20. Microplastics in the Mediterranean: Variability From Observations and Model Analysis

Abstract

In this study, the abundance and properties (size, shape, and polymer type) of microplastics (MPs) in sea surface water samples, collected during two sampling campaigns over 2018–2019, in four coastal areas of the Mediterranean Sea (Saronikos Gulf, LIgurian Sea, Gulf of Lion, and Gabes Gulf) were investigated. Coupled hydrodynamic/particle drift model simulations with basin-scale Mediterranean and high resolution nested models were used to provide a better understanding on the variability of the abundance/size of MPs, originating from wastewater and river runoff, in the four areas. Different size classes of MPs were considered in the model, taking into account biofouling induced sinking, as a possible mechanism of MPs removal from the surface. The Gabes Gulf showed the highest mean MPs abundance (0.073–0.310 items/m2), followed by Ligurian Sea (0.061–0.134 items/m2), Saronikos Gulf (0.047–0.080 items/m2), and Gulf of Lion (0.029–0.032 items/m2). Overall, the observed MPs abundance and size distribution was reasonably well reproduced by the model in the four different areas, except an overestimation of small size contribution in Saronikos Gulf. The basin-scale simulation revealed a strong decrease of smaller size MPs in offshore areas, due to biofouling induced sinking, with larger (floating) MPs being able to travel longer distances in the open sea. A significant impact of waves drift and advection of MPs from non-local sources was identified from model simulations, particularly in the Gulfs of Lion and Gabes, having a stronger effect on larger microplastics. Abstract

Background Laparoscopic cholecystectomy is considered the gold standard for the treatment of gallbladder lithiasis; nevertheless, the incidence of bile duct injuries (BDI) is still high (0.3–0.8%) compared to open cholecystectomy (0.2%). In 1995, Strasberg introduced the "Critical View of Safety" (CVS) to reduce the risk of BDI. Despite its widespread use, the scientific evidence supporting this technique to prevent BDI is controversial. Methods Between March 2017 and March 2019, the data of patients submitted to laparoscopic cholecystectomy in 30 Italian surgical departments were collected on a national database. A survey was submitted to all members of Italian Digestive Pathology Society to obtain data on the preoperative workup, the surgical and postoperative management of patients and to judge, at the end of the procedure, if the isolation of the elements was performed according to the CVS. In the case of a declared critical view, iconographic documentation was obtained, finally reviewed by an external auditor. Results Data from 604 patients were analysed. The study population was divided into two groups according to the evidence (Group A; n = 11) or absence (Group B; N = 593) of BDI and perioperative bleeding. The non-use of CVS was found in 54.6% of procedures in the Group A, and 25.8% in the Group B, and evaluating the operator-related variables the execution of CVS was associated with a significantly lower incidence of BDI and intraoperative bleeding. Conclusions The CVS confirmed to be the safest technique to recognize the elements of the Calot triangle and, if correctly performed, it significantly impacted on preventing intraoperative complications. Additional educational programs on the correct application of CVS in clinical practice would be desirable to avoid extreme conditions that may require additional procedures.

Abstract

Introduction Considering the multitude of bariatric procedures performed all over the world, the necessity of revisional surgery increased accordingly. Several authors argued that with the great diffusion of sleeve gastrectomy (SG), the number of patients who experienced a weight regain at long follow-up was congruous and physiologic, even if not negligible. Recent studies showed that one anastomosis gastric bypass (OAGB) was an effective and safe option also as redo surgery. The aim of the study was to evaluate effectiveness of OAGB as redo surgery after SG in term of anthropometric features and remission of comorbidities. Methods Bariatric patients experiencing weight regain and insufficient weight loss after SG and undergoing OAGB as redo surgery were retrospectively analyzed. During post-OAGB outpatient visit weight, %EWL, BMI, comorbidities, and vitamin deficiencies were evaluated. A further visit was scheduled for the evaluation of postoperative esophagitis/gastritis at upper endoscopy. Results Fifty-nine (Reviewer #3-1) patients underwent OAGB as redo-surgery consequently to the worsening of the bariatric outcomes at 29.42 +/- 7.29 months from SG (mean weight, BMI and %EWL were 120.89 +/- 16.79 kg, 43 +/- 4.39 and 19.84 +/- 30.29, respectively). Conversely, at a mean follow-up of 34.32 +/- 1.71 months (Reviewer #3-3) after OAGB, no weight regain or insufficient weight loss cases were recorded (mean weight 71.25 +/- 10.22 kg, mean BMI 24.46 +/- 2.06 kg/m2, mean %EWL 69.49 +/- 14.4, p < 0.0001) (Reviewer #3-2). Conclusion OAGB is a safe and effective bariatric procedure in terms of morbidity, mortality, and %EWL also as revision surgery after SG. Further larger studies are needed to address this issue.

31. Does antrum size matter in sleeve gastrectomy? A prospective randomized Study

Abstract

BACKGROUND: Laparoscopic sleeve gastrectomy (LSG) is a safe and effective bariatric procedure in terms of excess weight loss. Nevertheless, controversies still exist on several technical and operative aspects of LSG. The aim of this study is to evaluate variations in anthropometric features in subjects with a LSG gastric resection starting from 2cm or 6cm from the pylorus. Secondary aim was the evaluation of differences in morbidity, food tolerance, and GERD incidence studied with upper endoscopy (UE) and GERD Health-Related Quality-of-Life score.; METHODS: Patients were prospectively randomized into 2 groups: Group A (at 2cm proximally to the pylorus) and Group B (at 6cm proximally to the pylorus). All patients were followed-up at 3, 6, 12, and 24months. UE was performed in all patients at 12 or 24months.; RESULTS: One hundred and fifty met the inclusion criteria and were enrolled in the study. The anthropometric features (BMI, %EWL, %TWL) resulted statistically different in the first 12months (24.2±3.4 vs 27.5±4.3, 63.7±14.1 vs 59.6±12.5 and 42.9±7.4 vs 38.2±6.2), in favor of group A. At 24months the differences disappeared (25.2±4.4 vs 26.2±3.3, 62.8±13.1 vs 61.6±10.5 and 41.9±7.4 vs 40.2±6.2). An increased GERD was found in both groups postoperatively with a higher incidence in group A at 6-month follow-up.; CONCLUSION: Performing the LSG with a radical antrectomy could improve weight loss at 12-month follow-up but expose to lower food tolerance and higher transitory GERD. The differences seem to be reduced to a 24-month.

Background: For countries with strong border control and an epidemic that is not yet advanced, there is an opportunity to eliminate SARS-CoV-2 that is causing the COVID-19 pandemic. We show how public health policies and their effects can be modelled to estimate the probability of elimination of SARS-COV-2 under i) strict physical distancing policies implemented in New Zealand and Australia on the 26th and 28th March, respectively, continuing until such time that elimination is achieved; and ii) the same policy, but with physical distancing decaying over 60 days to 26th and 28th of May. Methods: We developed an agent-based SEIR model that simulated key aspects of both country’s populations, disease, spatial and behavioural dynamics, as well as the mechanism and effect of public health policy responses on the transmission of SARS-CoV-2. Findings: Under maintained strong physical distancing, we estimated a median elimination date of July 12th for Australia (95% SI: June 5th to August 28th) and June 3rd for New Zealand (95% SI May 4th to June 27th). A 90% probability of elimination is achievable in Australia on August 17th (95% SI: August 8th – August 30th) and on June 14th (95% SI: June 17th – June 29th) in New Zealand. However, under our scenario of decaying adherence to physical distancing from implementation to 60 days post restrictions, a rebound in SARS-CoV-2 infections (i.e., a second wave) was likely in Australia and possibly in New Zealand. At 100 days, the probability of elimination was reduced to 10% in Australia and 68% in New Zealand. Interpretation: The findings suggest that with the effective implementation and maintenance of public health policies limiting physical interaction, it was possible to estimate a pattern the elimination of SARS-CoV-2 transmission in both countries. It seems highly likely NZ will have successfully eliminated as of 8 June, but not in Australia; the latter is consistent with our lower probabilities that modelled less stringent restrictions applied in Australia.

Abstract

We show how public health policies and their effects can be modelled to estimate the likely progression of SARS-COV-2 under i) strict physical distancing policies implemented in New Zealand and Australia on the 26 th and 28 th March, 2020 respectively, and ii) from September 1st, 2020 in Victoria, Australia as this state planned its exit from a significant second wave of infections. Methods: We developed a dynamic, public health policy model using an agent-based framework that simulated key aspects of both country’s populations, disease, spatial and behavioural dynamics, as well as the mechanism and effect of public health policy responses on the transmission of SARS-CoV-2. We demonstrate the model’s development and application through the first wave of infections in New Zealand, and first and second wave of infections experienced in Australia. Findings: Under maintained strong physical distancing, we originally estimated a median elimination date of July 12th for Australia (95% SI: June 5th to August 28th) and June 3rd for New Zealand (95% SI May 4th to June 27th). We estimated a 90% probability of elimination was achievable in Australia on August 17th (95% SI: August 8th – August 30th) and on June 14th (95% SI: June 17th – June 29th) in New Zealand. However, under our scenario of decaying adherence to physical distancing from implementation to 60 days post restrictions, we predicted a resurgence in SARS-CoV-2 infections (i.e., a second wave) was likely in Australia and possibly in New Zealand. At 100 days, the probability of elimination was reduced to 10% in Australia and 68% in New Zealand. Having demonstrated the utility of our model above, the subsequent adaptation of the model in Victoria, Australia estimated that a threshold of less than 5 new cases per day over 14 days (from a peak of ~500 in August) could be achieved on or around the 17th of October. This would enable Victoria to move to, and sustain, lower levels of restrictions to movement and trade than had been possible under its strict lockdown policy. Interpretation: The findings suggest that during Australia and NZ’s first wave of infections, elimination of community transmission of SARS-CoV-2 in both countries through effective implementation and maintenance of public health interventions was possible. Subsequently, the structure and function of the agent-based policy model developed to analyse the effect of policies during wave 1 has been instrumental in supporting decision-making among public health authorities during Australia’s current second wave of infections. Abstract

A study was conducted to determine zinc (Zn) and cadmium (Cd) accumulation and localization in Idotea baltica hepatopancreas cells. Atomic adsorption spectrophotometry and x-ray microprobe analysis were performed. Zn and Cd were observed to have similar toxic effects but exhibited different regulation and detoxification mechanisms. It was hypothesized that the cytotoxic effects of the two heavy metals do not differ significantly.
