A person's target and option depend on different processes of thought. These cycles of thinking and motives either drive or drag the participant to the creative exercises of entrepreneurship. This research explored the entrepreneurial traits, reasons, difficulties, and obstacles faced by Ifugao citizens in the Philippines while doing business. The relationship between the above variables and the entrepreneurial aspirations of Ifugao individuals was also calculated. Data were collected from 263 Ifugao, both beneficiaries and non-beneficiaries of the programs of various government agencies in the province using snowball sampling through a research questionnaire and semi-structured interview. Using means, the Mann-Whitney U test, and the rank order correlation of Spearman, this paper finds that considering the difficulties and obstacles they face, Ifugao individuals retain the character of becoming an entrepreneur. Overall, five traits that are common to respondents have been identified. Ifugao people stay inspired, and it motivates them to persevere constantly in carrying out entrepreneurial practices. Finally, the findings of this study indicate that training, seminars, and workshops, the positive perceived image of entrepreneurs, and motives have a significant impact on the entrepreneurial purpose of Ifugao people, which is essential for entrepreneurial success. An innovative approach for the primary and definitive treatment of obstructive sleep apnea (OSA) in adult patients is presented: Bilateral Internal Ramus Distraction of the mandible (BIRD), which is a slow, progressive and more stable procedure to advance the mandibular bone. This study investigated whether this surgical approach is useful to cure OSA.Study design was of an interventional (surgical) one-arm trial of OSA patients assessed before and 12 months after BIRD. All patients were evaluated by pre- and post-operative polysomnography and three-dimensional scans. The amount of skeletal advancement, percentage of upper airway volume increase and postoperative value of mandibular occlusal plane were the predictor variables. Changes in the apnoea-hypopnoea index (AHI), oxygen desaturation index (ODI), and percentage of time with saturation under 90% (TC90) were the main outcome variables.Thirty-two subjects with a mean ± SD age of 41.9 ± 13.3 years and 87.5% male were included, and they were followed-up 32 ± 14.2 months. AHI was 47.9 ± 23.1 per hour before surgery and the Epworth Sleepiness Scale (ESS) was 13.4 ± 4.4. Postoperative AHI was 4.8 ± 5.6 per hour 12 months after surgery (P0.001), with 81.2% of the patients considered cured (AHI5) and 18.8% suffering from a mild-to-moderate residual OSA. ESS decreased to 1.9 ± 1.8 at the end of the surgical treatment (P0.001). 3D changes revealed an upper airway volume increase of 188.4% ± 73.5% (P0.001).Lengthening the mandibular ramus by distraction osteogenesis to cure OSA appears to be more effective and safer when compared to other surgical protocols, especially in very severe cases with initial AHI50/h. Titration of the mandibular advancement weekly using respiratory polygraphy allows better healing control and customization of the skeletal advancement, enhancing the aesthetic result.

Background Between 2016 and 2017 the National Agency of Medicines and Medical Devices regulated the use of nivolumab and pembrolizumab for the treatment of non-small-cell lung cancer (NSCLC). In clinical trials conducted, patients with ECOG 0–1 and a life expectancy of at least 3 months were included since the benefit of immunotherapy can be delayed and even present a response after progression (pseudo-progression). Purpose To analyse characteristics of patients with NSCLC treated with nivolumab and pembrolizumab for less than 3 months at our centre. Material and methods Observational descriptive study was conducted. Patients diagnosed with NSCLC treated for less than 3 months (with six or less cycles of nivolumab and four or less cycles of pembrolizumab) from the approval date of these drugs until October 2018 were included. Data from clinical and pharmacotherapeutic records was collected: age, sex, ECOG, histology, brain metastases, PDL-1 expression, number of previous lines, time elapsed since previous treatment if any and reason for discontinuation. Overall survival (OS) and progression-free survival (PFS) medians were calculated with SPSS 22.0 using the Kaplan–Meier method. Results Sixty-two patients were included (males 64.5%, mean age 68±9.7), 82.1% ECOG 0–1, 75.4% non-squamous histology and 14.5% with brain metastases). PDL-1 expression was positive in 100% of patients treated with pembrolizumab and in 4.8% of those treated with nivolumab (57.1% of them without determination). 75.8% had received previous treatment, 61.7% of them in less than 3 months and with less than three previous lines (97.2%). Forty-one patients were treated with pembrolizumab and 21 with nivolumab. The median treatment duration was 42 days (3–115). Seven patients discontinued due to drug toxicity. The global median OS and PFS were 361.5 and 61.5 days, with no statistically significant differences between both treatments (p=0.191 and p=0.279 respectively). Conclusion With the aim of improving the rational use of medicines and optimising results, these findings encourage us to carry out studies with a larger sample of patients in order to select the patients who would benefit most from these therapies. The possible presence of pseudoprogression in those who did not reach at least 3 months of treatment constitutes a limitation on observing the possible clinical benefits. Background Patients with a diagnosis of multiple myeloma (MM) have compromised innate and adaptive immunity, both humoral and cellular. The treatment of this pathology can produce immune alterations such as increasing the incidence of the Varicella–Zoster Virus (VZV) reactivation. The most accepted treatment is acyclovir at prophylactic doses. Purpose Our objective was to evaluate the adequacy of prophylaxis against VZV in patients with MM treated with daratumumab or carfilzomib. Material and methods Retrospective observational study in a third-level hospital. For the study, a population sample was obtained from the Farmatools Ambulatory Patient module who were in treatment with daratumumab or carfilzomib from January 2016 to April 2018. Clinical data was also obtained from discharge reports of the haematology service and active treatments in Horus.The registered variables were: name, patient identification number, dates of administration of daratumumab and carfilzomib, and doses and frequency of administration of acyclovir. In addition, the clearance of creatinine and renal pathologies were also recorded. Drug label of acyclovir indicates that a dose of 800 mg daily orally is recommended in immunocompromised patients. Results A total of 12 patients (seven males and five females) were included, of which eight patients were treated with daratumumab, two with carfilzomib and two patients were treated with both at different times. The mean daily dose of acyclovir was 689.58 mg (SD: 185.76 mg) and the median dose was 800 mg (200–800). One patient was treated with 200 mg daily for chronic kidney disease secondary to a chronic glomerulopathy (serum creatinine of 2 mg/dL) and another patient was treated with 400 mg daily because of moderate renal impairment (serum creatinine of 1.73 mg/dL). The rest of the patients (n=6) were treated with 800 mg daily. No patient developed VZV infection during the treatment of MM. Conclusion The use of prophylaxis with acyclovir against VZV in patients with MM under active treatment supposes a reduction in the rate of VZV reactivation to zero in our hospital. In our study, all patients had been prescribed an adequate acyclovir regimen individualised to the physiological features of each patient. References and/or acknowledgements Swaika A. et al. Background The presence of a clinical pharmacist in the medical services has been shown in numerous previous publications as a great additional factor in the quality and safety of pharmacological treatment. In many hospitals there is still no clinical pharmacist, and the implementation process is the critical stage to be overcome so that this professional activity could be consolidated within the multidisciplinary hospital team. Purpose It was proposed to evaluate the degree of activity achieved by a clinical pharmacist newly implemented in an internal medicine service during the first 2 months from the beginning of its activity. Material and methods The clinical pharmacist carried out the medication reconciliation (MR) of the patients with polypharmacy who were admitted to the internal medicine service. He also reviewed the patients’ treatments daily and carried out the patients’ MR at discharge. All of the interventions were recorded for an initial period of 2 months, and then analysed. Results The treatments of 119 patients were analysed. Each patient had an average of 10+6 medications at admission. A total of 145 pharmaceutical interventions were performed, corresponding to 19 different categories. The most common interventions were the absence of patients’ chronic treatment (26.9%), the need for additional treatment during admission (24.1%), the presence of unnecessary medications (17.9%), the need to reintroduce a medication at discharge (5.5%), insufficient doses (4.1%), allergies (4.1%), the need for nutritional evaluation (3.4%), excessive doses (2.8%), intravenous-to-oral switch therapy (2%) and contraindicated medications (1.4%). The major therapeutic groups for which interventions were performed were vitamin and mineral supplements (23.4%), proton pump inhibitors (7.6%), antiplatelet agents (5.5%), beta-blockers (5.5%), haematopoietic growth factors (5.5%), alpha-blockers (4.1%), anticholinergics (3.4%), antidepressants (3.4%), anticonvulsants (2.8%), androgen antagonists (2%) and antiarrhythmics agents (2%). Conclusion With the presence of a clinical pharmacist, an average of 1.2 interventions were performed for each patient reviewed. Through these interventions, it was possible to optimise the pharmacological treatment, providing the necessary medicines for each patient, adjusting the doses to their requirements and preventing medication-related problems. This paper presents the methodology used and the results obtained in the determination of diffusion coefficient of moisture into non-impregnated transformer insulating paper. In order to determine the diffusion coefficient, drying curves of paper samples were obtained from thermogravimetric experiments. The parameters of the diffusion coefficient were found by applying an optimization process based on genetic algorithms. The error function between measured and simulated curves was determined, and the parameters giving the best agreement between measured and estimated values were obtained. As a result, a new equation for the diffusion coefficient of moisture into non-impregnated insulating paper is proposed, depending on average moisture concentration, temperature and insulation thickness. The proposed coefficient, validated by good agreement between the experimental drying curves and those obtained by simulation using the diffusion coefficient, can be used for the determination of the time required to dry a power transformer in a factory.

Se elaboraron y compararon las tablas de vida de Hypothenemus hampei sobre las introducciones de café Coffea liberica , Coffea arabica CCC 534 y la variedad susceptible Caturra, con el fin de hallar fuentes de resistencia a este insecto bajo condiciones controladas ( 26 ºC ± 1, 75% ± 5 H. R.). Se realizaron dos experimentos: en el primero, se siguió el desarrollo de los estados inmaduros y se estimó la supervivencia. En el segundo, se determinó la supervivencia y fecundidad de los adultos usando hembras de 36 días de edad. La duración del ciclo para CCC 534, C . liberica y Caturra fue similar: 20 días en total, huevo cuatro, primer instar cuatro, segundo instar seis, prepupa dos y pupa cuatro días, aproximadamente. El número de huevos acumulados en las tres introducciones se ajustó a funciones cuadráticas. Los intervalos de confianza (95%), mostraron diferencias significativas entre las introducciones y el testigo: CCC 534 32 ± 2, C . liberica 28 ± 2 y Caturra 42 ± 2 huevos por hembra. Las proporciones sexuales fueron de 1: 9,2, 9,8 y 9,8 macho: hembra, respectivamente. No se encontraron diferencias en la supervivencia (entre 58 y 63%) hasta los 72 días para las tres introducciones. Hubo diferencias en la tasa reproductiva neta (Ro) entre Caturra (25 ± 1 huevos), CCC 534 (18 ± 2) y C. liberica (15 ± 2), en la tasa intrínseca de crecimiento (r) (0,073 ± 0,001; 0,065 ± 0,002; 0,057 ± 0,003, respectivamente) y en el tiempo de duplicación (10 ± 0,2; 11 ± 0,4; 12 ± 0,6 días); en el tiempo generacional solo hubo diferencias entre Caturra y C. liberica (45 ± 0,4; 47 ± 0,8, respectivamente). The life tables of Hypothenemus hampei were elaborated and compared on the coffee accessions Coffea liberica , Coffea arabica CCC 534 and the susceptible variety Caturra , in order to find sources of resistance to this insect under controlled conditions ( 26ºC ± 1, 75% ± 5 R. H.). Two assays were performed: in the first, the development of the immature stages was followed and survival was estimated. In the second, adult survival and fecundity were determined using 36- day old females. The duration of the cycle for CCC 534, C . liberica and Caturra was similar: total 20 days, egg four, first instar four, second instar six, prepupa two and pupa four days, approximately. The number of accumulated eggs in the three accessions fit quadratic functions. The confidence intervals (95%) showed significant differences between the accessions and the control: CCC 534 32 ± 2, C . liberica 28 ± 2 and Caturra 42 ± 2 eggs per female. The sex ratios were 9.2, 9.8, and 9.8 male: female, respectively. There was no difference in survival (range 58-63%) up to 72 days for the three accessions. There were differences in the net reproductive rate (Ro) among Caturra (25 ± 1 eggs), CCC 534 (18 ± 2), and C. liberica (15 ± 2); in the intrinsic rate of increase (r) (0.073 ± 0.001, 0.065 ± 0.002, 0.057 ± 0.003, respectively), and the duplication time (10 ± 0.2, 11 ± 0.4, 12 ± 0.6 days). In the generation time there was only a differences between Caturra and C. liberica (45 ± 0.4, 47 ± 0.8, respectively).

La evaluación del tractor Lister Pico contempló mediciones de laboratorio y de campo realizadas en varios centros de investigación del ICA durante 12 meses. Pruebas de laboratorio mostraron que la distribución de peso sobre los ejes corresponde a la de un vehículo multipropósito orientado principalmente a las labores de transporte y remolque de cargas, y que la máxima potencia del motor medida a través de su toma de fuerza fue de 19.4 KW (26.0 hp) satisfaciendo, en éste aspecto, las exigencias de las operaciones de labranza con implementos de categoría I. Los requerimientos de tiro y potencia obtenidos en las pruebas de campo se consideran normales para las condiciones de los suelos, de textura mediana principalmente y en algunos casos con poca agregación, en las cuales se efectuaron las evaluaciones, determinando valores moderados en la fuerza de tiro y eficiencias de tracción entre 0.4 y 0.6. Los rendimientos y eficiencias de campo alcanzados en estas pruebas fueron los esperados para el nivel de potencia del tractor y el tamaño de los predios preparados. Cuando se trabajó en lotes pequeños y con ligera pendiente (aproximadamente 10 por ciento), la eficiencia de campo registró valores menores del 60 por ciento, provocando bajos rendimientos de campo y traduciéndose en altos costos de mecanización/ha, en la preparación de suelos en este tipo de explotaciones agropecuarias Cacao-Theobroma cacao

Se efectuó un diagnóstico sobre los patrones de mecanización e implementos utilizados en el cultivo de la caña panelera, en la región de la Hoya del Río Suárez, Departamentos de Boyacá y Santander. El diagnóstico mostró que el tiempo empleado en las labores de desterronamiento y surcado que se ejecutan manualmente, es alto. A esto se suma la baja disponibilidad de mano de obra para realizarlas, respecto al arado reversible, único implemento disponible, se encontró que la calidad de sus materiales es deficiente. Con base en lo anterior se diseñaron, construyeron y evaluaron tres implementos para tracción animal: un arado reversible, un rastrillo de discos y una surcadora. Los resultados obtenidos al utilizarlos mostraron que el tiempo empleado en la surcada del terreno puede reducirse en un 56 por ciento, el desterronamiento, al efectuarse en forma mecánica, libera el empleo de 6 jornales por hectárea y la utilización del nuevo arado requiere menor esfuerzo por parte del operario Caña panelera-Saccharum officinarum - Saccharum officinarum L.

Se llevó a cabo la evaluación de dos llantas agrícolas (A) y (B) de producción nacional, Código R-2, 18,4/15-30, que presentaban diferencias en el diseño del labrado y en el número de venas, con el objeto de establecer sus características de tracción. Bajo igualdad de condiciones, los resultados señalan que la llanta (B) desarrolló una fuerza de tiro mayor a la alcanzada por la llanta (A) y una mayor eficiencia en el consumo de combustible, en los tres tipos de suelos seleccionados. Las diferencias numéricas encontradas entre los índices de tracción determinados para las dos llantas, deben ser explicadas en función de la interacción de las características de las venas tales como altura, espaciamiento y ángulo. Two kinds of rubber tires (A) and (B) Code: R-2, 18.4/15.30 with different treat design and number of iugs were evaluated with the objetive of determi- fling their tractive performances. Under similar conditions, results showed that tire (B) developed higgher tractive force and efficiency in fuel consumption than tire (A) in the soils evaluated. The numerical differences in the performance of the tires should be understood by the interaction of lug characteristics such as lug height, lug spacing and lug angle.

BackgroundThe presence of a clinical pharmacist in the medical services has been shown in numerous previous publications as a great additional factor in the quality and safety of pharmacological treatment. In many hospitals there is still no clinical pharmacist, and the implementation process is the critical stage to be overcome so that this professional activity could be consolidated within the multidisciplinary hospital team.PurposeIt was proposed to evaluate the degree of activity achieved by a clinical pharmacist newly implemented in an internal medicine service during the first 2 months from the beginning of its activity.Material and methodsThe clinical pharmacist carried out the medication reconciliation (MR) of the patients with polypharmacy who were admitted to the internal medicine service. He also reviewed the patients’ treatments daily and carried out the patients’ MR at discharge. All of the interventions were recorded for an initial period of 2 months, and then analysed.ResultsThe treatments of 119 patients were analysed. Each patient had an average of 10+6 medications at admission. A total of 145 pharmaceutical interventions were performed, corresponding to 19 different categories. The most common interventions were the absence of patients’ chronic treatment (26.9%), the need for additional treatment during admission (24.1%), the presence of unnecessary medications (17.9%), the need to reintroduce a medication at discharge (5.5%), insufficient doses (4.1%), allergies (4.1%), the need for nutritional evaluation (3.4%), excessive doses (2.8%), intravenous-to-oral switch therapy (2%) and contraindicated medications (1.4%). The major therapeutic groups for which interventions were performed were vitamin and mineral supplements (23.4%), proton pump inhibitors (7.6%), antiplatelet agents (5.5%), beta-blockers (5.5%), haematopoietic growth factors (5.5%), alpha-blockers (4.1%), anticholinergics (3.4%), antidepressants (3.4%), anticonvulsants (2.8%), androgen antagonists (2%) and antiarrhythmics agents (2%).ConclusionWith the presence of a clinical pharmacist, an average of 1.2 interventions were performed for each patient reviewed. Through these interventions, it was possible to optimise the pharmacological treatment, providing the necessary medicines for each patient, adjusting the doses to their requirements and preventing medication-related problems.References and/or AcknowledgementsWe thank the anonymous clinical pharmacists, who develop this profession every day.No conflict of interest

El crecimiento económico no es un proceso unilinear generado exclusivamente por fuerzas objetivas como el comercio, los mercados o el crecimiento de la población. Es también un proceso social moldeado por los intereses a menudo conflictivos de las distintas clases sociales que participan en él. El asunto crucial en una sociedad agraria es el acceso a la tierra -es decir, la definición de los derechos de propiedad- y el control de la mano de obra. Lo que parece haber acontecido en América Latina después de 1850 es que la ampliación de los mercados de ultramar ofreció nuevas oportunidades económicas a las que respondieron al par los• terratenientes y los campesinos. El resultado fue una competencia en busca de tierra y trabajo. En algunas partes, los terratenientes lograron ampliar sus propiedades y constituir una clase laboral dependiente cuando desalojaron de sus tierras a los campesinos. En otras, estallaron luchas abiertas entre campesinos y terratenientes. Los modelos de tenencia de la tierra y los tipos de actividad económica que aparecen en una región dada reflejan el desenlace de estas luchas. El antagonismo básico entre campesinos y terratenientes no se ha resuelto en ninguna parte: a medida que sigue ampliándose la economía agrícola exportadora prosiguen los conflictos. En efecto, muchos conflictos contemporáneos representan a la vez una prolongación y una transformación de conflictos previos. Las modalidades de conflicto rural en los años de crecimiento de las exportaciones después de 1850 fueron afectadas por modelos prevalecientes de utilización de la tierra. En áreas donde la tierra estaba ya ocupada por densas poblaciones indígenas, las disputas surgían a raíz de la constitución de grandes fincas comerciales. Ya fueran estas las haciendas azucareras de Morelos, en México, o las fincas cafeteras establecidas en Guatemala y El Salvador, o las ganaderías ovinas creadas en Perú y Bolivia, los conflictos enfrentaban terratenientes deseosos de ampliar su propiedad privada contra comunidades indígenas campesinas resueltas a conservar sus tierras ancestrales4• Como lo indican los estudios de David Browning, Andrew Pearse y otros, los campesinos solían resultar perdedores. Background: Bilateral internal ramus distraction (BIRD) is now part of select protocols for treating obstructive sleep apnea (OSA). Introducing a home monitoring protocol offers a valuable alternative to overnight laboratory polysomnography (PSG)., Purpose: The purpose of this study was to evaluate and compare OSA parameters obtained via home respiratory polygraphy (HRP) and hospital PSG in a cohort of patients undergoing mandibular distraction for OSA management., Study Design, Setting, Sample: Hospital Universitario La Princesa (Madrid) researchers conducted a prospective cohort study with patients diagnosed with moderate (apnea-hypopnea index (AHI) = 15 to 30) to severe (AHI>30) OSA undergoing BIRD followed by LeFort maxillary osteotomy. Exclusion criteria were as follows: severe systemic diseases, central apneas, smoking, poor dental hygiene, or prior OSA interventions., Predictor Variable: The predictor variable was PSG and HRP techniques., Main Outcome Variables: The main outcome variable comprises a collection of OSA parameters, including the AHI, oxygen desaturation index (ODI), and time spent below 90% oxygen saturation (T90). These data were measured both before and after distraction, as well as after 6 mm, 9 mm, and 12 mm of mandibular distraction., Covariates: The covariates were age, sex, cardiovascular risk parameters, and the Epworth sleepiness scale., Analyses: Pearson's correlation analyzed AHI, ODI, and T90 values from PSG and HRP. Wilcoxon Signed Rank-Sum Test compared 2 distraction stages, and the Friedman Test evaluated 3 stages (P < .05). Multiple regression analysis assessed if covariates were independent risk factors for postoperative persistent OSA., Results: The study included 32 patients (25% with moderate and 75% with severe OSA). Final AHI was 10.9 ± 8.9 (events/hour) with HRP, compared to 15.2 ± 13.4 with PSG (r = 0.7, P < .05). ODI was 9.0 ± 8.1 (des/h) with HRP and 8.7 ± 9.5 with PSG (r = 0.85, P < .05). T90 was 1.6 ± 2.2 with HRP and 1.3 ± 3.0 with PSG (r = 0.6, P < .05). Based on HRP data, AHI improved from D1 (34.0 ± 19.5) to D2 (20.8 ± 14.1) and D3 (12.5 ± 10.4) (P < .05). ODI decreased from D1 (26.1 ± 19.0) to D2 (16.0 ± 12.6) and D3 (9.4 ± 8.8) (P < .05). T90 reduced from D1 (8.2 ± 12.6) to D2 (4.1 ± 5.2) and D3 (1.9 ± 2.8) (P < .05). The Caenorhabditis elegans egg hatching methodology is a valuable tool for assessing the anthelmintic activity of drugs and compounds and evaluating anthelmintic drug efficacy. Isolated eggs from gravid adults are exposed to different concentrations of selected drugs and the percentage of egg hatching is determined with respect to the control condition. The assay allows the construction of concentration-response curves and determination of EC
50 or EC90 values for egg hatching inhibition. Also, it allows measurements of inhibition as a function of time of exposure. This approach addresses the urgent need for new anthelmintics, as resistance to current treatments poses a significant challenge in parasitic nematode infection. This resistance not only affects humans but also animals and plants, causing significant economic losses in livestock farming and agriculture. By using the free-living nematode C. elegans as a parasitic model organism, researchers can efficiently screen for potential treatments and assess drug combinations for synergistic effects. Importantly, this assay offers a cost-effective and accessible alternative to traditional methods, eliminating the need for specialized infrastructure, hosts, and trained animal maintenance personnel. Additionally, the methodology closely mimics natural conditions, providing insights into egg development and potential therapeutic targets. This method allows for evaluating the direct negative impact of drugs on egg hatching, which correlates with long-term anthelmintic effects, offering advantages in preventing or reducing the transmission and spread of worm infections by eggs. Cys-loop receptors integrate a large family of pentameric ligand-gated ion channels that mediate fast ionotropic responses in vertebrates and invertebrates. Their vital role in converting neurotransmitter recognition into an electrical impulse makes these receptors essential for a great variety of physiological processes. In vertebrates, the Cys-loop receptor family includes the cation-selective channels, nicotinic acetylcholine and 5-hydroxytryptamine type 3 receptors, and the anion-selective channels, GABA
A and glycine receptors, whereas in invertebrates, the repertoire is significantly larger. The free-living nematode Caenorhabditis elegans has the largest known Cys-loop receptor family as well as unique receptors that are absent in vertebrates and constitute attractive targets for anthelmintic drugs. Given the large number and variety of Cys-loop receptor subunits and the multiple possible ways of subunit assembly, C. elegans offers a large diversity of receptors although only a limited number of them have been characterized to date. C. elegans has emerged as a powerful model for the study of the nervous system and human diseases as well as a model for antiparasitic drug discovery. This nematode has also shown promise in the pharmaceutical industry search for new therapeutic compounds. C. elegans is therefore a powerful model organism to explore the biology and pharmacology of Cys-loop receptors and their potential as targets for novel therapeutic interventions. - Abstract
Nicotinic acetylcholine receptors (nAChRs) comprise a family of pentameric ligand-gated ion channels widely distributed in the central and peripheric nervous system and in non-neuronal cells. nAChRs are involved in chemical synapses and are key actors in vital physiological processes throughout the animal kingdom. They mediate skeletal muscle contraction, autonomic responses, contribute to cognitive processes, and regulate behaviors. Dysregulation of nAChRs is associated with neurological, neurodegenerative, inflammatory and motor disorders. In spite of the great advances in the elucidation of nAChR structure and function, our knowledge about the impact of post-translational modifications (PTMs) on nAChR functional activity and cholinergic signaling has lagged behind. PTMs occur at different steps of protein life cycle, modulating in time and space protein folding, localization, function, and protein-protein interactions, and allow fine-tuned responses to changes in the environment. A large body of evidence demonstrates that PTMs regulate all levels of nAChR life cycle, with key roles in receptor expression, membrane stability and function. However, our knowledge is still limited, restricted to a few PTMs, and many important aspects remain largely unknown. There is thus a long way to go to decipher the association of aberrant PTMs with disorders of cholinergic signaling and to target PTM regulation for novel therapeutic interventions. - Abstract
Anthelmintics are used to treat human and veterinary parasitic diseases and to reduce crop and livestock production loss associated with parasitosis. The free-living nematode Caenorhabditis elegans, a model system for anthelmintic drug discovery, has a serotonin (5-HT)-gated chloride channel, MOD-1, which belongs to the Cys-loop receptor family and modulates locomotory and behavioral functions. Since MOD-1 is unique to nematodes, it is emerging as an attractive anthelmintic drug target, but details of MOD-1 function are unclear. Here, we revealed novel aspects of MOD-1 function from the molecular level to the organism level and identified compounds targeting this receptor, which may provide new directions for anthelmintic drug discovery. We used whole-cell current recordings from heterologously expressed MOD-1 to show that tryptamine (Tryp), a weak partial agonist of vertebrate serotonin type 3 (5-HT
3 ) receptors, efficaciously activates MOD-1. A screen for modulators revealed that GABAergic ligands piperazine (PZE) and muscimol reduce 5-HT-elicited currents, thus identifying novel MOD-1 allosteric inhibitors. Next, we performed locomotor activity assays, and we found 5-HT and Tryp rapidly decrease worm motility, which is reversible only at low 5-HT concentrations. Mutants lacking MOD-1 are partially resistant to both drugs, demonstrating its role in locomotion. Acting as an antagonist of MOD-1, we showed PZE reduces the locomotor effects of exogenous 5-HT. Therefore, Tryp- and PZE-derived compounds, acting at MOD-1 through different molecular mechanisms, emerge as promising anthelmintic agents. - Abstract
The main objective of this study is to analyse the process of integration of health care implemented in the public health system (Osakidetza) of the Autonomous Community of the Basque Country (CAPV), and assess whether the steps taken to date have helped or hindered the work of health personnel in times of COVID-19. Based on a case study, an assessment is made of the way in which certain tools of the integration process have been applied, if they have worked well and if they have led to better management of the pandemic.For the purpose of this study, a qualitative methodology is chosen consisting of a case study and in-depth interviews with health personnel at the front line of the integration process and the fight against COVID-19.This study makes two fundamental contributions. First, it analyses the health integration process in recent years in the public health system of the Basque Country. Second, it gathers the perceptions of different agents related to the Basque Health System of the way in which the tools of the integration process implemented in recent years have worked during the pandemic, detailing the positive and negative perceptions in this regard.Our conclusions offer a series of strategic recommendations linked to comprehensive patient care and the use of tools related to teleconsulting: the unified medical record, electronic prescription, and non-face-to-face care channels.

- Abstract
The anthelmintic treatment of nematode infections remains the pillar of worm control in both human and veterinary medicine. Since control is threatened by the appearance of drug resistant nematodes, there is a need to develop novel compounds, among which phytochemicals constitute potential anthelmintic agents. Caenorhabditis elegans has been pivotal in anthelmintic drug discovery and in revealing mechanisms of drug action and resistance. By using C. elegans, we here revealed the anthelmintic actions of three plant terpenoids -thymol, carvacrol and eugenol- at the behavioral level. Terpenoids produce a rapid paralysis of worms with a potency rank order carvacrol > thymol > eugenol. In addition to their paralyzing activity, they also inhibit egg hatching, which would, in turn, lead to a broader anthelmintic spectrum of activity. To identify drug targets, we performed an in vivo screening of selected strains carrying mutations in receptors involved in worm locomotion for determining resistance to the paralyzing effect of terpenoids. The assays revealed that two Cys-loop receptors with key roles in worm locomotion -Levamisole sensitive nicotinic receptor (L-AChR) and GABA(A) (UNC-49) receptor- are involved in the paralyzing effects of terpenoids. To decipher the mechanism by which terpenoids affect these receptors, we performed electrophysiological studies using a primary culture of C. elegans L1 muscle cells. Whole cell recordings from L1 cells demonstrated that terpenoids decrease macroscopic responses of L-AChR and UNC-49 receptor to their endogenous agonists, thus acting as inhibitors. Single-channel recordings from L-AChR revealed that terpenoids decrease the frequency of opening events, probably by acting as negative allosteric modulators. The fact that terpenoids act at different receptors may have important advantages regarding efficacy and development of resistance. Thus, our findings give support to the use of terpenoids as either an alternative or a complementary anthelmintic strategy to overcome the ever-increasing resistance of parasites to classical anthelmintic drugs., Competing Interests: The authors have declared that no competing interests exist.

Nicotinic acetylcholine receptors (nAChRs) are ligand-gated ion channels involved in neuromuscular transmission. In nematodes, muscle nAChRs are targets of antiparasitic drugs. Bephenium is an anthelmintic compound whose molecular action in the free-living nematode Caenorhabditis elegans , which is a model for anthelmintic drug discovery, is poorly known. We explored the effect of bephenium on C. elegans locomotion and applied single-channel recordings to identify its molecular target, mechanism of action, and selectivity between mammalian and C. elegans nAChRs. As in parasites, bephenium paralyzes C. elegans A mutant strain lacking the muscle levamisole-sensitive nAChR (L-AChR) shows full resistance to bephenium, indicating that this receptor is the target site. Bephenium activates L-AChR channels from larvae muscle cells in the micromolar range. Channel activity is similar to that elicited by levamisole, appearing mainly as isolated brief openings. Our analysis revealed that bephenium is an agonist of L-AChR and an open-channel blocker at higher concentrations. It also activates mammalian muscle nAChRs. Opening events are significantly briefer than those elicited by ACh and do not appear in activation episodes at a range of concentrations, indicating that it is a very weak agonist of mammalian nAChRs. Recordings in the presence of ACh showed that bephenium acts as a voltage-dependent channel blocker and a low-affinity agonist. Molecular docking into homology-modeled binding-site interfaces represent the binding mode of bephenium that explains its partial agonism. Given the great diversity of helminth nAChRs and the overlap of their pharmacological profiles, unraveling the basis of drug receptor-selectivity will be required for rational design of anthelmintic drugs., (Copyright © 2018 by The American Society for Pharmacology and Experimental Therapeutics.)

- Abstract
The prevalence of human and animal helminth infections remains staggeringly high, thus urging the need for concerted efforts towards this area of research. GABA receptors, encoded by the unc-49 gene, mediate body muscle inhibition in Caenorhabditis elegans and parasitic nematodes and are targets of anthelmintic drugs. Thus, the characterization of nematode GABA receptors provides a foundation for rational anti-parasitic drug design. We therefore explored UNC-49 channels from C. elegans muscle cultured cells of the first larval stage at the electrophysiological and behavioral levels. Whole-cell recordings reveal that GABA, muscimol and the anthelmintic piperazine elicit macroscopic currents from UNC-49 receptors that decay in their sustained presence, indicating full desensitization. Single-channel recordings show that all drugs elicit openings of ∼2.5 pA (+100 mV), which appear either as brief isolated events or in short bursts. The comparison of the lowest concentration required for detectable channel opening, the frequency of openings and the amplitude of macroscopic currents suggest that piperazine is the least efficacious of the three drugs. Macroscopic and single-channel GABA-activated currents are profoundly and apparently irreversibly inhibited by ivermectin. To gain further insight into ivermectin action at C. elegans muscle, we analyzed its effect on single-channel activity of the levamisol-sensitive nicotinic receptor (L-AChR), the excitatory receptor involved in neuromuscular transmission. Ivermectin produces a profound inhibition of the frequency of channel opening without significant changes in channel properties. By revealing that ivermectin inhibits C. elegans muscle GABA and L-AChR receptors, our study adds two receptors to the already known ivermectin targets, thus contributing to the elucidation of its pleiotropic effects. Behavioral assays in worms show that ivermectin potentiates piperazine-induced paralysis, thus suggesting that their combination is a good strategy to overcome the increasing resistance of parasites, an issue of global concern for human and animal health.
