Dentine hypersensitivity is defined as short, sharp pain from exposed dentin in reaction to thermal, evaporative, tactile, osmotic, or chemical stimuli. This condition is associated with non-carious cervical lesions (erosion, abrasion and abfraction), gingival recessions, periodontal disease, among other dental problems. Due to its multifactorial nature, the diagnosis of dentin hypersensitivity is often difficult and confusing for the dental surgeon. There is no single strategy that can solve this condition in all patients and it is the responsibility of the dental surgeon to know its characteristics in order to propose an effective treatment plan for patients. Therefore, this review aims to show the main characteristics of dentin hypersensitivity, as well as the challenges associated with clinical management, for understanding and mastery in clinical practice. La hipersensibilidad dentinaria se define como un dolor corto y agudo de la dentina expuesta como reacción a estímulos térmicos, evaporativos, táctiles, osmóticos o químicos. Esta condición normalmente está asociada a lesiones cervicales no cariosas (erosión, abrasión y abfracción), recesiones gingivales, enfermedad periodontal, entre otros problemas odontológicos. Por su naturaleza multifactorial, el diagnóstico de la hipersensibilidad dentinaria suele ser difícil y confuso para el cirujano dentista. No existe una estrategia única que pueda resolver esta condición en todos los pacientes y es de responsabilidad del cirujano dentista conocer sus características para proponer un plan de tratamiento eficaz para sus pacientes. Por tal motivo, la presente revisión tiene como objetivo mostrar las principales características de la hipersensibilidad dentinaria, así como los desafíos asociados a su manejo clínico, para su entendimiento y dominio en la práctica clínica.

O presente artigo descreve um caso incomum de canino superior direito com duas raízes. Esse é o quarto caso relatado na literatura. Paciente do sexo feminino, 66 anos de idade, foi encaminhada para tratamento endodôntico do dente #13. A avaliação clínica mostrou que o dente #13 não apresentava resposta após o teste de vitalidade. Radiografias foram realizadas e revelaram que o dente #13 apresentava duas raízes. Em seguida, a paciente foi submetida a tratamento endodôntico. Esse relato de caso é relevante porque descreve uma variação anatômica rara, mas que pode se apresentar na clínica. Por tal motivo, o dentista precisa ter domínio da anatomia normal dos dentes para realizar corretamente o tratamento endodôntico, assim como conhecer as possíveis variações anatômicas, e analisar minuciosamente a radiografia de diagnóstico para ter uma noção individualizada da anatomia do dente a tratar.

Dental erosion is the progressive loss of the dental structure associated with acids of non-bacterial origin and with irreversible effects. The consumption of beverages and foods with high acid content corresponds to the main extrinsic factor of dental erosion, while the endogenous acids that project into the mouth through the esophagus during vomiting or regurgitation are part of the intrinsic factors that contribute to this injury. The prevention and treatment of eroded teeth are real challenges in dental practice. Therefore, this article aims to collect information available in the current literature on the etiology, clinical characteristics and treatment of erosive dental wear, to provide a practical guide for dental surgeons and students. La erosión dental se define como la pérdida progresiva de la estructura dental asociada a ácidos de origen no bacteriano y con efectos irreversibles. El consumo de bebidas y alimentos con alto contenido ácido corresponde al principal factor contribuyente extrínseco para la erosión dental, mientras que los ácidos endógenos que se proyectan, a través del esófago, en la boca, durante el vómito o regurgitación, forman parte de los factores intrínsecos que contribuyen con esta lesión. La prevención y tratamiento de dientes erosionados constituyen verdaderos desafíos en la práctica odontológica. Por tal motivo, el presente artículo tiene como objetivo recopilar información disponible en la literatura actual sobre la etiología, características clínicas y tratamiento del desgaste dental erosivo, para brindar una guía práctica dirigida a cirujanos dentistas y estudiantes.

Objetivos: Evaluar los efectos de agentes remineralizantes a base de fosfato de calcio enla obliteración de túbulos dentinarios y dispersión de energía en la dentina cervical.Material y métodos: Treinta especímenes de dentina cervical bovina fueron previamenteerosionados (ácido clorhídrico al 0,6 %, pH 2,3; 5 min) y divididos aleatoriamente en 3grupos (n = 10): Control – sin tratamiento, NP – Desensibilize Nano P (FGM®) y RD – MIPaste Plus (RecaldentTM). Los tratamientos fueron aplicados en cuatro sesiones conintervalos de 7 días. Los especímenes fueron analizados por microscopia electrónica debarrido (MEB) y espectroscopia de rayos X de energía dispersiva (EDX).Resultados: Losgrupos NP e RD mostraron mayor obliteración de túbulos dentinarios que el grupo control (p< 0,05). Las concentraciones de Ca/P fueron similares entre todos los grupos evaluados.Conclusiones: Los agentes remineralizantes a base de fosfato de calcio demostraron seruna alternativa promisoria en la obliteración de túbulos dentinarios.

No. of bitstreams: 0 Previous issue date: 2018-08-01 Objectives: To evaluate the effect of desensitizing agent containing calcium phosphate nanoparticles on the bond strength of etch-and-rinse adhesive system (Scotchbond Multi-Purpose), presence of precipitate, dentinal tubule obliteration and hybrid layer formation in dentin in comparison with potassium nitrate plus sodium fluoride or strontium chloride compounds. Methods: 150 bovine incisors were treated with (n = 10): G1, Desensibilize Nano P (Ca3(PO4)2+5%KNO3+0.9%NaF); G2, Desensibilize (10%SrCl2+5%KNO3); G3, Desensibilize KF2% (5%KNO3+0.2%NaF); G4, Ultra EZ (3%KNO3+0.25%NaF) and G5, no treated (control). Scanning electron microscopy was used to assess the incidence of precipitates (500×) and obliterated dentinal tubule counts (1.000×). The adhesive system was used after all desensitization treatments. The bond strength (n = 40) and the fracture pattern were evaluated. Confocal laser microscopy was used to quantify the hybrid layer formation in dentin. Results: G1 and G2 presented higher adhesive system bond strength (MPa) than G4 and G5, however no significant differences were observed in comparison with G3. Cohesive fracture was frequently found: G1 (58.5%), G2 (51.3%) and G3 (43.8%). G1 showed the highest incidence of precipitates and the highest number of blocked dentinal tubules. G1 and G2 presented similar hybrid layer formation and the highest hybrid layer formation values. Conclusions: Desensibilize Nano P (G1) favored the bond strength of the adhesive system to dentin, increased the precipitation of residues, obliteration of dentinal tubules, and hybrid layer formation in comparison with other agents. Clinical relevance: Desensitizers promote dentin obliteration, however, may affect dentin bonding. The optimal irrigating solution with antimicrobial and dentin cleansing properties for post space preparation for fiber posts is unclear. Peracetic acid is one option but is available in various chemical formulations that require evaluation. Purpose. The purpose of this in vitro study was to evaluate dentin surface cleanliness based on the presence of a smear layer and the number of open dentin tubules. It also investigates the chemical composition of residues after canal irrigation with a 1% peracetic acid solution (PA) at low or high concentration of hydrogen peroxide during the preparation of intracanal fiber posts. Material and methods. After filling the root canals of 40 mandibular incisors, a rotary instrument was used for intracanal preparation to place fiber posts. The teeth were divided into 4 groups (n=10) according to the post space irrigation protocol as follows: CG (control): distilled water; NA (NaOCl): 2.5% sodium hypochlorite; LH: PA with low concentration of hydrogen peroxide; and HH: PA with high concentrations of hydrogen peroxide. After irrigation, the teeth were sectioned, and the intracanal dentin surface was subjected to analysis using energy dispersive spectroscopy to evaluate chemical composition and to scanning electron microscopy (x500) to evaluate the presence of the smear layer. The number of open dentin tubules was measured by scanning electron microscopy analysis (x2000) using photo-editing software. ANOVA and the Tukey test (alpha=.05) were used to evaluate the data, except for the presence of a smear layer, for which the Kruskal-Wallis and Dunn tests were used (alpha=.05). Results. The highest concentrations of oxygen in the dentin residues were detected in LH and HH (P.05). El objetivo de esta revisión, fue describir de una manera sencilla las principales características de la Fibrina Rica en Plaquetas (FRP), su composición, propiedades y aplicación clínica. La FRP, biomaterial autógeno y concentrado plaquetario de segunda generación, es una matriz de fibrina que contiene leucocitos, plaquetas y factores de crecimiento, que son necesarios para los procesos de cicatrización, lo que brinda a este biomaterial, una gran utilidad en diversas áreas de la salud, incluyendo la odontología. Con esta revisión concluimos que la FRP es una alternativa real para mejorar la cicatrización de procedimientos quirúrgicos y potenciar otros biomateriales regenerativos en diversas áreas de la odontología, además de su accesibilidad y bajo costo. The aim of this review was to describe in a simple way the main characteristics of platelet rich Fibrin (PRF), its composition, properties and clinical application. PRF, biomaterial autologous and platelet concentrate of second generation, is a fibrin matrix containing leukocytes, platelets and growth factors, which are necessary for the healing process, giving to this biomaterial, very useful in various areas of health, including dentistry. With this review, we conclude that FRP is a real alternative to improve healing of surgical procedures and enhance other regenerative biomaterials in several areas of dentistry, in addition to accessibility and low cost.
