Abstract

The aim of the study was to compare the pharmacokinetics (PK) and pharmacodynamics (PD) of T‐glu (GP40321, test drug), and reference insulin glulisine in a hyperinsulinemic‐euglycemic clamp procedure. During this study, 34 healthy male volunteers underwent the hyperinsulinemic‐euglycemic clamp procedure following subcutaneous 0.3 U/kg injection of T‐glu or reference insulin glulisine in a randomized, double‐blind, crossover study. Plasma glucose levels were monitored every 5 minutes for 8 hours. Glucose infusion rate adjustment was based on the blood glucose measurements. Evaluation of PD was performed using the glucose infusion rate values, while PK was calculated using insulin concentrations measured via enzyme‐linked immunosorbent assay. The study results showed that the 90% CI for the geometric mean ratios of primary PK and PD of T‐glu and reference insulin glulisine were within 80%‐125% comparability limits, and that the safety profiles were comparable. Abstract

Petrographic characterization of stone and the forms of destruction in the " St. Astvatsatsin" Church of "Tatev" Monastic Complex have been mapped and studied. Mineralogical studies included a microscopic investigation of stone sections, united by external similarity into four groups. Each group's structural and textural characteristics, as well as its mineralogical composition, were investigated. The generalization of chemical-mineralogical and technological results made it possible to identify the rock materials used in the construction of the "St. Astvatsatsin." church. The work has been carried out using the visual inspection technique developed in the laboratory of the construction company "Horizon-95" LLC and the system for describing and assessing damage to stones by the International Scientific Committee for Stone (ICOMOS). The survey results were used in the "St. Astvatsatsin" church restoration. The work permits quantifying the forthcoming material costs associated with the rock material renewal. Individual stones were identified and marked for renewal or replacement, depending on the degree of damage. [ABSTRACT FROM AUTHOR]

Abstract

The article analyzes the backbone concepts of the spatial theory of sustainable development of modern countries – information space. The aim of the study is to substantiate the author's interpretation of the conceptual apparatus of spatial analysis, taking into account the principles of methodological institutionalism (comparative studies). In accordance with the purpose of the research, the following tasks are set and solved in the article: the essence of the information space in the system of sustainable development as a set of interactions of economic agents affecting the environment is disclosed; the attributive properties of the information space are formulated; the directions of its transformation under the influence of sustainable development processes are determined; the hypothesis about the transformation of the information space into information and communication under the influence of digitalization processes is formulated. It is concluded that the enrichment of the conceptual apparatus of the theory of spatial economics is a necessary prerequisite for the development of tools for implementing the sustainable development strategy in the digital and spatial dimensions.

Abstract

The article substantiates provisions aimed at improving the tools for implementing the strategy of sustainable spatial development of the Russian Federation, taking into account the processes of systemic digital transformation of the economy. The aim of the study is to substantiate a new methodological approach to the interpretation of the patterns of sustainable regional development, which considers the spatial and environmental factors as endogenous sources of sustainable growth. To achieve this goal, the stages of development of ideas about the regional economy and regional development were determined, the content of the spatial approach to the interpretation of economic phenomena and processes was clarified, recommendations were formulated aimed at improving the toolkit of regional policy, taking into account the structuredness and multipolarity of the Russian economic, environmental and information space. The result of the study is the conclusion about the heterogeneity of the space of Russian regions and the need to take into account environmental space quality indicators when developing program measures for sustainable development, as well as in assessing their effectiveness.

Abstract

Neuroosteoarthropathy is a rare, but devastating complication affecting patients with peripheral neuropathy. It is a progressive, destructive condition that is characterised by progressive bone and joint deterioration of the foot leading to permanent incapacity. Nowadays, diabetes mellitus is by far the most common etiology of CNO (Charcot neuro-osteoarthropathy), especially if it affects the foot or ankle. The cause of the condition is still controversial among experts. In particular, new data have emerged on the central role of RANK/RANKL/OPG in the pathogenesis of this complication, also known as Charcot foot. No pharmacological methods of therapy have proven effective to date. Very little progress has been made in treating this condition since the days of Jean-Martin Charcot. Today, the universally accepted treatment involves completely relieving pressure from the limb using a polymer retaining bandage. The aim of this review is to analyze research into the pathogenesis of CNO and our potential to better understand the nature of this condition. Particular attention is paid to modern concept of neuroosteopathology, according to which neuropathy is crucial to developing acute osseous pathology. Revealing new aspects of CNO pathogenesis can help extend therapeutic resources for treating this patient group.

Abstract

Cellular glass by its composition, structure and technical parameters is a high-performance multifunctional material, and its use in construction and engineering is extremely important. The problem of developing cellular glass power consumption technology on the base of natural amorphous aluminosilicates and silica rocks in a single technological process, combining the synthesis of a given glass composition and the formation of its cellular structure is solved by introduction of a nanodispersed modifier into the rock and by the mechanical activation of the charge, creating an impact possibility in the system at the atomic-molecular level. With the involvement of a complex modern theoretical and experimental methods, the processes of glass wool rock modifyed sintering and expanding were investigated, which served as a base for creating the cellular glass technology in the form of products and granules. The study of the secondary glass synthesis dynamics during sintering of volcanic glasses with sodium hydroxide solution showed that the hydrated amorphous neoplasms of sodium silicates synthesized at low temperatures and silica aluminosilicates with temperature rise gradually enriched with silica and formed hydrated secondary glasses, the dehydration of which expanded the mass upon reaching the pyroplastic state. Power consumption technologies of the new type cellular glasses based on the volcanic glass-wool and other amorphous silicate rocks, which allow to combine the processes of synthesis of low-melting glass and its expansion by sintering are challenging for the production and for application in construction and engineering.

Abstract

Aim: For people with suboptimally controlled type 2 diabetes (T2D) on basal insulin (BI), guidelines recommend several treatment advancement options. This study compared the clinical effectiveness of once‐daily iGlarLixi versus a multiple‐injection BI + rapid acting insulin (RAI) regimen in adults with T2D advancing from BI therapy in real‐world clinical practice. Materials and methods: Electronic medical records from the Observational Medical Outcomes Partnership (OMOP) database were analysed retrospectively using propensity score matching to compare therapy advancement with iGlarLixi or BI + RAI in US adults ≥18 years with T2D on BI who had ≥1 valid glycated haemoglobin (HbA1c) value at baseline and at the 6‐month follow‐up. The primary objective was non‐inferiority of iGlarLixi to BI + RAI in HbA1c change from baseline to 6 months (margin 0.3%). Results: Propensity score matching generated cohorts with balanced baseline characteristics (N = 814 in each group). HbA1c reduction from baseline to 6 months with iGlarLixi was non‐inferior to BI + RAI [mean difference (95% confidence interval): 0.1 (−0.1, 0.2)%; one‐sided p =.0032]. At 6 months, weight gain was significantly lower with iGlarLixi than with BI + RAI [−0.8 (−1.3, −0.2) kg; two‐sided p =.0069]. Achievement of HbA1c <7% without hypoglycaemia and weight gain were similar between groups [odds ratio (95% confidence interval): 1.15 (0.81, 1.63); p =.4280]. Hypoglycaemia was low in both groups, probably because of underreporting. Conclusions: In real‐world clinical practice, glycaemic outcomes 6 months after treatment advancement from BI are similar for people with T2D using iGlarLixi versus BI + RAI, with iGlarLixi leading to less weight gain.
