Abstract

Research shows fluctuations in drinking across the menstrual cycle among women with alcohol use disorder (AUD), but little work has investigated moderators of these fluctuations. This study examined drinking and craving intensity across the menstrual cycle, and the moderating effect of baseline depression and emotional distress during the midlate luteal phase and/or menses, among women receiving AUD treatment. Fifty-nine regularly cycling women reported menstrual history and baseline depression. Over 3 months of treatment, they kept daily logs of drinks, alcohol cravings, and menstruation (yes/no). Emotional distress during the midlate luteal phase and/or menses of their most recent menstrual cycle was also assessed during treatment. Menstrual cycle phase was estimated for each within-treatment day. Mixed model analyses tested main and interactive effects of menstrual cycle phase, baseline depression, and emotional distress during the midlate luteal phase and/or menses on daily drinks and craving intensity. Women drank most during the midlate luteal phase and menses compared with other phases. Among women with lower baseline depression, those with lower distress during the midlate luteal phase and/or menses reported more intense cravings during the midlate luteal phase (ΔM = .77, p = .000) and menses (ΔM = .51, p = .012); those with higher distress reported more intense cravings during menses, compared with all other phases (p < .01). Among women with higher baseline depression, craving intensity remained consistently high. Results document more drinking during the midlate luteal phase and menses and suggest that cycle-related distress and depression moderate the alcohol-menstrual association among women in AUD treatment. Abstract

Objective: Despite the growing research on the effectiveness of specialty courts, including drug treatment courts (DTCs) and veterans treatment courts (VTCs), little is known about the unique treatment needs of individuals with co-occurring mental health and substance use disorders (COD) who are commonly served by these courts. This pilot study examines individuals with COD across two DTCs and two VTCs in Massachusetts to understand the characteristics and treatment needs of these unique populations. Methods: Baseline data for 112 individuals enrolled in a community-based COD treatment program within two DTCs (N = 86) and two VTCs (N = 26) with a Monte Carlo simulation of 10,000 samples were analyzed to assess differences in treatment needs. Results: DTC participants experienced more lifetime convictions (p =.000), months incarcerated (p =.000), and had a longer lifetime history of illicit drug use (p =.004). Meanwhile, a higher proportion of VTC participants experienced trauma (p <.002) and chronic medical problems (p <.001), and VTC participants experienced more severe acute behavioral health symptoms as measured by the BASIS-32 (p =.021). Conclusions: Given the unique differences in treatment needs between DTC and VTC participants, these findings have implications for specialty court management such as treatment planning, referrals to meet participants' service needs, and even docket sizes. 30. Correlates of substance use among urban Latino immigrant high school freshmen

Abstract

Substance use in immigrant youth frequently has been associated with different aspects of acculturation. There is inconsistency in the literature, however, about the direction of these relationships. Furthermore, seldom has the role of acculturation been examined in the context of other common substance use correlates, such as peer use and relationship with parents. Finally, the possible contribution of sense of school belonging previously has not been considered along with acculturation, peer use, and parent relationships, in explaining substance use and intentions to use in immigrant adolescents. Thus, the current study examined the extent to which levels of language and cultural acculturation, years in the country, sense of school belonging, relationship with parents, and friends’ use would account for their use and intentions to use substances in a sample of 166 Northeastern ninth grade urban Latino immigrant adolescents. Regression analysis revealed that language acculturation, sense of school belonging, and friends’ use were significantly associated with adolescent substance use and intentions to use. Consistent with past research in the general adolescent population, Latino immigrant youth who had fewer friends who use substances and had higher levels of school bonding/sense of school belonging were less likely to report using substances and/or having intentions to use. Contrary to previous literature examining non-clinical immigrant youth, the current study found that higher use of native language (low language acculturation) also predicted students’ use and intentions to use.
