Energy harvesting represents one of the recent challenging subjects related to vibration and control. The scale of energy harvesters and storage can involve a wide power range, and the scale of some milliwatt is the elective field of piezoelectric applications. This paper investigates the power frontiers of the piezoelectric-based harvesters applied to automotive units. The analysis, supported by experimental data, aims at estimating the upper bound of the specific power of this technology for powering small devices on board cars. Passive optimally tuned piezoelectric harvester and semi-active controlled ones are compared, based on a new control strategy named VFC-Variational Feedback Control, recently developed by the authors. This new technique makes it possible to increase the total energy storage drained from car vibrations. However, the real advantage for their use relies on a sharp balance between the harvested power and the costs for the additional hardware mass transport. Numerical simulations of circuitry and experimental vibration data provides references to assess the energy convenience in installing this type of devices on board.

Background Although hemoptysis is a well known symptom of presentation of pulmonary embolism (PE), and it is included in different diagnostic scores for PE, the prevalence of PE in patients with hemoptysis and the yield of evidence based PE diagnostic pathway in these patients has not been specifically investigated. Objectives To determine the prevalence of PE and the yield (n° of positive tests/n° of tests), the efficiency (the proportion of patients in whom PE could be considered to be excluded) and the failure rate (the false negative rate) of the classic diagnostic algorithm for PE in patients with hemoptysis. Methods Unselected patients presenting with hemoptysis to 7 italian EDs were prospectively enrolled in a diagnostic multicenter study. Pre-test clinical probability for PE was assessed using the 2-level Wells score and the results of D-dimer testing using age-adjusted cutoffs. Presence of PE was evaluated by CT pulmonary angiogram (CTPA), when ordered as part of the clinical assessment by the treating ED physician, or by one month follow-up data. Results Out of 327 adult patients presenting to ED with hemoptysis, 15 (4.6%) were excluded because of diagnostic protocol violation and absence of one month follow-up. Among 312 included patients, PE was detected at presentation in 15 patients (4.8%; 95% confidence interval [CI]: 2.72–7.81%). During follow-up, 5 patients died (1.6%; 95% CI: 0.52%-3.7%) not due to PE. Among 165 patients with a “likely” score or a high D-dimer, 13 (7.9%; 95% CI: 4.72–13.8%) had PE. The efficiency was 32.8% and the failure rate was 1.7% in patients tested by D-dimer. Conclusions PE seems an uncommon cause of hemoptysis among patients presenting to the ED and the yield of classic diagnostic algorithm is low. PE-screening in patients with hemoptysis deserves further investigation aimed at improving the diagnostic yield. Funding Acknowledgement Type of funding sources: None.

The hormonal thyroid changes related to obesity, even when in the euthyroid state, may contribute to the unfavorable cardio-metabolic profile of obese patients. In this retrospective study, we aim to investigate the biochemical thyroid changes and the association between serum TSH, FT4, FT3 and cardio-metabolic risk factors in euthyroid obese youths.Four hundred ninety-one Caucasian euthyroid obese children and adolescents aged 9.93 ± 2.90 years were recruited. Each patient underwent clinical and auxological examination and laboratory workup including an OGTT and the measurement of thyroid function and lipid profile. Homeostasis model assessment of insulin resistance (HOMA-IR), triglyceride to high-density lipoprotein cholesterol ratio, total cholesterol to HDL ratio, atherogenic index of plasma, insulinogenic index, area under the glucose and insulin curves were calculated.We found that TSH was positively correlated with BMI-SDS values and significantly associated with hypercholesterolemia and hyperinsulinemia; FT4 resulted negatively correlated with BMI-SDS; FT3 was positively correlated with BMI-SDS and the area under the curve of insulin and negatively correlated with HDL. FT3 and FT4 resulted significantly associated with severe obesity. In addition, children with high-normal TSH values showed higher triglyceride to high-density lipoprotein cholesterol ratio values than those with normal TSH levels.Our data showed that thyroid hormones could influence obesity, lipid and glycemic parameters in euthyroid youths. These findings could carry implications regarding optimal TSH levels in obese children and confirm the importance of evaluating the thyroid function as possible adjunctive cardio-metabolic risk factor related to obesity.

Background Arterial hypertension is the most important risk factor for cardiovascular diseases, myocardial infarction, heart failure, renal failure and peripheral vascular disease. In the last decade, milk-derived bioactive peptides have attracted attention for their beneficial cardiovascular properties. Methods Here we combined in-vitro chemical assay such as LC-MS/MS analysis of buffalo ice cream, ex-vivo vascular studies evaluating endothelial and smooth muscle responses using pressure myograph, and translational assay testing in-vivo the vascular actions of PG1 administration in murine models. Results We demonstrate that a novel buffalo ice-cream-derived pentapeptide “QKEPM”, namely PG1, is a stable peptide that can be obtained at higher concentration after gastro-intestinal digestions (GID) of buffalo ice-cream (BIC). It owns potent vascular effect in counteract the effects of angiotensin II-evoked vasoconstriction and high blood pressure levels. Its effects are mediated by the inhibitory effect on AT1 receptor leading to a downregulation of p-ERK½/Rac1-GTP and consequent inactivation of NADPH oxidase activity. Conclusions These results strongly candidate PG1, as a novel bioactive peptide for the prevention and management of hypertension, thus expanding the armamentarium of preventive strategies aimed at reducing the incidence and progression of hypertension and its related cardiovascular complications. Since several innovations have recently changed the criteria of choice and management of peripheral venous access (new devices, new techniques of insertion, new recommendations for maintenance), the WoCoVA Foundation (WoCoVA = World Conference on Vascular Access) has developed an international Consensus with the following objectives: to propose a clear and useful classification of the currently available peripheral venous access devices; to clarify the proper indication of central versus peripheral venous access; discuss the indications of the different peripheral venous access devices (short peripheral cannulas vs long peripheral cannulas vs midline catheters); to define the proper techniques of insertion and maintenance that should be recommended today. To achieve these purposes, WoCoVA have decided to adopt a European point of view, considering some relevant differences of terminology between North America and Europe in this area of venous access and the need for a common basis of understanding among the experts recruited for this project. The ERPIUP Consensus (ERPIUP = European Recommendations for Proper Indication and Use of Peripheral venous access) was designed to offer systematic recommendations for clinical practice, covering every aspect of management of peripheral venous access devices in the adult patient: indication, insertion, maintenance, prevention and treatment of complications, removal. Also, our purpose was to improve the standardization of the terminology, bringing clarity of definition, and classification.

Docosahexaenoic acid is an omega-3 fatty acid that is essential for neurological development and function, and it is supplied to the brain and eyes predominantly from dietary sources1–6. This nutrient is transported across the blood–brain and blood–retina barriers in the form of lysophosphatidylcholine by major facilitator superfamily domain containing 2A (MFSD2A) in a Na+-dependent manner7,8. Here we present the structure of MFSD2A determined using single-particle cryo-electron microscopy, which reveals twelve transmembrane helices that are separated into two pseudosymmetric domains. The transporter is in an inward-facing conformation and features a large amphipathic cavity that contains the Na+-binding site and a bound lysolipid substrate, which we confirmed using native mass spectrometry. Together with our functional analyses and molecular dynamics simulations, this structure reveals details of how MFSD2A interacts with substrates and how Na+-dependent conformational changes allow for the release of these substrates into the membrane through a lateral gate. Our work provides insights into the molecular mechanism by which this atypical major facility superfamily transporter mediates the uptake of lysolipids into the brain, and has the potential to aid in the delivery of neurotherapeutic agents. Cryo-electron microscopy and molecular dynamics simulations reveal how MFSD2A transports essential omega-3 fatty acids across the blood–brain and blood–retina barriers as lysolipids.

Background: Subcutaneously anchored securement devices (or subcutaneous engineered securement devices) have been introduced recently into the clinical practice, but the number of published studies is still scarce. The Italian Group of Long-Term Central Venous Access Devices (GAVeCeLT)—in collaboration with WoCoVA (World Congress on Vascular Access)—has developed a Consensus about the effectiveness, safety, and cost-effectiveness of such devices. Methods: After the definition of a panel of experts, a systematic collection and review of the literature on subcutaneously anchored securement devices was performed. The panel has been divided in two working groups, one focusing on adult patients and the other on children and neonates. Results: Although the quality of evidence is generally poor, since it is based mainly on non-controlled prospective studies, the panel has concluded that subcutaneously anchored securement devices are overall effective in reducing the risk of dislodgment and they appear to be safe in all categories of patients, being associated only with rare and negligible local adverse effects; cost-effectiveness is demonstrated—or highly likely—in specific populations of patients with long-term venous access and/or at high risk of dislodgment. Conclusion: Subcutaneously anchored securement is a very promising strategy for avoiding dislodgment. Further studies are warranted, in particular for the purpose of defining (a) the best management of the anchoring device so to avoid local problems, (b) the patient populations in which it may be considered highly cost-effective and even mandatory, (c) the possible benefit in terms of reduction of other catheter-related complications such as venous thrombosis and/or infection, and—last but not least—(d) their impact on the workload and stress level of nurses taking care of the devices.

Purpose of Review: Autoimmune rheumatic diseases (ARDs) affect 8% of the population and approximately 78% of patients are women. Myocardial disease in ARDs is the endpoint of various pathophysiologic mechanisms including atherosclerosis, valvular disease, systemic, myocardial, and/or vascular inflammation, as well as myocardial ischemia and replacement/diffuse fibrosis. Recent Findings: The increased risk of CVD in ARDs leads to excess comorbidity not fully explained by traditional cardiovascular risk factors. It seems that the chronic inflammatory status typically seen in ARDs, promotes both the development of myocardial inflammation/fibrosis and the acceleration of atherosclerosis. Summary: CMR (cardio-vascular magnetic resonance) is the ideal imaging modality for the evaluation of cardiac involvement in patients with ARDs, as it can simultaneously assess cardiac function and characterize myocardial tissues with regard to oedema and fibrosis. Due to its high spatial resolution, CMR is capable of identifying various disease entities such as myocardial oedema /inflammation, subendocardial vasculitis and myocardial fibrosis, that are often missed by other imaging modalities, notably at an early stage of development. Although generally accepted guidelines about the application of CMR in ARDs have not yet been formulated, according to our experience and the available published literature, we recommend CMR in ARD patientS with new-onset heart failure (HF), arrhythmia, for treatment evaluation/change or if there is any mismatch between patient symptoms and routine non-invasive evaluation. © 2020, Springer Science+Business Media, LLC, part of Springer Nature.

SIGRIAN (National Information System for Agriculture Water Management) is a web GIS platform developed and managed by the Council for Agricultural Research and Economics, Centre for Politics and Bio economics (CREA-PB). This GEOdatabase is operating since 1998 and recently it has been established by the Italian Ministry of Agriculture (MIPAAF) as the reference repository for the irrigation data collection at national scale. SIGRIAN collects both geographical information concerning the hydraulic network schemes of the national water boards (Consortia and Water Associations) and information technically and economically related to the management of water resources in agriculture. SIGRIAN will be used as database for economic evaluations to address policy related to water resources in agriculture and to support the assessment of optimal water resource allocation.

Comprehensive studies of the biodiversity of the microbial epilithic community on monuments may provide critical insights for clarifying factors involved in the colonization processes. We carried out a high-throughput investigation of the communities colonizing the medieval church of San Leonardo di Siponto (Italy) by Illumina-based deep sequencing. The metagenomic analysis of sequences revealed the presence of Archaea, Bacteria, and Eukarya. Bacteria were Actinobacteria, Proteobacteria, Bacteroidetes, Cyanobacteria, Chloroflexi, Firmicutes and Candidatus Saccharibacteria. The predominant phylum was Actinobacteria, with the orders Actynomycetales and Rubrobacteriales, represented by the genera Pseudokineococcus, Sporichthya, Blastococcus, Arthrobacter, Geodermatophilus, Friedmanniella, Modestobacter, and Rubrobacter, respectively. Cyanobacteria sequences showing strong similarity with an uncultured bacterium sequence were identified. The presence of the green algae Oocystaceae and Trebuxiaceae was revealed. The microbial diversity was explored at qualitative and quantitative levels, evaluating the richness (the number of operational taxonomic units (OTUs)) and the abundance of reads associated with each OTU. The rarefaction curves approached saturation, suggesting that the majority of OTUs were recovered. The results highlighted a structured community, showing low diversity, made up of extremophile organisms adapted to desiccation and UV radiation. Notably, the microbiome appeared to be composed not only of microorganisms possibly involved in biodeterioration but also of carbonatogenic bacteria, such as those belonging to the genus Arthrobacter, which could be useful in bioconservation. Our investigation demonstrated that molecular tools, and in particular the easy-to-run next-generation sequencing, are powerful to perform a microbiological diagnosis in order to plan restoration and protection strategies.

Background and study aims Endoscopic drainage of walled-off pancreatic necrosis (WOPN) is feasible when contact with the gastric or duodenal wall is present; when WOPN cannot be accessed endoscopically, a percutaneous approach can be considered. Percutaneous use of esophageal self-expandable metal stents (SEMS) to establish access to a WOPN cavity was evaluated.

Superconducting nanowire single-photon detectors (SNSPDs) have shown to be promising in applications such as quantum communication and computation, quantum optics, imaging, metrology and sensing. They offer the advantages of a low dark count rate, high efficiency, a broadband response, a short time jitter, a high repetition rate, and no need for gated-mode operation. Several SNSPD designs have been proposed in literature. Here, we discuss the so-called parallel nanowires configurations. They were introduced with the aim of improving some SNSPD property like detection efficiency, speed, signal-to-noise ratio, or photon number resolution. Although apparently similar, the various parallel designs are not the same. There is no one design that can improve the mentioned properties all together. In fact, each design presents its own characteristics with specific advantages and drawbacks. In this work, we will discuss the various designs outlining peculiarities and possible improvements.

To describe for the first time in literature the specific methodology of use of negative pressure wound therapy (NPWT) for duodenal fistula through clinical cases. The constant increase of use of NPWT for complex surgical situations imposes tailored previously undescribed solutions for the technique.Herein, three cases of high output duodenal fistula successfully treated with Negative Pressure Wound Therapy (NPWT) are reported. The technical details for the application of NPWT to these fistulas are discussed and described.All three patients recovered without the necessity of further surgical operations.When using NPWT, management of high-output duodenal fistulas must rely on some degree of customization of the aspiration systems. The aim of the procedure is to put under depression the duodenal hole and surrounding tissues "all in one" and not to separate the complex wound in sectors as usually indicated. We suggest calling this technique Negative Pressure Fistula Therapy.

OBJECTIVE. The role of adipokines such as resistin, leptin, and adiponectin could be pivotal in the molecular crosstalk between the inflamed intestine and the surrounding mesenteric adipose tissue. Our aims were to a) evaluate their circulating concentrations in patients with active celiac disease (ACD) and compare them to those in patients with diarrhea-predominant irritable bowel syndrome (IBS-d) and healthy subjects; b) establish the impact of genetic variability in resistin; and c) evaluate whether a 1-year gluten-free diet (GFD) modifies circulating concentrations of resistin, leptin, and adiponectin in celiac patients. MATERIAL AND METHODS. The study included 34 ACD patients, 29 IBS-d patients, and 27 healthy controls. Circulating concentrations of resistin, leptin, adiponectin, IL-6, and IL-8 were evaluated at the time of enrollment. Resistin +299 G/A polymorphism was also analysed. In CD patients, biochemical measurements were repeated after a 1-year GFD. RESULTS. Along with higher IL-6 and IL-8 plasma levels, higher resistin and adiponectin concentrations were found in ACD and IBS-d patients compared with controls (p: 0.0351 and p: 0.0020, respectively). Resistin values proved to be predictable from a linear combination of IL-8 and +299 polymorphism. GFD affected resistin (p: 0.0009), but not leptin and adiponectin concentrations. CONCLUSIONS. Our data suggest that these adipokines are involved in modulating inflammatory processes in both CD and IBS-d patients. Alterations in the adipokine profile as well as the higher prevalence of the resistin +299 G/A SNP A allele compared to controls support the hypothesis that, at least in well-defined cases of IBS, a genetic component may also be supposed.

Background Functional impairment is associated with adverse outcomes in older people, as well as in patients on chronic hemodialysis. The aim of the present study was to determine the characteristics associated with functional impairment in chronic hemodialysis, and to evaluate if functional impairment represents a risk factor for reduced survival in chronic hemodialysis. Methods All 132 chronic hemodialysis referring to the Hemodialysis Service of the Catholic University, Rome, Italy between November 2007 and May 2015 were included. All patients underwent comprehensive geriatric assessment; functional ability was estimated using two questionnaires exploring independency in bathing, dressing, toileting, transferring, continence, feeding (ADLs), and independency in using the telephone, shopping, food preparation, housekeeping, laundering, traveling, taking medications, and handling finances (IADLs). Functional impairment was diagnosed in presence of dependence in one or more ADLs/IADLs. Mood was assessed using the 30-item Geriatric Depression Scale. Logistic regression was used to evaluate factors associated with functional impairment. The association between functional impairment and survival was assessed by Cox regression. Results ADLs impairment was present in 34 (26 %) participants, while IADLs impairment was detected in 64 (48 %) subjects. After a follow up of 90 months, 55 (42 %) patients died. In logistic regression, depressive symptoms were associated with ADLs and IADLs impairment (OR 1.12; 95 % CI = 1.02–1.23; OR 1.16; 95 % CI = 1.02–1.33; respectively). In Cox regression, ADLs impairment was associated with mortality (HR 2.47; 95 % CI–1.07–5.67) while IADLs impairment was not associated with reduced survival (HR .80; 95 % CI–.36–1.76). Conclusions Functional impairment is associated with depressive symptoms; also, impairment in the ADLs represents a risk factor of reduced survival in chronic hemodialysis. These associations and their potential implication should be assessed in dedicated studies.

At the global level, risk management tools are under discussion, in particular in the agricultural sector, in relation to its vulnerability to climatic risk. The main question points refer to the effectiveness of the most common current policy schemes, based on public support to insurances and compensation aids, in relation to the patterns of risk analysis in the context of climate change. Italy has a long tradition of risk management in agriculture because of the heterogeneity of climatic conditions. The present study has been conducted by Council for Agricultural Research and Economics to explore the potential of the current risk management scheme. One of the more relevant aspects studied is the demand for risk management in terms of exposure to disasters of the agricultural areas and their vulnerability. Crossing this analysis with the policy scheme, it is possible to assess its effectiveness in covering climatic risks. The results show that the current system based on economic tools needs a strong integration into a wider framework of risk assessment and policy strategies addressing climate change adaptation, in synergy with other structural and management measures.

With the help of molecular mechanics (program GenMol), suitable molecular geometries were obtained for photochromic spiropyrans and spirooxazines to evaluate substituent effects on the spectroscopic and physicochemical properties. The results were compared to X-ray structures and used to explain relevant 1H NMR data.

BACKGROUND: Inulin has been suggested to have beneficial effects on lipids, especially on triglyceridemia. Few data are available about the effects of inulin on Lipoprotein(a), a low-density lipoprotein-like particle considered as an independent risk factor for atherosclerosis. Adding inulin to pasta could be a preventive strategy for delaying the onset of atherosclerosis. AIM OF THE STUDY: was to evaluate the effects of inulin-enriched pasta on lipid profile and on Lipoprotein(a) in young healthy subjects. METHODS: Twenty-two young healthy male volunteers entered a randomized double blind cross-over study consisting of a 2-weeks run-in period, a baseline assessment, two 5-weeks study periods (11% inulin-enriched or control pasta), and an 8-weeks wash-out period in between. Serum lipid concentrations were evaluated by routine biochemical analyses and plasma Lipoprotein(a) concentrations by ELISA. The size of apolipoprotein(a) isoforms was determined by Western blot and immunodetection. RESULTS: Significant differences at baseline and in the treatment groups were found for HDL-cholesterol (P = 0.004), total cholesterol/HDL-cholesterol ratio (P = 0.006), triglycerides (P = 0.04), and Lipoprotein(a) (P = 0.02) concentrations (data analyzed by Friedman test). Dunn's multiple comparison test was used to assess the significance of differences between inulin-enriched pasta diet vs. baseline. HDL-cholesterol concentrations increased by 35.9%; total cholesterol/HDL-cholesterol ratio, triglycerides, and Lipoprotein(a) concentrations decreased by 22.2, 23.4, and 16.5% respectively. CONCLUSIONS: Inulin-enriched pasta administration induced significant effects on lipid pattern parameters in young healthy volunteers, including a significant reduction in Lipoprotein(a) concentrations.

Background The decision whether to perform endoscopy in children with suspected reflux oesophagitis is not a straightforward one. Few symptoms are specific for oesophagitis and the diagnosis is not always correlated even to visual findings on endoscopy. Aim The aim of this study was to define the role of endoscopy and especially of histology in the diagnosis of reflux oesophagitis and to examine the correlations between symptoms, endoscopic findings and histology in children with suspected gastroesophageal reflux disease. Patients and methods One hundred and thirty-six patients with a clinical diagnosis of reflux oesophagitis, aged 1–18 years (mean 8.43; standard deviation ±4.4), were enrolled from 12 Italian Paediatrics Gastroenterology Centres; symptom score, endoscopic and histologic oesophagitis scores were observed before and after therapy with proton pump inhibitors. Results Before therapy, a high correlation between the prevailing symptom score and endoscopic score was demonstrated, but not with histologic score: there was a significant tendency for histologic grade to exceed visual findings. After therapy, endoscopic score and histologic score were significantly improved. Conclusions Oesophageal biopsies increase the diagnostic accuracy of upper endoscopy. Histologic grading is often much more important than the endoscopic appearance, so that endoscopic oesophageal biopsies are very important aids in the diagnosis of oesophagitis. Appropriate clinical evaluation of symptoms must occur before endoscopic examination.

Rationale: Both insulin and IGF-1 have been implicated in the control of retinal endothelial cell growth, neovascularization and diabetic retinopathy. Recent findings have established an essential role for IGF-1 in angiogenesis and demonstrated a new target for control of retinopathy that explains why diabetic retinopathy initially increases with the onset of insulin treatment Objective: This cross-sectional study was designed to give insights into relationship between Insulin-Growth-Factor 1 (IGF-1) levels and diabetic retinopathy (DR) in a sample of thalassemia major (TM) patients with insulin dependent diabetes mellitus (IDDM). This relation was not previously evaluated, despite the fact that both diseases co-exist in the same patient. The study also describes the clinical and biochemical profile of the associated complications in TM patients with and without IDDM. Design: A population-based cross-sectional study. Participants: The study includes 19 consecutive TM patients with IDDM and 31 age- and sex-matched TM patients without IDDM who visited our out-patient clinics for an endocrine assessment Methods: An extensive medical history, with data on associated complications and current medications, was obtained. Blood samples were drawn in the morning after an overnight fast to measure the serum concentrations of IGF-1, glucose, fructosamine, free thyroxine (FT4), thyrotropin (TSH) and biochemical analysis. Serologic screening assays for hepatitis C virus seropositivity (HCVab and HCV-RNA) were also evaluated; applying routine laboratory methods. Plasma total IGF-1 was measured by a chemiluminescent immunometric assay (CLIA) method. Ophthalmology evaluation was done by the same researcher using stereoscopic fundus biomicroscopy through dilated pupils. DR was graded using the scale developed by the Global Diabetic Retinopathy Group. Iron stores were assessed by direct and indirect methods. Results: Eighteen TM patients with IDDM (94.7 %) and ten non-diabetic patients (32.2 %) had IGF-1 levels below the 2.5th percentile of the normal values for the Italian population. The mean serum IGF-1 concentrations were significantly lower in the diabetic versus the non-diabetic TM groups (p < 0.001). DR was present in 4 (21 %) of 19 TM patients with IDDM and was associated with the main classical risk factors, namely inefficient glycemic control and duration of the disease but not hypertension. Using the scale developed by the Global Diabetic Retinopathy Group, the DR in our patients was classified as non proliferative diabetic retinopathy (NPDR). Only a few numbers of microaneurysms [1-3] were detected. Our data also confirm the strong association of IDDM in TM patients with other endocrine and non-endocrine complications.

We investigated the role of strain on the magnetic properties of La0.7Sr0.3MnO3 epitaxial films grown on substrates with different crystallographic orientation. The magnetic characterization confirms the large magnetostriction due to the tensile stress applied by the SrTiO3 substrates. The data demonstrate that the easy direction of magnetization is aligned to the elongated, in-plane axis both in (110)- and (001)-oriented films. The in-plane anisotropy of (110) films is interpreted in the frame of a simple structural model. We provide evidence of a spatial separation between a conducting ferromagnetic and an insulating paramagnetic phase at the Curie temperature. The critical Josephson current (IC) in superconducting/ferromagnetic (S/F) multilayer-based junctions can be controlled by changing the relative directions of the magnetization in the F-layers. Recent experimental works [1, 2] show that an enhancement of IC is achieved in S/F weak links when the alternating F-layers are antiparallel aligned. We present preliminary experimental results concerning the dependence of IC on the relative orientation of the ferromagnetic layers in S/F1/I/F2/S tunnel junctions where the F-layers are obtained by changing the relative composition of NiCu alloys. The multilayers were grown by electron beam deposition, and processed by Focused Ion Beam lithography. The magnetic state of the devices was directly determined by measuring the current perpendicular to plane (CPP) magnetoresistance (MR) at high bias. IC was found to be larger when the F-layers are antiparallel aligned. The maximum change of IC corresponds to the maximum change of MR. The application of a magnetic field induces a transition in the shape of the currentvoltage curve that seems to suggest Coulomb blockade effect.

Background The aims were to investigate whether surgical stress can induce a positive or negative lipoprotein(a) acute response, to determine any association with apolipoprotein(a) phenotypes, and to establish whether any such response is dependent on changes in lipids and proinflammatory cytokines. In addition, the impact of interleukin (IL) 6 genetic variability on the cytokine response to surgery was examined. Methods This prospective, observational study included 41 patients with cancer referred for abdominal surgery. Preoperative (T0) plasma concentrations of lipoprotein(a), IL-6, tumour necrosis factor α, and serum concentrations of transforming growth factor β1 and lipids, were compared with values obtained 5 h (T1), 24 h (T2) and 5 days (T3) after surgery. Apolipoprotein(a) Kringle IV (KIV)-VNTR (variable-number tandem repeat) and IL-6 − 174 G/C polymorphisms were analysed. Results Lipoprotein(a) was found to act as a negative acute-phase reactant (30·0 per cent reduction at T2) (P = 0·009). Surgery had a more profound impact on subjects with low KIV-VNTR. After surgery, lipoprotein(a) correlated significantly with corrected low-density lipoprotein (LDL)-cholesterol (r = 0·408 at T2). IL-6 inversely correlated with lipoprotein(a) (r = −0·321 at T1) and LDL-cholesterol (r = −0·418 at T1). The IL-6 response could be predicted from a combination of the surgical severity and −174 G/C genotype. Conclusion Although temporal associations did not indicate causality, these data provide a hypothesis to explain the inverse relationship between lipoprotein(a) and IL-6.

This paper describes a sequential procedure for computer-aided tuning and diagnosis of multiple-coupled resonator filters. The method is based on a sequential parameter estimation and a systematic tuning procedure and employs three different filter models. A detuned model represents the initial status of the filter after a well-defined detuning procedure. The target filter is described by an ideal model, whereas the actual state of the filter at each tuning step is represented by a coarse adaptive model. The goal of the procedure is the convergence of the coarse model to the ideal model and will be obtained by systematically centering resonant frequencies and coupling coefficients. Practical examples comprising low- and high-degree filters confirm the effectiveness of the proposed approach in both tuning and fault diagnosis.

The lipoprotein Lp(a) with high plasma concentration is an independent genetic determinant for cardiovascular diseases. It was investigated as a quantitative factor of risk for myocardial infarction. A total of 345 Italian subjects, 127 Cases and 218 Controls, were studied. Lipids and lipoproteins were compared. Cases had atherogenic traits, such as lower HDL cholesterol and higher triglycerides than Controls. In particular, they had Lp(a) concentrations over the risk threshold, (median, 27 mg/dl in Cases vs 17 mg/dl in Controls; P = 0.0075, Mann-Whitney test) which confirmed the association of this parameter with the disease. Two main functional variants of the apo(a) gene, KringleIV and penta-nucleotide repeat, (PNR) were analyzed. Allele and genotype frequency distributions differed between Cases and Controls. Lp(a) concentrations differed according to PNR genotypes in Controls: subjects having alleles >8 showed lower Lp(a). This was not found in Cases. They had a higher prevalence of the smaller KringleIV alleles, the high Lp(a)-expressing ones. In Cases, genotypes consisting of two small KringleIV alleles were prevalently associated to PNR 8/9 and 8/10, thus preventing Lp(a) lowering. The putative apo(a) enhancer within LINE1 in the apo(a)-plasminogen intergenic region was investigated for functional polymorphisms. No variants that could be associated to the Lp(a) variability were found.

An alternating pump system simulating the left ventricle in a mechanical cardio-vascular model, has been modified to show Frank Starling regulating mechanism. An inextensible thin plastic wall chamber has been put around the syringe's core and connected to the internal syringe's chamber by two lateral holes. During the filling and the expulsion phases the external chamber changes his volume and the net output depends on the filling pressure. The regulating mechanism shows a beat to beat response and the dependence of systolic aortic pressure, systolic ventricular work, stroke output with respect ventricular filling pressure show a "physiological" behavior. This solution will be introduced in the mechanical whole model to study the changes of aorto-ventricular interaction by changing aortic compliance.

A model to describe the linear regime in a Superconducting Tunnel Junction detector has been developed. In the limit of low temperatures, the only effective processes which involve quasi-particles and phonons are the phonon scattering mechanisms and the phonon escape from the area of the detector. The response of the STJ is linear if τ esc −1 >; τ ph −1 for low energy phonons and τ esc −1 τ ph −1 for high energy phonons. The estimate of the mean energy loss ϵ by radiation per excess quasi-particle in the STJ is in agreement with previous determinations. A comparison with some experimental results taken from literature has been produced.

Enterocutaneous fistulas (ECFs) are an uncommon surgical problem, but they are characterized by a difficult management. Vacuum-assisted closure (VAC) therapy is a method utilized for chronic and traumatic wound healing. At first, VAC therapy had been contraindicated in the treatment of intestinal fistulas, but as time went by, VAC therapy revealed itself to be a "Swiss knife multi-tool". This paper presents some clinical cases of enterocutaneous (ECF) and enteroatmospheric fistulas (EAF) treated with VAC therapy™.The history of 8 patients treated for complex fistulas was revised. Four of them presented with enterocutaneous and four with enteroatmospheric fistulas. All were treated with VAC therapy with variations elaborated to help in accelerated closure of intestinal wall lesions.Four out of four ECFs closed spontaneously. In the EAF group, in three cases the fistula turned slowly into an entero-cutaneous fistula, and in one out of four it closed spontaneously. The mean length of VAC therapy™ was 35.5 days and that of spontaneous closure was 36.4 days.The results of our study encourage the use of VAC therapy™ for the treatment of enterocutaneous fistulas. VAC therapy™ use has a double therapeutic value: (1) it promotes the healing of the skin and allows also the management of EAFs; (2) in selected cases, those in which it is possible to create a deep fistula tract ("well") it is possible to assist to a complete healing with closure of the ECFs.

Electrical transport measurements and transient optical pump-probe experiments have been performed on epitaxial films of the electron doped Nd 1.83Ce0.17Cu4-δ compound for studying the non-equilibrium carrier dynamics in this material. Samples have been grown on (001)-oriented SrTiO3 substrates by dc sputtering in a mixed atmosphere of both Ar and O2. X-ray diffraction analysis and scanning electron microscope equipped with a wavelength dispersive spectroscopy detector have been used to characterize the structure and the composition of the thin films. Time-resolved femtosecond pump-and-probe spectroscopy has been also carried out on our samples in the temperature range 4.2K-300 K.
