4. The innovation and clinical application of ischemia-free organ transplantation

Abstract

The incidence rate of liver cancer is increasing year by year globally. Liver transplantation has become one of the therapeutic methods for patients with liver cancer. In the past, ischemia-reperfusion injury (IRI) was unavoidable in liver transplantation, compromising recipient and graft survival. At the same time, the imbalance between the supply and demand of organs limits the use of transplantation in patients with liver cancer. Recently, advances have been made in machine perfusion techniques to reduce graft IRI. However, none of the techniques can completely abrogate graft IRI. In 2017, the concept of ischemia-free organ transplantation (IFOT) was proposed, and our group conducted the first case of ischemia-free liver transplantation (IFLT). We then extended the concept of IFOT to kidney transplantation in 2018 and to heart transplantation in 2021. Here, we review the history, strengths, and weaknesses, and the future direction of IFOT, particularly in patients with liver cancers.

9. Multidisciplinary Cooperation in a Simultaneous Combined Liver and Kidney Transplantation Patient of Primary Hyperoxaluria

Abstract

Primary hyperoxaluria type 1 is an autosomal recessive hereditary glyoxylate metabolism disorder characterized by excessive production of oxalate, caused by the deficiency of liver specific peroxisomal enzyme: alanineglyoxylate aminotransferase. For patients with end-stage renal disease, combined liver and kidney transplantation was needed. This report describes one patient, with a diagnosis of end-stage renal disease and primary hyperoxaluria 1 confirmed by PCR and direct sequencing with genomic DNA, received the simultaneous combined liver and kidney transplantation after seven months’ waiting. However, there were several complications observed post surgery, such as protracted bleeding, common bile duct anastomotic stenosis, biliary calculi and recurrence of urolithiasis. All these were well solved by relevant department, and finally a satisfactory outcome was achieved. Multidisciplinary cooperation plays an important role on the PH1 patient management, especially when multiple complications are encountered. Abstract

Bone health related skeletal disorders are commonly diagnosed by X-ray imaging, but the radiation limits its use. Light excitation and optical imaging through the near-infrared-II window (NIR-II, 1000-1700 nm) can penetrate deep tissues without radiation risk, but the targeting of contrast agent is non-specific. Here, we report that lanthanide-doped nanocrystals can be passively transported by endothelial cells and macrophages from the blood vessels into bone marrow microenvironment. We found that this passive targeting scheme can be effective for longer than two months. We therefore developed an intravital 3D and high-resolution planar imaging instrumentation for bone disease diagnosis. We demonstrated the regular monitoring of 1 mm bone defects for over 10 days, with resolution similar to X-ray imaging result, but more flexible use in prognosis. Moreover, the passive targeting can be used to reveal the early onset inflammation at the joints as the synovitis in the early stage of rheumatoid arthritis. Furthermore, the proposed method is comparable to {\mu}CT in recognizing symptoms of osteoarthritis, including the mild hyperostosis in femur which is ~100 {\mu}m thicker than normal, and the growth of millimeter-scale osteophyte in the knee joint, which further proves the power and universality of our approach in diagnosis of bone diseases

12. Optimized Matching Lift Unit Transmission Ratio of Engine Driven Ducted Fan

Abstract

As a kind of VTOL technology, ducted fan is not only used by many kinds of aircrafts, but also one of the trends of the future aircraft lift system, and attracts more and more attention. For an engine driven ducted fan lift unit, involving the engine and ducted fan matching problem, the form of transmission and transmission ratio are the key design parameters. In order to design and develop a ducted fan aircraft reasonably, a thrust test platform was set up to connect the engine with the ducted fan through the belt driving. The matching relationship between the engine and the transmission system was experimentally studied and the optimal transmission ratio was determined. The results showed that the optimal transmission ratio for the engine 1 is 2.2:1, and for the engine 2, the optimal transmission ratio should be 2.95:1 based on the current ducted and movable blade aerofoil design. At this time, the lift will exceed 130 kg•f, meeting the aircraft's original design requirements.

Abstract

The late third-millennium BC Longshan period was a crucial time for state formation in central China. During these centuries, long-distance networks expanded and shared material culture and then cultural practices spread across wider areas precipitating social and ideological developments that presaged the rise of states and cities on the Central Plain. In this research, the authors use multiple (strontium, oxygen and carbon) isotope analyses from the dental enamel of 67 individuals buried at the Xiajin cemetery, Shanxi Province. The results indicate significant long-distance migration among females during the Longshan period, which the authors interpret as evidence of exogamous marriage for political alliance-building--a phenomenon found more widely across Eurasia at the start of the Bronze Age. 14. An MPI-based parallel genetic algorithm for multiple geographical feature label placement based on the hybrid of fixed-sliding models

Abstract

Multiple geographical feature label placement (MGFLP) has been a fundamental problem in geographic information visualization for decades. The nature of label positioning is proven an NP-hard problem, where the complexity of such a problem is directly influenced by the volume of input datasets. Advances in computer technology and robust approaches have addressed the problem of labeling. However, what is less considered in recent studies is the computational complexity of MGFLP, which significantly decreases the adoptability of those recently introduced approaches. In this study, an MPI parallel genetic algorithm is proposed for MGFLP based on a hybrid of fixed position model and sliding model to label fixed-types of geographical features. To evaluate the quality of label placement, a quality function is defined based on four quality metrics, label-feature conflict, label-label conflict, label ambiguity factor, and label position priority for points and polygons. Experimental results reveal that the proposed algorithm significantly reduced the overall score of the quality function and the computational time of label placement compared to the previous studies. The algorithm achieves a result in less than one minute with 6 label-feature conflicts, while Parallel-MS (Lessani et al., 2021) obtains the result in more than 20 minutes with 12 label-feature conflicts for the same dataset., Comment: 35 Pages, 11 Figures

Abstract

In this work, an image classification based on deep learning for quantitative field determination of aluminum ions (Al.sup.3+) was developed. Carbon quantum dots with yellow fluorescence were synthesized by a one-pot hydrothermal method which could specifically recognize Al.sup.3+ and produce enhanced green fluorescence. Using the convolutional neural network model in deep learning, an image classification was constructed to classify Al.sup.3+ samples at different concentrations. Then, a fitting method for classification information was proposed for the first time which could convert discontinuous, semi-quantitative concentration classification information into continuous, quantitative, and accurate concentration information. Recoveries of 92.0-110.3% in the concentration range of 0.3-320 [mu]M were obtained with a lower limit of detection of 0.3 [mu]M, exhibiting excellent accuracy and sensitivity. It could be completed in 2 min simply without requiring large equipment. 30. High Spatial and Temporal Resolution NIR-IIb Gastrointestinal Imaging in Mice

Abstract

Conventional biomedical imaging modalities, including endoscopy, X-rays, and magnetic resonance, are invasive and cannot provide sufficient spatial and temporal resolutions for regular imaging of gastrointestinal (GI) tract to guide prognosis and therapy of GI diseases. Here we report a non-invasive method for optical imaging of GI tract. It is based on a new type of lanthanide-doped nanocrystal with near-infrared (NIR) excitation at 980 nm and second NIR window (NIR-IIb) (1500~1700 nm) fluorescence emission at around 1530 nm. The rational design and controlled synthesis of nanocrystals with high brightness have led to an absolute quantum yield (QY) up to 48.6%. Further benefitting from the minimized scattering through the NIR-IIb window, we enhanced the spatial resolution by 3 times compared with the other NIR-IIa (1000~1500 nm) contract agents for GI tract imaging. The approach also led to a high temporal resolution of 8 frames per second, so that the moment of mice intestinal peristalsis happened in one minute can be captured. Furthermore, with a light-sheet imaging system, we demonstrated a three-dimensional (3D) imaging of the stereoscopic structure of the GI tract. Moreover, we successfully translate these advances to diagnose inflammatory bowel disease (IBD) in a pre-clinical model of mice colitis.
