2. Sleep Problems in Turkish Children with Conduct Disorder: A Single-centre, Cross-sectional, Case-control Study

Objective:This study aimed to determine the sleep habits and sleep problems of patients with conduct disorder (CD) and to compare them with healthy controls.Materials and Methods:This cross-sectional study included 45 children diagnosed with CD and 50 healthy controls. Sleep was evaluated by children’s sleep habits questionnaire (CSHQ). Qualitative variables between groups were compared using chi-square test, whereas quantitative variables were compared using t-test. Correlation analyses were conducted to evaluate relationships between the quantitative variables. Linear regression analysis was conducted to evaluate the predictors of severity of CD symptoms. P was set at 0.05.Results:The average ages of children in CD and control groups were 11.04±1.20 and 10.00±1.55 years, respectively. Moreover, 71.1% in the CD group were boys (vs. 78.0% in the control group). The groups were comparable in terms of socio-demographic features (all p>0.05). In children and adolescents, especially younger ones, recurrent abdominal pains may develop as a response to psychosocial stressors. They may be considered as functional and be one of the common reasons for clinical referrals. A rare cause of recurrent, organic abdominal pain is abdominal epilepsy. Abdominal epilepsy may be considered in differential diagnosis of patients that were thought to have functional complaints. In this case, a 16-year-old adolescent girl whose chief complaints included loss of consciousness and recurrent abdominal pains lasting for a year is presented.

The chronotype, which reflects differences among individuals in daily activity patterns and sleep-wake cycles, is actually linked to being healthy. The most important factor that determines the chronotype is the environmental light-dark cycle. Chronotype is the study of classification as morning or evening types according to the physiological and genetic characteristics of the human. There are many factors that determine chronotypes. Although the evidence shows a negative picture for evening types, it should be emphasized that only eveningness cannot cause depression or other psychiatric disorders alone and additional effects may play a role. Conducting studies on mechanisms underlying the chronotype-psychopathology connection is important for interventions that can help reduce the incidence of psychiatric disorders in evening types; these interventions may include sleep training, dietary advice, and cognitive-behavioral techniques. The purpose of this review is to provide an overview of psychiatric disorders in children and adolescents and their relationship with the morning and evening types.

- Abstract
Amaç: Çalışmamız Koronavirüs hastalığı-2019 (COVID-19) salgınının birinci ayında çocukların yeme düzenlerini, ebeveynlerinin koronavirüsü bulaşma riski ile ilgili kaygılarını ve bu kaygı ile ilişkili etkenlerin değerlendirilmesini ve salgın öncesi ile karşılaştırılmasını amaçlamıştır. Gereç ve Yöntem: Çalışma çok merkezli (n=8) ve kesitsel olarak planlanmıştır. Çalışma merkezlerine başvuran 6-18 yaş arası çocukların ailelerinden çalışmaya katılmayı kabul edenlere bilgilendirilmiş gönüllü olur formu ve anketleri içeren bilgiler, e-posta veya mesajlaşma yoluyla iletilmiştir. Bulgular: Çalışmaya ortalama 11,1 yaşında (standart sapma=3,4) 893 çocuk dahil edilmiştir. Çalışmaya dahil edilen çocukların %50,7’si (n=453) kızdır. Ailelerin çoğu, salgının birinci ayında çocuklarının yeme düzeninde değişiklik bildirmezken (%63,6, n=568), %21,4’ü çocuklarının yeme düzeninin daha iyi, %15,0’ı ise daha kötü olduğunu bildirmiştir. Ebeveynlerin %40.0’ı kendileri veya çocuklarına virüs bulaşmasından “çok kaygılı” olduğunu belirtmiştir. INTRODUCTION: The aim of this study was to compare children with Specific Learning Disorders (SLD) and matched controls in terms of nonverbal communication skills, understanding of emotions, electrodermal activity (EDA) changes and response latencies measured with affective stimuli. METHOD: The study was designed as a single-center, cross-sectional, controlled study. KA-SI Empathic Tendency Scale-Child Form, Test of Perception of Affect-Via Nonverbal Cues (TPANC) were used for both groups. The TPANC test was performed in 4 different sessions (for picture, video, audio and textual stimuli), and during each session, the EDA was measured from the non-dominant hand. Stimuli were given at 15 second intervals in the tests and phasic changes within 3–8 seconds after the stimulus was accepted as skin conductance responses related to the stimulus in accordance with the literature. The responses and response latency during the TPANC test were recorded by the clinician. RESULTS: 31 children with SLD and 31 healthy controls were included in the study. The mean ages for SLD and controls were 10 years 6 months and 10 years 1 month; respectively. This study aimed to investigate the effects of drug holidays during summer vacations among children with attention deficit/hyperactivity disorder (ADHD) who were treated with methylphenidate in terms of ADHD symptoms and executive functions.The study was a prospective cohort study that includes pre-treatment, post-treatment and post-drug holiday evaluations. ADHD symptom severity was evaluated with the Clinical Global Impression Scale (CGI), the Conners' Parental Rating Scale-Short Form (CPRS) and the Conners' Teacher Rating Scale- Short Form (CTRS). The Stroop Color Word Test- TBAG Form (SCWT) was used to evaluate executive functions. Fifty-one patients participated in the study according to the inclusion and exclusion criteria. Methylphenidate (MPH) was started at 0.5 mg/kg/day and titrated weekly to a maximum of 1.2 mg/kg/day. During the follow-up period, 22 (43.0%) of the patients stopped treatment.Completion times for all SCWT subtests were significantly reduced after treatment (Our results suggest that psychomotor speed and resistance to interference improved in children with ADHD receiving methylphenidate treatment. Drug holidays did not affect psychomotor speed while beneficial effects on resistance to interference were reduced with drug holidays.

In children and adolescents, especially younger ones, recurrent abdominal pains may develop as a response to psychosocial stressors. They may be considered as functional and be one of the common reasons for clinical referrals. A rare cause of recurrent, organic abdominal pain is abdominal epilepsy. Abdominal epilepsy may be considered in differential diagnosis of patients that were thought to have functional complaints. In this case, a 16-year-old adolescent girl whose chief complaints included loss of consciousness and recurrent abdominal pains lasting for a year is presented.

Background: Our study aimed to compare the pre- and post-treatment sleep habits of children with Attention Deficit and Hyperactivity Disorder (ADHD) who use methylphenidate and to evaluate the effect of the treatment on sleep problems. Methods: This study includes pre-treatment and post-treatment evaluations. Children's sleep habits and problems were assessed with the Children's Sleep Habits Questionnaire (CSHQ). We gave scales for the parents and teachers of 51 patients who participated in the study. Results: When the sub-scales of CSHQ applied before and after the treatment were analyzed, we found that only the parasomnia subscale score was statistically significantly higher before the treatment (p=0.026); and there was no statistically significant difference between other subscale scores (p>0.05). Compared to whether there is a sleep problem before and after treatment, there was no significant difference between them (p=0.993). Conclusion: In our study, after methylphenidate treatment, we found that parasomnia decreased. We found that there was no difference between the pre- and post-methylphenidate treatment in the presence of sleep problems. Considering the inconsistent results of studies on the effect of methylphenidate on sleep problems, studies requiring longer-term treatments and evaluating sleep habits and problems with more objective methods are needed.

The chronotype, which reflects differences among individuals in daily activity patterns and sleep-wake cycles, is actually linked to being healthy. The most important factor that determines the chronotype is the environmental light-dark cycle. Chronotype is the study of classification as morning or evening types according to the physiological and genetic characteristics of the human. There are many factors that determine chronotypes. Although the evidence shows a negative picture for evening types, it should be emphasized that only eveningness cannot cause depression or other psychiatric disorders alone and additional effects may play a role. Conducting studies on mechanisms underlying the chronotype-psychopathology connection is important for interventions that can help reduce the incidence of psychiatric disorders in evening types; these interventions may include sleep training, dietary advice, and cognitive-behavioral techniques. The purpose of this review is to provide an overview of psychiatric disorders in children and adolescents and their relationship with the morning and evening types. Günlük aktivite düzenleri ve uyku-uyanıklık döngülerindeki bireyler arası farklılıkları yansıtan kronotip, aslında sağlıklı olmakla bağlantılıdır. Kronotipi belirleyen en önemli olay çevresel ışık-karanlık döngüsüdür. Kronotip, insanın fizyolojik ve genetik özelliklerine göre, sabahçıl ya da akşamcıl kronotipler şeklinde sınıflandırılarak incelenmesidir. Kronotipleri belirleyen birçok etmen bulunmaktadır. Her ne kadar kanıtlar, akşamcılık için olumsuz bir tablo çizse de yalnızca akşamcılığın tek başına depresyona veya diğer psikiyatrik bozukluklara neden olamayacağı ve psikiyatrik bozuklukların ortaya çıkmasında ek etkilerin rol oynayabileceği olasılığı vurgulanmalıdır. Kronotip-psikopatoloji bağlantısının altında yatan mekanizmalarla ilgili araştırmaların yapılması, akşamcıl kronotipte psikiyatrik bozuklukların görülme sıklığını azaltmaya yardımcı olabilecek müdahaleler için önemlidir; bu müdahaleler uyku eğitimi, diyet tavsiyesi ve bilişsel-davranışsal teknikleri içerebilir.
