Abstract

La norma ISO 15189:2012 exige una serie de requisitos en cuanto a la gestión de muestras clínicas, el aseguramiento de la calidad del proceso y la gestión de la información del laboratorio. Además, la entidad acreditadora, ENAC en España, tiene establecidas las condiciones para el uso de la marca en el informe de resultados del laboratorio acreditado. Las recomendaciones planteadas aplican a las actividades incluidas en el proceso posanalítico del laboratorio, así como al personal implicado. Se indican los criterios para que el laboratorio defina y documente el tiempo y las condiciones de retención de las muestras, para diseñar un control de calidad interno que verifique si las actividades posanalíticas alcanzan la calidad esperada, los requisitos que debe cumplir la gestión de la información y la necesidad de disponer de un plan de contingencia que asegure la comunicación de los resultados en todo momento. Asimismo, se describe el uso correcto de la marca de acreditación en los informes. Diversos gobiernos y sociedades científicas abogan por la obligatoriedad de la acreditación de los laboratorios clínicos. Siendo la norma ISO 15189 la más específica para demostrar su competencia técnica, es indispensable el conocimiento y la comprensión de sus requisitos para su correcta implantación.

Abstract

ISO 15189:2012 establishes the requirements for clinical sample management, ensuring quality of process and laboratory information management. The accreditation authority, ENAC in Spain, established the requirements for the authorized use of the label in reports issued by accredited laboratories. These recommendations are applicable to the postanalytical processes and the professionals involved. The Standard requires laboratories to define and document the duration and conditions of sample retention. Laboratories are also required to design an internal quality control scheme to verify whether postanalytical activities attain the expected standards. Information management requirements are also established and laboratories are required to design a contingency plan to ensure the communication of laboratory results. Instructions are finally provided about the correct use of the accreditation label in laboratory reports. A range of nations and scientific societies support that clinical laboratories should be required to obtain accreditation. With ISO 15189 being the most specific standard for demonstrating technical performance, a clear understanding of its requirements is essential for proper implementation.

Abstract

The objective of this paper is to share some considerations about the management of postanalytical processes in relation to the review, reporting and release of test results in accordance with UNE-EN ISO 15189:2013 Standard requirements. The scope of this paper includes postanalytical activities and the personnel involved (laboratory management and staff). We describe the criteria and information required to review and validate analytical results and ensure that clear reports are sent to requesters. These criteria also guarantee that results are transcribed in a reliable way and that all necessary information is provided for the correct interpretation of results. Likewise, the requirements for the correct release of laboratory results are described, with special emphasis on the release of alarming or critical results. In some European countries, clinical laboratories are required to hold partial or full ISO 15189 accreditation, which is a global trend. Therefore, understanding ISO 15189 requirements is imperative for a progressive and more effective implementation of the Standard.

Abstract

El objeto de este trabajo es establecer unas consideraciones para facilitar la gestión del proceso posanalítico respecto a la revisión, notificación y comunicación de los resultados, de acuerdo con los requisitos de la Norma UNE-EN ISO 15189:2013. El ámbito de aplicación incluye las actividades del proceso posanalítico del laboratorio clínico, así como el personal implicado en él (dirección y personal del laboratorio). Se indican los criterios y la información necesaria para realizar la revisión y validación de los resultados de las pruebas analíticas y así enviar a los destinatarios informes claros, asegurando siempre una transcripción fidedigna de los resultados e incluyendo toda la información necesaria para su correcta interpretación. Asimismo, se describen los requisitos para una correcta comunicación de los resultados del laboratorio, haciendo especial hincapié en la comunicación de aquellos resultados alarmantes o críticos. En algunos países de Europa es obligatoria la acreditación, total o parcial, de los laboratorios clínicos, siguiendo la Norma ISO 15189 y esta parece ser la hoja de ruta marcada en otros muchos países. Por ello, es indispensable la comprensión de sus requisitos para realizar una implementación progresiva y más fácil.

Abstract

Introduction: Reticulocyte count and novel derived parameters provide insight into the effectiveness of erythropoiesis and may be useful tools in the classification and diagnosis of anemias. However, there is no standardisation, so we consider it necessary that each laboratory evaluates the parameters according to its own methodology and instrumentation and establishes its own reference ranges. Our aim was to establish the reference intervals (RIs) of reticulocyte profile provided by the Beckman Coulter DxH 900 haematological autoanalyzer in our reference population. Methods: One hundred and seventy‐five healthy adults (18 to 62 years) were included. Subjects were collected from the blood donation centre of the Hospital Clínico San Carlos (Madrid, Spain) upon informed consent. Whole blood was collected and assayed for 14 haematological parameters on the Beckman Coulter DxH 900 analyzer in the haematology laboratory of the Clinical Analysis Department. RIs were established as per Clinical and Laboratory Standards Institute EP28‐A3c guidelines using three different statistical approaches. Results: RIs estimated using the non‐parametric method and the Harrell‐Davis bootstrap method were very similar. RIs estimated by the robust method were narrower. Gender partitioning was required for two haematological parameters (low haemoglobin density (LHD) and microcytic anaemia factor (MAF)). The rest of the parameters did not need to be partitioned according to Lahti's method. Conclusion: RIs have been established for 14 hematologic parameters of the reticulocyte profile for the Beckman Coulter DxH 900 haematology analyzer using a healthy cohort of adult subjects. 18. Learning curve of robotic mitral repair: prospective two-centre study of proficiency and clinical outcomes.

Abstract

OBJECTIVES Robotic-assisted mitral valve repair surgery has a steep learning curve, and it is not clear whether previous experience in minimally invasive mitral valve surgery (MIMVS) facilitates this process. We aimed to investigate the initial experience of 2 cardiac centres starting their robotic programmes, evaluating the impact of previous MIMVS experience. METHODS Retrospective analysis was performed for the 1st consecutive cases operated due to severe degenerative mitral valve regurgitation using the robotic surgical platform in 2 European centres, 1 transitioning from conventional surgery (centre 1) and the other from mini thoracotomy MIMVS (centre 2). Cumulative sum analysis was used to evaluate the learning process using both surgical times and a combined primary outcome including relevant intra- and postoperative results. RESULTS First 62 patients in each centre were included. All median surgical times were shorter in centre 2: cardiopulmonary bypass: 238 vs 115 min, P  < 0.001; cross-clamp: 143 vs 82 min, P  < 0.001; and total intervention: 313 vs 228 min, P  < 0.001. The combined primary outcome showed no significant differences (9.7% vs 8%; P  = 1). However, the turning point making the end of the learning phase was detected at the 60th case in centre 1 and at the 50th in centre 2. Regarding surgical time, the learning curve was steeper in centre 1 with both cardiopulmonary bypass and cross-clamp overcoming the learning phase after 32 cases, as compared to 16 cases in centre 2. CONCLUSIONS A successful robotic-assisted mitral repair programme can be safely started coming from either conventional open approach or mini thoracotomy MIMVS. Abstract

Multinuclear aluminum complexes are useful catalysts for the ring opening polymerization (ROP) of ϵ‐caprolactone (ϵ‐CL), often outperforming monometallic analogues in the production of biodegradable polycaprolactone (PCL). These complexes require control over metal‐metal distance, ligand structure, and substituents. Herein, we report on the preparation of mono‐, bi‐, and octametallic aluminum complexes through direct chelation of multidentate bis(phosphino)‐2,2‐dimethyl‐1,3‐diamino ligands Me2C[CH2NHR2P(O)]2; R=Ph(1), iPr(2), tBu(3). Ligands 1–3 yielded bimetallic complexes with two types of coordination pockets, as well as bimetallic systems with identical coordination environments. In addition to the diversity of structural patterns within the present series, the catalytic performances of [AlR2{κ2‐N,N',–(Me2C{CH2NPh2P(O)}2}‐(κ2‐O,O'‐AlR2)] (R=Me(4), Et(5), iBu(6)) outperformed that of the monometallic [AlEt2{κ2‐O,O'‐(Me2C(CH2NtBu2P(O))2H} (10), showing controlled and immortal ROP of ϵ‐CL in the presence of BnOH as co‐initiator. DFT calculations suggest that the primary active site for 4–6 is the N−Al–N center. Gaining insight into the underlying principles that lead to high activity and controlled production of PCL in bimetallic systems can help design tailor‐made complexes with enhanced performance. Abstract

Atherosclerosis is a dynamic process. There is little evidence regarding whether quantification of atherosclerosis extent and progression, particularly in the carotid artery, in asymptomatic individuals predicts all-cause mortality. This study sought to evaluate the independent predictive value (beyond cardiovascular risk factors) of subclinical atherosclerosis burden and progression and all-cause mortality. A population of 5,716 asymptomatic U.S. adults (mean age 68.9 years, 56.7% female) enrolled between 2008 and 2009 in the BioImage (A Clinical Study of Burden of Atherosclerotic Disease in an At Risk Population) study underwent examination by vascular ultrasound to quantify carotid plaque burden (cPB) (the sum of right and left carotid plaque areas) and by computed tomography for coronary artery calcium (CAC). Follow-up carotid vascular ultrasound was performed on 732 participants a median of 8.9 years after the baseline exam. All participants were followed up for all-cause mortality, the primary outcome. Trend HRs are the per-tertile increase in each variable. Over a median 12.4 years' follow-up, 901 (16%) participants died. After adjustment for cardiovascular risk factors and background medication, baseline cPB and CAC score were both significantly associated with all-cause mortality (fully adjusted trend HR: 1.23; 95% CI: 1.16-1.32; and HR: 1.15; 95% CI: 1.08-1.23), respectively (both P < 0.001), thus providing additional prognostic value. cPB performed better than CAC score. In participants with a second vascular ultrasound evaluation, median cPB progressed from 29.2 to 91.3 mm3. cPB progression was significantly associated with all-cause mortality after adjusting for cardiovascular risk factors and baseline cPB (HR: 1.03; 95% CI: 1.01-1.04 per absolute 10-mm3 change; P = 0.01). Subclinical atherosclerosis burden (cPB and CAC) in asymptomatic individuals was independently associated with all-cause mortality.
