Background:Magnetic resonance imaging (MRI) has been used as a diagnostic and prognostic instrument to evaluate the results of conservative treatment for plantar fasciitis. However, there are scarce data available relative to changes in the plantar fascia after operative treatment. The primary objective of this study is to evaluate the imaging changes in patients with recalcitrant plantar fasciitis treated operatively by means of proximal medial gastrocnemius release.Methods:Thirteen patients with recalcitrant plantar fasciitis were studied with MRI preoperatively and 1 year after operative treatment. Quantitative (plantar fascia thickness) and qualitative variables (hyperintensity in the plantar fascia, insertional calcaneus bone edema, a plantar fascia tear, and the presence of perifascial collections) were assessed by 2 musculoskeletal radiologists. Clinical results were also measured with American Orthopaedic Ankle & Society (AOFAS), visual analog scale (VAS) pain, and 36-Item Short Form Health Survey (SF-36) scales.Results:The mean plantar fascia thickness was 6.59 mm preoperatively and 6.37 mm postoperatively ( P = .972). No statistically significant differences were found in any of the qualitative variables on comparing the pre- and postoperative periods. Patients reported clinical improvements in pain VAS, AOFAS measurement, and the physical subdomains of the SF-36 scale.Conclusion:Quantitative and qualitative variables assessed for the plantar fascia on MRI did not show any significant change after medial gastrocnemius release despite clear clinical improvement.Level of Evidence:Level II, perspective cohort study.

La fascitis plantar és una patologia molt freqüent en la població. El diagnòstic és fonamentalment clínic. Es defineix com patològic un gruix de la fàscia plantar per sobre de 4 mm (mesurat mitjançant ecografia o ressonància magnètica nuclear). En diferents publicacions s’ha relacionat aquest paràmetre amb l’estat clínic del pacient, i s’ha utilitzat per avaluar la resposta al tractament, relacionant la bona resposta clínica del pacient amb la disminució del gruix de la fàscia plantar. És una entitat que es caracteritza per la seva benignitat resolent-se la simptomatologia en el 90% dels casos dins del primer any. Un 10% dels pacients desenvolupen una fascitis plantar recalcitrant (FPR) que és la cronificació d’aquest procés i es caracteritza per l’absència de resposta als tractaments conservadors habituals, fent-se necessària la indicació de cirurgia per resoldre aquests casos. Actualment no hi ha dades suficients per poder recomanar un determinat tipus de tractament quirúrgic. L’objectiu de la primera feina ha estat valorar si hi ha una correlació entre la presentació clínica dels pacients que pateixen de fascitis plantar recalcitrant i el seu gruix mesurat per ecografia i ressonància magnètica nuclear. S’ha realitzat un estudi descriptiu transversal valorant el dolor, el nivell funcional i la percepció de salut en pacients amb fascitis plantar recalcitrant i s’ha relacionat amb el gruix de la fàscia plantar. Els resultats descriuen una absència de correlació entre la intensitat de la clínica i el gruix de la fàscia plantar, restant importància a aquest paràmetre en la valoració de pacients amb FPR. El segon treball va consistir en valorar quin tractament quirúrgic per a la fascitis plantar recalcitrant era més eficaç. Es va comparar la clàssica fasciotomía parcial plantar, tractament més freqüentment utilitzat, amb l’alliberament del bessó medial del múscul gastrocnemi. Els resultats van concloure que aquesta segona tècnica més novell pot arribar a un grau de satisfacció dels pacients important, amb resultats similars a la fasciotomía parcial, evitant les potencials i temudes complicacions d’aquesta. Los resultados describen una ausencia de correlación entre la intensidad de la clínica y el espesor de la fascia plantar, restando importancia a este parámetro en la valoración de pacientes con FPR. El segundo trabajo consistió en valorar qué tratamiento quirúrgico para la fascitis plantar recalcitrante era más eficaz. Se comparó la clásica fasciotomía parcial plantar, tratamiento más frecuentemente utilizado, y la liberación del gemelo medial del músculo gastrocnemio. Los resultados concluyeron que esta segunda técnica más novel puede alcanzar un grado de satisfacción de los pacientes importante, con resultados similares a la fasciotomía parcial, evitando las potenciales y temidas complicaciones de esta. Plantar fasciitis (PF) is a common disease among people. The diagnosis is basically clinical. A thickness of the plantar fascia as measured by means of Ultrasound (US) or Magnetic resonance imaging (MRI) that is greater than 4mm is defined as pathological. In recent publications, plantar fascia thickness (PFT) has been related to the clinical status of the patient. It has also been used to evaluate the responsiveness to the treatment. A reduction in PFT has been associated with a favourable evolution of the disease. PF is usually a benign entity that sees a resolution of the symptoms within the first year in 90% of the cases. The other 10% develop recalcitrant plantar fasciitis (RPF). It represents the transition to the chronic state of the PF and is characterized by its lack of responsiveness to conservative treatments, thereby making a surgical solution necessary. At present, there is not enough data to support recommending one surgical option over another. The aim of the first work was to evaluate whether there is any correlation between clinical presentation of patients with RPF and PFT measured with US or MRI. A transversal descriptive study was carried out to look for any correlation between pain, functionality, and self-rated health perception with PFT. The results evidence a lack of a correlation between the clinical status of the patients and PFT. This finding reduces the importance of PFT as an evaluation tool in RPF. The second work aimed to discriminate which surgical treatment would be more effective for RPF. The classic open plantar partial fasciotomy (OPF) was compared to proximal medial gastrocnemius release (PMGR). A high rate of satisfaction has been seen in patients with the latter procedure. In general, the results of PMGR are quite similar to those of OPF with the added benefit of avoiding the mechanical complications related to the fasciotomy. Universitat Autònoma de Barcelona.
