Abstract

Aim: Epidemiology of wild-type transthyretin cardiac amyloidosis (ATTRwt-CA) remains poorly defined. A better characterization of pathways leading to ATTRwt-CA diagnosis is of key importance, and potentially informative of disease course and prognosis. The aim of this study was to describe the characteristics of contemporary pathways leading to ATTRwt-CA diagnosis, and their potential association with survival. Methods and results: This was a retrospective study of patients diagnosed with ATTRwt-CA at 17 Italian referral centres for CA. Patients were categorized into different ‘pathways’ according to the medical reason that triggered the diagnosis of ATTRwt-CA (hypertrophic cardiomyopathy [HCM] pathway, heart failure [HF] pathway, incidental imaging or incidental clinical pathway). Prognosis was investigated with all-cause mortality as endpoint. Overall, 1281 ATTRwt-CA patients were included in the study. The diagnostic pathway leading to ATTRwt-CA diagnosis was HCM in 7% of patients, HF in 51%, incidental imaging in 23%, incidental clinical in 19%. Patients in the HF pathway, as compared to the others, were older and had a greater prevalence of New York Heart Association (NYHA) class III–IV and chronic kidney disease. Survival was significantly worse in the HF versus other pathways, but similar among the three others. In multivariate model, older age at diagnosis, NYHA class III–IV and some comorbidities but not the HF pathway were independently associated with worse survival. Conclusions: Half of contemporary ATTRwt-CA diagnoses occur in a HF setting. These patients had worse clinical profile and outcome than those diagnosed either due to suspected HCM or incidentally, although prognosis remained primarily related to age, NYHA functional class and comorbidities rather than the diagnostic pathway itself.

Abstract

Peru has a great potential for forestry plantations, nevertheless, importations of this type of products are getting very expensive. Soils under forest are acidic and infertile to support sustainable economically valuable forest tree species such as teak. Information is lacking on proper fertility management in forest plantations such as teak. Therefore, the objective of this study was to explore the effect of lime, and organic and inorganic fertilizers on the early growth and nutrient composition of 12-month old teak grown on acidic soil of Peru. The soil under field study was acidic with a pH of 4.99, low in fertility. A factorial design of 2x3: Lime (No Lime and Liming) and fertilizer (organic, inorganic and mixture), with 9 repetitions was adopted. Tree biometric parameters (height, diameter, biomass) and N, P, K, Ca, Mg, Fe, Cu, Mn, Zn foliar concentrations were determined during early the growth of teak. Treatments with addition of dolomite lime favored higher biometric parameters, and use of organic fertilizers promoted more growth than inorganic fertilizers. In the case of nutrients, no major differences were observed between limed and unlimed treatments while organic fertilizer promoted Ca, K and S nutrition. The results show that the application of lime and organic fertilizers is essential for the successful management and establishment of teak plants in acidic soils of Peru. [ABSTRACT FROM AUTHOR]

16. POBLACION DE Helicotylenchus sp. y Aphelenchus sp. EN LA RHIZOSFERA DE CLONES DE CACAO (Theobroma cacao L.) BAJO LOS SISTEMAS DE MANEJO TRADICIONAL Y DE BOSQUE MEJORADO

Abstract

Se estableció un experimento en la estación experimental El Choclino del Instituto de Cultivos Tropicales del Departamento de San Martín, Perú con la finalidad de evaluar la población de nematodos en clones de cacao bajo sistemas de manejo tradicional (SMT) y de bosque mejorado (SBM). De los sistemas en evaluación se eligió 10 genotipos de cacao (ICT-2142, CCN-51, ICT-1112, ICT-1026, ICT-2162, ICT-2171, ICS-95, UF-613, U-30 y H-35) y un testigo (híbrido). Para realizar el muestreo se procedió a marcar puntos de muestreo en cada planta de la parcela en forma de zigzag con dirección a la proyección de la copa del árbol a las profundidades 0-20, 20-40 y 40-60 cm. La extracción de nematodos del suelo se realizo mediante el método del tamizado y de la bandeja. Antes de establecer los sistemas de cultivo del cacao se encontró una población de 87 indiv/100cc de suelo de los cuales Helicotilenchus sp., representa el 12.5% y Aphelenchus sp., el 5.7%. Dos años después de la instalación de los sistemas de cultivo del cacao a las profundidades evaluadas se encontró en la rhizosfera una población promedio de 172 indiv/100cc de suelo en el SBM, de los cuales el 16.11% corresponde a Helicotilenchus sp., con una tasa de crecimiento de 8.38 indiv/100cc de suelo/año y 0.93% corresponde a Aphelenchus sp., con una tasa de crecimiento de -1.67 indiv/100cc de suelo/año. Mientras la población de nematodos en el SMT fue de 321 indiv/100cc de suelo, de los cuales el 6.3% represento a Helicotilenchus sp., con una tasa de crecimiento de 4.63 indiv/100cc de suelo/año y 22.78% represento a Aphelenchus sp., con una tasa de crecimiento de 34.03 indiv/100cc de suelo/año. En el SBM en los primeros 20 cm. de profundidad predomina el genero Helicotilenchus sp., encontrándose una mayor población en la rhizosfera del genotipo U-30 (78 indiv/100cc de suelo) seguido de los genotipos ICT-1021 y H-35 (50 y 40 indiv/100cc de suelo, respectivamente), mientras en el SMT la población del genero Helicotilenchus sp., fue mayor en los genotipos ICT-2142, ICT-1026 y ICS-95 (44, 36 y 31 indiv/100cc de suelo, respectivamente). La población de Helicotilenchus sp., disminuye considerablemente con la profundidad. En tanto la población de Aphelenchus sp., es mayor en el SMT en los primeros 20 cm. de profundidad, encontrándose en las rhizosferas de los genotipos ICT-1021, ICS-95 y H-35 (133, 89 y 71 indiv/100cc de suelo, respectivamente), a la profundidad de 20 - 40 cm. la población de este genero es 60 y 56 indiv/100cc de suelo (ICS-95 y ICT-1021, respectivamente), mientras a la profundidad de 40 - 60 cm. la población de Aphelenchus sp., disminuye considerablemente. La población de Helicotilenchus sp., es mayor en el sistema SBM, mientras la población de Aphelenchus sp., predomina en el sistema SMT. Se encontró una alta población de nematodos con el sistema de manejo tradicional.

41. Effect of sodium ipodate and iodide on free T4and free T3concentrations in patients with Graves’ disease

Abstract

Graves’ hyperthyroid patients were treated daily for 10 days with 1 g sodium ipodate, a cholecystographic agent which exerts a blocking effect on the peripheral conversion of T4to T3, or with 12 drops of saturated solution of potassium iodide (SSKI). Serum concentrations of free T4(FT4) and free T3(FT3) were measured before, during and 5 and 10 days after the administration of each drug. Sodium ipodate treatment induced a rapid decrement of serum FT4concentrations which declined from 48.9 ± 6.6 pg/ml to 26.0 ± 2.7 pg/ml. In these patients serum FT3concentrations declined from 12.4 ± 2.0 pg/ml to 2.5 ± 0.4 pg/ml. Ten days after sodium ipodate withdrawal, serum FT4and FT3concentrations returned to baseline values. In patients treated with SSKI serum FT4concentrations declined from 51.1 ± 8.8 pg/ml to 11.3 ± 1.4 pg/ml and FT3 from 15.7 ± 2 pg/ml to 2.6 ± 0.3 pg/ml. Moreover, after therapy interruption serum free thyroid hormone concentrations returned to baseline values in these patients. Serum FT4pattern during the study was not different between the two groups of subjects whereas serum FT3concentrations were significantly lower in patients treated with sodium ipodate. These findings indicate that SSKI and sodium ipodate are effective in inducing a rapid decrement of serum free thyroid hormone concentrations. Therefore the employment of these drugs may be useful in the treatment of patients with thyroid storm and those undergoing thyroidectomy.

42. Effects of excess iodine administration on thyroid function in euthyroid patients with a previous episode of thyroid dysfunction induced by interferon-alpha treatment

Abstract

OBJECTIVE
To determine the effects of pharmacological quantities of iodide (SSKI) on thyroid function in euthyroid patients previously treated with recombinant interferon-alpha (rIFN-alpha) for chronic viral hepatitis B and C (HCV), a cytokine which may induce thyroid dysfunction. DESIGN
Thyroid function tests were carried out in 16 euthyroid patients, 8 of whom had previously developed thyroid dysfunction during rIFN-alpha therapy for HCV, before, during and after the administration of 10 drops of saturated solution of potassium iodide (SSKI) (=~350 mg iodide). PATIENTS
All 16 patients had been treated in the past with rIFN-alpha for HCV. Eight patients had developed rIFN-alpha induced abnormalities in thyroid function (5 inflammatory thyrotoxicosis, 1 Graves' disease, and 2 impaired thyroid organification of iodide) and 8 had not developed thyroid dysfunction. MEASUREMENTS
After baseline serum free T4 (FT4) and free T3 (FT3) concentrations, basal and TRH stimulated TSH concentrations, and TSH-receptor (TSH-R-Ab) and thyroid peroxidase (TPO-Ab) antibodies were measured, 10 drops saturated solution of potassium iodide (SSKI, =~350 mg iodide) were given daily for 60 days and the above parameters assessed during and after SSKI was discontinued. RESULTS
Five of 8 patients with a previous history of rIFN-alpha induced thyroid dysfunction developed mild iodide induced abnormalities of thyroid function (subclinical hypothyroidism (slightly elevated basal and TRH stimulated serum TSH concentrations with normal serum FT4 and FT3 concentrations) or hyperthyroidism) compared with the 8 patients who had no previous evidence of thyroid dysfunction during rIFN-alpha therapy. CONCLUSIONS
In view of the present observations, it is prudent to avoid the administration of excess iodine to euthyroid subjects with a previous episode of thyroid dysfunction during rIFN-alpha therapy, adding a new group of patients susceptible to iodine induced thyroid disease.

43. Simultaneous inhibition by pirenzepine of the GH responses to GnRH and TRH in insulin-dependent diabetics and in patients with major depression

Abstract

Abstract. The present study was undertaken in order to establish whether muscarinic cholinergic receptors are involved in the anomalous GH response to GnRH in men with insulin-dependent diabetes mellitus and in male patients with major depression. For this purpose, 16 male diabetics, 18 depressed men and 9 normal controls were tested with GnRH (25 µg iv) with and without previous treatment with the muscarinic cholinergic receptor blocker pirenzepine (40 mg iv 10 min before GnRH). Additional experiments with TRH (200 µg iv 10 min after pirenzepine) were performed in the same subjects and used for comparison between responders to TRH and GnRH. The administration of GnRH stimulated GH release in 12 out of the 16 diabetics and in 8 out of the 18 depressed patients, but not in the normal controls. Control and diabetic non-responders to GnRH did not respond to TRH. In contrast, all diabetic responders to GnRH, except 2, showed paradoxical GH responses to TRH. All depressed responders to GnRH and 3 of the non-responders, were responsive to TRH. The pattern and magnitude of the secretory responses to TRH and GnRH were similar in depressed and diabetic patients. When the effects of GnRH and TRH were restudied in the presence of pirenzepine, neither GnRH nor TRH enhanced the serum concentrations of GH in any patient. These data indicate that a muscarinic cholinergic mechanism is involved in the anomalous responses of GH to GnRH and TRH in diabetic men and in male patients affected by major depression.

Abstract

In order to determine whether dopamine plays a role in the control of neuropituitary function in pregnant women during labour, blood levels of nicotine (NSN)- and estrogen (ESN)-stimulated neurophysins were measured in 119 women treated orally with placebo (n = 59, control group) or 5 mg bromocriptine, a potent dopaminergic receptor agonist (n = 60, experimental group). Serum samples were taken before drug ingestion (basal sample) and at delivery. The serum basal concentrations of NSN and ESN were similar in both groups of pregnant women in labour. At delivery, serum ESN levels were similar in all women regardless of the treatment, whereas NSN concentrations were significantly lower in the bromocriptine-treated women than in those who were given placebo. In additional experiments the effect of 5 mg bromocriptine on the serum concentrations of NSN and ESN was tested for 6 hours after drug ingestion in 10 healthy, non-pregnant women and in 8 women in the 3rd trimester of pregnancy. Bromocriptine did not modify the circulating levels of NSN and ESN in either of these 2 groups of women. Since NSN and ESN are thought to be associated with vasopressin and oxytocin, respectively, these results indicate that in non-pregnant women and in pregnant women during late pregnancy dopaminergic stimulation with a dopaminergic receptor agonist does not inhibit the release of either vasopressin or oxytocin during rest. In contrast, dopaminergic receptor stimulation appears to play an inhibitory role in the regulation of vasopressin, but not oxytocin secretion in pregnant women in labour.

Il quadro normativo del tempo (costituito dall’art. 22 l. 6 dicem- bre 1971, n. 1034, dall’art. 37 r.d. 26 giugno 1924, n. 1054 e dall’art. 44 r.d. 17 agosto 1907, n. 642) si limitava a regolare i termini e le modalità formali di proposizione del ricorso incidentale, prevedendo l’inefficacia dello stesso in caso di rinuncia o di dichiarazione di tardività del ricorso principale.

L’istituzione della dirigenza costituisce, peraltro, l’esito più avanzato di un processo di riforma risalente nel tempo, informato alla necessità di rendere maggiormente imparziale ed efficiente l’amministrazione pubblica, attuando in forme più innovative la c.d. distinzione tra politica e amministrazione.

In epoca precodicistica mancava del tutto una disciplina puntuale e specifica riferita ai motivi aggiunti. Il tema era peraltro poco considerato anche nell’elaborazione dottrinaria, tanto che i primi studi in chiave monografica appaiono solo nel corso degli anni Ottanta del secolo scorso. In siffatto contesto, fu, dunque, la giurisprudenza a farsi carico del problema del frazionamento della domanda, svolgendo una funzione di supplenza, sostanzialmente creativa, delle mancanze del legislatore.
