria por la sociedad, los pacientes, la sostenibilidad del sistema sanitario y, en definitiva, por sí misma. Debemos ser capaces de transmitir esto y darle visibilidad y, para ello, es imprescindible que no bajemos los brazos y trabajemos desde la unión y el compromiso en defensa de los valores que aporta la farmacia asistencial y el farmacéutico como verdadero experto en medicamentos.

¿Por qué muchos pacientes no toman bien sus medicamentos? ¿Estamos explicando bien los efectos de los tratamientos y su forma de seguirlos? ¿Informamos de los posibles efectos secundarios de determinadas terapias? ¿Comentamos suficientemente las consecuencias para la salud de un posible abandono? ¿Compartimos datos entre los distintos agentes de la cadena del medicamento? ¿Lanzamos mensajes homogéneos para no confundir al paciente? ¿Contamos con la opinión de los pacientes para que sean conscientes de por qué tienen que seguir sus tratamientos, cómo, hasta cuándo y qué pueden y no pueden esperar de ellos? Todas éstas y otras muchas son cuestiones que deberíamos plantearnos para encontrar una respuesta a una evidencia cada día más preocupante: la falta de adherencia terapéutica se ha convertido en una realidad tangible y es, hoy por hoy, uno de los principales retos que deben enfrentar los sistemas sanitarios para su buen funcionamiento.

Tras el éxito de nuestro último Congreso Nacional de Farmacéuticos Comunitarios celebrado el pasado noviembre en Barcelona, desde SEFAC seguimos trabajando en acercarnos aún más si cabe a nuestros socios y simpatizantes, y así hemos puesto en marcha la Jornada SEFAC en cada delegación, que conjuga el hecho de ser una sociedad científica fuerte a nivel nacional, con las diferentes realidades y peculiaridades existentes a nivel más local que representan nuestras delegaciones. A la vez, estas jornadas pretenden profundizar, a pesar de las amenazas existentes, en las grandes oportunidades y retos estimulantes que se presentan al farmacéutico comunitario.

¡Objetivo superado! De esta forma podemos resumir el V Congreso Nacional de Farmacéuticos Comunitarios, que se celebró en Barcelona del 15 al 17 de noviembre. Gracias a todos los que habéis hecho posible que el objetivo se haya cumplido y superado con creces, sobre todo a mis más próximos colaboradores en el comité organizador. Puede sonar a autocomplacencia, pero sin dejar de lado la búsqueda imprescindible de la mejora continua que debe guiar nuestras actuaciones, como presidente del comité organizador del congreso, mis impresiones y, sobre todo, las de muchos compañeros farmacéuticos que estuvieron presentes en Barcelona, me transmiten que el congreso ha servido para incrementar la ilusión y el orgullo de ser farmacéuticos comunitarios. Una ilusión y un orgullo que percibo más que nunca en primera persona porque, como bien sabrá el lector, la Asamblea General de SEFAC, celebrada el 15 de noviembre, me designó nuevo presidente de la Sociedad en sustitución de Marichu Rodríguez. Quiero agradecer desde estas líneas su inmensa labor y dedicación (así como la del resto de compañeros de su Junta Directiva que no continúan) en estos últimos cuatro años en los que SEFAC ha vivido un magnífico crecimiento.

BackgroundProgressive supranuclear palsy (PSP) -Richardson's Syndrome and Corticobasal Syndrome (CBS) are the two classic clinical syndromes associated with underlying four repeat (4R) tau pathology. The PSP Rating Scale is a commonly used assessment in PSP clinical trials; there is an increasing interest in designing combined 4R tauopathy clinical trials involving both CBS and PSP.ObjectivesTo determine contributions of each domain of the PSP Rating Scale to overall severity and characterize the probable sequence of clinical progression of PSP as compared to CBS.MethodsMulticenter clinical trial and natural history study data were analyzed from 545 patients with PSP and 49 with CBS. Proportional odds models were applied to model normalized cross-sectional PSP Rating Scale, estimating the probability that a patient would experience impairment in each domain using the PSP Rating Scale total score as the index of overall disease severity.ResultsThe earliest symptom domain to demonstrate impairment in PSP patients was most likely to be Ocular Motor, followed jointly by Gait/Midline and Daily Activities, then Limb Motor and Mentation, and finally Bulbar. For CBS, Limb Motor manifested first and ocular showed less probability of impairment throughout the disease spectrum. An online tool to visualize predicted disease progression was developed to predict relative disability on each subscale per overall disease severity.ConclusionThe PSP Rating Scale captures disease severity in both PSP and CBS. Modelling how domains change in relation to one other at varying disease severities may facilitate detection of therapeutic effects in future clinical trials.

Objectives: To have a group of COPD patients undergo a simple program of home-based exercise training, using the shuttle walking test (SWT) to standardize the intensity of training.Methods: Sixty patients participated, randomly distributed into two groups (rehabilitation and control) of 30 patients each. The following evaluations were carried out at baseline and at 12 weeks: (1) pulmonary function studies; (2) SWT; (3) submaximal intensity resistance test; (4) cycle ergometer test; (5) quality of life; and (6) dyspnea. The rehabilitation group underwent a lower-extremity training program. Walking was selected as the type of exercise. The intensity of training was set at 70% of the maximum speed attained on the SWT. Divided sessions were held, lasting 1 h, 6 days/wk, at home, with a checkup every 2 weeks. The duration of the program was 12 weeks.Results: The following patients completed the study: 20 patients (66.6%) from the rehabilitation group (mean [+/- SD]) age, 64.3 +/- 8.3 years; mean FEV(1), 41.7 +/- 15.6% of predicted); and 17 patients (56.6%) from the control group (mean age, 63.1 +/- 6.9 years; mean FEV(1), 40 +/- 16.4% of predicted). We found no changes in pulmonary function or effort parameters (SWT or cycle ergometer) in the rehabilitation group at 12 weeks. A twofold increase (1,274 +/- 980 to 2,651 +/- 2,056 m; p < 0.001) was achieved in the submaximal intensity resistance test, with less dyspnea at the conclusion of the test (p = 0.05). Significant improvement also was achieved in basal dyspnea and, both statistically and clinically, in the quality of life. Significant changes were not achieved in the control group patients.Conclusions: A simple home-based program of exercise training achieved improvement in exercise tolerance, posteffort dyspnea, basal dyspnea, and quality of life in COPD patients. In recent decades, the invasive seaweed Rugulopteryx okamurae has had a huge environmental impact on marine biodiversity, fisheries, GHG emissions and public health along much of the Iberian Peninsula and islands coastline. Due to the enormous amount of algae biomass that is expelled to the beaches where it slowly rots, some circular economy business initiatives, such as composting, are emerging. In the present study, we compared the quality of compost obtained from earthworms (Dendrobaena veneta), cockroaches (Eublaberus sp.), mealworms (Tenebrio molitor) and black soldier fly larvae (Hermetia illucens). Batches fed with 100% organic kitchen waste (control group) were compared with batches fed with 50% algae and 50% kitchen waste (treatment group). Our results show that the most sensitive species (D. veneta and T. molitor) to R. okamurae toxins compost adequately. The C/N ratio, electrical conductivity (EC), pH, total organic matter (TOM), C, K, K2O, Mg, MgO, N, P, P2O5, B, Cu, Ni and Zn of the compost obtained were determined. A high quality compost was obtained in which only the EC values are slightly elevated. Particularly good was the compost obtained with H. illucens and Eublaberus sp. This quality is in agreement with previous research on the mass balance of composting. Objectives: This study aimed to provide information on the training profile and characteristics of injuries sustained by obstacle course racing competitors., Methods: This research is a nationwide cross-sectional, self-administered online survey conducted in 2023., Results: We analyzed the data collected from 201 obstacle course racing participants (mean age: 33.8 ± 7.1 years; 60.7% men). On average, athletes had 2.9 ± 2.4 years of obstacle course racing experience and participated in approximately four races in the previous year. Most of these races covered distances of 5-10 km (65.2%). Among the participants, 28.4% reported injuries, with the upper extremities being the most frequently affected. Approximately 19% of these injuries required medical supervision, and 2% led to hospitalization., Conclusion: Injury risk in obstacle course racing participants was associated with participation in a higher number of obstacle course racing competitions, reduced time between these competitions and having performed specific obstacle training.

Predator-prey interactions are the interspecific relationships of greatest interest in ecology. Spiders are among the most diverse and ubiquitous terrestrial predators on the planet. Their large dietary breadth is often linked with the development of specific predatory behaviors and morphological adaptations. However, studies on the predatory behavior of spiders have mostly focused on specialist species, leaving behind the ethological variability occurring in generalist species that allow them to respond to the different prey types. For three species of generalist wandering spiders, we searched images of predation events on the Internet to determine the most common prey. Subsequently, the focal predator species were then used in behavioral experiments. Using high-speed videos, handling patterns for different prey types (spider and cricket) were analyzed. Our results show a notable difference in handling patterns between prey types. We found that the spider prey was often rotated around the axis allowing the predator to bite in the ventral region of the prey and thus avoid a counterattack. Contrary, crickets were arbitrarily rotated. Among the main arboviruses affecting public health in tropical regions are dengue, zika, and chikungunya, transmitted mainly by mosquitoes of the Aedes genus, especially Aedes aegypti . In recent years, outbreaks have posed major challenges to global health, highlighting the need for integrated and innovative strategies for their control and prevention. Prevention strategies include the elimination of vectors and avoiding mosquito bites; this can be achieved through the use of bioinsecticides and repellents based on plant phytochemicals, as they offer sustainable, ecological, and low-cost alternatives. Mexico has a variety of plants from which both extracts and essential oils have been obtained which have demonstrated significant efficacy in repelling and/or killing insect vectors. This review examines the current knowledge on plant species found in Mexico which are promising options concerning synthetic compounds in terms of their repellent and insecticidal properties against mosquitoes of the genus Aedes and that are friendly to the environment and health.

Objectives: To propose the grounds for "diabetic sarcopenia" as a new comorbidity of diabetes, and to establish a muscle screening algorithm proposal to facilitate its diagnosis and staging in clinical practice., Method: A qualitative expert opinion study was carried out using the nominal technique. A literature search was performed with the terms "screening" or "diagnostic criteria" and "muscle loss" or "sarcopenia" and "diabetes" that was sent to a multidisciplinary group of 7 experts who, in a face-to-face meeting, discussed various aspects of the screening algorithm., Results: The hallmark of diabetic sarcopenia (DS) is muscle mass atrophy characteristic of people with diabetes mellitus (DM) in contrast to the histological and physiological normality of muscle mass. The target population to be screened was defined as patients with DM with a SARC-F questionnaire > 4, glycosylated haemoglobin (HbA1C) ≥ 8.0%, more than 5 years since onset of DM, taking sulfonylureas, glinides and sodium/glucose cotransporter inhibitors (SGLT2), as well as presence of chronic complications of diabetes or clinical suspicion of sarcopenia. Diagnosis was based on the presence of criteria of low muscle strength (probable sarcopenia) and low muscle mass (confirmed sarcopenia) using methods available in any clinical consultation room, such as dynamometry, the chair stand test, and Body Mass Index (BMI)-adjusted calf circumference. DS was classified into 4 stages: Stage I corresponds to sarcopenic patients with no other diabetes complication, and Stage II corresponds to patients with some type of involvement. Within Stage II are three sublevels (a, b and c). Stage IIa refers to individuals with sarcopenic diabetes and some diabetes-specific impairment, IIb to sarcopenia with functional impairment, and IIc to sarcopenia with diabetes complications and changes in function measured using standard tests Conclusion: Diabetic sarcopenia has a significant impact on function and quality of life in people with type 2 diabetes mellitus (T2DM), and it is important to give it the same attention as all other traditionally described complications of T2DM. Common ground is the knowledge, beliefs, and suppositions shared between partners in an interaction. Previous research has focused extensively on what partners know they know together, that is, "common knowledge." However, another important aspect of common ground is what partners know they do not know together, that is, "common ignorance." A new coordination game was designed to investigate children's use of common ignorance. Without communicating or seeing each other's decisions, 4- to 8-year-olds needed to make the same decision as their partner about whether to try to retrieve a reward. To retrieve it, at least one of them needed to know a secret code. The knowledge/ignorance of both partners was ostensively manipulated by showing one partner, both partners, or neither partner the secret code in four conditions: common knowledge (both knew the code), common ignorance (neither partner knew the code), common privileged self knowledge (only children knew the code), and common privileged other knowledge (only their partner knew the code). Children's decisions, latency, and uncertainty were coded. Results showed that the common ignorance states were generally more difficult than the common knowledge states. Unexpectedly, children at all ages had difficulty with coordinating when their partner knew the code but they themselves did not (common privileged other knowledge). This study shows that, along with common knowledge, common ignorance and common privileged self knowledge and other knowledge also play important roles in coordinating with others but may develop differently., (Copyright © 2024 Elsevier Inc. In this study, two different approaches based on taxonomic assemblages and on copepod functional groups were used to investigate the mesozooplankton assemblage structure and its relationship with environmental variables in the main estuaries of the Gulf of Cádiz (Guadalquivir, Guadiana and Tinto-Odiel) during the dry-warm season. In general, the mesozooplankton assemblages were dominated by copepods, especially the calanoid Acartia tonsa, which reached its highest abundance in the inner zones while the adjacent coastal zones were characterized by a mixture of copepods and cladocerans, especially Penilia avirostris. Regarding the trait-based approach, three copepod functional groups were identified, principally sorted by their feeding strategy. Group 1 (composed of omnivorous copepods displaying a mixed feeding strategy and broadcast-spawners) was found mainly in the inner areas, while Groups 2 (omnivorous cyclopoids, sac-spawners that feed via active ambush) and 3 (herbivores-omnivores employing a filter feeding strategy and mostly broadcast-spawners) were predominant in the adjacent coastal zones. The relative abundance of copepod functional groups suggested that Group 1 could be considered the most important contributor to secondary production in the estuarine systems of the Gulf of Cádiz. In relation to environmental factors, salinity was the most influential variable on mesozooplankton assemblages in both approaches. Our results suggest that the studied estuaries, although taxonomically different, have mesozooplankton assemblages that perform similar ecological functions. In recent years, the function of noncoding RNAs (ncRNAs) as regulatory molecules of cell physiology has begun to be better understood. Advances in viral molecular biology have shown that host ncRNAs, cellular factors, and virus-derived ncRNAs and their interplay are strongly disturbed during viral infections. Nevertheless, the folding of RNA virus genomes has also been identified as a critical factor in regulating canonical and non-canonical functions. Due to the influence of host ncRNAs and the structure of RNA viral genomes, complex molecular and cellular processes in infections are modulated. We propose three main categories to organize the current information about RNA-RNA interactions in some well-known human viruses. The first category shows examples of host ncRNAs associated with the immune response triggered in viral infections. Even though miRNAs introduce a standpoint, they are briefly presented to keep researchers moving forward in uncovering other RNAs. The second category outlines interactions between virus-host ncRNAs, while the third describes how the structure of the RNA viral genome serves as a scaffold for processing virus-derived RNAs. Our grouping may provide a comprehensive framework to classify ncRNA-host-cell interactions for emerging viruses and diseases. In this sense, we introduced them to organize DENV-host-cell interactions.

The severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) is an emerging zoonotic virus of public and animal health concern, of which felids have been suggested as potential reservoirs. Although SARS-CoV-2 exposure has been detected in domestic and wild captive animals belonging to Felidae family, surveillance has not been carried out in free-ranging wild felids so far. The aim of the present study was to assess SARS-CoV-2 exposure in the Iberian lynx (Lynx pardinus), the most endangered felid in the world. Between 2019 and 2022, we conducted a seroepidemiological study of SARS-CoV-2 in 276 free-ranging and captive Iberian lynxes. Our results evidenced limited (0.4%; 95%CI: 0.0-1.1) but not negligible exposure to this emerging virus in this endangered felid species, increasing the SARS-CoV-2 host range. This study aimed to systematically review the available evidence on the effects of Pilates training programs on blood pressure in hypertensive patients. Randomized clinical trials and comparative studies were searched in four electronic databases until September 2023 (updated December 2023). The methodological quality of included studies was assessed using the Physiotherapy Evidence Database and Methodological Index for Non-Randomized Studies scales. A total of 4 randomized clinical trials and 7 comparative studies were included, showing a low (n = 1), hight (n = 6), and good (n = 4) methodological quality. Data synthesis indicated that participants who performed Pilates program obtained significantly reduces on systolic blood pressure and diastolic blood pressure, of -4.76 mmHg (95% CI: -6.55 to -2.97, p < 0.001) and -3.43 mmHg (95% CI: -4.37 to -2.49, p < 0.001), respectively, in comparison with those included in the comparison groups. When the analysis was performed by comparing hypertensive, and normotensive patients, the results remained non-significant for blood pressure (systolic blood pressure: 0.96 mmHg (95% CI: -2.85 to 4.77, P = 0.49); diastolic blood pressure: 1.18 mmHg (95% CI: -1.23 to 3.58, P = 0.34); mean blood pressure: 1.73 mmHg (95% CI: -1.96 to 5.42, P = 0.36). Evidence suggests Pilates is safe for hypertensive patients and can be part of their rehabilitation, but it may not necessarily offer superior results or improve exercise adherence compared to other modalities., (© 2024. In this paper, we study a generalized eco-epidemiological model of fractional order for the predator-prey type in the presence of an infectious disease in the prey. The proposed model considers that the disease infects the prey, causing them to be divided into two classes, susceptible prey and infected prey, with different density-dependent predation rates between the two classes. We propose logistic growth in both the prey and predator populations, and we also propose that the predators have alternative food sources (i.e., they do not feed exclusively on these prey). The model is evaluated from the perspective of the global and local generalized derivatives by using the generalized Caputo derivative and the generalized conformable derivative. The existence, uniqueness, non-negativity, and boundedness of the solutions of fractional order systems are demonstrated for the classical Caputo derivative. In addition, we study the stability of the equilibrium points of the model and the asymptotic behavior of its solution by using the Routh-Hurwitz stability criteria and the Matignon condition. Numerical simulations of the system are presented for both approaches (the classical Caputo derivative and the conformable Khalil derivative), and the results are compared with those obtained from the model with integro-differential equations. Finally, it is shown numerically that the introduction of a predator population in a susceptible-infectious system can help to control the spread of an infectious disease in the susceptible and infected prey population.

In recent years, a population of small RNA fragments derived from non-coding RNAs (sfd-RNAs) has gained significant interest due to its functional and structural resemblance to miRNAs, adding another level of complexity to our comprehension of small-RNA-mediated gene regulation. Despite this, scientists need more tools to test the differential expression of sfd-RNAs since the current methods to detect miRNAs may not be directly applied to them. The primary reasons are the lack of accurate small RNA and ncRNA annotation, the multi-mapping read (MMR) placement, and the multicopy nature of ncRNAs in the human genome. To solve these issues, a methodology that allows the detection of differentially expressed sfd-RNAs, including canonical miRNAs, by using an integrated copy-number-corrected ncRNA annotation was implemented. This approach was coupled with sixteen different computational strategies composed of combinations of four aligners and four normalization methods to provide a rank-order of prediction for each differentially expressed sfd-RNA. By systematically addressing the three main problems, we could detect differentially expressed miRNAs and sfd-RNAs in dengue virus-infected human dermal microvascular endothelial cells. Although more biological evaluations are required, two molecular targets of the hsa-mir-103a and hsa-mir-494 (CDK5 and PI3/AKT) appear relevant for dengue virus (DENV) infections. Marinas contribute to the degradation of coastal ecosystems, constitute non-indigenous species (NIS) hotspots and function as steppingstones in invasion processes. These often enclose highly modified water bodies that promote the concentration of pollutants and propagules, favoring NIS abundance. In these habitats, floating structures are often the most invaded by fouling NIS. This study aims to address the effect of floating substrate (buoys vs pontoons) on fouling assemblages, with special focus on NIS, in 6 marinas of Cadiz Bay during summer and winter seasons. Since the effect substrate type can depend on the water physicochemical conditions and environmental state and pressures of marinas, an environmental assessment was carried out for each marina using literature, physicochemical water measurements and environmental risk assessments. Despite the registered seasonal variation in fouling assemblages and the environmental variability among the studied marinas, the type of substrate played a key role in fouling assemblages' structure and abundance. The higher abundance of fouling assemblages in buoys than pontoons favor NIS prevalence in marinas and increase the risk of NIS dispersal, particularly considering that buoys are more likely to detach and drift at sea than pontoons. This study aims to investigate the potential relationship between energy expenditure, physical fitness level, physical activity, and body mass index among children taking part in a 30 min school recess. A total of 259 participants from three schools were included in this study. Data on energy expenditure during recess, age, gender, anthropometric measurements, global physical fitness, and physical activity index were recorded. The evaluation sessions occurred twice a week on alternate days over two weeks. A significant gender difference was observed in energy expenditure during recess, favoring boys ( p < 0.01). The participants classified as very active exhibited significantly higher scores compared to those categorized as sedentary and moderately active, respectively ( p < 0.01), with a poor but significant correlation (rho: 0.208; p < 0.001). There were significant differences between energy expenditure and cardiorespiratory fitness and energy expenditure and global physical fitness ( p < 0.01) with fair (rho: 0.289; p < 0.001) and poor (rho: 0.196, p = 0.001) correlation, respectively. In contrast, there were no significant differences by body mass index categories ( p = 0.571). These results suggest that gender, physical activity index, and global physical fitness were found to influence energy expenditure during a 30 min recess. However, no significant relationships were found with the body mass index.
