Los tumores de células germinales extragondales (TCGE) son poco frecuentes, sobre todo en mujeres. Aunque no existe evidencia de asociación entre este tipo de tumor y la infección por VIH, pueden coincidir en el mismo paciente debido a que, en ambos, la máxima incidencia se da en el mismo segmento de edad. Presentamos el caso de una mujer de 27 años, politoxicómana, con infección por VIH de reciente diagnóstico, que ingresa por clínica de infección e insuficiencia respiratoria y desarrolla durante su hospitalización, diarrea, crisis tónico-clónica generalizada y hemiparesia izquierda. En las pruebas complementarias practicadas se evidencia patrón intersticial pulmonar difuso, masa mediastínica con adenopatías intratorácicas, lesión ocupante de espacio (LOE) cerebral y engrosamiento intestinal difuso. La infección respiratoria mejoró con tratamiento antibiótico de amplio espectro que incluía cotrimoxazol y levofloxacino, pero la LOE cerebral no mejoró con tratamiento antitoxoplasma. La presentación clínica inicialmente simulaba la de un linfoma diseminado, en una paciente VIH +, sin embargo, tras recibir el resultado de la biopsia de una adenopatía supraclavicular y de b-HCG se realizó el diagnóstico de tumor de células germinales extragonadal. En la revisión bibliográfica realizada (MEDLINE) no hemos encontrado ningún caso de TCGE en mujeres jóvenes con infección por VIH. Extragodanal germ cell tumors (EGCT) are not common, especially among women. Although there is no evidence of relationship between this sort of tumor and HIV infection they can appear at the same time in a patient, because in both cases the maximum incidence occurs in patients in the same age group. We present the case of a 27 years old woman, poly-drug user, with a recently diagnosis of HIV infection, who was admitted to clinic because of infection and shortage of breath, and developes during her hospitalization diahorrea, generalized tonic-clonic seizure and left hemiparesis. Complementary tests showed us diffuse interstitial pulmonary pattern, mediastinal mass with intrathoracic adenopathies, cerebral tumor and diffuse intestinal enlargementent. The breath infection got better with a wide-ranging antibiotic treatment, which included cotrimoxazol and levofloxacin, but the brain tumor didn't get better with the antitoxoplasma treatment. The clinical presentation simulated in the beginning a disseminated lymphoma, in a HIV+ patient; nevertheless, after receiving the result of the biopsy of a supraclavicular adenopathy and a b-HCG, an extragodanal germ cell tumor was diagnosed. We haven't found any case of EGCT in young women infected with HIV in our bibliographical review (MEDLINE).

- Abstract
Most people in rural sub-Saharan Africa lack access to electricity and rely on traditional, inefficient, and polluting cooking solutions that have adverse impacts on both human health and the environment. Here, we propose a novel integrated agroforestry-bioenergy system that combines sustainable biomass production in sequential agroforestry systems with biomass-based cleaner cooking solutions and rural electricity production in small-scale combined heat and power plants and estimate the biophysical system outcomes. Despite conservative assumptions, we demonstrate that on-farm biomass production can cover the household's fuelwood demand for cooking and still generate a surplus of woody biomass for electricity production via gasification. Agroforestry and biochar soil amendments should increase agricultural productivity and food security. In addition to enhanced energy security, the proposed system should also contribute to improving cooking conditions and health, enhancing soil fertility and food security, climate change mitigation, gender equality, and rural poverty reduction., (© 2024. 11. Reduced bronchoalveolar macrophage phagocytosis and cytotoxic effects after controlled short-term exposure to wood smoke in healthy humans.

- Abstract
Background: Exposure to wood smoke has been shown to contribute to adverse respiratory health effects including airway infections, but the underlying mechanisms are unclear. A preceding study failed to confirm any acute inflammation or cell influx in bronchial wash (BW) or bronchoalveolar lavage (BAL) 24 h after wood smoke exposure but showed unexpected reductions in leukocyte numbers. The present study was performed to investigate responses at an earlier phase, regarding potential development of acute inflammation, as well as indications of cytotoxicity., Methods: In a double-blind, randomised crossover study, 14 healthy participants were exposed for 2 h to filtered air and diluted wood smoke from incomplete wood log combustion in a common wood stove with a mean particulate matter concentration of 409 µg/m
3 . Bronchoscopy with BW and BAL was performed 6 h after exposure. Differential cell counts, assessment of DNA-damage and ex vivo analysis of phagocytic function of phagocytosing BAL cells were performed. Wood smoke particles were also collected for in vitro toxicological analyses using bronchial epithelial cells (BEAS-2B) and alveolar type II-like cells (A549)., Results: Exposure to wood smoke increased BAL lactate dehydrogenase (LDH) (p = 0.04) and reduced the ex vivo alveolar macrophage phagocytic capacity (p = 0.03) and viability (p = 0.02) vs. filtered air. BAL eosinophil numbers were increased after wood smoke (p = 0.02), while other cell types were unaffected in BW and BAL. In vitro exposure to wood smoke particles confirmed increased DNA-damage, decreased metabolic activity and cell cycle disturbances., Conclusions: Exposure to wood smoke from incomplete combustion did not induce any acute airway inflammatory cell influx at 6 h, apart from eosinophils. However, there were indications of a cytotoxic reaction with increased LDH, reduced cell viability and impaired alveolar macrophage phagocytic capacity. These findings are in accordance with earlier bronchoscopy findings at 24 h and may provide evidence for the increased susceptibility to infections by biomass smoke exposure, reported in population-based studies., (© 2023. - Abstract
One hundred and five Labrador dogs were randomly divided into two groups to determine the number of animals that develop elbow dysplasia when treated with an oral supplement compared to untreated ones. Efficacy of the oral treatment was also evaluated once illness was diagnosed. The supplement (Hyaloral) contained hyaluronic acid, hydrolysed collagen, glucosamine, chondroitin sulphate, and gamma oryzanol. Clinical evaluation of the elbow joints was completed at months 3, 6, 12, and 20 by orthopaedic evaluations, radiography, serologic and blood analysis, and veterinarian evaluation of dysplasia symptoms. All side effects were recorded. In the control group, 33.3% of the dogs developed radiographic evidence of elbow dysplasia compared to 18.5% in the treated group. Symptoms of dysplasia at 12 months differed between the treated (12.5%) and control (61.5%) animals, and were significantly different at 20 months (p < 0.05). Differences in lameness along with movement and swelling of the elbows between groups were observed after 12 months. The treated group had improved significantly by the last visit (p < 0.05). No adverse side effects were reported. In conclusion, oral treatment with Hyaloral may have a potential cumulative action that provides protection against dysplasia and significantly improves symptoms of elbow dysplasia.
