Background We describe herein the case of a patient whose primary complaints were episodic vertigo and “depersonalization,” a sensation of detachment from his own body. Purpose This case study aims to further clinical knowledge and insight into the clinical evaluation of vertiginous patients with complaints of depersonalization. Research Design This is a case study. Data Collection and Analysis A retrospective chart review of vestibular function testing done on a vertiginous patient with complaints of depersonalization was performed. Results Vestibular function testing revealed absent cervical and ocular vestibular evoked myogenic potentials on the left side with normal vHIT or video Head Impulse Test, videonystagmography, and rotational chair results, suggesting peripheral vestibular impairment isolated to the left saccule and utricle. Conclusion The otolith end organ impairment explains the patient's postural deviation to the left side during attempts to ambulate. We recommend that clinicians should be attentive to patient complaints of depersonalization and perform vestibular evoked myogenic potential testing to determine whether evidence of at least a unilateral peripheral otolith end organ impairment exists.

Purpose The purpose of the current investigation was to determine the interrelations among vestibular disorders from a data set generated from the patient perspective as compared to previous data generated from the physician's perspective. Method The data for the current investigation originated from a previously published study describing the development of the Dizziness Symptom Profile (DSP; Jacobson et al., 2019 ). The DSP is a 31-item patient self-report tool designed to help primary care physicians in the development of a differential diagnosis using the patient's level of agreement with each dizziness and symptom-related statement. Responses to these items converge on common vestibular diagnoses and were previously found to agree with ear specialist differential diagnoses 70.3% of the time. Data were collected for 131 subjects ( M age = 56.7 years, 72 women) seen for evaluation in a tertiary dizziness specialty clinic. For this study, the data were analyzed using descriptive statistics to determine the frequency of single diagnoses, multiple diagnoses, co-occurring diagnoses, and patterns of co-occurrence. Results Results indicated that 52.7% of patients endorsed a single vestibular diagnosis and 47.3% endorsed two or more vestibular diagnoses. Benign paroxysmal positional vertigo (BPPV) and vestibular migraine were the most common single diagnoses and also the most common co-occurring diagnoses. As the number of diagnoses endorsed on the DSP increased, so did the percentage of time that BPPV and vestibular migraine would occur. Conclusions Results support and extend the work of others but using data generated from the perspective of the patient. A slight majority of patients endorsed a single disorder, but almost as many patients endorsed more than one vestibular diagnosis. BPPV and vestibular migraine were the most common single vestibular diagnoses and also the most common co-occurring vestibular diagnoses; vestibular migraine was more common when multiple diagnoses were endorsed. Results suggest it is common for patients to volunteer symptoms that cannot be explained by a single vestibular diagnosis. This finding is in agreement with physician-generated diagnosis data. Clinicians should consider the possibility of co-occurring diagnoses in complicated patients or in patients who are not responding optimally to management of a single vestibular disorder. The DSP is a tool that encourages clinicians to consider multiple co-occurring vestibular disorders as the source of patient complaints.

Objective: The purpose of this investigation was to determine the relationship between the horizontal semicircular canal video head impulse test (hvHIT) and the caloric test in a sample of school-age children evaluated due to complaints of dizziness or imbalance. In adults, these two tests provide different but complementary information regarding vestibular function and/or pathology. However, the most common causes of vestibular loss in children are different than those in adults, and it is not clear how these two tests of horizontal semicircular canal function are related in a pediatric population. Method: This is a retrospective study of electronic records of pediatric patients (< 18 years old) who were referred for vestibular function testing and completed both hvHIT and caloric testing. Agreement between both test results was evaluated by examining measures of sensitivity, specificity, and calculating Cohen's kappa. Results: Results from 117 patients (mean age = 13.7 years, age range: 8–17 years, 37 males) were analyzed. Most patients (97/115 [84.3%]) demonstrated both normal hvHIT and caloric test results. Compared to caloric testing, the hvHIT had poor sensitivity (38.9%) and excellent specificity (98.0%). Cohen's kappa analysis revealed moderate agreement between the methods (coefficient = 0.49 [0.24, 0.73]). Where disagreement between the two tests occurred, the final diagnosis was not contingent on either test result. Conclusions: There is a moderate agreement between the hvHIT and the caloric test in pediatric patients presenting to a tertiary care medical center for a vestibular evaluation. Most patients in this cohort presented with normal findings. The specificity of hvHIT was excellent, although the sensitivity was low. This may be related to the low prevalence of peripheral vestibular system disease in this study cohort, and these results do not generalize to centers that serve populations of children where vestibular disorders are more prevalent (e.g., children with sensorineural hearing loss).

Purpose The purpose of this project was to explore the association between the perception of motion during caloric testing and two tasks associated with central vestibular processing: postural stability and visuospatial memory. Method This was a prospective study of 25 patients who were found to have nonvestibular etiologies of their symptoms and normal vestibular function test results and who underwent caloric testing with a mean maximum slow phase eye velocity for each irrigation of 15° or greater. Following each caloric irrigation, patients were asked whether they had any sensation of movement. Patients were grouped based on the presence or absence of motion during the caloric exam (motion perception vs. absent perception). Postural stability was assessed using computerized dynamic posturography, and visuospatial memory was assessed using a memory match card game application. Results There were no significant differences between groups on any measures of peripheral vestibular function. However, the Absent Perception Group showed greater postural instability during Condition 5 of posturography and performed significantly worse on a task of visuospatial working memory. Both age and absence of motion perception predicted abnormal performance on measures of postural stability and visuospatial working memory. Conclusions There appears to be clinical implications to a lack of motion perception during the caloric exam in patients with an otherwise normal peripheral vestibular system. Based on the current findings, we are unable to determine whether differences in postural stability and visuospatial memory were due to age or a central vestibular processing deficit.

Objectives The objectives of this study were (a) to investigate the optimal tone burst duration when recording the cervical vestibular evoked myogenic potential (cVEMP) and the ocular vestibular evoked myogenic potential (oVEMP) and (b) to determine whether monopolar recording influences the latency or amplitude of the cVEMP or the oVEMP. Method Fifteen subjects ( M = 27.7 years, SD = 6.73 years) participated in this study. The participants had no prior history of otological or neurological disease. Both oVEMPs and cVEMPs were recorded at a fixed stimulus level for stimulus durations of 2, 5, 10, and 25 ms. For both cVEMP and oVEMP, responses were obtained using a traditional differential recording montage and a monopolar recording montage. Results The cVEMP and the oVEMP had the greatest amplitude in the 2-ms stimulus condition. There was a statistically significant decrease in amplitude for durations greater than 2 ms. Monopolar and bipolar cVEMP and oVEMP latencies and amplitudes were not significantly different. Conclusion As stimulus duration increased beyond 2 ms, the amplitude of the response decreased for both the cVEMP and the oVEMP. There was no significant change in latency with increasing stimulus duration for either response. These results suggested the optimal stimulus duration for both the oVEMP and cVEMP is 2 ms, and there is no apparent advantage of using a bipolar recording technique.

Objectives Dizziness, vertigo, and unsteadiness are common complaints of patients who present to primary care providers. These patients often are referred to otology for assessment and management. Unfortunately, there are a small number of specialists to manage these patients. However, there are several dizziness disorders that can be successfully managed by primary care providers if the disorder is properly identified. To assist in the identification of several of the most common dizziness disorders, we developed the dizziness symptom profile (DSP). The DSP is a self-report questionnaire designed to generate one or more differential diagnoses that can be combined with the patient's case history and physical examination. Design This report describes three investigations. Investigations 1 and 2 (i.e., exploratory and confirmatory investigations, N = 514) describe the development of the DSP. Investigation 3 (N = 195) is a validation study that describes the level of agreement between the DSP completed by the patient, and, the differential diagnosis of the otologist. Results The final version of the DSP consists of 31 items. Preliminary findings suggest that the DSP is in agreement with the differential diagnoses of ear specialists for Meniere's disease (100% agreement), vestibular migraine (95% agreement), and benign paroxysmal positional vertigo (82% agreement). Conclusions Early results suggest that DSP may be useful in the creation of differential diagnoses for dizzy patients that can be evaluated and managed locally. This has the potential to reduce the burden on primary care providers and reduce delays in the diagnosis of common dizziness and vertigo disorders.

This investigation was conducted to measure the test-retest reliability of the Dizziness Symptom Profile (DSP). The DSP was developed to assist primary care providers, general otolaryngologists, and other health care providers in the development of a differential diagnosis for patients who present with dizziness, vertigo, or unsteadiness. The DSP yields a score ranging from 0 to 100% for each of 7 subscales. Each subscale represents a different diagnosis including benign paroxysmal positional vertigo, Ménière's disease, persistent postural-perceptual dizziness (PPPD), superior semi-circular canal dehiscence, vestibular migraine, vestibular neuritis, and general unsteadiness.Subjects were 150 adult patients (mean age 56.79 years, SD 15.69 years) referred to the Balance Disorders Clinic at Vanderbilt University Medical Center. Subjects completed two administrations of the DSP. The mean interval between test administrations was 1.58 days (SD 1.78 days). The response modes for the DSP were both a 0 to 100 mm visual analog scale (scored 0 mm = "strongly disagree" to 100 mm = "strongly agree") and, by extrapolation, the original 5-point Likert scale where the anchors were "strongly disagree" (scored 0 points) and "strongly agree" (scored 4 points).Pearson correlation coefficients were calculated to assess test-retest reliability for individual DSP items, and ranged from r = 0.67 to 0.91 (mean 0.80; p0.001). Cronbach's α coefficients were calculated to assess internal consistency reliability of items comprising the seven subscales. Each subscale had an acceptable level of internal consistency (Cronbach's α coefficients0.7) with the exception of PPPD which approached 0.7. Intraclass correlation coefficient estimates and their 95% confidence intervals were also calculated to assess the relative reliability of the subscales. All 7 subscales showed moderate to strong test-retest reliability, with intraclass correlation coefficients ranging from 0.85 to 0.94. Minimal detectable change (MDC) scores were calculated to assess absolute variability/measurement error for the seven subscale scores (which range from 0 to 100%). MDC values ranged from 16% (PPPD) to 25% (unsteadiness).(1) The test-retest reliability of the DSP is moderate to strong. (2) MDC values for each subscale were determined. (3) The DSP coupled with the Dizziness Handicap Inventory enables the clinician to evaluate the constructs of dizziness impairment, and disability/handicap. (4) The DSP may help provide a window to the natural history of dizziness disease(s). (5) The DSP provides a less biased assessment of the symptoms reported by the patient.

To evaluate the relationship between normal and abnormal ocular vestibular evoked myogenic potentials (oVEMP) and cervical vestibular evoked myogenic potentials (cVEMP) in patients with and without vestibular migraine (VM).Retrospective review of oVEMP and cVEMP results in patients with vestibular disorders who were assessed clinically and completed vestibular function studies. Data were extracted from a deidentified RedCap Repository.Tertiary care multispecialty medical center.Subjects were 212 consecutive adults meeting prespecified inclusion criteria who were evaluated in the Balance Disorders Clinic at Vanderbilt University Medical Center between 2011 and 2017. Patients with bilaterally absent VEMPs were excluded from the study.None.Proportions of subjects with or without VM in one of the following four test outcomes: normal cVEMP/normal oVEMP, abnormal cVEMP/abnormal oVEMP, abnormal cVEMP/normal oVEMP, and normal cVEMP/abnormal oVEMP.There was a significant relationship between VM and cVEMP and oVEMP test outcomes.Patients with VM are more likely than subjects with vestibular disorders other than migraine to exhibit normal cVEMP responses in the presence of unilaterally abnormal oVEMP responses. Such a VEMP pattern may be a biomarker of VM and further supports a possible pathophysiologic relationship between the utriculo-ocular reflex and VM.

Purpose The purpose of the investigation was to determine whether the “belly–tendon” electrode derivation produced ocular vestibular evoked myogenic potentials (oVEMPs) in children that were superior to those recorded with the infraorbital–chin electrode derivation. Method This was a prospective observational study using a sample of convenience at a tertiary care medical center. Subjects were 13 children between the ages of 5 and 12 years. We compared the latency of the N1 wave and the N1–P1 peak-to-peak amplitude for the belly–tendon electrode derivation to the infraorbital–chin electrode derivation. Results The belly–tendon electrode derivation was associated with superior N1–P1 amplitudes compared to the infraorbital–chin electrode derivation. However, the large amplitude was also associated with greater variability compared to the infraorbital–chin derivation. There were no significant electrode derivation–based differences in N1 latency. Additionally, there was no predictable relationship between age and oVEMP amplitude. Conclusion As shown previously in adults, the belly–tendon electrode derivation produces significantly larger oVEMP amplitudes compared to the infraorbital–chin electrode derivation.

The Dizziness Handicap Inventory (DHI) is a 25-item self-report questionnaire developed to measure the disabling and handicapping impact of dizziness. The present investigation was conducted in an effort to re-assess the factor structure of the DHI.Retrospective study.Tertiary care center.Subjects were 1,991 patients who were evaluated in the Mayo Clinic-Rochester Vestibular and Balance Laboratory.Exploratory factor analysis: an exploratory bifactor analysis (EFA) with bifactor rotation was used to analyze a random sample of 999 patients. Analyses were used to determine the dominance of the general factor (i.e., total score) relative to the group factor (i.e., subscales). Confirmatory factor analysis: a confirmatory bifactor graded response model was fit with appropriate item-to-group relationships that was discovered by our exploratory analyses. To validate the bifactor model that was identified with the exploratory analyses, a bifactor model with three grouping factors (i.e., Physical manifestations, Catastrophic impact of dizziness, and the Emotional impact of dizziness) were fit to a different random sample of 992 patients using the new item-to-group factor specifications.In the confirmatory analyses, all items had a positive factor loading on the general factor. There were 14 items that loaded on the general factor only. The rest of the items (n = 11) loaded on both the general factor and one of three group factors.Conclusions of the study revealed several findings: 1) reporting the result as a total score (i.e., a single general factor) is warranted, and, 2) there is statistical support for the existence of three subscales representing: the Physical manifestations, Catastrophic impact, and Emotional impact of dizziness and vertigo.

In recent years there has been renewed interest in evaluating vestibular function in young children. This revival is due, at least in part, to emphasis on evidence-based practice in health care and the realization that although many children describe or demonstrate symptoms and signs associated with vestibular dysfunction, a solid evidence base has not been established for which children will benefit from testing. Furthermore, there is a paucity of information regarding age-appropriate normative data for components of the vestibular test battery that are considered to be essential. Although there are many challenges associated with evaluating vestibular function in children, information that will help guide clinicians in their decisions of which children to test, which tests to select and/or modify, and how to interpret results is emerging. This chapter describes the five most common causes of vertigo and dizziness in the pediatric population and the reported vestibular findings associated with these disorders.

To determine whether a reference electrode placed over the contralateral medical canthus is electrically indifferent to the ocular vestibular evoked myogenic potential (oVEMP).Prospective observational study.Tertiary academic center.Nineteen otologically normal subjects.oVEMPs were recorded using a 500 Hz tone burst at 125 dB pSPL using both a conventional dual infra-orbital derivation and contralateral infraorbital electrode referenced to a medial canthus electrode known as the "belly-tendon" derivation.The belly-tendon derivation produced significantly larger oVEMP amplitudes. This appears to be due to a polarity inversion of the first negative peak that occurs when the reference electrode is placed over the medial canthus. Thus, the belly-tendon derivation was associated with reference contamination, but in this instance the reference contamination produced an augmented oVEMP amplitude.Neither the dual infra-orbital nor belly-tendon electrode montages are electrically neutral. Reference contamination is the source of both a decrease in amplitude using conventional recording techniques and increased amplitude using the belly-tendon montage. The results support the contention that the belly-tendon montage generates a reversed polarity response, i.e., algebraically subtracted (i.e., added in the differential amplifier) from the response recorded at the infraorbital midline enhancing the oVEMP amplitude. It should be noted that the amplitude gained when using the belly-tendon montage may also be due to electromyographic (EMG) generated by extraocular muscles other than the inferior oblique generator, resulting in a loss of specificity. The authors recommend the belly-tendon montage be used in clinical situations where the oVEMP is absent or low in amplitude.

Purpose We report 3 patients with Ménière's disease and describe how the combination of audiometry, video head impulse testing, and caloric results may prove helpful in the diagnosis of Ménière's disease. Method Three patients with “definite” Ménière's disease were evaluated in a tertiary care medical center. Each patient underwent videonystagmography, horizontal canal video head impulse testing, and audiometry. Results All 3 patients demonstrated moderate, flat, sensorineural hearing losses; significant caloric asymmetries; and bilaterally normal video head impulse testing. This pattern of findings suggests differential preservation of high-frequency function (video head impulse testing) with impairment of low-frequency function (unilaterally abnormal caloric test results) in these patients. Conclusion Ipsilesional abnormal caloric testing in the presence of normal video head impulse testing is a pattern of findings observed in a cohort of patients who have “definite” Ménière's disease.

Introduction The absence of vertigo during the caloric test, despite a robust response, has been suggested to represent a central vestibular system phenomenon. The purpose of this investigation was to determine the prevalence of absent caloric-induced vertigo perception in an unselected group of patients and to assess possible predicting variables. Methods Prospective investigation of 92 unselected patients who underwent caloric testing. Inclusion criteria were that each patient generate a maximum slow phase velocity (maxSPV) ≥ 15 deg/sec and a caloric asymmetry of ≤10%. Following the caloric, patients were asked, “Did you have any sensation of motion?” Results Results showed 75% of patients reported motion with a mean age of 56.51 years compared to a mean age of 66.55 in the 25% of patients reporting an absence of motion. A logistic regression was performed and the overall model was statistically significant accounting for 29% of the variance in caloric perception. The significant predictor variables were patient age and maxSPV of the caloric response. The effect size for both variables was small with an odds ratio of .9 for maxSPV and 1.06 for age. Conclusions The current investigation showed that both age and maxSPV of the caloric response were significant predictors of vertigo perception during the caloric exam. However, the association between age and caloric perception is not conclusive. Although there is evidence to suggest that these findings represent age-related changes in the central processing of vestibular system stimulation, there are additional unmeasured factors that influence the perception of caloric-induced vertigo.

Purpose The purpose of the present investigation was to determine whether there are significant differences in patient/healthcare provider perceptions of patient's dizziness severity, dizziness disability/handicap, anxiety, and signs of autonomic system activation. Materials and methods This was a prospective investigation of 30 patient–provider dyads drawn as a sample of convenience from an otology clinic in a large, tertiary care, medical center. Patients completed both the Dizziness Handicap Inventory (DHI) and the Vestibular Symptom Scale (VSS) prior to vestibular function testing. Providers were instructed to complete the same measures following the patient's clinic visit from what they estimated was the patient's point of view. The two measures were analyzed for concordance and discordance. Results Patient/provider differences in DHI and VSS vertigo subscale scores were not significantly different. However, difference scores on the VSS anxiety/autonomic subscale indicated that providers significantly under-estimated patient anxiety and symptoms of autonomic system activation. Conclusions The results suggest that providers may be missing information pertinent to the role anxiety and autonomic system activation may play in patient visits for complaints of dizziness. We suggest that this problem can be mitigated by administrating to patients prior to their clinic visit a standardized measure that quantifies patient self-report dizziness, vertigo, anxiety and autonomic system arousal. Patterns of response by patients on these measures can enable providers to diagnose correctly dizziness disorders that are rooted in clinically significant anxiety either related to, or unrelated to, a history of vestibular system impairment.

To determine whether the responses of elders compared with younger patients differed significantly on a structured dizziness case history.Retrospective case reviews.Outpatient balance function testing center.Two-hundred thirty-three adults who underwent vestibular function testing and completed a structured case history. The mean age of the adult group (18-64 yr) was 46.4 years. The mean age of the old adult group (65 yr and older) was 76.2 years.Patient's self-reported symptoms on a structured case history questionnaire.Younger adults reported significantly more complaints of true vertigo and associated nausea and vomiting compared with older patients. Older patients tended to report symptoms of unsteadiness or falling. Despite the lack of vertiginous symptoms, BPPV was common in older adults.The clinician should be aware of differences in self-report dizziness symptoms in older patients. Older patients who do not have vertigo may be told that their symptoms are normal for their age, when, in fact, they may have an undiagnosed vestibular system impairment.

Background: The cervical vestibular evoked myogenic potential (cVEMP) is an acoustically synchronized, signal averaged, brief inhibitory response of a contracted muscle usually resulting from an acoustic stimulus. The cVEMP is recorded from the tonically contracted sternocleidomastoid muscle (SCM). The presence and amplitude of the cVEMP is related to both the integrity of the sacculo-collic pathway and magnitude of electromyographic (EMG) activity at the time of recording. Measurement variables include the absolute latency of the primary positive going component (referred to as P13) and interaural (i.e., left versus right) latency differences. Also measured is the peak-to-peak interaural amplitude asymmetry (IAA; percent difference in amplitude, left versus right). It is known that the amplitude of the cVEMP is positively correlated with the magnitude of tonic EMG from which the evoked potential is extracted. Thus, if EMG amplitude is uncontrolled, one cannot determine whether cVEMP asymmetries are occurring due to unilateral end organ disease or asymmetric tonic EMG activity. Two methods have been suggested to control for tonic EMG activity. These include (1) patient self-monitoring of EMG activity with biofeedback and (2) mathematical correction (i.e., amplitude normalization) of the left and right cVEMP waveforms. Currently, it is unknown how effective amplitude normalization techniques are at reducing cVEMP amplitude asymmetry in the presence of varying levels of EMG. Purpose: The purpose of this investigation was to determine whether the use of amplitude correction techniques would reduce significantly the P13-N23 IAA data in otologically and neurologically intact adults when the level of EMG was varied between right and left sides. Research Design: A prospective, repeated measures design was used for three different investigations in which cVEMPs were recorded and then processed using amplitude correction. Study Sample: Subjects were 20 otologically and neurologically health young adults between 21 and 29 yr of age. Intervention: cVEMPs were recorded at four different EMG target levels ranging from 100 to 400 μV. The absolute peak-to-peak amplitude of P13-N23, absolute latency of P13, and the left/right amplitude asymmetry of P13-N23 were measured both with and without the use of EMG amplitude correction techniques. IAAs were calculated using 10 different conditions of varying EMG asymmetry with and without amplitude correction. Data Collection and Analysis: Data were analyzed using repeated measures analysis of variance (ANOVA) to detect tonic EMG level-dependent differences separately for P13 latency, P13-N23 peak-to-peak amplitude, and mean root mean square (RMS) amplitude cVEMP responses. The amplitude of cVEMP responses from the left and right side were used to calculate IAA for subsequent analyses. Linear regression analyses compared level of tonic EMG with cVEMP amplitude. A one-way multivariate analysis of variance (MANOVA) was used to determine if IAAs were significantly reduced following amplitude correction. Any differences found were investigated using unplanned linear contrasts. Results: The uncorrected cVEMP amplitude and RMS EMG all increased significantly with increases in the EMG target levels. With amplitude correction, cVEMP amplitude did not change significantly with changes in RMS EMG or EMG target levels. Conclusions: These findings suggest that the use of amplitude correction techniques represent an effective method of neutralizing the factor of variability in tonic EMG level on the cVEMP that would be otherwise uncontrolled. Indeed when correction is employed in cases of extreme tonic EMG asymmetry, the upper limit of percent IAA is roughly half of that when EMG correction techniques are not used. Our findings are also in agreement with those of Bogle et al (2013) showing that the input/output growth function for P13/N23 amplitude is not linear but, in fact, saturates at supra-maximal stimulation levels. Accordingly, and contrary to what has been published previously, achieving maximum muscle activation may produce a paradoxically inferior signal-to-noise ratio and in some cases result in an artificially small (or undetectable) corrected cVEMP amplitude. cVEMP amplitude either asymptotes (if maximum EMG amplitude saturation occurs at the same stimulus intensity as yields the maximum cVEMP amplitude), or the cVEMP can become smaller if EMG amplitude can increase further beyond the stimulus intensity that yields that largest P1-N1 amplitude. In the latter case the noise increases further to reach maximum and creates a disadvantageous signal (cVEMP) to noise (tonic EMG) ratio.

Background: It has been estimated that as many as 50 million Americans do experience or have experienced tinnitus. For approximately 12 million of these individuals, tinnitus makes it impossible for them to carry out normal everyday activities without limitation. These are the patients that present to audiology clinics for assessment and management. The tinnitus evaluation includes the measurement of acoustical characteristics of tinnitus and the impact that this impairment has on health-related quality of life (HRQoL). Tinnitus is a disorder that often occurs as a result of auditory system impairment. The impairment for some can impart an activity limitation and a participation restriction (i.e., tinnitus-related disability or handicap, respectively). The goal of tinnitus management is to reduce, or eliminate, activity limitations and participation restrictions by reducing or eliminating a patient’s perception of tinnitus or their reaction to tinnitus. Implicit in this statement is the assumption that there exist standardized measures for quantifying the patient’s tinnitus perception and their reaction to it. If there existed stable and responsive standardized tinnitus measures, then it would be possible to compare a patient’s tinnitus experience at different time points (e.g., before and after treatment) to assess, for example, treatment efficacy. Purpose: The purposes of the current review are to (1) describe psychometric standards used to select outcome measurement tools; (2) discuss available measurement techniques and their application to tinnitus evaluation and treatment-related assessment within the domains established by the World Health Organization’s International Classification of Functioning, Disability and Health; (3) list and briefly describe self-report tinnitus questionnaires; (4) describe how valuation of tinnitus treatment can be assessed using economic models of treatment effectiveness; and (5) provide future directions including the development of a tinnitus outcomes test battery and treatment-related study designs. Research Design: Retrospective literature review Conclusions: Although psychometrically robust measures of tinnitus HRQoL do exist, there is no unanimity in, for example, what tests should be included in the tinnitus assessment, and how studies of HRQoL should be conducted. The current authors suggest that future studies employ more rigorous designs and contain (minimally) the following characteristics: (1) utilization of randomized control groups and blinding; (2) appropriate statistical testing including “dropouts” that should be used in an “intention to treat” analysis rather than elimination from the final data set; (3) long-term follow-up assessment to evaluate responsiveness; (4) appropriate inclusion criteria to avoid “ceiling” and “floor” effects; and (5) suitable sample sizes based on the application of power analyses.

ObjectiveThe purpose of this study was to describe the variability and test–retest reliability of a commercially available subjective visual vertical (SVV) system known as Virtual SVV (Interacoustics). In addition, the study aimed to compare the reliability of the Virtual system with a previously established bucket test of SVV.Study DesignFifteen participants with normal hearing, normal middle ear function, and normal utricular function were included in the study. Each participant underwent static SVV testing using both the Virtual system and the bucket test. Subjects completed 2 testing sessions to determine test–retest reliability. For each test, data were collected with the head at 0°, tilted 45° to the right, and tilted 45° to the left.SettingThis study was conducted in a balance function laboratory embedded in a large, tertiary care otology clinic.ResultsThe mean SVV values obtained with the Virtual system were within 1°–2° from 0 with the head positioned at 0°, which is in agreement with many other studies of SVV with the head at 0° (Akin & Murnane, 2009; Halmagyi & Curthoys, 1999; Zwergal, Rettinger, Frenzel, Dieterich, & Strupp, 2009). Using the intraclass correlation coefficient, test–retest reliability of the Virtual system was excellent in the 45° left position and fair to good in the 45° right and 0° position. Test–retest reliability of the bucket test was poor in all head positions.ConclusionsThe Virtual system is a more reliable measure of static SVV than the bucket test. Therefore, the Virtual system could be utilized as a screening device for utricular dysfunction in busy clinical settings.

Purpose The purpose of this investigation was to identify the optimal recording parameters for evoking the ocular vestibular evoked myogenic potential (oVEMP) using air-conduction stimuli. Method Subjects were 17 otologically and neurologically intact adults (age: M = 24.18 years, SD = 1.91 years). The oVEMP responses were elicited using a 500-Hz tone burst air-conduction stimulus presented at an intensity of 95 dB nHL. The setting was a balance function laboratory that was part of a large tertiary care otology clinic. Results The oVEMP electrode montage and body position that yielded the largest oVEMP amplitude was the belly-tendon montage (Sandhu, George, & Rea, 2013), recorded with the subject in the sitting position. The N1 latency recorded with the belly-tendon montage was significantly shorter than that recorded for the infraorbital montage in both the sitting and supine positions. Conclusion The belly-tendon recording montage with the subject sitting yields significantly larger oVEMP amplitudes and shorter N1 latencies than do traditional bipolar infraorbital recordings.

OBJECTIVE The present investigation was conducted in an effort to assess the level of congruence between patients and spouses for the patient's self-reported vertigo severity and dizziness handicap. STUDY DESIGN Prospective study. PATIENTS Fifty consecutive patients, and their spouses, evaluated in our Balance Disorders Laboratory in the Division of Vestibular Sciences. MAIN OUTCOME MEASURES Dizziness Handicap Inventory (DHI), Spouse version of the DHI (DHI-SP), Vertigo Symptom Scale (VSS), Spouse version of the VSS (VSS-SP), and the Hospital Anxiety and Depression Scale (HADS). RESULTS The mean DHI and DHI-SP total scores were not statistically different from one another (t49 = 1.58, p = 0.16) and were strongly correlated (r = 0.79, p < 0.01). The VSS and VSS-SP scores were statistically different (t49 = 2.33, p = 0.02) but were still moderately correlated (r = 0.56, p < 0.01). Spouses tended to overestimate vertigo severity. We observed an increase in the frequency of occurrence of clinically significant anxiety and depression not only in patients but in spouses as well. Furthermore, anxious patients tended to be married to anxious spouses, and depressed patients tended to be married to depressed spouses. Finally, the mean DHI scores were significantly greater for patients with clinically significant anxiety and/or depression, but the presence of patient anxiety and/or depression did not affect spousal congruence. CONCLUSION The results attest to the congruence of patient and spouse perceptions of vertigo severity and dizziness handicap.

Objectives Chronic subjective tinnitus is a prevalent condition that causes significant distress to millions of Americans. Effective tinnitus treatments are urgently needed, but evaluating them is hampered by the lack of standardized measures that are validated for both intake assessment and evaluation of treatment outcomes. This work was designed to develop a new self-report questionnaire, the Tinnitus Functional Index (TFI), that would have documented validity both for scaling the severity and negative impact of tinnitus for use in intake assessment and for measuring treatment-related changes in tinnitus (responsiveness) and that would provide comprehensive coverage of multiple tinnitus severity domains. Design To use preexisting knowledge concerning tinnitus-related problems, an Item Selection Panel (17 expert judges) surveyed the content (175 items) of nine widely used tinnitus questionnaires. From those items, the Panel identified 13 separate domains of tinnitus distress and selected 70 items most likely to be responsive to treatment effects. Eliminating redundant items while retaining good content validity and adding new items to achieve the recommended minimum of 3 to 4 items per domain yielded 43 items, which were then used for constructing TFI Prototype 1.Prototype 1 was tested at five clinics. The 326 participants included consecutive patients receiving tinnitus treatment who provided informed consent-constituting a convenience sample. Construct validity of Prototype 1 as an outcome measure was evaluated by measuring responsiveness of the overall scale and its individual items at 3 and 6 mo follow-up with 65 and 42 participants, respectively. Using a predetermined list of criteria, the 30 best-functioning items were selected for constructing TFI Prototype 2.Prototype 2 was tested at four clinics with 347 participants, including 155 and 86 who provided 3 and 6 mo follow-up data, respectively. Analyses were the same as for Prototype 1. Results were used to select the 25 best-functioning items for the final TFI. Results Both prototypes and the final TFI displayed strong measurement properties, with few missing data, high validity for scaling of tinnitus severity, and good reliability. All TFI versions exhibited the same eight factors characterizing tinnitus severity and negative impact. Responsiveness, evaluated by computing effect sizes for responses at follow-up, was satisfactory in all TFI versions.In the final TFI, Cronbach's alpha was 0.97 and test-retest reliability 0.78. Convergent validity (r = 0.86 with Tinnitus Handicap Inventory [THI]; r = 0.75 with Visual Analog Scale [VAS]) and discriminant validity (r = 0.56 with Beck Depression Inventory-Primary Care [BDI-PC]) were good. The final TFI was successful at detecting improvement from the initial clinic visit to 3 mo with moderate to large effect sizes and from initial to 6 mo with large effect sizes. Effect sizes for the TFI were generally larger than those obtained for the VAS and THI. After careful evaluation, a 13-point reduction was considered a preliminary criterion for meaningful reduction in TFI outcome scores. Conclusions The TFI should be useful in both clinical and research settings because of its responsiveness to treatment-related change, validity for scaling the overall severity of tinnitus, and comprehensive coverage of multiple domains of tinnitus severity.

Background: Stimulus-evoked electromyographic changes can be recorded from the extraocular muscles. These short-latency negative-polarity evoked myogenic potentials are called ocular vestibular evoked myogenic potentials (oVEMPs). To date there has not yet been a large-scale study examining the effects of age on the amplitude, latency, threshold, and interaural differences of the oVEMP to air-conducted stimuli. Further, before the oVEMP can become a useful clinical tool, the test–retest reliability of the response must be established. The oVEMP response, once more completely understood, may provide diagnostic information that is complementary to the cervical vestibular evoked myogenic potential (cVEMP; i.e., sternocleidomastoid muscle). Purpose: To describe the normal characteristics of oVEMP in a cohort of age-stratified subjects, to assess the test–retest reliability of the oVEMP, and to determine if reference contamination occurs using a common recommended infraorbital reference electrode derivation. Research Design: A prospective, descriptive study design was used for an investigation with a threefold purpose in which oVEMP recordings were made from the extraocular muscles (e.g., inferior oblique muscle). Study Sample: Fifty otologically and neurologically normal adults and children served as subjects. Subjects ranged in age from 8 to 88 yr. Data Collection and Analysis: In Investigation 1, oVEMPs were recorded from the ipsilateral and contralateral inferior oblique muscles for all subjects. The stimulus was a 95 dB nHL 500 Hz tone burst. Next, oVEMP thresholds were obtained. Amplitude, latency, and thresholds were tabulated, and descriptive statistics were used to calculate normative values. Age-related differences in oVEMP component latencies, amplitudes, interaural amplitude asymmetries (IAAs), and thresholds were determined using an analysis of variance. In Investigation 2, oVEMPs were recorded twice in 10 subjects, once (test) and once approximately 10 weeks later (retest). Test–retest reliability for the oVEMP peak-to-peak amplitude, n1 latency, p1 latency, n1 threshold, and IAA were assessed with intraclass correlation coefficients (ICCs) calculated using a two-way random-effects, absolute-agreement model. In Investigation 3, a four-channel oVEMP recording was conducted in 10 subjects. Both observational methods and paired-sample t-tests were used to evaluate the effect that reference electrode location had on the oVEMP. Results: oVEMP responses were present bilaterally in 90% of our subjects. The upper limit of oVEMP amplitude asymmetry, defined as the mean plus two standard deviations, was 34% (mean = 14%, SD 10), and the mean n1 latency was 12.5 (SD 1.0) msec. The amplitude of the response significantly decreased and the threshold significantly increased with increasing age, with the greatest age effects occurring in subjects 50 yr and older. Test–retest reliability was acceptable (ICCs for the measurement variables ranged from .53 to .87). Using conventional recommended recording techniques, evidence of reference contamination occurred for all subjects, resulting in a mean amplitude reduction of 30% (range = 18%–43%). Conclusions: Age results in systematic changes in oVEMP measurement parameters. The test–retest reliability is acceptable, and reference contamination averaging 30% is guaranteed using a second infraorbital electrode as the inverting input (i.e., reference electrode) for bipolar recordings. The oVEMP can be used as a complementary diagnostic tool to the cVEMP in evaluating subjects with suspected peripheral vestibular disorders.
