U sklopu ovogodišnje Noći istraživača, manifestacije financirane u okviru programa Horizon Europe koja ima za cilj približiti znanost i istraživanje javnosti, znanstvenici iz Centra za Proteomiku i Zavoda za Histologiju pripremili su 3 slikovnice naslova: „Što je rak?“, „Što su protutijela“ i „Što rade znanstvenici“. Navedene slikovnice imaju za cilj na jednostavan i razumljiv način objasniti djeci i mladima osnovne koncepte koji se tiču biologije raka, protutijela kao alata za istraživanje ili terapiju, te znanstvenika kao profesije koja sudjeluje u „borbi“ protiv raka. Naime, djeca, čak i ako nisu u stanju razumjeti rak, posebno su pažljiva i jako pogođena dijagnozom raka, a istraživanja pokazuju da otvorenost i iskrenost pomažu djeci da se nose s problemima. Osnovne informacije o raku pokrivene su u prvoj slikovnici “Što je rak?”, autorice dr.sc. Paole Kučan Brlić, koja je namijenjena djeci predškolske dobi i nižih razreda osnovne škole. Druga slikovnica, “Što su antitijela?”, autorice Dore Gašparini, dr.med., namijenjena je starijoj djeci te objašnjava kako funkcioniraju protutijela i kako se ona mogu koristiti za liječenje raka ili u znanstvenim istraživanjima. Zadnja slikovnica, „Što rade znanstvenici“, autorica dr.sc. Jelene Železnjak i dr.sc. Marije Mazor, predstavlja i promovira znanstvenike kao profesiju koja se, između ostalog, bavi istraživanjem i liječenjem raka, te objašnjava kako se postaje znanstvenik. - Abstract
Povezanost vitamina D i kože započinje sintezom vitamina D u koži, a neprestano se istražuju i uočavaju novi mehanizmi kojima najistraživaniji vitamin utječe na najveći organ ljudskoga tijela. Cilj je ovoga rada dati pregled dosadašnjih spoznaja o interakciji vitamina D i kože te utjecaju razina vitamina D na kožne bolesti. Po nekim istraživanjima suboptimalne razine vitamina D nalaze se u čak 30 – 90 % stanovništva, a procjenjuje se da postoji manjak vitamina D u oko jedne milijarde ljudi na cijelome svijetu. U 25,6 – 68 % pacijenata sa psorijazom postoji nedostatak vitamina D, a manjak vitamina D nazočan je u čak 97 % pacijenata. Nadalje, niz opservacijskih studija i metaanaliza dokazalo je da su serumske razine kalcidiola niže u djece i odraslih s atopijskim dermatitisom, a pojedina istraživanja dokazala su da u čak 70,5 % pacijenata s vitiligom postoji nedostatak vitamina D. Niske razine vitamina D povezuju se i s progresijom malignog melanoma, no nepoznato je u kojem trenutku izloženosti UV zračenju vitamin D prestaje djelovati protektivno i započinje biti rizični čimbenik za razvoj tumora kože. Unatoč brojnim provedenim istraživanjima, još uvijek nije poznata priroda odnosa vitamina D i pojedinih kožnih bolesti, odnosno je li on uzročnoposljedičan ili slučajan, dok se u drugih dermatoza, poput psorijaze, dokazi o povezanosti s razinom vitamina D primjenjuju u svakodnevnom radu s pacijentima – u dijagnostici i liječenju., The relationship between vitamin D and the skin starts with the synthesis of vitamin D in the skin. New mechanisms on how the most researched vitamin affects the biggest organ in the human body are constantly investigated and observed. The aim is to review the current knowledge on vitamin D and skin interaction, as well as the effect of vitamin D levels on skin diseases. According to some studies, suboptimal vitamin D levels are found in as many as 30 – 90% of the population, with an estimated vitamin D insufficiency in about one billion people worldwide. In 25.6 – 68% of patients with psoriasis, there is a deficiency of vitamin D and insufficiency of vitamin D in as many as 97% of patients. Furthermore, a series of observational studies and meta-analyses have shown that the serum levels of calcidiol are lower in children and adults with atopic dermatitis, and some studies have shown that up to 70.5% of patients with vitiligo have vitamin D deficiency. Low levels of vitamin D are associated with the progression of malignant melanoma, but it is unknown at which point UV exposure ceases to be protective through vitamin D and begins to be a risk factor for the development of skin tumours. Despite many studies that have been conducted, the nature of the relationship between vitamin D and certain skin diseases is still unknown, whether it is causal or accidental, while in others, such as psoriasis, evidence of the correlation with vitamin D levels is used in daily work with patients – in diagnosis and treatment.

Šećerna bolest, kronična progresivna metabolička bolest, globalni je javnozdravstveni problem. U pojedinaca čije razine glukoze u plazmi ne zadovoljavaju dijagnostičke kriterije za šećernu bolest, a previsoke su da bi se smatrale urednim nalazom postoji stanje koje nazivamo predijabetes. Budući da je predijabetes reverzibilno stanje, u kojem se još uvijek može spriječiti razvoj šećerne bolesti, cilj je mnogih znanstvenika odrediti točku u kojoj stanje predijabetesa prelazi u šećernu bolest. Znanstvenici su prepoznali infekcije kao jedan od potencijalnih rizičnih čimbenika za razvoj šećerne bolesti tipa 2, odnosno kao ‘događaj’ koji pretvara stanje predijabetesa u šećernu bolest. Virusna infekcija dovodi do smanjenja osjetljivosti na inzulin, koji gušterača ne može kompenzirati povećanjem lučenja inzulina, što dovodi do hiperglikemije i naglog nastupa šećerne bolesti u bolesnika s predijabetesom. Ako bi ova saznanja bila ponovljiva u humanim studijama većega opsega, ovo bi zapažanje moglo utjecati na način kako liječnici pružaju zdravstvenu skrb bolesnicima s predijabetesom i s virusnim infekcijama. Virusne infekcije uobičajeno ne zahtijevaju liječenje, no klinički pristup zasigurno bi se promijenio ako bi se time mogao spriječiti njihov utjecaj na toleranciju glukoze. Iako je šećerna bolest značajan zdravstveni problem, tek nam je aktualna pandemija uzrokovana koronavirusom identificiranim 2019. godine otkrila koliko, zapravo, malo znamo o imunološkim mehanizmima infekcije u bolesnika sa šećernom bolesti. Sada postajemo svjesni koliko je važno istraživati mehanizme, koji objašnjavaju imunoendokrinološke odnose, i usmjeriti znanstvene napore upravo u polje imunometabolizma kako bismo dobili što više informacija koje bi nam pomogle u svakodnevnoj skrbi bolesnika sa šećernom bolesti., Diabetes mellitus, chronic progressive metabolic disorder, is a global public-health problem. In individuals whose glucose levels do not reach diagnostic criteria for diabetes mellitus but are too high to be considered normal, there is a condition called prediabetes. Since prediabetes is a reversible condition, in which the development of diabetes mellitus can be prevented, many investigators aim to determine the point at which prediabetes shifts to diabetes. Scientists recognized infection as one of the potential risk factors for the development of diabetes mellitus type 2, more precisely as an ‘event’ that derails glycemic control from prediabetes to diabetes. The viral infection leads to decreased insulin sensitivity, which cannot be compensated by insulin production in the pancreas resulting in hyperglycemia and abrupt onset of diabetes in patients with prediabetes. If these findings were replicable in large-scale human studies, this observation could impact the way clinicians would provide healthcare to patients with prediabetes and viral infection. Viral infections usually do not require treatment, but the clinical approach would change if their impact on glucose tolerance could be prevented. Even though diabetes mellitus is a significant health issue, only current coronavirus disease 2019 pandemic has unfolded how little we know about the immunological mechanisms in patients with diabetes mellitus. We are now becoming aware of how important it is to investigate mechanisms, that elucidate immunoendocrinologic interactions, and to direct scientific efforts particularly in the field of immunometabolism to obtain as many information as possible, that would help us in the daily care of patients with diabetes mellitus.

Chronic pain is a painful condition defined by its duration where pain persists three months or more. Pain is connected with the high price of health care, work inability and disability. Moreover, it has significant consequences for patients and their families, working organizations and the society as a whole. The prevalence of chronic pain can range between 11.0% and 51.3% in general population. Pain is usually coherent with distress and a range of psychological symptoms such as depression, anxiety, altered attention and cognition manifesting as fear. Comprehensive pain management should always include the treatment of associated psychological symptoms. Multidisciplinary approach in treating chronic pain and its comorbidities and proper education of primary care physicians and different specialists involved in the management of chronic pain are crucial for better clinical outcomes. Topical capsaicin acts as a highly selective agonist of transient receptor potential vanilloid 1 of C and Aį nociceptors. Repeated applications or high concentrations give rise to a long- lasting effect termed defunctionalisation. In addition, the reduction of central sensitization through reduced C- nociceptor input contributes to capsaicin's indirect mechanism of action. Capsaicin provides effective durable pain relief and reduction of intensity and area of pain in adult patients with chronic pain with a faster onset of analgesia and considerably fewer systemic adverse effects than the conventional treatment. While offering high levels of pain relief, additional improvements in sleep, fatigue, depression and quality of life have been noticed. Topical administration avoids dangerous systemic adverse effects and enables the combination with other drugs and analgesics with limited drug-drug interactions. Adding capsaicin to the standard chronic pain treatment might improve, fasten and ease the challenging path of managing chronic pain consequently providing the patient and their society with better quality of life.

Od toga kako prepoznati osobu s genetičkim poremećajem, kako nastaju i koje vrste genetičkih poremećaja postoje pa sve do toga na koji ih način možemo dijagnosticirati. Nadalje, nakon svake ilustracije na pojedinoj stranici nalaze se definicije 79 pojmova iz medicinske genetike koje čine tezaurus za studente, liječnike i pacijente koji se na bilo koji način susreću s genetičkim poremećajima. Naposljetku, ova knjiga sadrži i primjere rečenica u koje su ubačeni stručni pojmovi iz medicinske genetike, a koji su namijenjeni studentima prilikom savladavanja komunikacijskih vještina na kolegiju Medicinska genetika, ali i liječnicima prilikom informiranja svojih pacijenata o (mogućem) genetičkom poremećaju. Introduction: Synthetic anabolic androgenic steroids (AAS), compounds mimicking the action of endogenous testosterone in enhancing training performance, have been extensively studied during the last century. AAS abuse has become a major public health concern with an estimated worldwide lifetime prevalence of 1–5%1 . Long-term administration of AAS in supraphysiological doses may have detrimental effects on the cardiovascular system, presumably through direct action on cardiac myocyte androgen receptors. In severe cases, life-threatening conditions such as myocardial infarction, aortic dissection or cardiomyopathy, particularly dilated cardiomyopathy as the most common form, may occur. Hereby, we report a rare case of AAS-induced cardiomyopathy with an emphasis on the multidisciplinary approach. Case report: 46-year-old male bodybuilder presented with exercise intolerance unrelated to maximum training load and post-workout water retention 6 weeks before the visit. History revealed previous administration of testosterone enanthate 500 mg every 8 to 12 days during the period of 4 years. After a month-long cessation, he started taking testosterone undecanoate 1000 mg in 6-week intervals. The cardiorespiratory part of the physical examination showed normal findings and blood pressure of 125/80 mmHg. The patient was of athletic build with no signs of increased hairiness and no palpable testicular mass. An electrocardiogram showed a normal electrical axis and sinus bradycardia. Laboratory assessment (Table 1) was followed by echocardiography which was in accordance with the diagnosis of AAS-induced cardiomyopathy (Figure 1). Further diagnostic assessment of osteoporosis, hepatic, renal and psychological complications was performed. Conclusion: Long-term administration of AAS with unknown pharmacokinetic and pharmacodynamic properties should be considered as a cause of newly diagnosed cardiomyopathy, especially in previously healthy individuals with an athletic background. - Abstract
Memory CD8 T cells play an important role in the protection against breakthrough infections with severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2). Whether the route of antigen exposure impacts these cells at a functional level is incompletely characterized. Here, we compare the memory CD8 T cell response against a common SARS-CoV-2 epitope after vaccination, infection, or both. CD8 T cells demonstrate comparable functional capacity when restimulated directly ex vivo, independent of the antigenic history. However, analysis of T cell receptor usage shows that vaccination results in a narrower scope than infection alone or in combination with vaccination. Importantly, in an in vivorecall model, memory CD8 T cells from infected individuals show equal proliferation but secrete less tumor necrosis factor (TNF) compared with those from vaccinated people. This difference is negated when infected individuals have also been vaccinated. Our findings shed more light on the differences in susceptibility to re-infection after different routes of SARS-CoV-2 antigen exposure.

23. A cross-sectional study in type 2 diabetes patients reveals that elevated pulse wave velocity predicts asymptomatic peripheral arterial disease associated with age and diabetes duration.

- Abstract
Background: Peripheral arterial disease (PAD) reduces functional capacity and raises cardiovascular risks, but underdiagnosis is common, resulting in less comprehensive care than other cardiovascular conditions. While diabetes has long been viewed as a key risk factor for PAD, recent studies indicate that its impact is influenced by the presence of concurrent cardiovascular risk factors. The aim of this study is to elucidate the intricate relationship between the prevalence of PAD, diabetic complications, and cardiovascular risk factors among asymptomatic patients with type 2 diabetes mellitus (T2DM)., Methods: Ninety-one patients with T2DM and no symptoms or previous diagnosis of PAD were recruited from the outpatient diabetic clinic. Clinical data were extracted from electronic medical records, and the screening for PAD was conducted using MESI mTABLET., Results: Screening for PAD among asymptomatic individuals with T2DM revealed that 5.49 % of patients exhibit a low ankle-brachial index (ABI). Patients who had previously experienced major adverse cardiovascular events or exhibited albuminuria displayed lower ABI values. Furthermore, a striking 45.05 % of the participants displayed an abnormally high carotid-femoral pulse wave velocity (cfPWV) value, with elevated PWV values correlating with advanced age and longer diabetes duration., Conclusions: The prevalence of elevated cfPWV is more pronounced than that of decreased ABI in T2DM patients with asymptomatic PAD and is associated with older age and longer diabetes duration, therefore measurement of both ABI and PWV is crucial for the cardiovascular risk assessment protocol for patients with T2DM and timely PAD diagnosis., Competing Interests: The authors declare that they have no known competing financial interests or personal relationships that could have appeared to influence the work reported in this paper., (© 2023 The Authors.)

- Abstract
Chronic pain is a painful condition defined by its duration where pain persists three months or more. Pain is connected with the high price of health care, work inability and disability. Moreover, it has significant consequences for patients and their families, working organizations and the society as a whole. The prevalence of chronic pain can range between 11.0% and 51.3% in general population. Pain is usually coherent with distress and a range of psychological symptoms such as depression, anxiety, altered attention and cognition manifesting as fear. Comprehensive pain management should always include the treatment of associated psychological symptoms. Multidisciplinary approach in treating chronic pain and its comorbidities and proper education of primary care physicians and different specialists involved in the management of chronic pain are crucial for better clinical outcomes. Topical capsaicin acts as a highly selective agonist of transient receptor potential vanilloid 1 of C and Aδ nociceptors. Repeated applications or high concentrations give rise to a long-lasting effect termed defunctionalisation. In addition, the reduction of central sensitization through reduced C-nociceptor input contributes to capsaicin's indirect mechanism of action. Capsaicin provides effective durable pain relief and reduction of intensity and area of pain in adult patients with chronic pain with a faster onset of analgesia and considerably fewer systemic adverse effects than the conventional treatment. While offering high levels of pain relief, additional improvements in sleep, fatigue, depression and quality of life have been noticed. Topical administration avoids dangerous systemic adverse effects and enables the combination with other drugs and analgesics with limited drug-drug interactions. Adding capsaicin to the standard chronic pain treatment might improve, fasten and ease the challenging path of managing chronic pain consequently providing the patient and their society with better quality of life.
