- Abstract
Artificial intelligence (AI) algorithms have enormous potential applications and cover the entire medical imaging lifecycle, from image creation, diagnosis, classification and even prediction of particular pathologies. A problem for the development and clinical application of AI algorithms is the limited availability of extensive and representative training data including expert labeling (Manual Labeling). Current supervised AI methods require a data pre-processing stage for labeling data to train, validate and test algorithms with an optimal approximation to the ground truth. In this paper, a graphical interface was developed and implemented to label in a semi-automatic way Regions of Interest of Mid-Middle Lateral Oblique Screen-Film Mammography (MMLOSFM) through pre-processing techniques such as segmentation and image enhancement. Based on the analysis performed using four structural comparison indexes, it was demonstrated an efficient segmentation and labeling of MMLOSFM with a consistently high similarity (91.6% compared to the ground truth provided by the MIAS database).

Resumen: El presente estudio evaluó el potencial de uso de una escoria de alto horno (BFS) como material de conformación de capas granulares no tratadas en pavimentos (base, subbase y afirmado), y como agregado pétreo en la fabricación de mezclas asfálticas. Para tal fin, fueron ejecutados sobre la BFS, ensayos de caracterización, difractometría de rayos X (DRX), fluorescencia de rayos X (FRX) e imagenología en un microscopio electrónico de barrido (MEB). Como conclusión general se reporta que la BFS puede ser utilizada en la conformación de capas granulares no tratadas de subbase. Como material para base granular se recomendaría su uso en vías con bajos volúmenes de tráfico o con capas asfálticas gruesas, y como material de afirmado, en vías terciarias que soporten tráfico liviano. Para el caso de mezclas asfálticas no se recomienda utilizar como agregado pétreo la fracción gruesa de la BFS. Sin embargo, los resultados obtenidos indican que la fracción fina presenta buenas características para ser utilizado en la producción del mastic de dichas mezclas. Abstract: The present study evaluated the potential use of Blast Furnace Slag (BFS) as forming material of untreated granular layers in pavement (e.g. base, subbase and selected material), and as stone aggregate in the manufacture of hot mix asphalts - HMA. For that purpose, tests of characterization, X-ray diffractometry (XRD), X-ray fluorescence (XRF) and imaging in a scanning electron microscope (SEM) were executed on the BFS. As a general conclusion is reported that the BFS can be used in the formation of unbound granular layers of subbase. As granular base material, its use would be recommended on roads with low traffic volumes or with thick asphalt layers and as a selected material, in tertiary roads that support light traffic. In the case of HMA is not recommended to use the coarse fraction of the BFS as a stone aggregate. However, the obtained results indicate that the fine fraction shows good characteristics to be used in the production of the mastic of such mixtures.

Objective: to perform a diagnosis of the state of biomedical equipment maintenance management in the emergency areas of three (3) institutions of level three of medical complexity in the city of Medellín, Colombia. The realm is within the cleaner production in health research project. Methodology. A survey was designed based on the Cleaner Production Sector. Guide: in hospitals, clinics and health centers, where a check list was obtained to review the state of maintenance management of the most complex biomedical equipment used in the emergency room. Results: maintenance management of the most complex biomedical equipment is very good in hospital 1 and excellent in hospitals 2 and 3. Metrology and maintenance management in the biomedical equipment of the emergency area of all three hospitals is excellent. Conclusion: maintenance management is very important for financial savings in institutions, since they can thus reduce equipment downtime without hindering the provision of services, along with reducing costs in the purchase of spare parts and generation of waste, which causes a negative impact on the environment., Objetivo: Realizar um diagnóstico do estado da gestão da manutenção de equipamentos biomédicos na área de urgências de três (3) instituições de nível três de complexidade médica da cidade de Medellín, Colômbia, dentro do projeto de investigação produção mais limpa em saúde. Metodologia: Desenhou-se uma pesquisa a partir da Guia Setorial de Produção mais limpa, em hospitais, clínicas e centros de saúde, onde se obteve uma lista de verificação para a revisão do estado da gestão de manutenção dos equipamentos biomédicos mais complexos utilizados em urgências. Resultados: A gestão de manutenção dos equipamentos biomédicos mais complexos é muito boa no hospital 1 e excelente nos hospitais 2 e 3; a metrologia e gestão de manutenção nos equipamentos biomédicos da área de urgências dos três hospitais é excelente. Conclusão: A gestão de manutenção é importantíssima para a poupança económica nas instituições, já que podem-se reduzir tempos mortos dos equipamentos sem afetar a prestação dos serviços, aparte de que se reduzem despesas na compra de repostos, e geração de resíduos causantes de impactos negativos ao meio ambiente., Objetivo: Realizar un diagnóstico del estado de la gestión del mantenimiento de equipos biomédicos en el área de urgencias de tres (3) instituciones de nivel tres de complejidad médica de la ciudad de Medellín, Colombia, dentro del proyecto de investigación producción más limpia en salud. Metodología: Se diseñó una encuesta a partir de la Guía Sectorial de Producción más Limpia, en hospitales, clínicas y centros de salud, donde se obtuvo una lista de chequeo para la revisión del estado de la gestión de mantenimiento de los equipos biomédicos más complejos utilizados en urgencias. Resultados: La gestión de mantenimiento de los equipos biomédicos más complejos es muy buena en el hospital 1 y excelente en los hospitales 2 y 3; la metrología y gestión de mantenimiento en los equipos biomédicos del área de urgencias de los tres hospitales es excelente. Conclusión: La gestión de mantenimiento es muy importante para el ahorro económico en las instituciones, ya que se pueden reducir tiempos muertos de los equipos sin afectar la prestación de los servicios, aparte de que se reducen gastos en la compra de repuestos, y generación de residuos causantes de impactos negativos al medio ambiente.

This paper presents an extensive review of the pertinent literature regarding asphalt and asphalt mixture aging. Aging affects flexible pavement performance and is produced by intrinsic and extrinsic variables as well as exposure time. Intrinsic variables include asphalt and aggregate properties, a mixture's asphalt content, binder film thickness and air void content; extrinsic variables are associated with production (short-term aging) and exposure to environmental field conditions (long-term aging). Taken together, both variables demonstrate that aging results from three distinct mechanisms: volatilisation, oxidation and steric hardening. Temperature, pressure and photo degradation treatments are used to simulate aging in the laboratory and empirical and semi-empirical models are created to represent and study aging. Aging increases asphalt complex modulus and decreases the phase angle. Mixtures become stiffer while fatigue life becomes reduced. Carbonyl and sulfoxide group formation in asphalt are often studied as such chemical changes show oxidation in aged asphalts. The prevailing models used to predict asphalt aging are discussed, though more comprehensive research into asphalt aging is still needed. Este artículo presenta una revisión bibliográfica del fenómeno de envejecimiento que experimentan los asfaltos y las mezclas asfálticas. El envejecimiento afecta el desempeño de los pavimentos flexibles y la magnitud de sus efectos se debe a la combinación de las características de los materiales (variables intrínsecas) y los efectos ambientales (variables extrínsecas) que actúan sobre ellos. Las variables intrínsecas incluyen las propiedades del asfalto y los agregados, el contenido de asfalto en la mezcla, el espesor de la película de asfalto y el contenido de vacíos. Las variables extrínsecas corresponden a dos momentos de la mezcla asfáltica; el primero, conocido como envejecimiento a corto plazo, se asocia al proceso de producción y colocación de la mezcla, y el segundo, conocido como envejecimiento a largo plazo, se asocia a la exposición a condiciones ambientales en el campo durante el tiempo de servicio. El proceso de envejecimiento ocurre por medio de tres mecanismos: volatilización, oxidación y endurecimiento estérico. Para simular el fenómeno en el laboratorio, se llevaron a cabo pruebas de envejecimiento acelerado mediante temperatura, presión y fotodegradación y a partir de ellas se crearon modelos empíricos y semiempíricos. Como resultado de esas pruebas, se ha observado el incremento en el módulo complejo y la reducción del ángulo de fase de los asfaltos. En el caso de las mezclas, se aprecia que la rigidez también se incrementa y las mezclas tienden a ser frágiles, por lo que la vida de fatiga se reduce. Los cambios químicos se manifiestan en la formación y aumento en el tiempo de grupos funcionales de carbonilo y sulfóxido. Finalmente, algunos modelos utilizados para predecir el envejecimiento se discuten. Todo lo anterior permite afirmar que la investigación del fenómeno de envejecimiento es una importante tarea de la ingeniería de pavimentos.

Purpose We aimed to evaluate the impact of a multidimensional infection control strategy for the reduction of the incidence of catheter-associated urinary tract infection (CAUTI) in patients hospitalized in adult intensive care units (AICUs) of hospitals which are members of the International Nosocomial Infection Control Consortium (INICC), from 40 cities of 15 developing countries: Argentina, Brazil, China, Colombia, Costa Rica, Cuba, India, Lebanon, Macedonia, Mexico, Morocco, Panama, Peru, Philippines, and Turkey. Methods We conducted a prospective before-after surveillance study of CAUTI rates on 56,429 patients hospitalized in 57 AICUs, during 360,667 bed-days. The study was divided into the baseline period (Phase 1) and the intervention period (Phase 2). In Phase 1, active surveillance was performed. In Phase 2, we implemented a multidimensional infection control approach that included: (1) a bundle of preventive measures, (2) education, (3) outcome surveillance, (4) process surveillance, (5) feedback of CAUTI rates, and (6) feedback of performance. The rates of CAUTI obtained in Phase 1 were compared with the rates obtained in Phase 2, after interventions were implemented. Results We recorded 253,122 urinary catheter (UC)-days: 30,390 in Phase 1 and 222,732 in Phase 2. In Phase 1, before the intervention, the CAUTI rate was 7.86 per 1,000 UC-days, and in Phase 2, after intervention, the rate of CAUTI decreased to 4.95 per 1,000 UC-days [relative risk (RR) 0.63 (95 % confidence interval [CI] 0.55-0.72)], showing a 37 % rate reduction. Conclusions Our study showed that the implementation of a multidimensional infection control strategy is associated with a significant reduction in the CAUTI rate in AICUs from developing countries. © Springer-Verlag 2012.

In this paper the problem of spatially sharing a common fish resource between two quite different harvesting activities is modelled. The problem is studied from the viewpoint a of regulator in charge of deciding how to share the resource. We assume that the regulator has to fix the percent of the area opened to each of the agents and in this way design two patches. One of the patches should be used only by small scale fisheries and plays the role of a reserve that contains the reproduction area. In the second patch a more efficient and intensive activity should be permitted. This problem is motivated by a study case in Chile. The model is introduce stepwise and takes into account the different impact of the activities and a natural biomass transfer into the patches. We end up with an optimal control problem that try to find social relevant solutions. The resulting control problem is treated using a calculus of variation approach and some conditions ensuring the existence of interior solutions are proposed. Finally some academical tests are done in order to see the feasibility of the proposed conditions.

The present work applies Gabor filters bank for texture analysis of skin lesions images, obtained by ultrasound biomicroscopy. The regions affected by the lesions were differentiated from surrounding tissue in all the analyzed cases; however the accuracy of the traced borders showed some limitations in part of the images. Purpose: To evaluate the impact of country socioeconomic status and hospital type on device-associated healthcare-associated infections (DA-HAIs) in neonatal intensive care units (NICUs). Methods: Data were collected on DA-HAIs from September 2003 to February 2010 on 13,251 patients in 30 NICUs in 15 countries. DA-HAIs were defined using criteria formulated by the Centers for Disease Control and Prevention. Country socioeconomic status was defined using World Bank criteria. Results: Central-line-associated bloodstream infection (CLA-BSI) rates in NICU patients were significantly lower in private than academic hospitals (10.8 vs. 14.3 CLA-BSI per 1,000 catheter-days; p < 0.03), but not different in public and academic hospitals (14.6 vs. 14.3 CLA-BSI per 1,000 catheter-days; p = 0.86). NICU patient CLA-BSI rates were significantly higher in low-income countries than in lower-middle-income countries or upper-middle-income countries [37.0 vs. 11.9 ( p < 0.02) vs. 17.6 ( p < 0.05) CLA-BSIs per 1,000 catheter-days, respectively]. Ventilator-associated-pneumonia (VAP) rates in NICU patients were significantly higher in academic hospitals than in private or public hospitals [13.2 vs. 2.4 ( p < 0.001) vs. 4.9 ( p < 0.001) VAPs per 1,000 ventilator days, respectively]. Lower-middle-income countries had significantly higher VAP rates than low-income countries (11.8 vs. 3.8 per 1,000 ventilator-days; p < 0.001), but VAP rates were not different in low-income countries and upper-middle-income countries (3.8 vs. 6.7 per 1,000 ventilator-days; p = 0.57). When examined by hospital type, overall crude mortality for NICU patients without DA-HAIs was significantly higher in academic and public hospitals than in private hospitals (5.8 vs. 12.5%; p < 0.001). In contrast, NICU patient mortality among those with DA-HAIs was not different regardless of hospital type or country socioeconomic level. Conclusions: Hospital type and country socioeconomic level influence DA-HAI rates and overall mortality in developing countries. [ABSTRACT FROM AUTHOR]
