Individual responses were collected, analyzed and presented into a face-to-face meeting in order to reach a consensus. Cost categories considered were: drug acquisition and administration, photo/radiotherapy session and maintenance, clinical follow-up visits and laboratory tests. Costs were expressed in euros from 2018. The Spanish National Health System perspective was considered, taking into account direct health costs and time horizons of 1, 3 and 6 months.Results: Costs for the skin-directed treatments (SDT) assessed at 1, 3 and 6 months, were: Topical carmustine [€ 6,593.36, € 19,780.09 and € 27,592.78]; Phototherapy with psoralens and ultraviolet A light (PUVA) [€ 1,098.68, € 2,999.99 and € 3,187.60]; Narrow-band ultraviolet B phototherapy [€ 1,657.47, € 4,842.10 and € 4,842.10]; Total skin electron beam therapy (TSEBT) [€ 6,796.45, € 7,913.34 and € 7,913.34]. Cost for topical corticosteroids, being considered an adjuvant option, were € 17.16, € 51.49 and € 102.97. Costs for the assessed systemic treatments alone or in combination with SDT at 1, 3 and 6 months, were: Systemic retinoids [€ 2,026.03, € 5,206.63 and € 7,426.42]; Systemic retinoids + PUVA phototherapy [€ 3,066.50, € 8,271.26 and € 10,046.58]; Interferon alfa + PUVA phototherapy [€ 1,541.09, € 5,167.57 and € 6,404.55].Conclusion: According to the Spanish clinical practice, phototherapies in monotherapy were the treatments with the lowest associated costs regardless of the time horizon considered. TSEBT turned out as the treatment with the highest associated costs when considering 1 month. However, while considering 3 and 6 months the treatment with the highest associated costs was topical carmustine. Due to the anatomy and location of the nasopharynx, surgery is rarely used to treat primary NPC cancers. Treatment by radiotherapy, combined or not with chemotherapy, are efficient for primary tumors but often do not protect against fatal relapses or metastases.Methods: Search for new therapeutic molecules through high content screening lead to the identification of Ivermectin (IVM) as a promising drug. IVM is a US Food and Drug Administration-approved macrocyclic lactone widely used as anthelmintic and insecticidal agent that has also shown protective effects against cancers.Results: We show here that IVM has cytotoxic activity in vitro against NPC cells, in which it reduces MAPKs pathway activation through the inhibition PAK-1 activity. Moreover, all macrocyclic lactones tested and a PAK1 inhibitor are cytotoxic in vitro for EBV-positive and EBV-negative NPC tumor cells. We have also shown that IVM intraperitoneal repeated injections, at US Food and Drug Administration-approved doses, have no significant toxicity and decrease NPC subcutaneous tumors development in nude mice.Conclusion: Macrocyclic lactones appear as promising molecules against NPC targeting PAK-1 with no detectable adverse effect. Abstract Reindeer herding (RDH) is a livelihood strategy deeply connected to Sami cultural tradition. This article explores the implications of two theoretical and methodological approaches for grasping complex socioenvironmental relationships of RDH in Subarctic Sweden. Based on joint fieldwork, two teams – one that aligns itself with political ecology (PE) and the other with social-ecological systems (SES) – compared PE and SES approaches of understanding RDH. Our purpose was twofold: 1) to describe the situation of Sami RDH through the lenses of PE and SES, exploring how the two approaches interpret the same empirical data; 2) to present an analytical comparison of the ontological and epistemological assumptions of this work, also inferring different courses of action to instigate change for the sustainability of RDH. Key informants from four sameby in the Kiruna region expressed strong support for the continuation of RDH as a cultural and economic practice. Concerns about the current situation raised by Sami representatives centered on the cumulative negative impacts on RDH from mining, forestry and tourism. PE and SES researchers offered dissimilar interpretations of the key aspects of the RDH socio-economic situation, namely: the nature and scale of RDH systems; the ubiquitous role of conflict; and conceptualizations of responses to changing socioenvironmental conditions. Due to these disparities, PE and SES analyses have radically divergent sociopolitical implications for what ought to be done to redress the current RDH situation. Michael Polanyi es tal vez mejor conocido por su epistemología poscrítica centrada en los coeficientes personales y tácitos de todo conocimiento. Acuña el término poscrítico para describir un planteamiento filosófico que toma como punto de partida la naturaleza fiduciaria de todo conocimiento. Rechaza plenamente el planteamiento crítico, típico de la modernidad, que toma como su fundamento la duda metodológica. Para un pensador poscrítico la creencia metodológica es fundamental y la duda es complementaria y de segunda fila. A pesar de su justa fama por su epistemología, arguyo que su propósito central es metafísico. Siguiendo este hilo, mi proyecto investiga la metafísica de Polanyi más a fondo de lo que se ha hecho hasta la fecha. Propongo que su realismo metafísico debe ser interpretado como un realismo copernicano y lo pongo en conversación con tres realismos contemporáneos: el ontic structural realism de James Ladyman, la object-oriented ontology de Graham Harman, y el neo-aristotelismo/hilemorfismo contemporáneo de Anna Marmodoro, Robert Koons, William Simpson, etc. Desarrollo ciertos conceptos derivados de su epistemología más allá de lo que propuso explícitamente el mismo Polanyi, concluyendo que su planteamiento es platónico. Propongo, entonces, un platonismo poscrítico que afirma una metafísica de tres capas: 1. Una metafísica jerárquica diferenciada por la potencia eleática que ejerce la entidad (la producción de manifestaciones futuras no predecibles). 2. Una ontología plana de entidades comprensivas que están compuestas de particulares subsidiarios, unidos por/en el significado conjunto de la entidad. Dada la intercambiabilidad en la obra de Polanyi de entidad y significado, mantengo que su posición implica que ser es significar. 3. Una mereología jerárquica que describe las relaciones entre los particulares subsidiarios que forman parte de toda entidad comprensiva. Existir, entonces, es formar parte de la estructura descrita por el concepto de la entidad comprensiva y por lo tanto ser comprensible. Este estado se puede analizar más a fondo en términos de la mereología jerárquica. Existir así también es tener un impacto causal, cuya potencia determina el rango de la entidad en la jerarquía metafísica. Desarrollo el concepto de la entidad comprensiva, proponiedo que actúa como un arnés télico que engancha los particulares subsidiarios que se incorporan en su ser para realizar su telos. En su acción de enganche, la entidad comprensiva realiza el papel de una causa formal; en su realización del telos, es una causa final. Investigo el tema de los universales en la obra de Polanyi, concluyendo que son una especie de entidad comprensiva. Arguyo que esto, tomado junto con la afirmación platónica (hecha por Polanyi) de la existencia de universales no instanciados, delinea un planteamiento platónico poscrítico que rechaza teorías de Dos Mundos. Mi análisis incluye una comparación de las teorías del acceso a los universales de Polanyi, Platón, y Aristóteles. Siguiendo a Lloyd Gerson concluyo que el mejor modo de enfrentarnos con nuestra experiencia de sameness-in-difference es postular formas platónicas que son la base metafísica de las naturalezas de las entidades del mundo y a la vez la base de los universales aristotélicos, que son el mecanismo falible por el cual rastreamos las naturalezas basadas en las formas. Todo esto tiene lugar tras un reconocimiento anamnético (propuesto en el Fedón de Platón) que explico en términos de la epistemología de Polanyi. Este planteamiento con respeto a las formas y universales tiene cierta afinidad con la síntesis neo-platónica de Aristóteles con Platón de la antigüedad. Adicionalmente, propongo una subclase de formas que denomino las formas primordiales. Formas primordiales "Límite, por ejemplo" son las que se instanciarán necesariamente en cualquier mundo posible que incluye un estrato material.

28. Optic Tract as an Upper Limit in Amygdalectomy: Microsurgical Study

BACKGROUND: In surgeries involving resection of the amygdala, despite clear relations established with the medial, lateral, anterior, posterior, and inferior segments, the upper limit remains controversial. The optic tract (OT) has been anatomically considered as a good landmark immediately inferior to the striatopallidal region. This anatomic structure has barely been explored by microsurgical study, generating uncertainty about the exact relationship with the surrounding structures.; OBJECTIVE: To describe the OT in its entire length through microsurgical study, showing its superior, inferior, medial, and lateral relationships and highlighting its value as a landmark in superior amygdala resection.; METHODS: Microsurgical anatomic dissection of the OT, from its origin in the chiasm to the lateral geniculate nucleus was performed in 8 alcohol-fixed human hemispheres, showing its different segments and relations. Photographs were taken from different angles to facilitate surgical orientation.; RESULTS: We performed a dissection of the OT, showing its position relative to caudate and hippocampal formations. We exposed the structures related to the OT superiorly (striatopallidal region and superior caudate fasciculus), inferiorly (head of the hippocampus, amygdala, anterior choroidal artery, perforating artery branch of the anterior choroidal artery, terminal stria, and basal vein), medially (internal capsule and midbrain), and laterally (temporal stem [uncinate and inferior fronto-occipital fascicle], anterior perforated substance, and superior caudate fasciculus).; CONCLUSION: To date, there is a paucity of articles describing the anatomy of the OT from a neurosurgery perspective. In this study, we describe the microsurgical anatomic path of the OT, as a reliable upper limit landmark for amygdala resection. Copyright © Congress of Neurological Surgeons 2022. 32. Identification of Essential Protein Domains From High-density Transposon Insertion Sequencing

A first clue to gene function can be obtained by examining whether a gene is required for life in certain standard conditions, that is, whether a gene is essential. In bacteria, essential genes are usually identified by high-density transposon mutagenesis followed by sequencing of insertion sites (Tn-seq). These studies assign the term “essential” to whole genes rather than the protein domain sequences that confer the essential functions. However, genes can code for multiple protein domains that evolve their functions independently. Therefore, when essential genes code for more than one protein domain, only one of them could be essential. In this study, we defined this subset of genes as “essential domain-containing” (EDC) genes. Using a Tn-seq data set built-in Burkholderia cenocepacia K56-2, we developed an in silico pipeline to identify EDC genes and the essential protein domains they encode. We found forty candidate EDC genes and demonstrated growth defect phenotypes using CRISPR interference (CRISPRi). This analysis included two knockdowns of genes encoding the protein domains of unknown function DUF2213 and DUF4148. These essential domains are conserved in more than two hundred bacterial species, including human and plant pathogens. Together, our study suggests that essentiality should be assigned to individual protein domains rather than genes, contributing to a first functional characterization of protein domains of unknown function.

Purpose The complex relations of the paraclinoid area make the surgical management of the pathology of this region a challenge. The anterior clinoid process (ACP) is an anatomical landmark that hinders the visualization and manipulation of the surrounding neurovascular structures, hence in certain surgical interventions might be necessary to remove it. We reviewed the anatomical relationships that involve the paraclinoid area and detailed the step-by-step techniques of intra and extradural clinoidectomy in cadaveric specimens. Materials and methods A literature review was done describing the most relevant anatomic relationships regarding the anterior clinoid process. Extradural and intradural clinoidectomy techniques were performed in six dry bone heads and in ten previously injected cadaverous specimens with colored latex (Sanan et al. in Neurosurgery 45:1267-1274, 1999) and each step of the procedure was recorded using photographic material. Finally, an analysis of the anatomical exposure achieved in each of the techniques used was performed. Results The main advantage of the intradural clinoidectomy technique is the direct visualization of the neurovascular structures adjacent to the ACP when drilling, at the same time, opening the Sylvian fissure will allow the direct visualization of the ACP variants. The main advantage offered by the extradural technique is that the dura protects adjacent eloquent structures while drilling. Among the disadvantages, it is noted that the same dura that would protect the underlying structures also prevents the direct visualization of these neurovascular structures adjacent to the ACP. Conclusion We reviewed the anatomy of the paraclinoid area and made a step-by-step description of the technique of the anterior clinoidectomy in its intra- and extradural variants in cadaveric preparations for a better understanding.

BACKGROUND: Simulation allows surgical trainees to acquire surgical skills in a safe environment. With the aim of reducing the use of animal experimentation, different alternative nonliving models have been pursued. However, one of the main disadvantages of these nonliving models has been the absence of arterial flow, pulsation, and the ability to integrate both during a procedure on a blood vessel. In the present report, we have introduced a microvascular surgery simulation training model that uses a fiscally responsible and replicable pulsatile flow system. METHODS: We connected 30 human placentas to a pulsatile flow system and used them to simulate aneurysm clipping and vascular anastomosis. RESULTS: The presence of the pulsatile flow system allowed for the simulation of a hydrodynamic mechanism similar to that found in real life. In the aneurysm simulation, the arterial flow could be evaluated before and after clipping the aneurysm using a Doppler ultrasound system. When practicing anastomosis, the use of the pulsatile flow system allowed us to assess the vascular flow through the anastomosis, with verification using the Doppler u ltrasound system. Leaks were manifested as "blood" pulsatile ejections and were more frequent at the begin-n ing of the surgical practice, showing a learning curve. CONCLUSIONS: We have provided a step-by-step guide for the assembly of a replicable and inexpensive pulsatile flow system and its use in placentas for the simulation of, and training in, performing different types of anastomoses and intracranial aneurysms surgery.
