Abstract

In unforeseen situations, such as nuclear power plant's or civilian radiation accidents, there is a need for effective and computationally inexpensive methods to determine the expression level of a selected gene panel, allowing for rough dose estimates in thousands of donors. The new generation in-situ mapper, fast and of low energy consumption, working at the level of single nanopore output, is in demand. We aim to create a sequence identification tool that utilizes Natural Language Processing (NLP) techniques and ensures a high level of negative predictive value (NPV) compared to the classical approach. The training dataset consisted of RNASeq data from 6 samples. Having tested multiple NLP models, the best configuration analyses the entire sequence and uses a word length of 3 base pairs with one-word neighbor on each side. For the considered FDXR gene, the achieved mean balanced accuracy (BACC) was 98.29% and NPV 99.25%, compared to minimap2's performance in a cross-validation scenario. Reducing the dictionary from 1024 to 145 changed BACC to 96.49% and the NPV to 98.15%. Obtained NLP model, validated on an external independent genome sequencing dataset, gave NPV of 99.64% for complete and 95.87% for reduced dictionary. The salmon-estimated read counts differed from the classical approach on average by 3.48% for the complete dictionary and by 5.82% for the reduced one. We conclude that for long Oxford Nanopore reads, an NLP-based approach can successfully replace classical mapping in case of emergency. The developed NLP model can be easily retrained to identify selected transcripts and/or work with various long-read sequencing techniques. Abstract

BackgroundThe antimetabolites methotrexate (MTX) and mycophenolate mofetil (MMF) are commonly used as initial corticosteroid-sparing treatment for uveitis. There is little data examining risk factors for failing both MTX and MMF. The objective of this study is to determine risk factors for failing both MTX and MMF in patients with non-infectious uveitis.Main bodyThis is a sub-analysis of the First-line Antimetabolites as Steroid-sparing Treatment (FAST) uveitis trial, which was an international, multicenter, block-randomized, observer-masked, comparative effectiveness trial comparing MTX and MMF as initial treatments for non-infectious uveitis. This study was undertaken at multiple referral centers in India, the United States, Australia, Saudi Arabia and Mexico between 2013 and 2017. A total of 137 patients who completed all 12 months of follow-up from the FAST trial, were included in this study. The primary outcome was failing both antimetabolites over the 12 months of the trial. Potential predictors included: age, sex, bilateral involvement, anatomic location of the uveitis, presence of cystoid macular edema (CME) and retinal vasculitis at baseline visit, uveitis duration, and country/study sites as risk factors for failing both MTX and MMF. The presence of retinal vasculitis posterior to the equator on fluorescein angiogram was associated with failing both MTX and MMF.ConclusionRetinal vasculitis may be a risk factor for failing multiple antimetabolites. Clinicians could consider more quickly advancing these patients to other medication classes, such as biologics.

Abstract

PurposeTo evaluate changes in health-related and vision-related quality of life (VRQoL) among patients with noninfectious uveitis who were treated with antimetabolites.DesignSecondary analysis of a randomized controlled trial.ParticipantsPatients with noninfectious uveitis from India, the United States, Australia, Saudi Arabia, and Mexico.MethodsFrom 2013 through 2017, 216 participants were randomized to receive 25 mg weekly oral methotrexate or 1.5 g twice daily oral mycophenolate mofetil. Median changes in quality of life (QoL) were measured using Wilcoxon signed-rank tests, and differences between treatment groups were measured using linear mixed models, adjusting for baseline QoL score, age, gender, and site. Among Indian patients, VRQoL scores from a general scale (the National Eye Institute Visual Function Questionnaire [NEI-VFQ]) and a culturally specific scale (the Indian Visual Function Questionnaire [IND-VFQ]) were compared using Pearson correlation tests.Main outcome measuresVision-related QoL (NEI-VFQ and IND-VFQ) and health-related QoL (HRQoL; physical component score [PCS] and mental component score [MCS] of the Medical Outcomes Study 36-Item Short Form Survey [SF-36v2]) were measured at baseline, the primary end point (6 months or treatment failure before 6 months), and the secondary end point (12 months or treatment failure between 6 and 12 months).ResultsAmong 193 participants who reached the primary end point, VRQoL increased from baseline by a median of 12.0 points (interquartile range [IQR], 1.0-26.1, NEI-VFQ scale), physical HRQoL increased by a median of 3.6 points (IQR, -1.4 to 14.9, PCS SF-36v2), and mental HRQoL increased by a median of 3.0 points (IQR, -3.7 to 11.9, MCS SF-36v2). These improvements in NEI-VFQ, SF-36v2 PCS, and SF-36v2 MCS scores all were significant (P < 0.01). The linear mixed models showed that QoL did not differ between treatment groups for each QoL assessment (NEI-VFQ, IND-VFQ, PCS SF-36v2, and MCS SF-36v2; P > 0.05 for all). The NEI-VFQ and IND-VFQ scores for Indian participants were correlated highly at baseline and the primary and secondary end points (correlation coefficients, 0.87, 0.80, and 0.90, respectively).ConclusionsAmong patients treated with methotrexate or mycophenolate mofetil for uveitis, VRQoL and HRQoL improved significantly over the course of 1 year and did not differ by treatment allocation. These findings suggest that antimetabolites could improve overall patient well-being and daily functioning.

Abstract

Purpose: In this short tale, we describe a year of Pierre Chris Curry's ionizing radiation (IR) exposure, assessing and summarizing how much he has been exposed to over a year of his fictive life, cumulating the different types of exposures (either due to natural radiation, occupational and medical exposure), while staying reasonably credible. We have limited ourselves to IR exposure. As a recognized specialist in interventional cardiac surgery, Pierre provides lectures at international conferences requiring overseas flights. When not traveling, Pierre lives in Brittany in an area where there is high natural background radiation, owing to significant concentrations of radon, a radioactive gas produced from the natural radioactive decay of uranium found in rocks, granite in this case. Conclusion: Natural exposures correspond to half of Pierre's total yearly exposure. Therefore, where you live, and your life habits have an important impact on your radiation exposure levels. Medical exposures take the second place in the ranking, but these exposures are punctual. Although his professional exposure is the lowest percentage, this represents a chronic exposure which is continuous over the duration of his working life, and alongside the natural exposure, is building on over-time. Although Pierre calculated total yearly dose was 58.4 mSv, significantly higher than the average in countries such as France, UK or even USA, his excess risk of death from cancer was still very low 0.292%. Multiple natural language processing models were examined, differing in the type of dictionary components (word length, step, context) as well as the encoding length and number of sequences required for algorithm training. The best configuration analyses the entire sequence and uses a word length of 3 base pairs with one-word neighbor on each side. For the considered FDXR gene, the achieved mean balanced accuracy (BACC) was 98.29% and NPV was 99.25%, compared to minimap2's performance in a cross-validation scenario. The next stage focused on exploring the dictionary components and attempting to optimize it, employing statistical techniques as well as those relying on the explainability of the decisions made. Reducing the dictionary from 1024 to 145 changed BACC to 96.49% and the NPV to 98.15%. Obtained model, validated on an external independent genome sequencing dataset, gave NPV of 99.64% for complete and 95.87% for reduced dictionary. The salmon-estimated read counts differed from the classical approach on average by 3.48% for the complete dictionary and by 5.82% for the reduced one. Conclusions: We conclude that for long Oxford nanopore reads, a natural language processing-based approach can reliably replace classical mapping when there is a need for fast, reliable and energy and computationally efficient targeted mapping of a pre-defined subset of transcripts. Abstract

Antimicrobial resistance poses a major threat to global health and food security and is primarily driven by antimicrobial use in human and veterinary medicine. Understanding its epidemiology at farm level is crucial for effective control measures. Despite the significant reduction in antibiotic use in conventional livestock production, the swine sector traditionally has a higher level of antibiotic use in veterinary medicine. Consequently, multidrug resistance (MDR) among microbial isolates of swine origin has been relatively frequent. The aim of this study was to assess the presence of multidrug-resistant (MDR) bacteria, enteric pathogens and resistance genes to the main antibiotics used in clinical practice, both within the environment and in animals across pig farms characterized by varying degrees of sanitary status. A total of 274 samples were collected. Of these, 34 samples were collected from the environment (wall swabs, slat swabs and slurry pit), and 240 samples were collected from animals (sows' and piglets' rectal faeces). All samples were analysed for MDR bacteria and enteric pathogens. The study revealed a high frequency of extended-spectrum beta-lactamases (ESBL)-producing Enterobacterales and Campylobacter spp., with ESBL-producing Enterobacterales predominating in high health status farms (environment and animals) and Campylobacter spp. in both high health status and low health status environments. Additionally, a high percentage of methicillin-resistant Staphylococcus aureus (MRSA) was found, mainly in environmental samples from high health status farms, and Clostridioides difficile was distributed ubiquitously among farms and samples. Furthermore, though less frequently, vancomycin-resistant Enterococcus faecium (VRE) was isolated only in high health status farms, and Gram-negative bacilli resistant to carbapenems were isolated only in environmental samples of high health status and low health status farms. This study underscores the importance of surveillance for MDR bacteria in farm animals and their environment, including their waste. 32. Preparation and characterisation of nanostructured bulk Bi2Te3 thermoelectric materials using ultrasound milling

Abstract

Thermoelectric materials have been intensively investigated during the last years for energy harvesting applications. The main drawback of this technology is the low efficiency of the current materials. Significant advances in this respect have been recently achieved by nanostructuring, to mainly reduce the thermal conductivity. In this approach a bulk sample is milled into a nanostructured powder that is then compacted to form a nanobulk sample. The main objective of this thesis is to explore and introduce a new technique, ultrasound milling, based on the crushing of bulk samples by means of ultrasound effects taking place in a liquid medium, for the preparation of nanostructured bulk materials. Bismuth telluride alloys are the industrial standard in thermoelectrics and has been chosen as the material to perform this investigation. The most suitable conditions for the preparation of the nanostructured powders by the ultrasound milling technique have been identified. The optimised powders were used to prepare compacted nanobulk samples. The thermoelectric properties of these samples were finally characterised at room temperature and their performance related to their microstructure. Extraordinarily low thermal conductivity was obtained for both n- and p-type samples prepared (0.5 and 0.35 W/Km respectively), which are within the lowest reported values for any thermoelectric alloy. This reduction was accompanied with a significant decrease in electrical conductivity which led to a non-significant improvement in the figure of merit (Z). However, high ZT values (1 and 1.4 for n- and p-type bismuth telluride respectively) were identified in non-treated samples after a very simple grinding process which was employed as pre-treatment. The figure of merit of these two materials prepared by this simple methodology is close to the best reported values for Bi2Te3. Our results identify ultrasound milling and the simple crushing method as promising tools for the fabrication of nanostructured bulk thermoelectric materials.
