Abstract

We consider solutions to the aggregation equation with Newtonian potential wherethe initial data are the characteristic function of a domain with boundary of class C1+γ ,0 < γ < 1.Such initial data are known to yield a solution that, going forward in time, retains a patch-likestructure with a constant time-dependent density inside an evolving region, which collapses on itselfin a finite time, and which, going backward in time, converges in an L1sense to a self-similarexpanding ball solution. In this work, we prove C1+γ regularity of the domain’s boundary on thetime interval on which the solution exists as an L∞ patch, up to the collapse time going forward intime and for all finite times going backward in time.

Abstract

Objective: Fetoscopic closure of spina bifida using heated and humidified carbon dioxide gas (hhCO2) has been associated with lower maternal morbidity compared with open closure. Fetal cardiovascular changes during these surgical interventions are poorly defined. Our objective was to compare fetal bradycardia (defined as fetal heart rate (FHR) < 110 bpm for 10 min) and changes in umbilical artery (UA) Doppler parameters during open vs fetoscopic closure. Methods: This was a prospective cohort study of 22 open and 46 fetoscopic consecutive in‐utero closures conducted between 2019 and 2023. Both cohorts had similar preoperative counseling and clinical management. FHR and UA Doppler velocimetry were obtained systematically during preoperative assessment, every 5 min during the intraoperative period, and during the postoperative assessment. FHR, UA pulsatility index (PI) and UA end‐diastolic flow (EDF) were segmented into hourly periods during surgery, and the lowest values were averaged for analysis. Umbilical vein maximum velocity was measured in the fetoscopic cohort. At each timepoint at which FHR was recorded, maternal heart rate and systolic and diastolic blood pressure were measured. Results: Fetal bradycardia occurred in 4/22 (18.2%) cases of open closure and 21/46 (45.7%) cases of fetoscopic closure (P = 0.03). FHR decreased gradually in both cohorts after administration of general anesthesia and decreased further during surgery. FHR was significantly lower during hour 2 of surgery in the fetoscopic‐repair cohort compared with the open‐repair cohort. The change in FHR from baseline in the final stage of fetal surgery was significantly more pronounced in the fetoscopic‐repair cohort compared with the open‐repair cohort (mean, −32.4 (95% CI, −35.7 to −29.1) bpm vs −23.5 (95% CI, −28.1 to −18.8) bpm; P = 0.002). Abnormal UA‐EDF (defined as absent or reversed EDF) occurred in 3/22 (13.6%) cases in the open‐repair cohort and 23/46 (50.0%) cases in the fetoscopic‐repair cohort (P = 0.004). There were no differences in UA‐EDF or UA‐PI between closure techniques at the individual stages of assessment. Conclusions: We observed a decrease in FHR and abnormalities in UA Doppler parameters during both open and fetoscopic spina bifida closure. Fetal bradycardia was more prominent during fetoscopic closure following hhCO2 insufflation, but FHR recovered after cessation of hhCO2. Changes in FHR and UA Doppler parameters during in‐utero spina bifida closure were transient, no cases required emergency delivery and no fetoscopic closure was converted to open closure. These observations should inform algorithms for the perioperative management of fetal bradycardia associated with in‐utero spina bifida closure. © 2024 International Society of Ultrasound in Obstetrics and Gynecology. 3. The External Consultant as the Wonderful Wizard from Oz--A Parody.

Abstract

Using the metaphor of the "Wizard of Oz," this paper satirizes the use of an unofficially identified and often highly paid group of educational consultants to reform and revitalize special needs career/vocational education. It begins with the premise that leaders in the educational system believe that they can somehow install three internal, essential ingredients for reform and revitalization through the use of "consultants from Oz" who possess specific rational answers to many educational, vocational, and social problems that challenge the educational system today. The main characters from L. Frank Baum's novel, "The Wonderful Wizard of Oz," personify the human longing to seek out external sources that will somehow be able to provide answers to internal questions. This mindset generally fails to recognize and support the strong internal identities and expertise that each school and school system already possesses. The paper concludes that too many educators forget, or may never have realized, that the most important lesson the Wizard of Oz taught to Dorothy, the Scarecrow, the Tin Woodman, and the Lion was to use the power that they had within themselves to make things better. 5. Pasteur's Quadrant as the Bridge Linking Rigor with Relevance

Abstract

A tension exists between educational practitioners and researchers, which is often attributed to their dichotomous and oftentimes polarizing professional ideologies or Discourse communities. When determining what works in education, researchers tend to emphasize evidencebased practices (EBPs) supported by research that is rigorous and internally valid, whereas practitioners tend to value practice-based evidence (PBE) that is relevant and externally valid. The authors argue that these separate mindsets stem from the classical view of research as being either rigorous or relevant. In his canonical Pasteur's Quadrant, Stokes (1997) proposed that rigor and relevance are complementary notions that, when merged, further the production, translation, and implementation of instructional practices that are both rigorous (i.e., evidence-based) and relevant (i.e., practice-based). The authors propose educational design research (EDR) and communities of practice (CoPs) as frameworks through which to realize the promise of Pasteur's quadrant.

7. Behavioral Cusps: A Person-Centered Concept for Establishing Pivotal Individual, Family, and Community Behaviors and Repertoires

Abstract

Cumulative-hierarchical learning (CHL) and behavior, a premise first introduced by Staats in 1975, describes how higher-level behavioral patterns and structures can emerge from interactions among a set of lower-level actions. Proponents of CHL emphasize the importance of pivotal response interventions, behavior repertoires, generative learning, and the development of behavioral cusps, the human equivalent of the computer world's killer application, or "killer app" ("Wikipedia," n.d.). Rosales-Ruiz and Baer (1997) defined a behavioral cusp as an entry point for pivotal behavioral change that, once initiated, so profoundly alters, displaces, or transforms one's behavioral repertoire that it renders preexisting behavioral repertoires obsolete. In this article, we demonstrate how behavioral cusps can be used to construct CHL behaviors and repertoires of persons with autism and other pervasive developmental disorders within and across five pivotal behavioral elements. We also describe how behavioral cusps could be used to improve the quality of the collaborative conversations during person-centered planning sessions.

Basic figures of merit were measured at IR flux levels simulating those expected in space-based astronomical observations. Results include dark current less than 20 e(-)/s, G-eta as high as 3.8, eta/beta of 20%, and read noise below 100 rms(e(-)).

Abstract

As part of a program of high spatial resolution imaging of gravitationally lensed sources in the visible and IR, images of 2237 + 030 were obtained in the Gunn r and infrared J, H, K, and 3.3 micron filters. The results of the photometry of the four bright quasar components provide evidence of extinction through the lens and a determination of the extinction law in the galaxy is made. The energy distribution shows evidence of a sharp decrease of the spectral index at wavelengths longer than a rest wavelength of 1 micron. Assuming that microlensing amplification in the IR is of similar strength as in the visible, the data constrain the suggested microlensing event of August-September 1988 to a time scale of 100 days.

45. Subarcsecond resolution observations of the central parsec of the Galaxy at 2.2 microns

Abstract

Four separate lunar occultations of the Galactic center in the infrared K-band have been observed, and the results are used to estimate the size of the sources and to derive photometry. It is found that IRS 16NE, 16NW, and IRS 29 are smaller than about 510 AU and IRS 16SW-W and C are smaller than 85 AU. All sources brighter than K = 10 mag in the central pc are smaller than 510 AU and therefore seem to be stars. IRS 7 is smaller than about 100 AU. IRS 1W and 13, resolved into pairs of stars, are probably the exciting stars for these H II regions. The shape of the differential luminosity function of the stars in the central sq pc is similar to that of stars in the central 150 sq pc. There is a steady increase in star density toward the central few arcsec of the Galaxy.

Abstract

A tension exists between educational practitioners and researchers, which is often attributed to their dichotomous and oftentimes polarizing professional ideologies or Discourse communities. When determining what works in education, researchers tend to emphasize evidence-based practices (EBPs) supported by research that is rigorous and internally valid, whereas practitioners tend to value practice-based evidence (PBE) that is relevant and externally valid. The authors argue that these separate mindsets stem from the classical view of research as being either rigorous or relevant. In his canonical Pasteur's Quadrant, Stokes (1997) proposed that rigor and relevance are complementary notions that, when merged, further the production, translation, and implementation of instructional practices that are both rigorous (i.e., evidence-based) and relevant (i.e., practice-based). The authors propose educational design research (EDR) and communities of practice (CoPs) as frameworks through which to realize the promise of Pasteur's quadrant.
